        Treatment outcomes of patients with stage II pure endometrioid-type endometrial cancer: a Taiwanese Gynecologic Oncology Group (TGOG-2006) retrospective cohort study

        Objective: Choice of hysterectomy and adjuvant treatment for International Federation of Gynecology and Obstetrics (FIGO) 2009 stage II endometrioid endometrial cancer (EEC) is still controversial. Aims of this study were to evaluate survival benefits and adverse effects of different hysterectomies with or without adjuvant radiotherapy (RT), and to identify prognostic factors. Methods: The patients at 14 member hospitals of the Taiwanese Gynecologic Oncology Group from 1992 to 2013 were retrospectively investigated. Patients were divided into simple hysterectomy (SH) alone, SH with RT, radical hysterectomy (RH) alone, and RH with RT groups. Endpoints were recurrence-free survival (RFS), overall survival (OS), disease-specific survival (DSS), adverse effects and prognostic factors for survival. Results: Total of 246 patients were enrolled. The 5-year RFS, OS, DSS and recurrence rates for the entire cohort were 89.5%, 94.3%, 96.2% and 10.2%, respectively. Patients receiving RH had more adverse effects including blood loss (p<0.001), recurrent urinary tract infections (p=0.013), and leg lymphedema (p=0.038). Age over 50-year (HR=9.2; 95% confidence interval [CI], 1.2–70.9) and grade 3 histology (HR=7.28; 95% CI, 1.45–36.6) were independent predictors of OS. Grade 3 histology was an independent predictor of RFS (HR=5.13; 95% CI, 1.38–19.1) and DSS (HR=5.97; 95% CI, 1.06–58.7). Patients receiving adjuvant RT had lower locoregional recurrence (p=0.046), but no impact on survival. Conclusion: Different treatment modalities yield similar survival outcomes. Patients receiving SH with RT had lower locoregional recurrent with acceptable morbidity. Age and tumor grading remained significant predictors for survival among patients with FIGO 2009 stage II EEC.

        Single cell lysis and DNA extending using electroporation microfluidic device

        The purpose of cell lysis is to obtain intracellular substances such as DNA and proteins for analysis. Commonly used methods include chemical (Chemical solution decomposition) and physical (electricity or mechanical force). This study proposes an integrated system using an electroporation and microfluidic device made by micro-photolithography to lyse a single cell and stretch its DNA. The PDMS, as the manufacturing material of the microfluidic device, consists of 2 parts: the cell lytic zone, in which the immobilized cells trapped within the dense microstructure are lysed at a single-cell level, and the DNA stretching and recovery zone. This study showed that in a hypotonic environment (75 mM glucose solution), when electric field conditions were 100 Vpp and 1 kHz, the target cell was lysed and its DNA was released into the solution. When injected with proteinase K, the DNA flowed along the rectangular microstructure and was stretched to a length exceeding 840 μm.

        Laser-induced heating for cell release and cellular DNA denaturation in a microfluidics

        In this study, a microfluidics is integrated with a laser to develop a microfluidic system for singlecell capture and cellular DNA denaturation. The results show that the cells encountered low hydrodynamic stress at low velocities and, thus, could be captured by a cell capture structure. Laser heating of the solution at low velocity produced bubbles that pushed the cells away from the capturing areas. The released cells were subsequently lysed, and cellular DNA was extended and immobilized on the surface of pillars before undergoing laser heating to induce partial denaturation.

        Telbivudine-Induced Myopathy: Clinical Features, Histopathological Characteristics, and Risk Factors

        Background and Purpose Oral nucleos(t)ide analogs (NAs) are the mainstay treatment for chronic hepatitis B (CHB). Myotoxicity is an important extrahepatic effect related to NA treatment. Telbivudine is the NA for CHB that is frequently associated with muscle-related side effects. The risk factors for telbivudine-induced myopathy (TIM) are not yet clear. Methods This study characterized the clinical, magnetic resonance images (MRI), and pathological features of 12 TIM cases. A group of telbivudine-tolerant (TT) patients with CHB who received regular telbivudine treatment during the same period without the occurrence of myopathy was collected. Demographic and clinical factors were compared between the patients with TIM and the TT controls. Factors independently associated with TIM were identified using logistic regression analysis. Results The patients with TIM (males/females: 7/5, mean age: 57 years) developed myopathy after using telbivudine for a median period of 19.5 months. Muscle histopathology revealed abnormal proliferation, subsarcolemmal or sarcoplasmic accumulations, and ultrastructural defects of mitochondria. When compared with TT cases, patients with TIM had a lower estimated glomerular filtration rate and were more frequently positive for hepatitis B e antigen (HBeAg). Conclusions Mitochondrial abnormalities are characteristic histopathological features, and impaired renal function and HBeAg positivity are risk factors for TIM. Telbivudine-induced mitochondrial dysfunction and immune activation related to mitochondrial damage and HBeAg serostatus changes may underlie TIM. Constant clinical surveillance of myopathy during telbivudine treatment is needed due to the significant latency of its development. Dose adjustment for impaired renal function does not eliminate the risk of TIM occurrence.

      • Use of Acupressure to Improve Gastrointestinal Motility in Women after Trans-abdominal Hysterectomy

        Objective : To evaluate the effectiveness of acupressure on the gastrointestinal motility (GI) in women after trans-abdominal hysterectomy(TAH). Methods : Forty-one women undergoing trans-abdominal hysterectomy were recruited and randomly assigned to two groups. Twenty-one patients were assigned to the experimental group while remaining 20 patients were assigned as controls. A single blinded test was used for this study. Patients were not informed as to the tripe of treatment they would receive. The experimental group received twice daily acupressure for 3 minutes at three meridian points, Neiguan(PC-6), Zusanli(ST-36) and Sanyinjiao(SP-6), beginning 4 hours after surgery and continuing for 3 days. Control group received 3 minutes acupressure on non-meridian points on the tibia or radial bone surface approximately 3 to 4 cm from the corresponding three meridian points. Acupressure was performed twice a day, one at 10 Am and another at 2 Pm. GI contractions was measured by stethoscope prior to and after afternoon acupressure during 2- minute observational period. Results : Results show that acupressure of these three meridian points significantly (p<0.05) increased GI motility in the experimental groups, but not in the control group (p>0.05). Conclusions : Results showed that non-invasive acupressure of these meridian points could significantly improve the GI motility and could be incorporated into the technical curriculum and clinical education program of nursing schools. Family members of the patient could be taught to continue this procedure at home to enhance GI motility in patients undergoing trans-abdominal hysterectomy.

      • Various Digital Memory Behaviors of Functional Aromatic Polyimides Based on Electron Donor and Acceptor Substituted Triphenylamines

        <P>A series of aromatic polyimides (PIs) were synthesized via the polymerization of 3,3′,4,4′-diphenylsulfonyltetracarboxylic dianhydride with 4,4′-diaminotriphenylamine derivatives containing hydrogen, cyano, methoxy, or dimethylamine substituents. These PIs were thermally and dimensionally stable and produced high-quality thin films when applied in a conventional spin-coating process. Their structure and properties were characterized. Nanoscale thin films of the PIs demonstrated excellent electrical memory performance, with high stabilities and ON/OFF current ratios. The memory characteristics were found to be tunable by varying the substituents; nonvolatile write-once–read-many-times memory behavior, nonvolatile ON/OFF switching type memory behavior, and volatile dynamic random access memory behavior were observed. The memory characteristics were substantially influenced by the electron-accepting cyano- and electron-donating dimethylamine substituents but were apparently not affected by the electron-donating methoxy substituent. In addition, the film density was a significant factor influencing the observed memory behaviors, with larger film densities causing lower OFF-current levels. However, the critical switching-on voltage varied very little as the substituents were changed and was measured to be approximately ±2 V. All of the memory behaviors were found to be governed by a mechanism involving trap-limited space-charge-limited conduction and local filament formation. Overall, all of the PIs assessed in the present work were found to be suitable active materials for the low-cost mass production of high-performance, programmable unipolar memory devices that can be operated with very low power consumption, high ON/OFF current ratios, and high thermal and dimensional stability.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/mamobx/2012/mamobx.2012.45.issue-9/ma300311d/production/images/medium/ma-2012-00311d_0004.gif'></P>

        The Effects of Environmental Toxins on Allergic Inflammation

        The prevalence of asthma and allergic disease has increased worldwide over the last few decades. Many common environmental factors are associated with this increase. Several theories have been proposed to account for this trend, especially those concerning the impact of environmental toxicants. The development of the immune system, particularly in the prenatal period, has far-reaching consequences for health during early childhood, and throughout adult life. One underlying mechanism for the increased levels of allergic responses, secondary to exposure, appears to be an imbalance in the T-helper function caused by exposure to the toxicants. Exposure to environmental endocrine-disrupting chemicals can result in dramatic changes in cytokine production, the activity of the immune system, the overall Th1 and Th2 balance, and in mediators of type 1 hypersensitivity mediators, such as IgE. Passive exposure to tobacco smoke is a common risk factor for wheezing and asthma in children. People living in urban areas and close to roads with a high volume of traffic, and high levels of diesel exhaust fumes, have the highest exposure to environmental compounds, and these people are strongly linked with type 1 hypersensitivity disorders and enhanced Th2 responses. These data are consistent with epidemiological research that has consistently detected increased incidences of allergies and asthma in people living in these locations. During recent decades more than 100,000 new chemicals have been used in common consumer products and are released into the everyday environment. Therefore, in this review, we discuss the environmental effects on allergies of indoor and outside exposure.

      • Hardening steels by the generation of transient phase using additive manufacturing

        <P><B>Abstract</B></P> <P>In the present work, the tensile properties of 15–5 PH steel fabricated by selective laser melting (SLM) were examined with respect to the transient austenite phase. Compared with the 8%-transient-phase sample, the 18%-transient-phase one shows higher ultimate tensile strength and relatively low yield strength, as well as hardening behavior. We conducted <I>in-situ</I> neutron-diffraction study to examine the microstructure evolution for mechanistic understanding. After applying the external load, most non-equilibrium, retained austenite in the 8%-transient-phase sample transforms before the yield strength, whereas in the 18%-transient-phase sample only 50% of the austenite transforms. Accompanying with the phase transformation, a decrease in the dislocation density and the dislocation strain energy-assisted phase transformation of the ferrite phase are found in the 8%-transient-phase sample even before yielding, which, however, is not the case in the 18%-transient-phase sample. The study demonstrates the SLM enables tuning the amount of transient-phase ratio and coherency between phases to realize a mechanical property control for additive manufactured steel.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The tensile behavior was examined for 8%- and 18%-transient-phase additive manufacturing steels using in-situ neutron diffraction. </LI> <LI> The 18%-transient-phase sample exhibited higher ultimate tensile strength and low yield strength. </LI> <LI> The 18%-transient-phase specimen with more non-equilibrium γ-phase revealed a more profound strain hardening effect. </LI> </UL> </P>
