        혼합치열기 반대교합자에서 Ⅲ급 악간견인력의 효과에 관한 연구

        The purpose of this study is to observe how the Class Ⅲ intermaxillary elastics act upon the craniofacial structures of mixed dentition with anterior crossbites. The cephalometric headplates of 16 childrens treated only with Class Ⅲ elastics(C Ⅲ elastics group) and 23 childrens treated with Class Ⅲ elastics and Reverse pull headgear simultaneously(C Ⅲ elastics +RPHG group)were traced, digitized and statistically analyzed. The results were as follows. 1. Anterior displacement of maxilla was observed in both groups. 2. Simultaneous Class Ⅲ elastics and reverse pull headgear group showed the counterclockwise rotation of maxilla, however Class Ⅲ elastics group manifested no rotational change of maxilla. 3. Simultaneous Class Ⅲ elastics and reverse pull headgear group showed the downward and backward rotation of mandible, however Class Ⅲ elastics group manifested no rotational change of mandible. 4. Counterclockwise canting of occlusal plane, labial tipping of upper incisor and lingual tipping of lower incisor and mesial displacement of upper molar to pterygoid root vertical were observed in both groups.

        Fluoride, Bisphosphonate 및 Gallium이 상아질 흡수 억제에 미치는 영향

        Replacement resorption is followed by the delayed replantation of an avulsed tooth. Currently no effective treatment is substantiated for replacement resorption. The purpose of this study was to investigate the effect of stannous fluoride, bisphosphonate(etidronate disodium) and gallium nitrate, which have been shown to reduce dentin resorption, on human dentin. Osteoclasts were collected from tibeas of chich embryo. The cells were well agitated to prevent adhesion and seeded onto the sliced human dentin wafers which had been soaked in either culture media(control), or several different concentrations of stannous fluoride, etidronate disodium(1-hydroxyethylidene-1, 1-bisphosphonate disodium), and gallium nitrate. Resorption was measured by counting the number of resorptive pit produced by the cells. Results are ad follows. Stannous fluoride and etidronate disodium showed statistically significant reduction of dentin resorption(p<0.05) but the effect of stannous fluoride seemed to be its high cytotoxicity. Etidronate disodium did not show cytotoxicities in all experimented concentrations. Gallium nitrate did not show differences in resorption either between different concentrations of from the control group.

        흉요추 이행부 골절환자의 임상분석

        To clarify the prognostic factors influencing clinical outcome. the authors retrospectively analyzed 50 cases of thoracolumbar vertebral junctuon fracture treated at our hospital between September 1989 and October 1992. The age of the patients ranged from 23 to 75years(median 49years) and the male-to-female ratio was 33:17. Falling was the major mode of injury(n=27). The kyphotic angle between fractured vertebrae and the ratio of neural canal encroachment was measured in a simple X-ray and a computed tomography(CT) of the thoracic spine. An ANOVA test was performed to evaulate the predicting factors to determine the final outcome. The kyphotic angle and loss of height of vertebral body were not significantly correalted with the neurological status of the patients. However. the ratio of the compromised area of the neural canal was closely correlated with the neurological outcome(p value<0.05). The worst prognosis was found in patients with fracture-dislocation. In conclusion. it is suggested that early surgical intervention must be considered in the neurologically compromised patients whose CT demonstrates neural canal encroachment by bony fragments.

        The Internet of Things, the core of the Fourth Industrial Revolution, is growing by incorporating various technologies into various fields. Among them, smart home systems that apply IoT technology to home environments are being actively studied. The home hub market, a means of communication for smart homes, is also developing at the same time. As the home hub market develops, security threats are also increasing accordingly. In this paper, 'Smart Home Mirror' is implemented by combining mirror and IoT technologies so that users can use them to communicate with smart home applications and other devices. To meet security issues, users in it are recognized by using face recognition technology and AES between all data communications in it is applied. The proposed device’s usefulness is approved by implementing it in smart home environments.

        뇌동맥류 파열에 의한 급성 경막하 혈종이나 뇌내혈종을 동반한 혼수상태 환자에 대한 뇌혈관 조영술을 시행하지 않은 응급수술

        Neurologically based clinical grading scales offer excellent prognostic information for the patient suuffering diffuse subarachnoid hemorrhaging. These grading scales are less applicable to patients with life-threatening intraparenchymal or subdural hematomas after aneurysmal rupture. During 2 and half years. 9 patients(ICH;5.SDH:1. ICH+SDH:3) in a comatose state with brain stem compression syndromes documented by computerized tomographic scans have undergone emergent operation without angiography in our neurosurgical departement. Each patient was so critically ill that we believed it unwise to delay craniotomy for diagnostic angiography. The average delay from the detection to operation was 2 hours. The origin of the hematoma was identified as a berry aneurysm of the middle cerebral artery bifurcation in seven patients and a posterior communicating artery aneurysm in two patients. One patient had a good recovery. two patients recovered with hemipardsis and mild cognitive dysfunction. two patients were vegetative and died due to pneumonia. and four patients died due to brain swelling within two weeks postoperatively. Emergent craniotomy with empiric exploration of appropriate subarachnoid cisterns after hematoma decompression may be life-saving management in some cases. The delay imposed for diagnostic angiography may be avoided in attempts to save vital minutes of severe brain stem compression.

        Management of Unilateral Facet Dislocation of the Cervical Spine

        Objective : Unilateral facet dislocation of the cervical spine occurs by flexion and rotation injuries and cannot be easily reduced by axial traction. We analyzed 14 consecutive patients with unilateral facet dislocation of the cervical spine to increase knowledge about anatomical reduction of locked facet and factors for successful reduction. Methods : Fourteen patients [10 men and 4 women] with unilateral facet dislocation of the cervical spine were retrospectively analyzed. Plain X-ray, computerized tomography scan, and magnetic resonance imaging were performed. All patients underwent manual reduction and surgery with anterior interbody fusion and plate fixation. The manual reduction was performed by neck flexion and rotation to the opposite side of dislocation, followed by rotation and flexion of the head toward the side of dislocation and extension with relaxation of traction. Mean follow-up period was 17 months. The level of spine, amount of subluxation, combined facet fracture, and time from injury to initial reduction were analyzed using the data obtained from medical records. Results : Thirteen [93%] patients were reduced successfully. Immediate reduction was achieved in 7 patients but failed in 7 patients. Seven patients underwent delayed closed reduction under general anesthesia, and successful reduction was achieved in 6 patients. Only one patient with bone chips between articular facets failed to achieve anatomical reduction. Conclusion : In order to reduce the locked facet more easily and safely, we recommend manipulative traction with anterior interbody fusion and plate fixation under general anesthesia after being aware of spinal cord injury with magnetic resonance imaging.

        기립성 체위성 빈맥 증후군으로 진단된 소아청소년 환자의 임상적 특징 및 신경생리검사 소견

        Purpose : No domestic report has been published on postural orthostatic tachycardia syndrome (POTS) in adolescents, although it has been increasingly studied in adolescents since first reported in 1999. This study analyzed the clinical characteristics and neurophysiologic findings of POTS in Korean pediatric patients. Methods : We reviewed the medical records of pediatric patients (<18 years) with POTS who visited Gyeongsang National University Hospital in Jinju between January 2008 and March 2010. All patients underwent tilt-table and autonomic function tests. Result : We identified 18 patients (72.2% male, mean age 13.6 ± 2.1 years). The mean orthostatic heart rate increment was 43.0 ± 9.1 beats/min. Most patients (83.3 %) had mild autonomic dysfunction, and three (16.6%) had moderate autonomic dys- function. Half of patients had sudomotor abnormalities in the lower limbs, implying peripheral postganglionic sympathetic denervation in the legs. Of these patients, 66.6 % also had adrenergic impairment. The degree of autonomic dysfunction correlated with the orthostatic heart rate increment (P =0.047). Patients needing pharmacologic treatment had a higher orthostatic heart rate increment (P =0.017), composite auto- nomic severity score (P =0.002), and sudomotor score (P =0.006), compared with non-pharmacologically treated patients. Conclusion : Autonomic dysfunction or autonomic neuropathy seems to be the main pathophysiology of POTS in pediatric patients, especially adolescents. Further, the degree of autonomic dysfunction influenced the prognosis of POTS. Additionally, unlike previous adult series, most of our patients were male. This male predominance in adolescents suggests that the growth spurt is an important factor triggering POTS at this age.

        전자빔을 이용한 폴록사머 하이드로젤의 제조 및 구강 점막부착성 약물전달을 위한 특성 분석

        본 연구에서는 전자빔을 이용하여 폴록사머 하이드로젤을 제조하여 구강 점막부착성 약물전달체로서의 특성을 평가하고자 하였다. 온도감응성을 나타내는 폴록사머 고분자의 말단에 가교반응을 위한 비닐기를 도입 한 후, 방사선 가교반응을 이용하여 하이드로젤을 제조하였다. 점착성 향상을 위한 첨가제로 점착성 고분자인 카 보폴을 첨가하여 하이드로젤을 제조하였고, 카보폴이 점막부착성과 서방성 방출에 미치는 영향을 알아보았다. 결과적으로, 폴록사머 고분자의 말단 개질과 카보폴의 첨가에 의해 하이드로젤의 기계적 물성과 점막부착성이 향상되었고, 약물방출실험 결과 약물이 방출되는 속도가 지연되는 결과를 확인하였다. In this study, poloxamer hydrogels were prepared by electron beam irradiation and evaluated for potential application as a buccal mucoadhesive drug delivery system. Poloxamer, one of typical thermoresponsive polymers, was modified to have vinyl end groups for crosslinking reaction, and its hydrogels were fabricated by irradiation crosslinking reaction. Carbopol as a mucoadhesive polymeric additive was introduced to improve the mucoadhesive property of the hydrogels and its effect on the mucoadhesion and drug release properties was investigated. The results showed that the end group modification of poloxamer and the addition of carbopol improved mucoadhesive force and mechanical properties and led to a sustained drug release behavior.
