        To investigate the effect of environmental measurement on the improvement of working condition in small and medium sized industries and to evaluate the usefulness of this measurement which is covered by Occupational Safety and Health Act for the protection of workers from hazardous working condition, authors selected 107 small and medium sized manufacturing industries mainly from Chunan area and partly from lead using industries which were in special contract of agent specific occupational health service with Institute of Industrial Medicine, Soonchunhyang University. Environmental measurements were provided twice in interval of 6 months to study industries. At the end of second measurement questionnaires were collected to check the usefulness of this measurement for the workers and industries itself. The results obtained were as follows: 1. Out of 107 manufacturing industries, the most frequently measured index of harmful agent in environmental measurement was noise, and the next indices were dust, organic solvents and special chemicals in descending order. While the excess rates of measurement over Permissible Exposure Level(PEL) in the first measurement were 19.3% in noise, 22.8% in dust, 14.3% I organic solvents and 20.3% in special chemicals measurement, those in second measurement were 26.0% in noise, 14.7% in dust, 10.0% in organic solvents and 29.5% in special chemicals measurement, respectively. 2. The supply rate of protective equipment and the labeling condition of health and safety poster in the later period of study were improved than early period. 3. Most persons in charge of health and safety management(97.2%) had engaged in other administrative works and only 2.8% of study industries had independent occupational safety and health personnels who engaged only in health and safety of workers. 4. The purpose of environmental measurement answered by the eligible of persons each industries was to report at the regional office of labor affair. Only 7 out of 107 industries answered that their measurements were for the real improvement of working condition and 3 industries for the fulfilment of requirement of trade union. 5. While 86.0% in industries (92/107) felt the report of environmental measurement was helpful to set up protective equipment program to prevent workers from harmful condition, 66.4% in industries answered that it was useful for the establishment of health education program. The measurement of environmental conditions were also useful for the installation of ventilatory system of workplace, for the isolation of work process and for the improvement of work process. But 8.4% in industries answered that it was not useful at all.

        Aplastic anemia is one of the hematologic disease which is characterized by cytopenia in peripheral blood, hypocelluarity in bone marrow and marrow failure. Most common cause of aplastic anemia is unknown. Acquired causes of it are viral infection, drugs, and chemical agents. The symptoms of aplastic anemia are fatigue, headache, dizziness, exertional dyspnea, and pallor, and they are due to abrupt decrease in hemoglobin level. The incidence of acquired aplastic anemia is higher in Eastern population than Western. In geeral, men and women are affected with equal frequency, but the age distribution is biphasic, with the major peak in the teens and tweenties and a second rise in the elderly. We report a case of a 28-year-old man who experience aplastic anemia after ingestion of Cordyceps with the review of literatures.

        Vertebral Body Anterior Translation, a Novel Technique for Delayed Myelopathy Due to Osteoporotic Spine Fractures

        Background: Osteoporotic vertebral compression fractures (OVCFs) are often associated with delayed myelopathy. Surgical treatment of delayed myelopathy following an OVCF comprises spinal canal decompression and stable fixation of the vertebral column with an acceptable sagittal alignment. However, such surgical methods are not usually feasible because of medical comorbidities and osteoporosis. We devised a novel, simple technique to decompress the spinal canal and reconstruct the middle column by translating the fractured vertebral body anteriorly through a posterior approach and verified the validity of the new technique. Methods: We conducted a single-center, retrospective study. Patients who underwent vertebral body anterior translation (VBaT) between 2014 and 2017 due to delayed myelopathy after OVCFs were included. Through a posterior approach, discs between the fractured vertebra and the adjacent vertebrae were released. The fractured vertebra was translated anteriorly with pedicle screws and rods to realign the middle column. Radiological and functional improvement was analyzed. Results: There were 12 consecutive patients. The mean age was 70.3 ± 9.4 years. There were 8 female and 4 male patients. Follow-up period was 35.9 ± 13.1 months. Nine patients had pedicle screw augmentation with polymethyl methacrylate. The mean number of fusion segments was 3.4 (range, 2–4). There were 3 types of spinal canal invasion. Five patients had vertebral body vacuum clefts with posterior wall fractures. Five patients had vertebral body angulation with endplate protrusion. Two patients had 3 column fractures. In radiological analysis, the regional kyphotic angle was 35.1° ± 9.1° preoperatively and improved to 8.8° ± 6.8° postoperatively and 9.8° ± 6.1° at the final follow-up (p < 0.001). The anterior vertebral body height ratio was 27.6% ± 7.0% preoperatively and improved to 80.5% ± 13.7% postoperatively and 83.7% ± 12.5% at the final follow-up (p < 0.001). The spinal canal invasion ratio was 52.6% ± 9.1% preoperatively and improved to 25.2% ± 10.4% postoperatively (p < 0.001). Neurological deficit was improved in all patients by 1–3 grades according to Nurick’s grading system. Conclusions: In delayed myelopathy following an OVCF, although the posterior cortex invades the spinal canal, it is usually already in the union state. Therefore, it can bear compression force as a middle column if realigned to be in line with the adjoining vertebrae. VBaT demonstrated satisfactory reduction of kyphosis and maintenance of stability until the last follow-up.

        Preparation and Characterization of Hybrid Silica-Poly(ethylene glycol) Sonogel

        An inorganic-organic hybrid system, silica-poly(ethylene glycol) songel is reported. This system was prepared via sol-gel method by varying varous processing variables. e.g. ultrasonic radiation time, gelling temperanture, PEG content and its molecular weight. Various experimental techniques wee employed to analyze the morphological, mechanical and optical properties of the system. The results were discussed in the light of existing theories. The sonogel system exhibited the common features of inorganic-organic hybrids. $SiO_2-10$ wt% PEG sonogel exhibited the morphological and optical properties superior to those reported earlier for the classic gels and found suitable for device applications.

      • 肺吸蟲으로 感作한 마우스에서의 細胞性免疫

        肺吸蟲으로 感作된 ICR 마우스의 脾臟에 palque形成細胞가 生成되는지 그 與否를 究明하기 위해 本蟲의 代謝産物과 蟲體構成物을 Freund incomplete adjuvant와 emulsion을 만들어 마우스의 footpad에 皮下注射하여 5日, 10日, 15日, 20日 및 30日後에 頸部骨折로 잡아 Jerne plaque assay를 施行한 結果 肺吸蟲의 代謝産物과 蟲體構成物로 感作한 마우스의 脾臟에서 plaque 形成細胞가 感作후 5日에 나타나 10日에 그 數가 最大値를 나타내었으며 15日 以後부터 漸次로 적어졌다. 肺吸蟲의 蟲體構成物보다 代謝産物로 感作한 마우스의 脾臟에서 plaque形成細胞數가 많았으며 肺吸蟲과 構川吸蟲의 代謝産物과 蟲體構成物에 의한 plaque 形成細胞數는 大體로 肺吸蟲은 構川吸蟲보다 약 2倍 많았다. In an attempt to determine the presence of plaque forming cells(PFC) in the spleen of ICR strain mice, the emulsions prepared by mixing incomplete Freund's adjuvant with the metabolites and somatic constituents of Paragonimus westermani were injected into the footapads of mice. The mice were killed by neck fracture on 5, 10, 15, 20 and 30 days after the injection. Spleen cell suspensions obtained from the mice were used for the Jerne plaque assay. PFC were detected in the spleen of mice immunized with the metabolites and somatic constituents of P. westermani in 5 days after immunization. The peak number of PFC was found in 10 days and dropped in 15 days after stimulation. The mean number of PFC per spleen of mice immunize with the metabolites was higher than those with somatic constituents as a whole. At the sam time, the numbers of PFC in mice sensitized with P. westermani were higher than hthose wit Metagonimus yokogawai.

        Duplication and deletion of 21 hydroxylase gene among the normal Korean subjects and adrenogenital syndrome patients

        Steroid 21 hydroxylase deficiency is a major cause of congenital adrenal hyperplasia (CAH) and is caused by genetic impairment of the gene (CYP21B). In the human genome, CYP21B is located within the MHC class Ⅲ region on the short arm of chromosome 6. Most of the genes in this region are highly polymorphic and crowded. Also the CYP21B gene is accompanied by its pseudogene (CYP21A) and tandemly arranged with two genes of the fourth component of complement. A highly complex gene cluster in this area may predispose genetic instability of CYP21, i.e. mutations. In this study, we tried to investigate the frequency of duplication and deletion of CYP21 and patterns of the genetic alterations of these genes. We also compared the genetic alterations in normal subjects with those of the CAH patients. The results showed that 15% of the normal Korean population have duplication or deletion of CYP21. There was one normal subject heterozygous for the deletion of CYP21B. of the 5 CAH patients examined, 2 were found to show abnormal patterns. One was a large-scale gene conversion and the other a gene conversion associated with deletion involving both CYP21B and C4 locus Ⅱ gene. Through this study, we came to the conclusion that the duplication or even deletion of CYP21 and C4 might be quite a common event in the Korean population and these rearrangements must be regarded as polymorphisms. It could contribute to a high incidence of CAH by providing a genetic pool of instable CYP21.

        The Protective Effect of Quercetin-3-O-β-D-Glucuronopyranoside on Ethanol-induced Damage in Cultured Feline Esophageal Epithelial Cells

        Quercetin-3-O-β-D-glucuronopyranoside (QGC) is a flavonoid glucoside extracted from Rumex Aquaticus Herba. We aimed to explore its protective effect against ethanol-induced cell damage and the mechanism involved in the effect in feline esophageal epithelial cells (EEC). Cell viability was tested and 2',7'-dichlorofluorescin diacetate assay was used to detect intracellular H2O2 production. Western blotting analysis was performed to investigate MAPK activation and interleukin 6 (IL-6) expression. Exposure of cells to 10% ethanol time-dependently decreased cell viability. Notably, exposure to ethanol for 30 min decreased cell viability to 43.4%. When cells were incubated with 50μM QGC for 12 h prior to and during ethanol treatment, cell viability was increased to 65%. QGC also inhibited the H2O2 production and activation of ERK 1/2 induced by ethanol. Pretreatment of cells with the NADPH oxidase inhibitor, diphenylene iodonium, also inhibited the ethanol-induced ERK 1/2 activation. Treatment of cells with ethanol for 30 or 60 min in the absence or presence of QGC exhibited no changes in the IL-6 expression or release compared to control. Taken together, the data indicate that the cytoprotective effect of QGC against ethanol-induced cell damage may involve inhibition of ROS generation and downstream activation of the ERK 1/2 in feline EEC.

        담관낭선암 2예

        Biliary cystadenomas and cystadenocarcinomas are rare tumors that arise in the liver or less frequently in the extrahepatic biliary system. These tumors are associated with few symptom and usually normal laboratory data. The ultrasonogram, abdominal CT scan, angiography and aspiration cytology are valuable diagnostic tools. Regardless of the diagnostic modalities, cystadenomas and cystadenocarcinomas cannot be differentiated macroscopically. Resection of cystadenomas is usually curative and, since cystadenocarcinomas are believed to arise from benign cystadenomas, complete surgical resection, when possible, is warranted to avoid malignant transformation of these rare tumors. We report two cases of biliary cystadenocarcinoma confirmed by the operation and review these cases with references.
