        To consider the efficiency of system operation under the introduction of the PQ, this study investigated the condition of the current national liability supervising system, and the condition of the operation of the foreign supervising system. This study also investigated the current PQ criterion, judgement method, and the problem of the PQ intention decision process. This study was conducted to try to find a engineering competition improvement of national supervising system and PQ system development direction through the presentation of reasonable improvement plan. In conclusion, this study was tried to find a improvement guide through analysis of problem in current system, but we was not concluded that all problem was improved by replacement of system or rule. It was more important that order and service system were respect to PQ Judgement criterion and engaged on construction engineering service system. It was important that presentation of reasonable system. but these system was applicated justly and elastically under the condition of faithful contract performance. We expected that in these condition. improved system was used efficiently and positive effect through the system improvement.

      • Streptozotocin으로 유발된 당뇨병쥐의 신경전도속도에 대한 Evening primrose oil (EPO), Vit.C, Vit.E의 약물효과

        The effects of evening primrose oil (EPO). Vit.C and Vit.E on nerve conduction velocity (NCV) in streptozotocin -induced diabetic rats were investigated. 5 weeks- diabetes caused 27±0.5 % reduction in sciatic motor conduction velocity as compared with normal control. Dietary supplement of 5% EPO. 125mg/kg Vit.C. and lg/kg Vit.E prevented significantly the development of the motor nerve conduction velocity deficit about 21±0.5, 21±0.5, 26±0.5% respectively. The total serum cholesterol and triglyceride were decreased by administration 5% EPO. Vit.C, and Vit.E as compared with diabetic control. However, the serum glucose was not decreased by administration 5% EPO. 125mg/kg Vit.C but the serum glucose was decreased by administration lg/kg Vit.E as compared with diabetic control.

        우리나라 노인의 구강건강 주요지표

        Korean society is experiencing a very rapid change in population aging. Oral diseases, including dental caries and periodontal disease, are usually cumulative and make oral health worse with age. Preventing tooth loss through the prevention and treatment of caries and periodontal diseases are essential for retaining oral function later in life. This study aimed to identify the trend in oral health status among elderlies over 65 years old, using major oral health indicators of Health Plan 2020. The fifth, sixth, and seventh Korea National Health and Nutrition Examination Survey (KNHANES V, VI, VII) were used in the present study to estimate the number of remaining natural teeth, the rate of 20 or more natural teeth, the rate of chewing difficulty, and the rate of oral health check-up of elderly. The numㄴber of remaining natural teeth increased from 15.6 in 2010 to 17.1 in 2015, and the rate of 20 or more natural teeth also increased from 45.8% in 2010 to 53.7% in 2015. The rate of chewing difficulty was similar but declined a little from 44.3% in 2010 to 42.9% in 2017. Finally, the rate of oral health check-ups of the elderly increased significantly from 12.2% in 2010 to 21.1% in 2017.

        노년기 건강의 적, 노쇠(frailty)와 구강노쇠(oral frailty)

        With a dramatic increase in the older population, there is a growing interest in strategies for managing oral disorders in the elderly. This study aims to introduce oral frailty and propose dental and oral health services for older people. This study reviewed the literature on oral frailty and frailty in older people by searching academic databases, government documents, and related websites. There is a growing awareness that the recovery and maintenance of oral function help delay the onset of frailty and conditions requiring nursing care, thereby extending a healthy life expectancy. The Japanese Society of Gerodontology (JSG) proposed the concept of oral hypofunction. JSG defined oral hypofunction as a presentation of 7 signs or symptoms: oral uncleanness, oral dryness, a decline in occlusal force, decline in the motor function of the tongue and lips, decrease in tongue pressure, decline in chewing function, and reduction in the swallowing function. JSG suggested diagnosing oral hypofunction if the criteria for three or more signs or symptoms are met. More evidence should be gathered from clinical studies and trials in the Korean community care setting to develop better diagnostic criteria and management strategies for the Korean elderly. Better health in older people can be achieved by strengthening the dental and oral health provider’s accountability of healthcare systems. The evolving concept of oral frailty paves the way for effective dental care for the elderly in Korea.

        Doxorubicin Inhibits the Production of Nitric Oxide by Colorectal Cancer Cells

        Doxorubicin (DOX) is an active and broad spectrum chemotherapeutic agent. Increased inducible nitric oxide synthase (NOS) expression and/or activity have been reported in several human tumors. While the relationship between DOX treatment and the enzymatic activity of endothelial NOS has been well characterized, little is known about the effects of DOX on the expression of iNOS in human cancer cells. In the present study, we characterized the effects of DOX on the nitric oxide (NO) production by colorectal cancer cells, DLD-1. IFN-${\gamma}$/IL-1$\beta$ (CM) increased the production of NO, whereas pretreatment of DOX inhibited the production of NO in response to CM in a dose dependent manner. The increased expressions of iNOS mRNA and protein by CM were completely blocked by DOX without affecting the iNOS mRNA stability. However, DOX activated nuclear factor-kB (NF-kB) in response to CM. Furthermore, the expression of inhibitor kB$\alpha$ was reduced by DOX in a dose dependent manner. Collectively, DOX inhibited the production of NO by DLD-1 cells, which is not linked to well known transcription factor, NF-kB. Therefore, further studies on the possible mechanisms of inhibitory effects of NO production by DOX would be worth pursuing.

        북한 보건의료전략의 분석과 치과 분야에서의 적용 현황 고찰

        In preparing for the era of Korean reunification, it is essential to consider the integration of medical systems and human resources. While Korean dental practitioners are expanding their activities in various fields both domestically and internationally, there are many restrictions on the activities for improving the health of North Korean people due to political and historical reasons even nowadays. In addition, there is little is known about the current state of dental health in North Korea. We analyzed the reports published by the World Health Organization (WHO) and the Ministry of Public Health of North Korea prepared individually or in cooperation, and investigated the current status of the health care strategy applied to the dental field by conducting a full investigation of the 2018 『Rodong』newspaper. Based on the above, we tried to grasp the major health care strategies in North Korea and their application. Understanding the direction and status of North Korea"s health care system would be an important cornerstone for international cooperation and practical activities to improve oral health care of North Koreans in the future. And there is a need that studies should be steadily conducted in various methods to overcome the heterogeneity of the two Koreas in the long-term perspective.

      • Development of a fluorescence-image scoring system for assessing noncavitated occlusal caries

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>This study aimed (1) to develop a scoring system based on a quantitative light-induced fluorescence (QLF) score for the occlusal caries (QS-Occlusal) that standardizes the fluorescence properties of noncavitated lesions from QLF images, (2) to confirm the validity and reliability of QS-Occlusal, and (3) to determine whether it is possible to replace existing clinical examinations by image evaluations based on the developed QS-Occlusal for assessing occlusal caries lesions.</P> <P><B>Methods</B></P> <P>This clinical study investigated 791 teeth of 94 subjects. The teeth were assessed by visual and tactile examinations using ICDAS criteria and quantitative light-induced fluorescence-digital (QLF-D) image examinations. QS-Occlusal was divided into four stages (from 0 to 3) based on the progression level of the lesion and the fluorescence loss and red fluorescence on captured QLF-D images. Two trained examiners who were not involved in the visual examination evaluated occlusal fluorescence images using QS-Occlusal. The maximum loss of fluorescence (|Δ<I>F</I> <SUB>max</SUB>|) and the maximum change in the ratio of red and green fluorescence (Δ<I>R</I> <SUB>max</SUB>) were quantitatively analyzed by the QA2 software to detect differences between the QS-Occlusal groups. The modalities were compared in terms of sensitivity, specificity, and area under the receiver operating characteristics (AUROC) curve for three different thresholds of the ICDAS codes: 0 vs 1–4 (D<SUB>1</SUB>), 0–2 vs 3/4 (D<SUB>2</SUB>), and 0–3 vs 4 (D<SUB>3</SUB>).</P> <P><B>Results</B></P> <P>|Δ<I>F</I> <SUB>max</SUB>| increased significantly by about 4.7-fold (from 15.94 to 75.63) when QS-Occlusal increased from 0 to 3. Δ<I>R</I> <SUB>max</SUB> was about 6.2-fold higher for QS-Occlusal=1 (49.74) than for QS-Occlusal=0 (8.04), and 21.6-fold higher for QS-Occlusal=3 (<I>P<</I> 0.05). The new QS-Occlusal showed an excellent AUROC (ranging from 0.807 to 0.976) in detecting occlusal caries when optimum cutoff values were applied. The intra- and interexaminer agreements indicated excellent reliability, with ICC values of 0.94 and 0.86, respectively.</P> <P><B>Conclusions</B></P> <P>The QS-Occlusal proposed in this study can be used in the clinical detection of noncavitated lesions with an excellent diagnostic ability. This makes it possible to replace clinical examinations and intuitively evaluate the lesion severity and status relatively easily and objectively by applying this scoring system to fluorescence images.</P> <P><B>Highlights</B></P> <P> <UL> <LI> QLF technology can be a useful screening tool for noncavitated occlusal lesion. </LI> <LI> QLF score can be used to evaluate the severity and status of occlusal caries lesion nondestructively. </LI> <LI> QLF score may be able to replace existing clincal examination in the future with excellent diagnostic ability for detecting noncavitated caries. </LI> </UL> </P>

        In situ 연구를 통한 치약의 연마도 평가

        Objectives. The aim of this study was to evaluate the effect of two different abrasive toothpastes on the dentine surface using an in situ method. Methods. The dentifrices used in this study had relative dentine abrasion (RDA) values of 30 and 100 when measured by a radioactive dentine abrasion method. They were analyzed using a double blind, randomized, split mouth, in situ study. Dental hygienists trained the subjects in correct tooth brushing techniques with intra-oral appliance using a powered toothbrush prior to the experimental demonstration. The subjects wore bi-lateral, lower appliances from 9 a.m. to 3 p.m. (excluding one hour of lunchtime) for 14 days. Brushing was performed 3 times a day (9 a.m., 12 p.m. and 3 p.m.) for 14 days. The human dentine specimens installed on the appliance were brushed with different tooth brush heads depending on the type of toothpaste, and 1 g of the toothpaste was applied to each specimen for 60 seconds. After 14 days, specimens were removed from the appliances and average dentine loss was measured by a non-contact profilometer. The average dentine loss of each dentifrice was compared by a paired t-test using SPSS 17.0 software. Results. Sixteen subjects were included in the main analysis. The average age of subjects was 23.7 (range; 21∼29). The dentine loss depth of the high-abrasive toothpaste (6.60 μm) was statistically higher than that of the low-abrasive toothpaste (4.23 μm) (p<0.05). Conclusions. The low-abrasive toothpaste group (RDA 30) showed a significantly lower average dentine loss depth when compared to the high-abrasive toothpaste (RDA 100), and this difference observed in vitro also appeared to exhibit valid results when applied to the in situ study.

      • HMG-CoA Reductase Inhibitor의 간독성 평가

        First statin approved by U.S Food and Drug Administration(FDA) in 1987 was Lovastatin and the statins are now one of the most widely prescribed classes of medications. The statins have shown evidence of reducing coronary heart disease events, but on the other hand they have been implicated in causing complications in muscle, liver and kidney. There’re no papers about hepatotoxicity in Koreans even though there’re a lot of papers about muscle-toxicity at home and abroad. So we have studied to assess the risk of liver function test (LFT) abnormalities with Koreans using statin. We retrospectively reviewed of Electronic Medical Records(EMR) of 12,860 new users of six statins(Atorvastatin, Fluvastatin, Lovastatin, Pitavastatin, Rosuvastatin, Simvastatin) during the period 1 May 2003 through 30 April 2006 in Seoul National University Hospital(SNUH).The patients in their 60s were the largest population (36.6%) and 55% of them were female. The spread of patients of statins indicated 30.4%, 2.2%, 8.7%, 1.1%, 10.6% and 47.0% in Atorvastatin, Fluvastatin, Lovastatin, Pitavastatin, Rosuvastatin and Simvastatin, respectively. Assessing LFT abnormalities, among the patients, 26.1% were more than mild(AST or ALT >40), 5.6% were more than moderate(AST or ALT >80) and 1.7% were more than severe(ALT or ALT >120). Especially, we’ve analyzed the data of Severe(AST or ALT >120) LFT abnormalities by using ‘Ranking by Clinical Significance of the Definition of Liver Function Test’to estimate hepatotoxicity clearly. This trial has revealed same result that all six statins have effecting severe hepatotoxicity around 1% with papers reported abroad. Also severe hepatotoxicity was related with dose but with age(p<0.05). Severe hepatotoxicity is uncommon and is reported in less than 1% of the statins therapy but it is related with dose, it is important to check LFT periodically in case of high dose therapy.

        Effects of Early Phosphorus Intake on Respiratory Distress in Extremely Low-Birth-Weight Infants

        Purpose: This study was aimed to investigate the effect of early phosphorus intake on respiratory distress in extremely low-birth-weight infants (ELBWIs) with a high incidence of hypophosphatemia. Methods: We performed a retrospective study to target 164 ELBWIs admitted to the neonatal intensive care unit in Seoul National University Children’s Hospital. Birth characteristics, nutritional intake, and electrolyte levels during the first week were investigated as predictors that would affect the clinical outcomes. The correlations among invasive ventilation at postnatal age of 2 weeks, moderate-to-severe bronchopulmonary dysplasia (BPD), and phosphorous intake were analyzed. Results: Hypophosphatemia (phosphorus level <4 mg/dL) was observed in 72.0% of the subjects. The rates of invasive ventilation (P=0.001) and moderate-to-severe BPD (P=0.005) were significantly lower in the high phosphorus intake group (≥0.7 mM/kg/day) than in the low phosphorus intake group (<0.7 mM/kg/day). Phosphorus intake during the first week was a significant factor affecting invasive ventilation at 2 weeks of age (adjusted odds ratio [OR], 8.212; 95% confidence interval [CI], 2.256 to 28.896; P=0.001) and moderate-to-severe BPD (adjusted OR, 3.402; 95% CI, 1.274 to 9.084; P=0.015). Conclusion: Early insufficient phosphorus intake confers a significantly higher risk with invasive ventilation at 2 weeks of age and moderate-to-severe BPD. Therefore, early sufficient phosphorus supply may improve respiratory outcomes in ELBWIs.
