        간호사와 간호조무사의 업무범위에 관한 고찰

        In Korea, under the current medical law, nurses are medical personnel and are granted a license to perform medical activities such as medical assistance within that range. On the other hand, nursing assistants are not medical personnel, but they are allowed to perform nursing and medical assistance with qualification recognition. In this situation, due to the increasing demand for nursing, a 'proposal to enact the Nursing Act' has been discussed, which separates the nurse's job among medical personnel and stipulates necessary matters related to nursing in a separate law. However, the bill stipulates the work of nurses and nursing assistants, and it was difficult to find a large difference between the proposed nursing law enactment and the current medical law. Therefore, in this paper, the status of nurses and nursing assistants in relation to the principle of division of labor, especially in cases that actually occurred through Korean precedents, and the scope of medical assistance of nurses or nursing assistants under the guidance of doctors, dentists, and oriental doctors were reviewed to examine the duties or scope of work between each job. In addition, in Japan and the United States, we compared the systems corresponding to nurses and nursing assistants in Korea and how they understood the scope of work of each job. In order for the enactment of the nursing law to be justified in the future, it is necessary to check what the work of nurses and nursing assistants is and how to coordinate their scope in line with the changing policy reality. This can be referred to as the work setting according to the system of nursing personnel, as shown in Japanese and American legislation, including precedents.

        Suggestion of System Reforms for Assistant Nurses at the Dental Hospital or Dental Clinic

        The dental assistant system has not been established in Korea, and the assistant nurse workforce at the dental clinics has instead taken the role of the dental assistant. Therefore, for some time now, there have been some arguments between dental hygienists because of the limitation of their tasks by the law. The assistant nurse system should be reformed for dental works. The main theme would be considered from the reference for study, for supply, and the task of the dental assistant workforce compared with dental hygienists. From the study, the ideas for system reform for assistant nurses who have worked at the dental clinics would be suggested with the consideration of present dental situations. The co-operation of the workers at dental clinics should be required between dental hygienists and assistant nurses together by changing relative regulations. The dental expert or professional assistant nurse system should be established in the future.

        Relationship between Unit-Level Nurses' Expectations from Nursing Assistant Roles and Individual Nursing Assistants’ Information-Sharing Behaviors: A Multilevel Mediation Analysis

        Purpose: This study aimed to investigate the relationship between the expectations of unit-level nurses from nursing assistants (NAs), frequency of individual NAs' information-sharing behaviors with nurses, and the effect of NAs’ self-perceived roles on this relationship. NAs provide patient care along with nurses, and their information-sharing behaviors with nurses may be influenced by the expectations of the nurses. Methods: Nurses and NAs from 104 integrated community care (sub- and postacute) units were included in this Japanese cross-sectional study conducted from July to September 2018. Nurses' expectations from NAs and the NAs' self-perceived roles and information-sharing frequency were measured. Multilevel mediation analysis was performed for NAs’ information-sharing behaviors, such as expressing, asking, providing linguistic responses, and providing feedback. Results: Unit-level nurses' expectations from NAs were associated with the frequency of NAs' asking and responding behaviors with nurses (p < .05), mediated by the NAs' self-perceived roles. The frequency of NAs’ expressing behaviors and feedback were also associated with their self-perceived roles (p < .001). Conclusions: High expectations of unit-level nurses from NAs led to better perception by NAs of their roles and led to better information-sharing behaviors. Educating nurses on NAs’ roles may improve information-sharing between nurses and NAs that leads to safe and appropriate care to patients.

      • Field Education Model for Assistant Nurses using Edutech: Flipped Class

        Purpose - This study is to suggest a model of field education in the Assistant Nurses curriculum using edutech and to produce competent Assistant Nurses students reflecting the requirements of various medical fields. This model expects to upgrade the quality of the field education and to provide an Assistant Nurses school with standardized field education tools using edutech. Research design, data, and methodology - Throughout the review of the related thesis, most of them were studied on Assistant Nurses' job satisfaction, conflicts with other jobs in hospitals, and Assistant Nurses' job area in nursing hospitals. To study the current field education for Assistant Nurses students in hospitals, it used interviewing the heads of the hospital nursing department and reflecting on their interview results to develop the model of field education. Result - The field education model with edutech is processed with flipped class. Each area in flipped class is designed by applications and webs which is friendly to both teachers and students. Conclusion - This study presents a simple and easy process of field education using edutech. In the next study, it needs to find the precious results of comparison between students educated by the new model in field education in the Assistant Nurses' curriculum or not.

        Improvement of the items of the Korea nurse assistant licensing examination through a job analysis of nurse assistants

        Background: It aimed to analyze the duties of nurse assistants and to provide recommendations for improving the Korea Nurse Assistant Licensing Examination (KNALE). Methods: In order to understand the job characteristics, job content and requirements and KNALE topics were reviewed. Furthermore, the experts’ opinions concerning nurse assistants (specialized high school teachers, nursing and health professors, nursing academy directors, physicians, and oriental medicine doctors) were assessed. A seminar for convergence, a focused group interview with 10 nurses and 7 health scientists, was done. The online questionnaire for 265 nurses, nurse assistants, health science majors, and medicine majors were also conducted. Results: The survey from experts and nurse assistants revealed that the job characteristics of a nurse assistant are diverse and that the KNALE is structured in a way that does not match the professionalism required on the field. The job-based exam items should be included to improve the KNALE. Specifically, the number of practice-based items should be greater than that of theory-based items and should emphasize on “infection and injury” and “emergency response” areas. The inclusion of a former nurse assistant who can contribute to items production and item review is highly favored to improve the items’ suitability to nurse assistant’s job. Conclusion: It is necessary to improve the examination items by placing a greater emphasis on areas of “infection and injury” and “emergency response,” because a high level of such knowledge is demanded from nurse assistants who work in hospitals.

        간호업무 관련 의료법 개정의 의의와 향후 과제

        On 29 December 2015, the Medical Service Act regarding nursing services was amended and enforced from 30 December 2016. Provision on nurse's task was vague under previous Medical Service Act, which had a gap with actual nursing field. Nurses' roles may be replaced by nurse assistants so that boundaries between two professions were blurred. Also, nurse assistants were in the blind spot of supervision and audit which needs to quality control mechanism. In this regard, this study examined the revision of nursing services related Medical Services Act and reviewed significance and policy implication on revision. Firstly, nurse's mission and duties were specified. It recognized nurses role on health management professional which implies improved professionalism. Secondly, qualification management for nurse assistant was enforced. Nurse assistants are obtained accreditation of qualifications from the Minister of Health and Welfare after completing the curricula prescribed and passing the national examination. An education and training institutes for nurse assistants are assessed and accredited. Registered nurse assistants should declare their qualification as well as get refresher training. Thirdly, nurses give guidance on the assistance performed by nurse assistants which leads to division of mission and duties among nursing staffs. Future tasks of revised Medical Service Act are accumulating cases after amendment, developing supporting policy for management of nurse assistants, managing supply and demand on nursing staffs, and discussion of whole nursing professions including midwife, nurse practitioner, physician assistant, and others. 2015년 12월 29일 간호사 및 간호조무사와 관련된 의료법 일부 개정안이 국회 본회의를 통과하여 최종공포 되었다. 기존의 의료법에는 간호사의 업무가 추상적으로 명시되어 있어 실제 간호현장에 부합하지 않다는 것이 문제점으로 지적되어 왔고 간호사 수급불균형에 대한 대책으로 간호사의 역할을 간호조무사가 대체할 수 있도록 함으로써 간호사-간호조무사간의 업무 경계가 모호해진 상황이었다. 또한 간호조무사의 질 관리 및 감독이 사각지대에 있어 질적 수준의 제고가 필요하였다. 본 연구에서는 간호업무와 관련된 의료법 개정의 주요내용을 살펴보고 법 개정의 의의와 정책적 시사점을 도출하였다. 주요한 내용으로는 첫째, 간호사에 대한 업무는 의료법 제정 이후 64년 만의 첫 개정으로 개정된 의료법에서 간호의 전문성을 살리고 건강관리전문가로서의 간호사의 역할을 인정하는 방향으로 간호사의 업무규정이 구체화되었다. 둘째, 간호조무사에 대한 자격관리가 강화되었다. 간호조무사에게 보건복지부 장관 자격을 부여하였으며, 간호조무사 교육훈련기관의 지정·평가제도, 간호조무사의 자격신고 및 보수교육제도가 도입되었다. 셋째, 간호조무사의 업무에 대한 지도권이 간호사에게 있음을 법률에 명시함으로써 간호인력 별 역할분담 및 적정 간호인력 공급의 틀을 마련하였다. 간호업무와 관련된 의료법 개정의 향후 과제로서 간호인력 간 업무범위 구분에 따른 사례축적, 간호조무사 관리체계 세부지침 마련, 간호인력 수급관리 등이 이루어져야 할 것으로 보인다.

        전담간호사의 감성지능과 셀프리더십이 직무만족에 미치는 영향

        Purpose: This study aimed to identify the effects of emotional intelligence and self-leadership on job satisfaction among physician assistant nurses. Methods: The participants were 146 physician assistant nurses working at two university hospitals. Data were collected from August 1-September 31, 2020 and analyzed through t-test, ANOVA, Scheffé test, Pearson correlation, and hierarchical regression analysis using SPSS/WIN version 26.0. Results: Factors affecting job satisfaction were self-leadership (β=.30, p=.003), “less than 1 year of experience as a physician assistant nurse” (β=.27, p=.025), and emotional intelligence (β=.25, p=.007), and the explanatory power was 34.4% (F=6.03, p<.001). Conclusion: Our study shows that self-leadership and emotional intelligence play a significant role in the job satisfaction of physician assistant nurses; thus, strengthening these two factors is crucial to improve the nurses’ job satisfaction. The results of this study may serve as basic data for the development of strategies to enhance job satisfaction among physician assistant nurses.

        트랜스내셔널 관점에서 본 독일 한인간호이주의 역사

        While many studies have addressed the Korean nurse immigration to Germany in 1960s-70s in terms of the nurses’ personal histories from a national perspective, few studies have pointed to its transnational impact on the medical field. Given this gap, the paper discusses its significance in the medical history through examining the nursing culture and policy changes made in both countries initiated by the immigration of the qualified Korean nurses. For this end, the paper first discusses differences between Korea and Germany in the nursing culture and professional standards at the time of the nurse immigration. The study then examines the transnational changes in the two nations in the nursing professional culture and related policies including nurse education and qualification process. More specifically, the paper argues that the Korean female nurses not only supplied labor in need but also contributed to the medical policy changes in Germany. The prevalent stereotype of nursing as a non-professional field in the German society begin to change while experiencing professionally qualified Asian immigrant nurses and practiced treatments that had been conduced by German doctors by then. This observation actually leads to the policy level reforms in nursing education and qualification process to meet the need of high-quality nurses when the labor immigration was stopped in mid-1970s. The paper also points out that the influence of the nurse immigration on the Korean society is not merely economic; it brought the policy level reforms in nurse education and qualification as well. The immigration, resulting in a lack of nurses in the Korean society, triggered two main responses: the expansion of the existing nurse education system and the establishment of a new system called “nursing assistant.”
