Clinostomum complanatum에 의한 후두 감염 1예

A 22-year-old Korean woman visited our outpatient department with complaints of discomfort and pain on the left part of her throat, occurring 3 days after eating raw fish with chopped fish bone. Laryngeal endoscopy revealed a slow-moving worm, about 5 mm in length, which was found attached to the surface of the left aryepiglottic fold. The fluke was removed by forceps, under local anesthesia with 8% lidocaine spray. The worm was identified to be Clinostomum complanatum (C. complanatum). C. complanatum infection in human is rare and known to occur after eating raw fish, a secondary intermediate host. In humans, metacercariae is known to enter the stomach and then migrate to the throat through the esophagus, causing laryngitis. The only treatment method for this parasite infection is a mechanical removal.

Purpose : Recently, cancer has become a chronic disease that requires supervision because of early diagnosis and the development of therapeutic technology. As a result, cancer patients are interested in improving the quality of their lives besides the treatment of cancer itself. Therefore, it is necessary to conduct a qualitative research to understand the vivid experiences of cancer patients and structure their treatment experience. Among qualitative researches, grounded theory is developed based on the data collected in the field. The grounded theory research method is easy to analyze for the process and structure of the treatment experience. Therefore, the purpose of this study is to provide basic data on the integrated medical experience of cancer patients Methods : Participants were conveniently selected, and the criteria for selection were for those who had more than 1 month of hospitalization so that they could dictate their situation and experience in a meaningful manner. Data was collected through in-depth interviews and continued until the data were saturated through theoretical sensitivity and continuous comparison methods. The collected data were analyzed through the process of open-coding, axial coding, and selection coding, which are the research methods of grounded theory. Results : Cancer patients differ in their path, purpose, and attitudes depending on their respective situations and the internal and external resources of individuals. There is also a difference in the perception of their situation among the cancer patients and their families. Cancer patients were shown to recognize and cope with problems in the integrated medical treatment process, and have been classified into 6 different types after the results. Cancer patients showed positive changes in terms of physical, emotional, and lifestyle after their integrated medical treatment. Cancer patients perceived the integrated medical treatment process as a horizontal relationship structure and with diversity. Conclusions : The experience of integrated medical treatment of cancer patients is a process of rehabilitation that heals the body and restores life within the interaction of support system, contextual situation, and internal resources of the individual. Despite this, there is a need not only for the efforts of integrated medical service providers but also institutional support in the future with regards to the current weaknesses and points for improvement. In addition, there is a need for an objective criterion to measure the outcome of integrated medicine for the standardization of integrated medical services.

가상현실 속에서의 시간성 표현 연구: <성학십도 VR>의 <숙흥야매잠도 VR> 편을 중심으로

조선시대 철학서 『성학십도』는 사람이 마땅히 수행해야 할 내용을 열 가지 도상에 담고 있는데, 이를 현대적으로 재해석해 가상현실에 구현한 프로젝트가 <성학십도 VR> 프로젝트다. 본 연구는 열개의 독립적 VR로 창작된 콘텐츠 중 <숙흥야매잠도 VR>에서의 시간성을 논한다. 이는 제10도「숙흥야매잠도」를 VR로 제작한 것이다. 흔히 시간이라 하면 직선적이고 한 방향으로 흐르는 시간을 떠올린다. 하지만, 반복되는 순환적 시간이나 영원한 시간 등 과학과 철학 분야에서는 다양한 시간의 개념이 추상적으로 접근되고 탐구되었고, 수많은 예술 작업에서도 여러 시간적 표현을 시도하는 다양한노력도 있었다. 「숙흥야매잠도」에서는 아침-점심-저녁의 시간에 지속해서 경을 추구하기 위한 몸과 마음가짐을 수행을 통해 함양하라고 강조한다. 이는 아침에서 저녁으로 흐르는 크로노스(Chronos) 의 직선적 시간을 바탕으로 하지만, 또한 하루의 일과에서 수행을 반복과 지속하며 매 순간 경을 체험하는 카이로스(Kairos)적 접근, 그리고 순환과 영원으로서의 아이온(Aion)의 시간도 함께 담아내고있다. <숙흥야매잠도 VR>에서는 이런 다양한 시간성을 표현하기 위하여 구체적으로 시간을 드러내는 상징적 소재(조선시대 해시계인 앙부일구)를 활용하거나 다양한 시간대의 넘나드는 포털문(Portal)을 이용했다. 본고는 VR에서의 이들의 공간적 배치에 대해서 분석하고, 포털 내 과거와 현재를 넘나드는 영상이 사용자 인터랙션을 통해 어떻게 재생되는지 분석한다. 이는 <The Khronos Projector>와 같이 기존의 물리적 세계에서의 접근과는 다른, 가상 세계만의 시공간 표현의 탐색을포함한다. 철학서의 내용을 VR 콘텐츠로 재매개 하는 과정은 철학서가 담고 있는 시간성에 대해 분석함과 동시에 가상 세계에서만 접근해볼 수 있는 고유의 시간성을 검토하는 기회가 된다.

추력 히스테리시스를 동반하는 하이브리드 로켓의 PI 추력제어

하이브리드 로켓에서 발생하는 추력 히스테리시스를 완화하기 위한 PI 추력제어를 시도했다. PI 제어기 설계를 위해 근 궤적 방법을 이용하여 적절한 게인 값의 범위를 설정하고 자체 개발한 하이브리드 로켓 시뮬레이션 프로그램을 사용하여 응답의 오버 슛의 발생, 상승시간과 초기 응답의 지연 등을 확인하며 게인 값을 결정했다. 또한, 추력 실험을 수행하여 PI 제어기가 추력 히스테리시스의 완화와 추력의 정밀 제어에 미치는 영향을 최종적으로 확인했다. 응답의 오버 슛 발생을 감소시키고 상승시간 지연을 줄이기 위한 실험의 결과, 다른 경우에 비해 응답 오차가 50% 정도 감소했다. 그러나 PI 제어기로 과도 특성을 모두 고려할 수 없으므로 보다 정밀한 추력 제어를 달성하려면 추력 히스테리시스에 대한 비선형 모델과 선형 제어를 결합하는 피드 포워드(feed forward) 제어 방법이 필요하다[12]. PI thrust control was attempted to alleviate thrust hysteresis observed in hybrid rocket combustion. A proper range of gain values was firstly selected using the root locus method for PI controller design, and final values were determined by checking the occurrence of overshoot, rise time, and delay in the response curve using an in-house hybrid rocket simulation program. In addition, thrust tests were conducted to confirm the effect of PI controllers on the mitigation of thrust hysteresis and precise thrust control. Combustion tests confirmed that the occurrence of overshot of responses and rise time delays were significantly reduced by 50% in response error compared to other cases. However, PI controllers cannot be effective in alleviating all these transient characteristics, a feed forward control method that combines the application of a nonlinear model to thrust hysteresis and linear control is needed to improve the precision of thrust control.

한의대생의 글쓰기가 자존감에 미치는 영향

Objectives : Self-esteem is a necessary inner factor for the korean medicine undergraduate, who will perform a various treatment relationship as a future health care provider. Writing is closely related with improving self-esteem by providing insight towards oneself. Methods : The level of self-esteem and current state of writing among 251 undergraduates studying korean medicine in Dongshin University were looked into through a structural survey. Results : Self-esteem score was 3.12 on a scale of 4 which was relatively high, but they showed defensive self-esteem tendency. Meanwhile writing variables appeared to be more influential to the self-esteem of undergraduates than personal characteristics. Also among writing variables, emotional writing and writing 2 to 3 times or more a month was related to high self-esteem which defensive self-esteem tendency is low. Majority of the undergraduates, however, turned out to be writing essay and writing 2 to 3 times a month. Conclusions : The following result suggests the need of writing classes as well as writing involved in emotion for self-esteem and improve medical communication in the curriculum of korean medical college.
