        일부지역의 구강위생용품 사용실태에 관한 조사연구

        Objectivs: The purpose of this study is to provide basic data for the use of oral hygiene devices and to improve oral health care continuously by encouraging the use of oral hygiene devices. Methods: The survey was carried out by questionnaire research targeting 395 respondents in metropolitan area in June-August 2009. The collected data is analyzed by statistics program SPSS version 12.0. 1. General characteristics of subjects are analyzed by percentage and descriptive statistics. 2. Recognition about how to use oral hygiene devices, usage of oral hygiene devices, recognition of dental brush usage, Effect after using oral hygiene devices are analyzed by mean difference test(t, F verification). 3. Effect after using oral hygiene devices, relation of oral hygiene devices usage with subjective oral health status, oral hygiene devices usage and participation in education are analyzed by Correlation, Regression analysis. Results: 1. Investigation shows that most oral health devices are rarely used except toothbrush(4.57) in case of the use of oral hygiene devices. 2. In the case of awareness of oral hygiene devices usage, almost everyone knows how to use dental brushes(MEAN 4.16) the most. And the next ones are like these-tooth picks(MEAN 3.38), dental floss(MEAN 3.28), mouth rinse(MEAN 3.24). And they don`t know how to use the other oral hygiene devices usage. 3. respondents who use dental floss(β=.238, p<.001), interdental brush(β=.242, p<.001) and water pik(β =.180, p<.05) recognize that the result and effect after using them are more positive than the others in different case. 4. Respondents who use toothbrush(β=.119), dental floss(β=.120) and interdental brush(β=.136). frequently evaluate their subjective oral health status positively in case of frequency of using oral hygiene devices and subjective oral health status(p<.05). 5. Respondents who use toothbrush and interdental brush properly and frequently, who are older than the others show more concern about participating in education of "How to use oral hygiene devices" than the others(p<.05). Conclusions: In conclusion, to prevent oral disease, the opportunity of participating in education about correct knowledge and proper way should be expanded. And we should contribute to oral health improvement through developing, executing and informing systematic, popular oral health education with active one.

        수치화 된 자아를 활용한 헬스케어 웨어러블 디바이스 기능 분석 - 심뇌혈관 질환 중심으로 -

        심뇌혈관 질환은 국내 사망률 2위를 차지하고 있는 주요 만성질환이다. 이 질환은 생활습관의 개선으로 예방할 수 있어서 평소 건강관리가 중요하다. 하지만 국내에서 대부분 예방관리 프로그램은 안내 책자, 강의 등 소극적인 방법으로 대응하고 있어 질환 예방관리의 실효성이 떨어지고 있다. 현재 ICT 기술의 발달로 스마트 헬스케어 시장이 발달함에 따라, 개인의 건강상태를 측정하고 신체 데이터를 수치로 나타내는 수치화된 자아(Quantified self)가 웨어러블 디바이스를 통해 확산하고 있다. 이에 본 연구는 심뇌혈관 질환의 적극적인 예방과 관리를 위해 헬스케어 웨어러블 디바이스를 탐색하고 논의하였다. 먼저 현재 상용화되고 있거나 심뇌혈관 질환과 관련된 헬스케어 웨어러블 디바이스들의 사례들을 부착형태에 따라 손목형, 의복형, 부착형으로 분류한 뒤 기능 분석했다. 이를 종합해 웨어러블 디바이스를 통한 심뇌혈관 질환의 예방법과 관련 있는 건강활동 트래킹, 생체신호 트래킹, 의료진과 피드백 기능으로 분류해 비교분석 하였다. 이를 통해 질환에 대한 적극적인 자가 헬스케어의 행태가 개선될 수 있음을 기대할 수 있으며, 향후 본 연구를 통해 질환의 예방연구의 다학제적인 활성 기제로 작용할 수 있을 것이다. Cardio-cerebrovascular disease is one of the chronic diseases that often attack people in Korea, and in fact, it ranks second in terms of death rate. This disease can be prevented by improving lifestyle, usual health care is important. But, in Korea most of the prevention or management programs adopt passive methods like using guide books or giving lectures, so it is not very effective in preventing the disease. Presently, the smart health care market is being developed in Korea and overseas. As an example, quantified self is being spread through wearable devices which are intended to measure each individual’s health conditions and quantify body data into numbers for bettering habits. Accordingly, this author will explore and discuss wearable health care devices so as to prevent and manage cardio-cerebrovascular disease in a more active way. First, this study has classified wearable health care devices presently commercialized or related with cardio-cerebrovascular disease into wrist, clothes, or attaching types by the way of their attachment and analyzed them. After that, summing that up, this author performed cross-tabulations with other ways of preventing cardio-cerebrovascular disease. This will contribute to improving one’s health care behavior about disease more actively and also work as an active interdisciplinary mechanism in research dealing with how to prevent disease afterwards.

        심혈관계질환자의 구강관리용품 사용과 관련요인

        Objectives: This study was aimed at investigating the oral hygiene device use by cardiovascular disease patients and assessing related factors. Methods: We used data of 1,630 cardiovascular disease patients (including hypertension, stroke, and myocardial infarction) from the 7th Korean National Health and Nutrition Examination Survey (2016-2018). The χ² test and logistic regression analysis were performed to analyze the oral hygiene device use by the subjects and assess related factors. All statistical analyses were performed with the IBM SPSS Statistics, version 20.0. Results: The oral hygiene device use and related factors were significantly higher in patients with an educational level of middle school or below high school, with a score of 1.85 (95% confidence interval [CI] = 1.35–2.53), or college, with a score of 1.93 (95% CI = 1.19–3.14), compared to those with an educational level below primary school. Further, the oral hygiene device use and related factors were significantly higher in patients who answered ‘yes’, with a score of 1.96 (95% CI = 1.42– 2.73), compared to those who answered ‘no’ to the question on oral examination. Conclusions: More patients with cardiovascular disease did not use oral hygiene devices than those who did. It is necessary to expand the approach of preventive treatment to increase the utilization rate of oral hygiene devices.

      • 말기 심장 질환의 수술 치료법 및 이식형 양심실 보조 장치의 의의

        말기 심질환의 수술 치료에는 심장이식, 기계식 순환 보조 및 기타 수술이 있다. 그 중 심장이식은 최선의 치료로 인정되고 있으나 공여자 부족 현상과 적응 대상의 제한 등과 같은 요인 때문에 극소수의 환자가 도움을 받을 뿐이다. 따라서 획기적인 치료법이 개발되지 않는 한 기계식 순환 보조 장치의 필요성이 계속 요구된다. 기계식 순환 보조 장치를 임시 (IABP, VAD, ECMO, Centrifural pump, ECLS) 및 영구(TAH) 순환 보조 장치로 구분할 때, 이들 각각은 펌프와 도관 및 회로의 구성에 따라 사용 용도가 제한된다. 이식형 양심실 보조 장치는 이러한 기존 순환 장치들의 단점을 보완하고 장점을 취합한 새로운 개념의 순환 보조 장치로서, 심장 이식 혹은 심장 기능 회복까지의 가교와 같은 임시 순환 보조로 쓰일 수 있으며 영구 순환 보조로는 자기 심장 기능을 보존한 상태에서 이식형 인공 심장으로도 사용될 수 있다. 또한 다른 순환 보조 장치의 구동 시스템으로도 사용이 가능한 장점이 있어, 말기 심질환의 수술 치료에 있어 새로운 지평을 제시한다고 본다. Surgical treatments for end-stage heart diseases include cardiac transplantation, mechanical circulatory assist and others. Although the cardiac transplantation has been considered as the best option of the treatments for the end-stage heart diseases, only a very small number of the patients can undergo it due to donor shortage or limited indications. Accordingly, requests for mechanical assist devices will be remained until an evolutional treatment appears. Mechanical circulatory assist devices can be divided into a temporary (IABP, VAD, EMO, Centrifugal pump, and ECLS) and a permanent (TAH) one, and each has its own limitations in use depending on the composition of pump, cannulas and circuits. An implantable bi-ventricular assist device is a new concept of mechanical circulatory assist devices which takes the advantages as well as removes the shortcomings of the conventional circulatory assist devices. It can be used not only as a bridge to cardiac transplantation or recovery but also as a long-term life support system with preservation of the native heart. The latter can be nominated for a heterotopic total artificial heart. In addition, since the implantable bi-ventricular assist device can be used for a driving pump in various circulatory devices, it is most likely that the device can open a new horizon of the surgical treatment for end-stage heart diseases.

        『침묵의 시간』에 나타난 서사적 장치로서의 질병

        Time of Silence is a work which presents a new novelistic aesthetics through verbal experiments and segmented narrative techniques, breaking away from the dominant realist tradition of novels in the 1950s. Nevertheless, this work does not merely focus on introducing aesthetics changes, but it also offers a deep reflection of human beings and their society at a broader level. Although this novel is different from the social novels of previous times, it still contains similar social critical elements, and the figure of the main character is built up under an eminently existentialist perspective. In addition, the introduction of demythologizing elements contributing to the deconstruction of the Spanish national narrative established during the Francoist era is also remarkable. In this context, Time of Silence has been understood in various ways apart from its experimental dimension. This study focuses on the usage of disease as the narrative device which enables multi-layered interpretations. In this novel, disease encompasses not only physical disturbances, but also social and existentialist phenomena related to human existence which are implied metaphorically. In fact, various diseases appear in this work and carry a great weight in the narrative. A good example of this is the employment of cancer research as a basic driving force in the development of the narrative, as it is useful to show the particular relationship between individuals and society. The main character of the novel is deeply frustrated with his study on the effect of cancerous cells, which leads him to the “disease of reason” and “Spain’s disease”, metaphorical expressions for social phenomena and also for the portrayal of the main character in a state of lethargy. All this reminds us of the despair that Kierkegaard expressed in “Sickness Unto Death”. In this way, this study examines how the pathological factors presented in Time of Silence function as narrative devices and how they are closely connected to the problematic perceptions of individuals and society which this work tries to present. 『침묵의 시간』은 1950년대의 사실주의 소설 전통에서 벗어나 언어적 실험과 분절적 방식의 서술 기법으로 새로운 소설 미학을 보여준 작품이다. 그런데도, 이 작품은 형식적인 개혁에만 머물지 않고 보다 광의의 차원에서 인간과 사회에 관해 심도 있게 성찰한다. 이 소설은 이전 시대의 사회 소설과는 다르지만, 여전히 사회 비판적인 요소를 내포하고 있으며, 주인공의 모습이 실존주의적 전망 아래 해석된다. 또한, 프랑코 시대에 세워놓은 스페인적 신화를 해체하려는 탈신화적 성격 역시 두드러진다. 이렇게 『침묵의 시간』은 실험적인 성격 이외에도 다양한 측면에서 그 의미가 논의되어왔다. 본 연구는 이렇게 다층적 해석을 가능하게 하는 이 소설의 서사적 장치로서 질병이 갖는 의미에 주목하였다. 여기서 질병은 단지 의학적 질병뿐만이 아니라 그것의 은유로 암시된 사회적 현상과 인간 실존과의 관련성을 포괄한다. 구체적으로 작품에는 여러 질병에 관한 언급이 나오는데, 그중에서도 인간과 사회의 상호관계를 토대로 한 암 연구가 이야기 전개의 기본적인 동력으로 작용한다. 주인공은 이 연구에서 좌절하는데, 그것은 사회적 현상을 가리키는 비유적 표현으로서 ‘이성의 병’과 ‘스페인의 병’으로 연결되며, 무기력 상태에 빠진 주인공의 모습은 키르케고르가 ‘죽음에 이르는 병’이라고 이름한 절망을 떠올리게 한다. 이렇게 본 연구는 『침묵의 시간』에 제시된 병적인 요소가 중요한 소재일 뿐만 아니라 작품이 표방하는 개인과 사회에 대한 문제적 인식과 긴밀히 연결되는 서사적 장치로 기능함을 살펴보았다.

Considerations for prescribing and dispensing, administering and following up, education, and adherence; all of these factors impact on treatment success and all are intrinsically linked to the device selected. This review brings together relevant evidence, real-life data and practice tools to assist health care professionals in making decisions about the use of inhalers in the management of chronic obstructive pulmonary diseases. It covers some of the key technical device issues to be considered, the evidence behind the role of inhalers in disease control, population studies which link behaviors and adherence to inhaler devices as well as practice advice on inhaler technique education and the advantages and disadvantages in selecting different inhaler devices. Finally, a list of key considerations to aid health care providers in successfully managing the use of inhaler devices are summarized.

        Evaluation of Gait Assistive Devices in Patients with Parkinson's Disease

        Objective: There are no guidelines for choosing appropriate gait assistive devices. The aim of this study was to evaluate gait assistive devices in patients with Parkinson's disease. Method: We evaluated 15 individuals with Parkinson's disease who did or did not use one of two different devices including canes and two-wheeled walkers. Data were collected using the GAITRite system. Results: Participants in the group using canes and two-wheeled walkers had significantly increased double support time and decreased gait velocity, normalized gait velocity, and stride length compared with those who did not. Participants who used a two-wheeled walker had significantly decreased gait velocity, normalized gait velocity, and stride length compared with those who used a cane. Furthermore, participants who used a two-wheeled walker had significantly decreased coefficients of variation for step time, stride length, and swing time compared with those who used a cane. Conclusion: Our results indicated that the two-wheeled walker offered the most consistent advantages for decreasing the risk of falling
