Background : The cun measurement System, an essential and convenient method in locating acupoints, has been widely used in the practice of acupuncture. However, traditional cun measurement has been criticized for its lack of reliability. Objectives : The purposes of this study are to determine if one cun measured by the directional methods have a consistency with that of proportional methods and to investigate which factors are related with these differences, especially in forearm. Methods : The distance between the elbow crease and the wrist crease of forearm was compared to a reference value of one cun obtained by the directional method. In this method, one cun is one third of the distance between index finger and Small finger of a subject, measured at proximal interphalangeal joint. In addition, to investigate the factors influencing the differences between these two methods, we measured the height and body weight and calculated body mass index (BMI). Finally we analyzed the factors correlated with these lengths by linear regression test. Results : The results showed that one cun obtained by the directional methods were significantly different from one cun by the proportional methods in forearm. It was demonstrated that the length acquired with the directional method was more correlated with body weight and body mass index, while the length obtained by the proportional method was more correlated with the height.

Background: Letrozole is an oral non-steroidal inhibitor of the aromatase enzyme, which has proven to be a useful drug against breast cancer. Methods: This single-dose, randomized 2 × 2 crossover study was conducted in healthy male volunteers. Participants of each sequence group (each 13 volunteers for sequence group) received, in randomized sequence, a single oral 2.5-mg dose of generic letrozole (test) or branded letrozole (reference). Each treatment period was separated by a 5-week washout period. Blood samples were collected for up to 312 hours after drug administration, and drug concentrations were determined using validated LC/MS-MS. Pharmacokinetic properties were obtained using noncompartmental analysis. Drug tolerability was assessed throughout the study, using measurements of vital signs, physical examination, clinical chemistry testing, EKG, and interviews. Results: A total of 26 subjects completed the study. The geometric mean ratios (90% CI) of Cmax and AUClast were 0.92 (0.85 – 0.99) and 1.01 (0.97 – 1.04), respectively. No serious AEs were reported, and there were no clinically significant differences between test and reference groups. Conclusion: The findings from this study suggest bioequivalence between two formulations of letrozole in healthy male volunteers. The safety profile of two formulations had similar characteristics. Indobufen (Ibustrin®), a reversible inhibitor of platelet aggregation, exists in two enantiomericforms in 1:1 ratio. Here, we characterized the anti-platelet effect of S- and R-indobufen using responsesurface modeling using NONMEM® and predicted the therapeutic doses exerting the maximalefficacy of each enantioselective S- and R-indobufen formulation. S- and R-indobufen wereadded individually or together to 24 plasma samples from drug-naïve healthy subjects, generating892 samples containing randomly selected concentrations of the drugs of 0–128 mg/L. Collageninducedplatelet aggregation in platelet-rich plasma was determined using a Chrono-log Lumi-Aggregometer. Inhibitory sigmoid Imax model adequately described the anti-platelet effect. The S-formwas more potent, whereas the R-form showed less inter-individual variation. No significant interactionwas observed between the two enantiomers. The anti-platelet effect of multiple treatments with200 mg indobufen twice daily doses was predicted in the simulation study, and the effect of S- or Rindobufenalone at various doses was predicted to define optimal dosing regimen for each enantiomer. Simulation study predicted that 200 mg twice daily administration of S-indobufen alone willproduce more treatment effect than S-and R-mixture formulation. S-indobufen produced treatmenteffect at lower concentration than R-indobufen. However, inter-individual variation of the pharmacodynamicresponse was smaller in R-indobufen. The present study suggests the optimal doses ofR-and S-enantioselective indobufen formulations in terms of treatment efficacy for patients withthromboembolic problems. The proposed methodology in this study can be applied to the developnovel enantio-selective drugs more efficiently.

建物周圍를 지나가는 공기는 비나 눈 또는 굴뚝의 연기, 먼지 등을 운반하며, 建物內部의 通風시스템을 교란하고 建物의 冷暖房에도 영향을 끼친다. 따라서 本 硏究에서는 바람이 끼치는 영향을 조사하기 위해 간단한 사각형의 단면을 가지는 單一建物의 경우와 複數建物이 있는 경우의 2次元 流動場을 有限差分法으로 解析하였다. 이 硏究는 單一建物 周圍의 流動場 解析結果를 확장하여 複數建物周圍의 流動場을 解析하는데 主目的이 있다. 數値解析 結果, 單一建物인 경우에는 旣存의 實驗結果와 잘 一致하였으며, 複數建物인 경우에는 隣接한 建物의 높이 및 建物사이의 떨어진 거리에 따라 流動場은 큰 영향을 받음을 알 수 있다. Air around a building carries snow or chimney smoke and upsets the internal ventilation system through the infiltration effect. Therefore, infiltrated air affects the heating and cooling system directly. In order to investigate the effects of 2-dimensional wind, flow field over single building with rectangular shape has been analyzed by the finite difference method. The analysis has been extended to the case of two adjacent buildings. This study shows that the results of numerical analysis in single building coincide qualitatively with the earlier experimental results and in the case of two adjacent buildings, the height of backward building and the distance between building effects the flow field.
3D wide-field multispectral photoacoustic imaging of melanoma: a pilot study

Background: Melanoma stage is usually classified by the histopathological depth of tumor, measured as ‘the Breslow depth’, which determines surgical margin. However, it is difficult to accurately determine the actual Breslow depth before surgery using the existing ocular micrometer and biopsy technique. Objectives: The aim of this study is to evaluate the clinical use of 3D wide-field multispectral photoacoustic imaging, which can noninvasively measure depth and outline the boundary of melanomas. Methods: Six melanoma patients were examined in vivo using the 3D multispectral photoacoustic imaging system. For five cases of melanomas, spectrally unmixed photoacoustic depths were calculated and compared against histopathological depths. Results: Spectrally unmixed photoacoustic depths and histopathological depths match well within a mean absolute error of 0.36 mm. In particular, the measured minimum and maximum depths in the in situ and nodular type of melanoma were 0.6 mm and 9.1 mm, respectively. In the 3D photoacoustic image of one metastatic melanoma, feeding vessels were visualized in the melanoma, suggesting the neovascularization around the tumor. Conclusion: The 3D multispectral photoacoustic imaging not only provides well-measured depth and sizes of various types of melanomas, it also visualizes the metastatic type of melanoma. Obtaining accurate depth and boundary information of melanoma before surgery would play a useful role in the complete excision of melanoma.

Background: Dermatofibromas (DF) are fibrohistiocytic tumors of unknown etiology, and multiple DF (MDF) are relatively rare. MDF have been reported in the setting of autoimmune diseases, human immunodeficiency virus infection, or comorbidities treated with immunosuppressive drugs. Objective: The present study investigated whether underlying conditions with impaired immune function are associated with MDF. Methods: A total of 338 patients with DF was enrolled. We divided patients into two groups as MDF and solitary DF (SDF) groups. We retrospectively reviewed patient medical records and classified all patients by underlying diseases. Statistical significance of SDF and MDF for each conditions was analyzed. Results: The demographics and comorbidities were compared between MDF group (n=82) and SDF group (n=256). Among underlying conditions, systemic lupus erythematosus (SLE) (odds ratio, 10.397; 95% confidence interval, 2.743∼39.404; p<0.001) was significantly associated with MDF over SDF. Vitiligo and post status of kidney transplant were related more highly to MDF (p=0.014; p=0.014, respectively) than to SDF. Presence of overall comorbidities, autoimmune diseases, and immunosuppressive drug use were associated with DF number (p<0.001; p<0.001; p<0.001, respectively). Conclusion: We propose an association between MDF and comorbidities, especially SLE and immunosuppressant use. MDF lesions seem to be impacted by compromised immune function. This is of significance since it is essential to search for associated conditions in patients presenting with MDF in dermatologic clinical settings. (Korean J Dermatol 2022;60(1):25∼34)
