교정정책 마련을 위한 마약 밀매자의 마약 밀매 경험에 대한 사례연구: 남성회복자를 중심으로

본 연구는 현재 마약중독에서 회복되었고 마약 밀매를 하지 않는 남성으로 과거의 마약 밀매 경험을 탐색하여 밀매의 구조적 변화와 밀매 전략, 밀매조직을 살펴보고 이를 근거로 마약 밀매자의 중독회복과 마약 공급차단 정책 수립을 위한 교정차원의 정책마련의 기초자료를 제공하는데 목적이 있다. 연구 참여자 선정은 마약밀매 경험이 7년 이상, 마약중독에서 최소 3년 이상 회복을 유지하고 있는 5인을 대상으로 하였다. 자료수집은 1대1 심층 면담을 통해 자료를 수집하였으며, 수집 된 자료는 Creswell(2013)의 사례연구방법으로 분석하였다. 연구는 마약 밀매자의 마약밀매 과정을 시간 흐름에 따른 현상의 동태적 과정과 결과를 분석하고, 특히 맥락을 이해하고 자 하였다. 그 결과 4개의 분석영역, 11개의 범주와 39개 개념결집이 도출되었다. 첫째, 진입동기에서는 마이다스의 비즈니스와 사회적 지위의 초고속상승, 가난한 자의 꿈으로 나타났다. 둘째, 마약공급 조직에서는 항구적 상선구축, 불신과 배신구조, 믿음의 근원 현금으로 나타났다. 셋째, 소비시장 확장전략에서는 주변지인 공략, 다단계식 밀매조직, 중독의 무한루프에 가두기로 나타났다. 넷째, 사회관계에서는 평범의 가면 쓰기, 거짓 봉사로 나타났다. 이러한 연구결과를 토대로 마약밀매자로 부터 마약 예비중독자와 중독재발의 위험이 있는 개인을 보호할 수 있는 공급차단정책과 수요감 소정책을 통해 마약밀매자의 회복을 위한 교정정책차원의 논의 및 제언하였다. The purpose of this study is to explore the past drug dealing experience of males who were recovered from drug addiction and stopped drug dealing, examine the structural changes, strategies, and organizations of drug dealing, and provide basic data for a correction policy to help drug dealers’ addiction recovery and establish a drug supply reduction policy. The subjects of the study were five males who had more than seven-year experience of drug dealing and more than three-year recovery period. An in-depth one-on-one interview was conducted for data collecting. The data collected were analyzed using the case study methodology by Creswell(2013). This study was intended to analyze the dynamic process and results of phenomenon according to the flow of time of drug dealing and in particular understand the context. As a result, four analysis areas, 11 categories, and 39 concept clusters were derived. First, entry motivation included Midas business, a superspeed rise of social status, and poor person’s dream. Second, drug supply groups included building up a permanent supplier, a distrust and betrayal structure, and cash that is the root of trust. Third, strategies to expand a consumer market included targeting friends, a multi-level drug dealing group, and putting one into an infinite loop of addiction. Fourth, social relationship included putting on a mask of the ordinary and false voluntary service. Based on the results, this study discussed and suggested a correction policy to help drug dealer’s recovery through a supply reduction policy and a demand decrease policy to protect preliminary drug addicts and individuals who are in danger of drug addiction relapse from drug dealers.

대동맥궁 차단증을 동반한 동맥간의 일차 완전교정 1예 보고

대동맥궁 차단증을 동반한 동맥간은 매우 중한 자연경과를 갖고 있는 희귀한 선천성 심장 기형이다. 생후 25일, 체중 3.1 kg의 신생아에서 정중흉골절개술에 의한 일차 완전교정술을 시행하였다. A형 대동맥궁 차단증은 하행대동맥을 원위부 상행대동맥에 직접 문합하였으며, 폐동맥을 동맥간으로부터 분리하여 동맥간의 앞쪽에 위치한 다음 처리하지 않은 무판막 자가심낭도관을 사용하여 우심실 유출로를 재건하였다. 성공적 교정술 후 약 1년간의 양호한 추적결과를 경험하였기에 보고하는 바이다.

성장기 아동의 single tooth scissors bite의 교정 치료: 증례 보고

A scissors bite in the posterior teeth occurs when the upper teeth are positioned totally buccal to the lower teeth in centric occlusion, either unilaterally or bilaterally. This malocclusion can result from either excessive width of the maxilla or deficient width of the mandible, or sometimes combination of the both. Scissors bite, when left untreated without a proper dental intervention, interferes with the normal mandibular growth leading to a state where consequent disharmony in dental arch width evokes occlusal disturbances. Therefore, early preventive orthodontic treatment is necessary in patients with scissors bite. Scissors bite rarely involves anterior and posterior sites concuttently across the dental arch but usually affect single tooth. Even in the single tooth scissors bite cases, more likely to be met in the clinical fields, immediate dental intervention is indicated because continuous occlusal forces that exacerbate the already adverse axis of the posterior teeth. In this case study, patients with single tooth scissors bite, each 7, 14, 12, and 16 years old, were each treated with criss-cross elastic, fixed appliance, removable appliance, and miniscrews. With the proper selection of appliances appropriate to each specific cases, good treatment outcome can be achieved without resulting any side effects. 중심교합위의 상태에서 편측 혹은 양측으로 상악 구치부의 치아가 하악 구치부의 협측으로 위치한 경우를 scissors bite라 고 한다. 이는 상악의 폭경이 크거나 하악의 폭경이 좁은 경우 발생한다. Scissors bite의 경우 방치하게 되면 하악의 성장이 방해받게 되고 그 결과, 악궁 폭경의 부조화가 심해져 저작 장애를 일으킬 수 있으므로 조기 차단 교정이 반드시 필요하다. 실제 임상에서는 여러 치아의 scissors bite보다 최후방 구치 혹은 그 외 하나의 구치만의 single tooth scissors bite(STSB)을 보이는 경우가 많다. 이러한 STSB의 경우도 방치하게 되면 저작력으로 인하여 구치의 각도가 더욱 악화되므로 즉각적인 치료가 필요하다. 본 증례에서는 STSB를 보이는 7세, 14세, 12세, 16세의 환자에게 각각 criss-cross elastic, 고정성 장치, 가철성 장치, miniscrew를 사용하여 치료한 증례를 보고하였다. 각 환자의 증례에 적합한 장치를 잘 선택하여 사용한다면 치아를 정출시키는 등의 부작용을 야기하지 않고 양호한 결과를 얻을 수 있을 것이다
