Objective Electroconvulsive seizure (ECS) is a potent treatment modality for various neuropsychiatric diseases, including Parkinson disease (PD). Recent animal studies showed that repeated ECS activates autophagy signaling, the impairment of which is known to be involved in PD. However, the effectiveness of ECS on PD and its therapeutic mechanisms have not yet been investigated in detail. Methods Systemic injection of a neurotoxin 1-Methyl-4-phenyl-1,2,3,6-tetrahydropyridine hydrochloride (MPTP), which destroys dopaminergic neurons in the substantia nigra compacta (SNc), in mice was utilized to induce an animal model of PD. Mice were treated with ECS 3 times per week for 2 weeks. Behavioral changes were measured with a rotarod test. Molecular changes related to autophagy signaling in midbrain including SNc, striatum, and prefrontal cortex were analyzed with immunohistochemistry and immunoblot analyses. Results Repeated ECS treatments normalized the motor deficits and the loss of dopamiergic neurons in SNc of the MPTP PD mouse model. In the mouse model, LC3-II, an autophagy marker, was increased in midbrain while decreased in prefrontal cortex, both of which were reversed by repeated ECS treatments. In the prefrontal cortex, ECS-induced LC3-II increase was accompanied with AMP-activated protein kinase (AMPK)-Unc-51-like kinase 1-Beclin1 pathway activation and inhibition of mamalian target of rapamycin signaling which promotes autophagy initiation. Conclusion The findings revealed the therapeutic effects of repeated ECS treatments on PD, which could be attributed to the neuroprotective effect of ECS mediated by AMPK-autophagy signaling.

Down syndrome is a rare cause of neonatal cholestasis. Neonatal cholestasis in a patient with Down syndrome is usually associated with severe liver diseases, such as neonatal hemochromatosis, myeloproliferative disorder and intrahepatic bile duct paucity. We experienced a case of idiopathic neonatal cholestasis in a patient with Down syndrome, which resolved spontaneously.
Genetic Variation and Population Structure of Pinus densiflora

Pinus densiflora is a perannuial coniferous species to Asia. The level of genetic diversity ofthis species was surveyed using starch gel electrophoresis at putative six enzyme loci from 17 natural populations in Korea. Results from nineteen loci indicated that genetic diversity was higher and population divergence was found between genetic identity statistics and geographic distance between populations. Although the range in genetic diversity statistics among populations was low, genetic drift and gene flow did not appear to play major roles in structuring genetic variation. I concluded that the level of genetic diversity maintained within P. densiflora populations, and the level of population divergence was influenced by its status as forest and mode of gene flow.

Purpose: Chronic day-to-day symptoms of orthostatic intolerance are the most notable features of postural orthostatic tachycardia syndrome (POTS). However, we have encountered patients with such symptoms and excessive tachycardia but with no symptoms during the tilt-table test (TTT). We aimed to investigate whether POTS patients with chronic orthostatic intolerance always present orthostatic symptoms during the TTT and analyze the factors underlying symptom manifestation during this test. Methods: We retrospectively examined patients who presented with POTS at the Gyeongsang National University Hospital between 2008 and 2011. Diagnosis of POTS was based on chronic day-to-day orthostatic intolerance symptoms as well as excessive tachycardia during the TTT. The patients were divided two groups depending on the presentation of orthostatic symptoms during the TTT. Clinical data and the results of the TTT were compared between these groups. Results: In 22 patients, 7 patients (31.8%) did not present orthostatic symptoms during the test. Diastolic blood pressure (BP) was significantly lower in the symptom-positive group. The head-up tilt resulted in a significant increase in diastolic BP in the symptom-negative group (P=0.04), while systolic BP had a tendency to decrease in the symptom-positive group (P=0.06). Conclusion: Significant patients with POTS did not present orthostatic symptoms during the TTT despite having chronic daily symptoms. This finding may be important for establishing definitive diagnostic criteria for pediatric POTS. Development of symptoms during TTT might be related to low diastolic BP and abnormal compensatory responses to orthostasis.

Basal cell carcinoma (BCC) is a common malignant skin tumor. Risk factors for development of BCC compose of sun exposure, radiation therapy, family history of BCC, immunosuppression, genetic syndromes, and a fair complexion. Occasionally, BCC can also occur in sun-protected sites such as axilla or genitalia. However, BCC that arises at the umbilicus is rare. Few cases have been reported about the metastatic tumor on the umbilicus, which is often called Sister Mary Joseph``s node. It is mainly metastases from the stomach, large and small bowel, and the ovary as a result of primary adenocarcionoma. However, primary skin cancer on the umbilicus is rarely reported in the literature. Herein, we report a rare case of primary basal cell carcinoma on the umbilicus with no prior history of malignancy. The patient was a 60-year-old male and complained of pigmented plaque with surface ulceration on the umbilicus which first appeared six years ago. He reported that he had frequently scratched and irritated the umbilicus during baths. And the lesion aggravated a year ago with intermittent bleeding and discharge. The biopsy from the umbilicus confirmed basal cell carcinoma and the tumor was surgically removed. PET-CT showed no lymph node invasion or any signs of metastasis.
Cutaneous atypical mycobacterial infection in a body scrubber (“ddaemirri”)

Cutaneous atypical mycobacterial infection (AMI) can result from direct inoculation of mycobacteria other than M. tuberculosis (MOTT) into the skin of a healthy person with no previous history of Tb infection. Thus, cutaneous AMI is considered predominantly an occupational disease and is a challenging diagnosis to make. Herein, we report a case of 50-year-old female whose occupation is a body scrubber, also known as “ddaemirri”, and was diagnosed with cutaneous AMI with typical histopathology and appropriate empirical treatment. The patient complained of itching erythematous macules, nodules, and pustules on both arms showing sporotrichoid patterns. She had received antibiotics and steroid intralesional injections at local clinics for 3 years without improvement. Biopsies from arms showed suppurative granulomatous inflammation with some acid-fast bacilli. However, other laboratory examinations including tissue culture, Tb & NTM PCR and interferon gamma release assay were all negative. She then was treated with rifampicin, and minocin for 6 months and the symptoms improved dramatically. This is the first reported case of cutaneous AMI found in a body scrubber, who comes in close contact with many people. A high index of clinical suspicion is important to diagnoses and we should always take caution when visiting public places such as saunas.
