Objectives: The aim of this study is to evaluate the short-term results and complications of treating the intertrochanteric fracture with bipolar hemiarthroplasty (BHA) in elderly population. Methods: We retrospectively reviewed 31 unstable intertrochanteric fracture patients who were treated with BHA between January 2007 and August 2009 in older populations more than 65 years old. The 6 males and 25 females had a mean age of 79.8 years (range: 66-88) and a mean follow-up of 36.3 months (range: 24-55). We analyzed the radiological outcomes, functional recovery grade, using Jensen’s social function score and Harris hip score (HHS), and complications. Results: The average operation time and blood loss was 148.9 min (range, 110-215 min) and 455.2 mL (range, 200-1200 mL). Mean preoperative and postoperative hemoglobin (Hb) was 10.9 g/dL (range, 8.6-13.4 g/dL) and 10.5 g/dL (range, 5.1-14.1 g/dL) respectively. Average 1.3 pints of blood transfusion was performed. Ambulation with (or without) crutch was possible at mean 6.8 days postoperatively. The stability and alignment indices were adequate in all cases at final follow-up. On clinical results, the average HHS score, was changed from 79.7 points (range, 44-100) preoperatively to 73.0 points (range, 46-92) postoperatively, and the preoperative and postoperative Jensen’s score was 1.8 (range, 1-3) and 2.1 (range, 1-4) respectively. Conclusions: The BHA is an acceptable alternative for unstable intertrochanteric fractures in older population.

Generally, shear wall system appears to resist the lateral loads, because high-rise buildings require satisfaction of building drift limit due to lateral loads. However shear wall system used mostly concrete, in this paper steel plate shear wall use to resist the lateral loads instead of reinforced concrete wall system. But, there is lack of research and understanding of steel plate shear wall system so that no real building of such system exists in Korea. This paper demonstrates stability and serviceability of steel plate shear wall by preliminary experimental test.

Background and Objectives: We reviewed the long-term outcomes after tetralogy of Fallot (TOF) repair with trans-annular incision; and evaluated the effectiveness of pulmonary valve replacement (PVR) on outcomes. Methods: This was a retrospective review of clinical outcomes of 180 of 196 TOF patients who underwent total correction with trans-annular incision from 1991 to 1997 (PVR group: 81; non-PVR group: 99). Results: The median age of the patients was 14.0 months (interquartile range [IQR], 10.7–19.8 months) at TOF repair. Ten in-hospital deaths (5.1%) occurred. During the follow-up, 81 patients underwent PVR at the median age of 13.5 years (IQR, 11.2–17.1 years). The patients in PVR group showed better outcomes than non-PVR group in overall survival rate (100% in PVR vs. 88.7% in non-PVR, p=0.007), in all adverse events (arrhythmia, neurologic complications, 95.5% in PVR vs. 74.6% in non-PVR, p=0.024) at 20 years. Age at TOF repair younger than 1 year (hazard ratio [HR], 2.265; p=0.01) and previous shunt history (HR, 2.195; p=0.008) were predictive for requiring PVR. During follow-up, 10 late deaths (5 sudden deaths) occurred in the non-PVR group, mainly due to ventricular arrhythmia and right ventricular failure; there was 1 late death (not a sudden death) in the PVR group. Conclusions: Long-term survival after repair of TOF with trans-annular incision were acceptable. However, arrhythmias were frequently observed during 20 years of follow-up. The patient age <1 year at the time of TOF repair and shunt implantation prior to TOF repair were predictive factors for requiring PVR.

Background: Skin prick tests (SPTs) has relatively a good correlation with allergen-specific IgE against house dust mites and pollens, whereas their correlation to fungi remains low. Objective: We aimed to investigate the prevalence and correlation between fungus-specific IgE and SPTs in patients with chronic lower respiratory symptoms. The difference between fungus-specific IgE-positive and -negative groups was also examined. Method: A total of 566 patients were included in this study, all of whom had either negative SPT results, positive for one or more fungi, or positive for dusts or mixed threshings. Result: SPTs and allergen-specific IgE tests for fungus were positive in 9.4% and 11.8% of the patients, respectively. The positive concordance rate for fungus-specific IgE with SPTs was 47%. The k statistic for the agreement between SPTs and fungus-specific IgE test was 0.349. In all populations negative for SPTs, total eosinophil counts, total IgE, ECP and bronchial reversibility were higher in the fungus-specific IgE-positive group. In the group that tested only positive for dusts or mixed threshings with SPTs, total IgE and bronchial hyperresponsiveness were higher in the fungus-specific IgE-positive group (P<.05). Conclusion: The rate of successful detection of fungus using SPTs was low compared to a fungus-specific IgE. Thus, fungus-specific IgE may be useful to detect fungus allergies in patients with chronic lower respiratory symptoms. (Korean J Asthma Allergy Clin Immunol 2009;29:33-38)

Occurrence and effect of horizontal load, such as the recent frequent earthquakes and seismic load and wind load due to the increase of demand for high-rise buildings are no longer able to not consider the possibility of damage and secondary damage of the exterior panel. Therefore, in this study, by the exterior panels that using moving clips to follow the displacement that occurs in the structure of the building, there is the object of the present invention is to prevent damage and falling off of the exterior panels. Perform the finite element analysis and the actual large shaking table test of the exterior panels, to verify the structural performance, satisfies the non-structural elements seismic design for Korean building code.
