        규칙적인 재활운동프로그램이 뇌졸중 편마비 환자의 유병기간에 따른 관절가동범위와 균형능력 및 보행능력에 미치는 영향

        The purpose of this study was to compare the effects of regular rehabilitation exercise on duration of the range of motion, Balance and gait ability in Hemiplegia. The objects of this study covered 26 patients with hemiplegia, age from 40 to 55 yeare old and fell into two groups which was a group of 1years and 6months and the regular rehabilitation exercise was conducted 3 times a week for 50 minutes each for 12 weeks. The clinical assessment were obtained before and after 12weeks regular rehabilitation exercise on affected ankle range of motion. To compare the differences in balancing ability before and after the regular rehabilitation exercise, static balance ability was measured by using balance system SD. Also, the changes before and after the regular rehabilitation exercise were measured through physical composition and walking related to physical strength test. The followings conclusions have been obtained through t-test analysis of the results obtained through the research. The results of this study were as follows; Firstly, 6month duration Hemiplegia showed statistical significance that range of motion of the ankle dorsiflexion and plantarflexion(p<.01)(p<.01). Between group showed statistical significance(p<.01) (p<.05). Secondly, 6month duration Hemiplegia showed statistical significance that overall balance index (OBI), anteroposterior balance index(ABI), mediolateral balance index(MBI) (p<.01) (p<.01) (p<.01). Between group showed statistical significance(p<.05). Thirdly, 6month and 1years duration Hemiplegia showed statistical significance that gait ability(p<.05)(p<.01). Between group showed statistical significance(p<.05). This study suggests that regular rehabilitation exercise is an effective treatment in range of motion, balance and gait ability rehabilitation of patients with hemiplegia and 6month group to be more than effected.

        수중재활운동이 편마비 아동의 대근육 운동기능에 미치는 효과

        The purpose of this study was to examine the effects on the gross motor skills of the children with hemiplegia by aquatic rehabilitation exercise. 12 children with hemiplegia were selected as subjects of this study. They were divided into two groups; one group was exercise group(n=6), and the other group was control group (n=6). The results of this study were as follows; 1. Aquatic rehabilitation exercise showed effect on sitting trunk flection of children with hemiplegia. 2. Aquatic rehabilitation exercise showed effect on standing long jump, shuttle run change of body position, sit-up of children with hemiplegia. 3. Aquatic rehabilitation exercise showed effect on opened-eyes foot balance, closed- eyes foot balance of children with hemiplegia. 4. Aquatic rehabilitation exercise showed effect on movement skill test battery`s total score of children with hemiplegia. In conclusion, aquatic rehabilitation exercise has a positive effect on the body flexibility, muscle strength, agility and balance which is contribute to improve of physical fitness and health. Therefore, further studies were suggested that various and specific measures for the practical application of this program were to be applied.

        운동생리학 : 상·하지 점증 부하 최대운동이 성인 편마비 장애인의 심폐기능에 미치는 영향

        The purpose of this study was to find the cardiopulmonary function between upper extremities by arm ergometer exercise and lower extremities by cycle ergometer exercise in adult hemiplegia.The subjects selected for the experiment were composed of 20 male and female(ll male adult hemiplegia, 9 male normal subjects).The subjects were given an explanation of all laboratory procedures before they consented They performed continuous incremental work load test by way of arm ergometer(Cybex MET300™, U.SA) and cycle ergometer(Cybex MET100™, USA). They were warmed up with 60RPM at 0 watt for 2 minute and thereafter the work rate was gradually increased 125 watt every 2 minute and thereafter the work rate was gradually increased 12.5 watt every 2 minute until exhaustion in maximal arm ergometer exercise test. Also, They were pedalled with 60RPM at the begining with unloads for 2 minute and thereafter the work rate was gradually increased 125 watt every 2 minute until exhaustion in maximal cycle ergometer exercise test. Respiratory gas was analyzed by automated respiratory ags analyzer, Quinton(USA) every 20 second.Statistical techniques for data analysis were applied 2 way repeated measure ANOVA The 0.05 level of significance was used as the critical level for rejection of the null hypotheses for the study.On the basis of the results analyzed in this study, conclusions were drawn as follow1) Significant differences in HRpeak were observed between the 2 groups (male adult hemiplegia and normal male on arm ergometer and cycle ergometer (p=0.001) during graded maximal exercise.2) Significant differences in VO<sub>2</sub>peak were observed between the 2 groups ( male adult hemiplegia and normal male on arm ergometer and cycle ergometer(p=0.0002) during graded maximal exercise.3) Significant differences in VO<sub>2</sub>peak/kg were observed between the 2 groups (male adult hemiplegia and male normal on arm ergometer and cycle ergometer(p=0.0001) during graded maximal exercise.

        PNF Concept중 Scapula and Pelvis의 Symmetrical Reciprocal Pattern이 Hemiplegia환자의 보행속도와 균형감각에 미치는 효과

        Purpose: The purpose of this study was to determine the effect of symmetrical-reciprocal pattern of scapula and pelvis exercises using proprioceptive neuromuscular facilitation (PNF) on gait speed and balance in patients with hemiplegia. Methods: Among the adult patients with hemiplegia that were hospitalized at Michuhol Rehabilitation Center after being diagnosed with stroke, 10 that were capable of independent walking for more than five minutes and that understood and cooperated with the therapy and test methods of this research, were selected as subjects. The therapy was implemented based on the concept of PNF, and it was performed on a low mat and a height-adjustable mat, as proscribed by the fundamental procedure for PNF. Symmetrical-reciprocal pattern of scapula and pelvis exercises were applied to the patients in the decubitus position. The therapy scheme included stabilizing reversals, rhythmic stabilization, and a combination of isotonics, rhythmic initiation, and dynamic reversals. To investigate gait speed and body trunk mobility before and after the symmetrical-reciprocal pattern of scapula and pelvis exercises were applied, walking speed for a distance of 10 m was measured and balance was tested based on the Berg-Balance scale test table. The Berg-Balance scale test was performed by one therapist to minimize any error that could occur from the subjective evaluation method used by therapists. Results: Gait speed increased by 8.97 seconds after applying the symmetrical-reciprocal pattern of scapula and pelvis exercises using the concept of PNF, showing a significant difference (p<0.01). However, balance showed no significant difference after the therapy (p>0.14). Conclusion: Exercise therapy that uses the symmetrical-reciprocal pattern of scapula and pelvis with the concept of PNF can be said to be a useful therapeutic technique that can enhance the walking speed of patients with hemiplegia.

        The Analysis to Movement Patterns in Standing Up In Children with Spastic Hemiplegia

        Purpose This study aimed to evaluate the strategical changes of movement patterns and measure the MSS(mean segmental score)of children with spastic hemiplegia according to age by observing the standing up movements of spastic hemiplegia children. Method We performed visual analysis on the segmental movement pattern during standing up movement from supine position of 8 normal children and 8 children with spastic hemiplegia. We used Mann-Whitney U test to evaluate the changes in MSS analysis and motion patterns of upper limbs, axis, and lower limbs and the changes of motion strategy in relation to the changes of age in the two groups of children. Result The result are as follows; 1) There was significant difference in MSS analysis on "forward with rotation" of axial region between the two groups of children aged 4~7 years (p<O.02). 2) There was significant difference in MSS analysis on "Narrow-based symmetric squat" of lower limbs between the two groups of children aged 4~ 7 years and 8~ 12 years (p<O.01). 3) The children with spastic hemiplegia were using more flexor muscles when changing posture, and significant increase of asymmetrical motion pattern was observed when supporting the upper and lower limbs regularly. Conclusion Through the approach in this study, we could obtain knowledge about the regularity of motion patterns of children with cerebral palsy and as a result we could easily recognize the characteristics of posture and motion of children with cerebral palsy.

      • 냉·온수적용이 편마비환자의 손기민성에 미치는 영향

        Background : This study was to investigate effect of hot and cold water for the hand dexterity. Methods : A total of 37 subjects having hemiplegia(men=6, women=11) and normal(men=10, women=10) were participated in this study. After both hands was soaked in the small tube for 1 minute, we measured 'making dots in circles(BOTMP)', 'lifting small objects(JHFT)', 'disk turning(MMDT)' test. Results : In the case of normal's dominant hand, there were statistically significant differences in all tests after soaked in the hot water but there were statistically significant differences in 'disk turning' test after soaked in the cold water. In the case of hemiplegia's unaffected hand, there were statistically significant differences in 'disk turning' test after soaked in the hot water but there were statistically significant differences in 'making dots in circles', 'disk turning' test after soaked in the cold water. In the case of hemiplegia's affected hand, there were statistically significant differences in 'lifting small objects', 'disk turning' test after soaked in the hot water but there were statistically significant differences in 'making dots in circles', 'disk turning' test after soaked in the cold water. Conclusion : The hot and cold water were affected in the hand dexterity. Especially, in the case of both normal and hemiplegia, there were statistically significant differences in 'disk turning' test after soaked in the cold and hot water. Therefore, We were founded that affected in gross movement than fine movement.

        자세근 강화운동이 편마비환자의 보행에 미치는 영향

        The purpose of this study was to determine the effect of strengthening exercise on the postural muscles for patients with hemiplegia. Subjects and Methods Sixteen subjects with hemiplegia (mean 56.6±9.9 years) were recruited. Treatment outcomes were assessed with Timed Up and Go test ad 6MWT. Results After a 4-week training period, the mean time of TUG decreased significantly and 16% increased in 6MWT. Retraining gait in patients with hemiplegia while to promote the activity of anti-gravity muscles was supported resulted in better walking abilities. Conclusion This novel approach provides efficient approach for the therapy of gait disturbance in patients with hemiplegia.

        편마비 환자의 재활치료 효과를 위한 파크골프 도입에 대한 고찰

        The purpose of this study suggest that Park Golf for rehabilitation training of the patients with hemiplegia. The increase of the aged caused various problems. Among the problems, chronic illnesses the old people suffer from charge their family with mental, physical, social and economic burden as well as the person. Especially, the more old people are the more risks and attacks of cerebral vascular accident(CVA) exist. Therefore, the increase of the aged means the increase of cerebral vascular accident(CVA) patients. The cerebral vascular accident(CVA) patient get into difficulties to secure their livelihood due to troubles and disoders, so medical care facilities and rehabilitation training are really necessary to help them return to their normal life with mental, physical and social capability. The rehabilitation of patient with hemiplegia is ongoing process. It may possibly accomplish throughout the whole life because it takes a lot of time and effort. In addition, an ultimate aim of the rehabilitation is returning and adaptation to society. In this way, it is hardly to expect a satisfied results of the rehabilitation without motive. Therefore, patient with hemiplegia rehabilitation motive will is the most important. If interesting of park golf the patient with hemiplegia can be absorbed in easily are applied to the rehabilitation training, the patient are will to participate in the training without troubling.

        키네시오 테이핑과 요천추 보조기를 적용한 트레드밀 훈련이 편마비 환자의 균형과 보행에 미치는 영향

        Objective: This study was conducted to investigate the effect of balance and gait on the treadmill training in patients with hemiplegia applying kinesio-taping and lumbosacral orthosis. Method: Thirty patients with hemiplegia were randomly selected and divided into three groups.: treadmill training group, treadmill training applying kinesio-taping group, and treadmill training applying lumbosacral orthosis group and they were 30 minute and 6 week trainings. Results from the study are described as follows. Results: Treadmill group shows significant difference in the length of 10 m gait test, gait speed, gait cadence, and stride(p<.05). Treadmill Applying kinesio-taping group shows significant difference in the length of 10 m gait test, gait speed, gait cadence, and length of stride(p<.05). Treadmill applying lumbosacral orthosis group shows significant difference in BBS, TUG, 10 m gait test, gait speed, gait cadence, stride, and stance phase and swing phase of the affected side(p<.05). In the comparison with interventions, there was a significant difference in BBS, TUG, 10 m gait test, gait speed, gait and stance phase and swing phase of the affected side(p<.05). Conclusion: These findings demonstrate that there was a difference in the effect of kinesio-taping and lumbosacral orthosis on trunk stability of patients with hemiplegia during treadmill training and application of lumbosacral orthosis leads to a more effective result in balance and gait changes during treadmill training for patients with hemiplegia.
