Objective: To evaluate the effectiveness of the pulmonary recruitment maneuver (PRM) at the end of the operation to decrease laparoscopy-induced abdominal or shoulder pain after gynecological oncologic surgery. Methods: In total, 113 women undergoing laparoscopic surgery for malignant or premalignant gynecological lesions were assigned randomly to two groups: the PRM group (the patient was placed in the Trendelenburg position (30°) and the PRM, consisting of two manual pulmonary inflations to a maximum pressure of 40 cmH2O) (n=54) and the control group (n=52). Postoperative shoulder and abdominal pain was assessed 12, 24, and 48 hours later using a visual analog scale (0–10). In addition, the incidence of post-discharge nausea and vomiting was recorded until 48 hours after discharge. Results: Postoperative shoulder pain at 12 and 24 hours was significantly less severe in the PRM group (2.2±0.5 and 2.0±0.4) than in the control group (4.0±0.5 and 3.9±0.4; both p<0.001). The PRM significantly reduced the severity of upper abdominal pain at 12 and 24 h compared with the control group (3.1±0.4 and 2.9±0.4 vs. 5.9±0.5 and 4.9±0.5; both p<0.001). The analgesic requirement during the postoperative period was similar in the two groups (control group, 78.8%; PRM group, 75.9%; p=0.719). Conclusion: The PRM effectively and safely reduced postoperative shoulder and upper abdominal pain levels in patients undergoing laparoscopic gynecological oncologic surgery. Reverse-Phase High-performance Liquid Chromatography (RP-HPLC) based technique is one of the most sensitive techniques to detect the antibiotics present in honey. In the southern part of Tamilnadu, India, majority of the farmlands are occupied by plantations such as coconut, banana and rubber. A variety of antimicrobial compounds and antibiotics, which have been reported in pollen, nectar and other floral parts of the plant, gets accumulated in honey through honeybees (Apis mellifera). We have collected the nectar samples from banana (Musa paridasiaca) and rubber (Ficus elastica) flowers and the honey from honey hives of banana and rubber cultivated areas. The extracted nectar and honey samples are subjected to RP-HPLC analysis with authentic antibiotic standards. Nectar and honey samples showed 4-17, 11-29 ${\mu}g/kg$ of streptomycin, 2-29, 3-44 ${\mu}g/kg$ of ampicillin and 17-34, 26-48 ${\mu}g/kg$ of kanamycin respectively.

Clinical study with Ovral, containing 0.5mg of norgestrel and 0.05mg ethynyl estradiol, has been performed with a grant from Wyeth Laboratories Philadelphia. Two-hundred and twenty-eight married fertile women have been selected for contraceptive treatment with Ovral those who had seeked contraceptive advice at the Family Planning Clinic, Deoartment of Obstetrocs and Gynecology of Seoul National Univeersity Hospital. They have received the treatment with Ovral for 21 days each cycle and a total of 1,112 cycles has been accumulated with these 228 wimen. Individual cycles varied from one to 11 cycles. Continuation rates by the 3rd and 6th cycle were 70.2 and 39.9 per cent respectively,and only 3.9 per cent of the total have continued the use of pills up to 11th cycle. Most of the side effects have developed during the first 3 cycles of use. These were mild and transi- tory in nature. Most frequently encountered side effects were indigestion, nausea & vomiting and loss of appetive in the order of frequency. Poor digestion occurred in 2.7 per cent of the total cycles; and nausea vomiting in 2.25 per cent. Spotting occured in 0.3per cent of the total cycles; break-through bleeding in 0.2 per cent; head- ache and dizziness in 0.72 and 0.54 per cent respectively. Chloasma have developed in 1.5 per cent of the total cycles, which is rather higher than those obtained from the othe studies. Change in body weight have not been recognized in 44.0 per cent and changes of + 1kg in weight have been noted in another 34.2 per cent of the total cases. Not a single case of pregnancy has been noted.

The study showed that the EE had a significant, dose dependent anti-edematogenic, anti-ulcerogenic and hypoglycaemic activity, whereas the crude alkaloid extract exhibited a significant only. Both the extracts possess dose dependent inhibitory activity on carrageenan-induced edema, inhibiting prostaglandins or mediators involved in prostaglandin synthesis, the second phase of inflammation. The EE was significantly effective in protecting pylorus-ligation-induced gastric ulcers at a higher dose level. The active principle of EE seems to be a selective inhibitor of the COX II (prostaglandin synthesis) without important effect on COX I since, EE exhibited both anti-edematogenic and anti-ulcerogenic effect. The EE was effective in reducing the plasma glucose level in normal albino rats in a dose dependent manner, producing hypoglycaemic effect by stimulating the release of insulin from the ${\beta}-cells$ and/or increasing the uptake of glucose from the plasma.

자궁선근증에 수반된 더그러스와 및 난소자궁내막증의 1예

자궁내악과 같은 조직이 자궁공이외의 부위에 발육증식한것을 자궁내막증 endometriosis라한다. 선골증 adenomyosis는 자궁근증내에 자궁내막이 침입증식한것으로서 내자궁내막증 endometriosis interna라하고 자궁이외의 부위에 발생증식한 외자궁내막증 endometriosis externa와 구별 되어 있다. 본증의 호발부위는 자궁이고 Douglas공에 광범하게 발생한것은 구미에서는 그리 희유한것은 아니다. 그러나 일반으로 동양인에 있어서는 자궁전골증은 그리 희유한 것은 아니나 외자궁내막증은 희유하다. 환자들은 42세 부인의 자궁선근증에 수반된 Douglas공 및 난소에 발생한 외자궁내막증의 1례를 경험하였기에 이를 보고하는 바이다. 결 론 자궁선근종에 합병된 더그라스공 및 난소자궁내막증을 외과적으로 절제한후 그 경과가 양호했던 우리나라에서는 희유한 흥미있는 1례를 보고하는 바이다.
