        돈황의서에 나타난 삼음삼양 혈위에 나타난 고찰

        Background : There are many books on ancient medicine among the Dunhuang books found at a cave in Dunhuang, most of which were transcribed during Tang dynasty, reflecting ancient theories and practices. Objective and Material : To investigate the usage of the terms of three pairs of yin and yang as acupoint names, 4 books on acupuncture and moxibustion among Dunhuang books were examined. Results and Conclusion : Investigation of the usage of the terms of three pairs of yin and yang in the scripts and illustrations of 4 Dunhuang books which are also the names of 12 main meridians revealed that the terms of three pairs of yin and yang were used as the names of specific acupoints so called three pairs of yin and yang acupoints. The significance of the terms of three pairs of yin and yang as the names of specific acupoints are discussed focusing on the early form and development of meridian theory.

        Functional neurology is a function-oriented clinical neurology with a focus on the viability, functionality, and balance of the neurologic system, which may be considered as a form of yin-yang balance medicine like Korean medicine. While it incorporates knowledge systems such as developmental neurology, neuropsychology, comparative neuroanatomy, and others, it views the neurologic system and the body as an individually different, self-regulating mechanism with the help of the active balancing mechanism within the nervous system and between the individual and the environment, which view is at the core of its effort to improve and serve the human dignity based on the best possible functioning nervous system. This article reviews core concepts of the functional neurology, discusses yin-yang balance medicine perspectives and clinical applications of it.

      • C. M. Guzay의 Quadrant Theorem에 대한 고찰

        Objectives: The quadrant theorem is a theorem proposed by C. M. Guzay in the field of functional, holistic dentistry. There are not much of scientific literature on the quadrant theorem. This study briefly reviewed basic concepts of quadrant theorem. Methods: A publication by Guzay and research articles were searched and reviewed. The quadrant theorem is depicted as a series of illustrations and accompanied explanations. Results: The primary concept of the quadrant theorem was presented in 1952. Based on geometric biophysics of the occlusion and related anatomical functions, physiological pivotal axis of the mandible is analyzed to occurs at the dens (the sub-atlas area). Composite muscular activity links the mandibular posture with C1-C2, which is then linked with the spinal posture. Twenty illustrations are progressively presented on the physiognomy, occlusion, and analysis of anatomical functions. The balanced distribution of the forces gives the durability of the functions in life. Conclusions: The quadrant theorem provides a functional linkage between the mandibular posture and the upper cervical vertebrae.

        경락과 혈위는 무엇인가? - 경락과 혈위의 개념에 대한 의사학적 고찰

        Meridian and acupuncture point (MAP) is a core theory of acupuncture and essential building blocks of oriental medicine There still continue theoretic or experimental arguments and controversies on the origination or original concept of MAP without any definite approval or disapproval of a hypothesis The theory of MAP is an historic product and has never been outside of historic influences This study discusses the original concept of meridian and acupuncture point theory and its historical evolution based on the review of classic literatures on meridian including the mawangdui medical texts of Han dynasty The concept of MAP served as a empirical reference system in clinical settings irrespective of the anatomical entity of MAP 경맥개념의 발전 초기에 사지 원위부 맥동처 1곳과 두면/체간의 1곳을 잇는 선이 먼저 그려졌다 완과부의 맥동처를 본맥(本脈)으로 하고 두면이나 구간의 한 부위를 표맥(標脈)으로 하는 구심성 체계였다 32) 삼음삼양 혈위의 맥동을 살피고 일련의 병증을 그 맥동 변화에 따라 범주화하여 경맥병증의 개념이 나오고 경맥병증을 설명하는 체계로서 경맥의 순환노선이 설정되었으며 경맥 순행노선상의 병증에 대해 해당 맥동처에 폄석과 구법을 시술하여 치료를 하는 형태로 소박한 맥진/경맥 개념이 형성되었다 여기에서 『영추』의 맥진과 12경맥 경맥병증에 대한 침구치료가 형태를 갖추어 나왔던 것이다이렇게 발전해 가는 과정에는 고대의 해부학적 지식을 바탕으로 한 추론과 가설이 일부 역할을 담당했을 것이다 經' 개념이 주류를 이루기 전에 脈'의 개념에는 소박한 혈맥 맥동처라는 의미가 함께 포함되어 있었다 그리고 「사기 편작창공열전(史記 扁鵲倉公列傳)」 등 고대문헌의 해부관련 기록을 보면 옛 사람들이 어느 정도의 해부학적 지식을 갖추고 있었음을 알 수 있으며 실제로 혈맥과 신경은 사지에서 기본적으로 경맥처럼 종축방향의 주행을 갖고 있다

        도플러 초음파검사를 이용한 안면마비 환자의 안면부 경혈의 혈류 관찰

        Objectives : The aim of this study was to investigate whether Doppler ultrasound device is applicable for the evaluation of facial palsy patients. Methods : A total of 25 patients participated in the first assessment and 22 of them finished a follow-up assessment one week later. An assessment comprised of a Minimax-Doppler-K device examination on bilateral acupoints ST5, SI19, ST2, and BL1, House-Brackmann grading, and subjective symptom questionnaire. Measurement reliability was assessed and clinically meaningful variations of Doppler measurement values were explored. Results : Doppler ultrasound examination revealed a significant difference of some measurement values between values of the symptom side and the contralateral side, between groups of severe paralysis and moderate paralysis, and between groups of short duration(0-3 weeks) and long duration (3weeks-5 months) by non-parametric analyses (p<0.05). Several re-examination values showed a significant correlation with the first examination values by Spearman’s correlation tests (p<0.05). Conclusions : This study revealed several possibilities for the clinical application of this device. Further validity tests and device improvements for a user’s convenience would be helpful for its practical application.

        Positional Release Techniques (PRTs) are an umbrella term for manual therapies harnessing spontaneous musculoskeletal balancing mechanism of the body facilitated by finding and maintaining therapeutic position. PRT has its origin in the Strain Counterstrain (SCS) technique by Dr. Jones but encompasses diverse related techniques that stemmed off from the SCS. PRT emphasizes postural balance within the body and innate healing potential of the body including the postural balance of the temporomandibular joint (TMJ). This study briefly reviews concepts, history, and contemporary study reports on PRT with a focus on the yin-yang balance based approach of PRT.

        Applied Kinesiology (AK) is an evaluation system on the functional balance and integration of the body. Functional aspects of the triad of health are evaluated through muscle testing to see if the balance and integration is in an optimal status or is influenced by a given environmental stimulation. Triad of health refers to the structural, chemical, and psychological aspects of the body. Muscle testing in AK is a functional neurological assessment of muscular response to a given environmental stimulation in a given individual. Meridian system and stomatognathic system as in TMJ balancing medicine serve as one of the theoretical basis of AK. This article reviews core concepts of AK, discusses yin-yang balance medicine perspectives and clinical applications of it.

        구안와사 후유증의 분포와 인식도에 대한 단면조사

        Background : Sequelae symptoms of peripheral facial palsy include not only partial recovery of the paretic muscles but contracture, spasm, synkinesis and atrophy which cause significant functional, esthetic, and psychosocial disturbances to the lives of patients. Objective : This study is to investigate the prevalence, time of onset, patient's self assessment on the degree of understanding of the sequelae and the association of the degree of palsy with the appearance of sequelae. Methods : 106 patients with peripheral facial palsy were sequentially interviewed and examined. Results : 29 patients(27.4%) of 106 patients showed sequelae symptoms, of whom 19 patients(65.5%) showed sequelae symptoms during 4 to 6 months after the onset of palsy. The degree of understanding on the sequelae at the time of interview showed improvement compared to that at the time of first consultation to any physician but not to reach an sufficient understanding. No relation between the degree of palsy and the appearance of sequelae was observed.

        한약과 양약의 개념 설정 어떻게 할 것인가?

        Objectives : The purpose of this study is to find a reasonable solution to a current status of drug classification between the drugs of western conventional (allopathic) medicine and Korean medicine. A clear and distinct concept on the drugs of allopathic medicine and Korean medicine based on reasonable concepts and broad consensus is a pressing issue in Korea, and will facilitate the development of herbal medicinal products and pharmaceutical industry. Methods : Considering the issue of drug classification from domestic and international regulations, we reviewed the current Drug Law of Korea and China, Guidance for Industry, Botanical Drug Products of USA, Directive 2004/24/EC of the European parliament and of the council. Results : In Korea, the drug classification of allopathic medicine and Korean medicine is quite vague even though differential licensure system is enforced for the clinicians of allopathic medicine and Korean medicine field. According to the definition in the Drug Law, the scope of Korean medicine drug is so broad that even a drug made of single-compound material, as well as herbal extract of crude mixture, is regarded as a drug of Korean medicine, as long as the material may be separated from medicinal herbs, animal tissues, or mineral resources. Only new compound not found in natural resources are outside of the scope of Korean medicine drug. In USA and EU, medicinal products manufactured from herbs are approved by separate regulations for the herbs with special waivers. In China, the category of new medicine and the definition of allopathic medicine and traditional chinese medicine are clearly specified and classified. Conclusions : As medicines are validated therapeutic materials for efficacy and toxicity, we suggest that generally the concept of conventional medicines is based on a single compound that has been synthesized and individually validated and that of Korean medicines is based on a compound extracted from natural materials or a complex of compounds that has been validated as a whole in its totality.
