        미소안면침의 얼굴 축소 및 피부상태 개선 효과

        Objectives : The purpose of this study is to evaluate the effects of Miso facial acupuncture on facial reduction and improvement of skin condition. Methods : From March to May 2012, 20 women in their twenties to forties with no facial diseases were recruited. Miso facial acupuncture was performed on the both side of their face twice a week, total seven times. The 3D face photographs of each participant were checked 5 times ; before treatment, after 1st treatment, 4th treatment, 7th treatment and 10 days from the last treatment using RS-400FL(Morpheus Co. Korea) and REAL FACE. The surface distances of the 4 lines on the face were measured using Renai MEF program. The improvement of skin condition was evaluated by water content, trans epidermal water loss(TEWL), erythema index and melanin index using MPA 5. They were checked 4 times ; before treatment, 4th treatment, 7th treatment and after 10 days from the last treatment. We analyzed data using Paired t-test(p<0.05) and Wilcoxon signed rank test(p<0.05). Results : One case was dropped out due to pain and edema after 4th treatment. The surface distances of the 4 lines on face were significantly reduced and maintained after 10 days from last treatment. Water content was significantly increased and maintained after 10 days from last treatment. TEWL was significantly decreased. Erythema index was significantly decreased from the 3rd evaluations. Melanin index was slight decreased with no significance at the 2nd evaluations. Conclusions : These results show that Miso facial acupuncture can reduce sizes of the face and improve skin condition such as water content, TEWL and erythema index except melanin index.

        민감성 피부에서 부식성 및 비부식성 자극물질에 의한 피부자극에 관한 비교연구

        Background: Sensitive skin is largely agreed to be a lay term used by individuals who have reduced tolerance to frequent or prolonged use of cosmetics and toiletries, with symptoms ranging from subjective neurosensory forms of discomfort such as stinging, burning, itching, tightness and smarting sensations to visible signs of irritation such as erythema and scaling. Although the concept of sensitive skin is intriguing to many dermatologists, the diagnosis is dependant on self-perceived symptoms. Models to define sensitive skin are not standardized, and the term sensitive skin can be misleading. Objective: The purpose of this study was to evaluate difference of influence to skin barrier function between corrosive and non-corrosive agents in sensitive skin. Methods: 0.1%, 0.25%, 0.5%, 2% solution of sodium lauryl sulfate (SLS) and 1%, 2.5%, 5%, 20% solution of nonanoic acid (NAA) were applied on volar forearm skin for 24 hours in healthy controls and self-perceived sensitive skin subjects without underlying problems. Clinical score, transepidermal water loss (TEWL), and erythema-index (E-index) were compared between healthy controls and the sensitive skin group. Results: Clinical scores, TEWL values and E-indices showed a rising curve concentration for each irritant in a similar pattern. There were no significant differences in clinical score and TEWL value between non-sensitive and sensitive skin. In contrast, E-indices evaluated from sensitive skin subjects were much higher than those in a healthy controls. Conclusion: An elevated TEWL value indicates barrier impairment. An increase in the erythema parameter shows that cutaneous vascular reactions have an unspecifically exaggerated reaction to various irritants and this induces multifarious inflammatory reactions and vasodilation. Therefore, the results of this study suggest that sensitive skin without underlying disease is derived from cutaneous vascular hyperreactivity and various inflammatory reactions, rather than barrier impairment.       • Steroid induced rosacea: A retrospective review in a single institution from 2011 to 2021

        Background: Rosacea is a common chronic inflammatory disorder affecting facial skin. Uncontrolled use of topical steroid has caused rosacea. Currently, no accurate method is available for assessing the severity of rosacea but color imaging systems can reliable for evaluating the severity of rosacea. Objectives: The aim was to determine correlation between the potency of topical steroid that applied to face and the severity of steroid induced rosacea. Methods: We conducted a retrospective analysis of 19 patients with clinical suspicion of steroid induced rosacea who visited the Korea University Ansan Hospital from 2011 to 2021. For each patient, skin color analysis system (SpyderCUBE and SpyderCHECKR@24) was used to measure the erythema severity index and facial area index showing erythema. Additionally, a history was taken of topical steroids type and duration used prior to the onset of rosacea. Spearman’s rank correlation analysis test was used for the statistical analysis. Results: Coefficient of correlation between the potency class(class 1 ~ class 7) of used topical corticosteroids and a erythema severity index is 0.419(p-value = 0.042); with the erythema area index is 0.300(p-value = 0.101). Conclusion: Our study revealed that higher potency of topical steroid can induce more severe erythematous steroid induced rosacea. Because conventional rosacea scale could not quantitatively indicate the degree of erythema or the area involved, skin color imaging system was used for analysis.

        Tape Stripping과 Sodium Lauryl Sulfate에 의한 피부 장벽기능의 손상 및 회복에 관한 연구

        N/A Background:The main function of the stratum corneum is to serve as a barrier preventing the penetration of irritants and transepidermal water loss (TEWL). Previous studies have shown that the cutaneous barrier disruption by application of acetone or sodium lauryl sulfate (SLS) and tape stripping leads to various metabolic changes. Recent studies showed that there was some difference in the recovery rate between tape-stripped groups and acetone-treated groups, and an individual irritant not only exerted rather specific morphologic effects on the cellular components of the keratinocyte but also induced distinct morphological alterations of the different components of the stratum corneum. In other words, this data suggested that individual irritants could show different cutaneous barrier recoveries. Objective:This study was done to compare the recovery rate after cutaneous barrier disruption between a tape-stripped group and a SLS irritated group, and to test the relationship among baseline TEWL, TEWL after the SLS irritation, recovery of TEWL, and number of strippings needed to increase TEWL up to 20g/m2/h. Methods:The permeability barrier of normal human skin was disrupted by tape stripping and patch of SLS and the recovery rate was assessed by measure of TEWL and erythema (E-) index. Results:The number of tape strippings needed to increase TEWL up to 20g/m2/h was 32.7±9.6 (20-50).         Clinical Study on Skin Improvement Effect in Adult Women of Age 40 to 50 Using Cosmetics Containing Sea Cucumber Extract

        The purpose of the study was to investigate the effects of sea cucumber extract on skin as a natural cosmetics functional material. Subjective evaluation of cosmetics before and after were conducted with questionnaires regarding moisture content, sebum content, melanin index, and erythema index. Experiments were conducted on improvement efficacy using skin clinical trials and questionnaires to evaluate changes in perception of skin condition and efficiency of products. With the aim of minimizing skin irritation, the efficiency of the solvent used for extraction was an important factor, and the sea cucumber extract was harvested with efficient extraction conditions at a ratio of 1:10 of 50% ethanol. The study aimed to identify the suitability of sea cucumber extract as a functional cosmetics material to improve the moisturizing ability of skin and its effect on the skin by adding marine natural animal sea cucumber extract. Clinical studies on cosmetics skin containing sea cucumber extract, excellent skin improvement effect from all items of clinical experiment in experimental and control groups. Sea cucumber extract was proved to be a stable, non-adverse physiologically active substance against abnormal symptoms or side effects of skin reactions and skin problems. In addition, the study found excellent results that can lead to its use as a cosmetics material. This is expected to contribute to the development of various cosmetics industries.

        반사 분광광도법 ( Reflectance spectrophotometry ) 을 이용한 화염상 모반의 색소 레이저 치료 효과의 평가

        Background: At present, flashlamp-pumped pulsed dye lasers are treatment of choice for the treatment of congenital capillary malformations like nevus flammeus. Clinical methods have been generally used to evaluate their effectiveness in many papers. Objective: To evaluate the effectiveness of the laser therapy in nevus flammeus patients, we used reflectance spectrophotometry as a supportive objective method in addition to the clinical evaluation. Methods: Thirty-two patients with definite nevus flammeus skin lesion, mainly on head and neck, were treated with flashlamp-pumped pulsed dye laser(SPTL-1b) at 8 week interval. Patients without previous treatment and the others with previous treatments were grouped separately to compare the influence of previous treatment to laser therapy. Erythema/Melanin indices were measured before and after laser therapy in both normal and lesional skin. Based on these indices, relative blanching effect were calculated. Results: Clinically, they showed 58.6% improvement in their skin color in average and Erythema/Melanin indices measured on normal skins were 13.09+3.91 a.u. and 34.65+2.82 a.u.. Erythema indices measured on lesional skins were 26.07+10.26 a.u. and 20.73+8.81 a.u. in non-treated group and 24.53+6.14 a.u. and 21.18+5.47 a.u. in pre-treated group, alternatively. Relative blanching effect in both groups were calculated as 35.9% and 31.6%, meaning no significant difference(p>0.3). Clinical evaluation and relative blanching effect showed good correlation(r=0. 691). Conclusion: Reflectance spectrophotometry can be useful in evaluating successful laser treatments in nevus flammeus patients. Reflectance measurements, an objective estimate of blanching, correlate well with the clinical results, and are helpful in monitoring and predicting the therapeutic outcome in dye laser treated patients.         This study aims to identify erythema index, melanin index of facial skin of middle-aged women by non-invasive method. This measurement intends to grasp the skin condition on each facial part of middle-aged women, check the correlation between the measured values and determine the relation between objective measured values and the general characters The results were as follows: 1. For objective measurement by part of a face, erythema index and melanin index were higher on cheek, forehead and eye rims, forehead and cheek, respectively. 2. erythema index in facial skin showed significant correlation with the place of residence, educational background, monthly income of a household, driving that respondents observed by themselves and variables of skin troubles recognized by respondents. 3. melanin index in facial skin was significantly related to age, place of residence, educational background, monthly income of a household, driving that respondents observed by themselves and variables of skin troubles recognized by respondents. To sum up, the objectively measured values of skins, erythema index, melanin index vary on parts of a face and the significant relations were observed among measured values. Moreover, diverse variables such as place of residence, age, exposure to ultraviolet rays on skin conditions.
