목적: 스테로이드로 골 다공증을 유도한 가토에서 신연 골형성을 연구하였다. 대상 및 방법: 성숙한 가토에서 스테로이드 투여 용량에 따라 1군(스테로이드를 투여하지 않은 군), 2군(0.4 ㎎/㎏/day 2주간 투여한 군), 3군(0.4㎎/㎏/day 4주간 투여한 군), 4군(0.8 ㎎/㎏/day 2주간 투여한 군), 5군(0.8 ㎎/㎏ 4주간 투여 한 군)으로 나누었다. 스테로이드 투여 후 경골 간부에 외고정 기구를 고정한 후 절골술을 시행하였으며 4주간 골신연술을 시행하였다. 1주마다 단순 방사선 사진 촬영과 골밀도 및 퍼센티지 골밀도를 측정하고, 최종 신생골 형성 정도에 따라 3개의 군으로 나누어 조직학적 분석을 하였다. 결과: 양호한 가골을 형성한 가토는 모두 1군이었다. 그들의 요추 골밀도는 평균 5.1% 감소되었으며, 신연 1주 후 신연부 퍼센티지 골밀도는 50 이상이었다. 결론: 성숙한 가토에서 요추 골밀도가 10% 이상 감소되었거나, 신연 1주 후 퍼센티지 골밀도가 50 미만인 경우 사지 연장술 중 문제점의 발생이 예상된다. 인체에 있어서도 유사한 수치가 적용될 수 있으리라 생각된다. Purpose: This study investigated the results of distraction osteogenesis in steroid-induced osteoporotic rabbits. Materials and Methods: Mature rabbits were assigned to the following five steroid dosage groups: Ⅰ (none); Ⅱ (0.4 ㎎/㎏/day for 2 weeks); Ⅲ (0.4 ㎎/㎏/day for 4 weeks); Ⅳ (0.8 ㎎/㎏/day for 2 weeks); Ⅴ (0.8 ㎎/㎏/day for 4 weeks). After the steroid injection, the tibias were osteotomized and distracted for 4 weeks. The rabbits were classified radiographically into three groups according to the quality of new bone formed, and the Bone mineral density (BMD) and percentage BMD for these groups were then compared. The microscopic results supported the radiographic groupings. Results: The rabbits that formed good calluses were all from group Ⅰ, and their average lumbar BMD remained high (mean decrement, 5.1%). These rabbits had a percentage BMD ≥50 after 1 week of the distraction. Conclusion: Problems can be expected during limb lengthening in mature rabbits that shows a lumbar BMD decrease ≥10%, or a percentage BMD <50 after 1 week of the distraction. Similar guidelines may be indicated for humans.

Background: Ginseng is a commonly used herbal medicine in treating various medical conditions. Chronic gut inflammation is a recognized factor for the development of colorectal cancer (CRC). In thisproject, Asian ginseng berry polysaccharide preparations were used to assess their effects on CRC andrelated immune regulation mechanisms. Methods: Ginseng berry polysaccharide extract (GBPE) and purified ginseng berry polysaccharideportion (GBPP) were used to evaluate their activities on human HCT-116 and HT-29 CRC cell proliferation. Interleukin-8 secretion analysis was performed on HT-29 cells. Naive CD4 cell isolation and T-helper celldifferentiation were performed and determined using flow cytometry for Th1 and Treg in addition to cellcycle and apoptotic investigation. Results: GBPE and GBPP significantly inhibited interleukin-8 secretion and cancer cell proliferation,inhibited CD4þIFN-gþ cell (Th1) differentiation, and decreased CD4þFoxP3þ cell (Treg) differentiation. Compared to the GBPE, GBPP showed more potent antiinflammatory activities on the malignant cells. This is consistent with the observation that GBPP can also inhibit Th1-cell differentiation better, suggestingthat it has an important role in antiinflammation, whereas Treg cells hinder the body’s immuneresponse against malignancies. Supported by cell cycle and apoptosis data, GBPE and GBPP, at variousdegrees, remarkably enhanced the anticancer activities of 5-fluorouracil. Conclusion: Data from this project suggested that Asian ginseng berry potentially has clinical utility inmanaging enteric inflammation and suppressing CRC through immunomodulation mechanisms.

목적: 천골 및 천장관절을 포함한 불안정성 골반골 골절 환자의 치료에 있어 해면골 유관 나사를 이용한 조기 후방 내고정술의 유용성에 대해 보고하고자 한다. 연구 대상 및 방법: 16예의 천골 및 천장 관절을 포함한 불안정성 골반 골절 환자를 대상으로 하였다. 수상기전에 따른 분류상 측면 압박이 4예, 전후면 압박이 7예, 수직 전단력이 4예, 혼합형이 1예였다. 치료는 전방 외고정술과 함께 후방 내고정술을 병행한 경우가 9예(해면골 유관 나사 7예, 경 장골 막대 2예), 전방 외고정술만을 시행한 경우가 5예, 수술적 고정 없이 보존적 요법으로 치료한 경우가 2예였다. 치료결과는 방사선학적 분석과 함께 Matta등의 기준에 따른 임상 결과를 분석하였다. 결과: 전방 외고정술과 함께 후방 내고정술을 동시에 시행한 군(만족 7예, 불만족 2예) 이 전방 외고정술만을 시행한 군(만족 1예, 불만족 4예)이나 보존적 요법으로 치료한 군(불만족 2예) 보다 우수한 임상적 결과를 보였고 방사선학적으로도 부정 유합이나 불유합의 빈도가 적었다. 결론: 해면골 유관 나사를 사용한 후방 내고정은 비 침습적이고 시술이 용이하여 조기에 시행될수 있으며 불안정성 골반골 골절 후 따를 수 있는 여러 가지 합병증들을 막을 수 있는 좋은 치료 방법이라고 생각된다. Purpose: To evaluate the usefulness of early posterior internal fixation(I/F) by cannulated cancellous screw(CCS) in unstable pelvic bone fractures involving the sacrum and sacroiliac joint. Materials and methods: Sixteen cases were evaluated retrospectively. Classification according to the injury mechanism, using Young`s criteria, was as follows: 7 anteroposterior compression, 4 lateral compression, 4 vertical shear and 1 combined mechanism. With respect to treatment, 9 cases were treated by combined anterior external fixation(E/F) and posterior I/F with CCS(7 cases) and transiliac rod(2 cases). Five cases were treated by anterior E/F only, and 2 cases were treated without surgery. We analyzed clinical results using Matta`s criteria, and radiologic abnormalities. Results: The management by posterior I/F with CCS and transiliac rod including anterior E/F has shown superior clinical results(7 satisfactory, 2 unsatisfactory) over the management by only anterior E/F(1 satisfactory, 4 unsatisfactory) or conservative methods(2 unsatisfactory). The rate of malunion and nonunion was also low in the former method in the radiographic analysis. Conclusion: Posterior I/F using CCS is an excellent surgical procedure in the treatment of unstable pelvic bone fracture due to its lessened invasiveness, early applicability, simple surgical technique and its direct effects on the prevention of several complications.
Simultaneous Expression of the Protease Inhibitors in a Rice Blast-Resistant Mutant

We have previously identified genes for four different protease inhibitors (PIs) that were induced upon rice blast infection in a rice blast resistant mutant SHM-11. Our expression analysis of the PIs indicated that induction of the PIs was the highest 24 hr after rice blast inoculation in the rice mutant SHM-11. Three PIs in the group of serine PIs were highly expressed while a cystein PI was weakly expressed upon rice blast inoculation. Four PIs were weakly induced 48 hr after pathogen inoculation in rice blast susceptible wild type rice plant. The simultaneous expression of three serine PIs was apparent from SHM-11 and two of them were induced in rice blast resistant Taebaegbyeo. One of them was induced in rice blast resistant Hwayeongbyeo while none of them were expressed in rice blast susceptible Nagdongbyeo and rice blast resistant Dongjinbyeo. Our results suggest that the expression of PI gene is rice cultivar specific and may be linked with the rice blast resistance in a specific rice mutant by the simultaneous expression of the PI genes.
