      • Methylprednisolone Acetate를 이용한 고립성 골낭종의 치료

        We treated 19 cases of unicameral bone cysts with methylprednisolone acetate (MPA) from January 1988 to December 1995. We evaluated the effect of MPA injections through simple follow-up radiographs according to Oppenheim's, classification and retrospectively reviewed the sites of cyst, age of the patients at diagnosis, the incidence of pathologic fracture, cystic nature, and cystic proximiy to the growth plate, We then analyzed the relationship between these variables with the results of MPA injections. According to Oppenheim's classification, the results with the use of MPA injections were as follows: healed in six cases, improved in seven cases, incomplete obliteration in five cases and recurred in one case, If healed and improved were considered satisfactory results, then 13 cases(68.4%) were satisfactory at the last follow-up. Sites of cyst, age of the patients at diagnosis, incidence of pathologic fracture, cystic nature and cystic proximiy to the growth plate appeared not to influence the results of MPA injections statistically. On these data, we thought that the treatment of unicameral bone cysts with MPA injection was the most effective first choice of treatment before operative options such as curettage and bone graft.

        제2 중수골 기저부의 견열 골절

        제2, 제3 중수골의 기저부에 요측 신건에 의한 견열 골절은 드물게 일어나는 것으로 알려져 있으며, 제2 중수지 기저부에 장요수근 신건의 부착부의 견열 골절과 제3 중수골 기저부의 단 요 수근 신건의 부착부의 견열 골절이 보고되었다. 제1 중 수골 기저부의 골절의 치료에 대해서는 여러 문헌에서 흔히 다루어지고 였으나 이들 골절의 치료에 대한 보고가 드물다. 이에 저자들은 제2 중수골 기저부의 견열 골절 2예를 관혈적 정복 및 소형 금속판을 이용한 내고정술로 좋은 결과를 얻었기에 문헌 고찰과 함께 보고한다. Avulsion fractures involving insertions of the radial extensors at the base of the index and middle metacarpals are quite rare. However, avulsion fractures at the insertion site of the extensor carpi radialis longus on the index metacarpal and avulsion fractures involving the extensor carpi radialis brevis insertion at the base of the middle finger metacarpal have been reported. Although there are many reports of fractures of the base of the first metacarpal, there are few recommendations available for treatment of these injury. We report two avulsion fractures at the extensor carpi radialis longus insertion, which were treated with an open reduction and internal fixation of the avulsed bony fragment using a miniplate and screw.

        소아 상완골 외과 골절에서 도수 정복술과 개방 정복술의 비교

        목적: 소아 상완골 외과 골절을 분류하고, 치료 방법과 골절의 형태에 따른 결과 및 합병증을 분석하였다. 대상 및 방법: 1995년 5월부터 2001년 5월까지 소아 상완골 외과 골절로 진단 받고 1년 이상 추시가 가능했던 92예를 대상으로 하였다. 남자가 66예, 여자가 26예이었으며, 79예가 9세 미만이었다. Milch Ⅰ형이 12예, Ⅱ형이 80예였고, Jakob Ⅰ기 15예, Ⅱ기 59예, Ⅲ기가 18예였다. 9예에서 비수술적 치료로 장상지 석고 고정술을 시행하였고, 83예에서 수술적 치료를 시행하였다. 도수정복 및 K-강선 내고정이 18예, 관혈적 정복 및 내고정이 65예였다. 결과: 운반각 변형은 외반주 변형 및 내반주 변형이 의심되는 7예를 제외하고는 임상적으로 의미가 없었으며, 7° 증가에서 18° 감소까지(평균 -1.3 감소) 운반각 변형을 보였다. Hardacre 등에 의한 치료 결과 판정에서 우수가 46예, 양호가 41예, 그리고 불량이 5예였다. 추시 중에 발생한 합병증으로 골극 형성이 8예, 외과 과잉 성장 및 내반주 4예, 외반주가 3예, 무혈성 골괴사 3예, 조기 성장판 유합 2예, 불유합 2예, 지연 유합 1예, fish-tail 변형 1예, 국소 염증 5예, 관절운동범위 제 한 2예, 그리고 일시적 요골신경 마비가 1예였다. 결론: 상완골 외과 골절은 소아 주관절 주위 골절 중 비교적 높은 합병증 발생률을 보였고, 임상적으로 유의한 합병증은 Jakob Ⅱ기의 일부 및 대다수의 Ⅲ기에서 발생하였다. 따라서 Jakob stage Ⅲ 골절에서 관혈적 정복을 시도하는 경우 좀더 세심한 연부조직의 조작과 해부학적 정복에 노력해야 할 것으로 판단된다. Purpose: The fracture of humeral lateral condyle in children was classified and the complications and results were analyzed according to the treatment modality and the types of fracture. Materials and Methods: 92 patients who could be followed up one year or more, and were treated for a humeral lateral condyle fracture from May 1995 to May 2001 were studied. 66 cases were male, 26 cases were female, and 79 cases were under 9 years of age. The Milch stage Ⅰ encountered in 12 cases, and Ⅱ in 80 cases. Jacob stage Ⅰ were encountered in 15 cases, stage Ⅱ in 59 and stage Ⅲ in 18 cases. Conservative treatment was performed in 9 cases and surgery was performed in 83 cases. A closed reduction and internal fixation with K-wire was performed in 18 cases, and an open reduction and internal fixation was performed in 65 cases. Results: The change in the carrying angle was not clinically significant except for 7 cases, with a cubitus varus and cubitus valgus deformity. The change in the carrying angle showed an average -1.3 (from +7 to -18). The results according to the Hardacre criteria were 'excellent' in 46 cases, 'good' in 41 and 'poor' in 5. Regarding the complications, bony spur formation was encountered in 8 cases, overgrowth of the lateral condyle and cubitus varus in 4, cubitus valgus in 3, avascular osteonecrosis in 3, prema-ture epiphyseal fusion in 2, nonunion in 2, delayed union in 1, fish-tail deformity in 1, local infection in 5, range of motion limitation in 2 and transient radial nerve palsy in 1 case. Conclusion: Humeral lateral condylar fractures have a high rate of complications and clincally significant complication appeared mainly in stage Ⅱ and Ⅲ. This suggests that more a careful dissection of the soft tissue and anatomical reduction in Jakob stage Ⅲ is required in cases of an open reduction.

        종양괴사인자로 유도된 기도 뮤신 생성, 뮤신 유전자 발현 및 NF-κB p65의 핵으로의 이동에 대한 크리신의 영향

        Chrysin and chlorogenic acid are natural products derived from Scutellariae Radix and Lonicerae Flos, respectively. We examined whether chrysin and chlorogenic acid affect airway mucin production induced by TNF-α in NCI-H292 cells. Cells were pretreated with each agent for 30 min and then stimulated with TNF-α for 24 h. Of the two compounds, chrysin suppressed airway MUC5AC mucin production. Also, chrysin suppressed MUC5AC mucin gene expression and translocation of NF-κB p65 induced by TNF-α. This result suggests that chrysin can regulate the production and gene expression of mucin induced by TNF-α through the inactivation of NF-κB in airway epithelial cells.

        신연 골 연장술시 연령에 따른 국소성장 인자가 근육재생에 미치는 영향

        목적: 골 견인 시 견인율과 연령에 따라 성장인자가 근육 재생에 미치는 영향을 평가하였다. 대상 및 방법: 젊은 쥐 6마리, 성숙 쥐 6마리를 대상으로 경골의 골 견인이 전 경골근과 가자미 근에 미치는 영향을 측정하였다. 젊은 쥐는 6주(평균 250 gm), 성숙 쥐는 6개월(평균 450 gm)된 Sprague-Dawley 쥐를 사용하였으며, 면역조직화학 검사를 통하여 국소성장인자인 IGF-1, PDGF, bFGF가 발현되는 양상을 평가하였다. 신연율은 0.7 ㎜/일(2회)로 전 예에서 우측 경골을 견인시켰고, 좌측은 대조군으로 사용하였다. 전 예에서 경골의 길이는 총 20%를 신장하였다. 결과: 전 경골 근에서 골 신연시 IGF-1 및 PDGF의 발현은 연령의 증가에 따라 감소하였다(p<0.05). 가자미근에서는 IGF-1와 PDGF가 모두 성숙 쥐(6개월)에서 골 신연시 증가하는 양상을 보였으나 통계적인 유의성은 없었다. BrdU 양성인 세포는 myosatellite cell 뿐만 아니라 골격근의 기저 막에서 발현되었고, IGF-1과 PDGF도 myosatellite cell에서 발현하였다. 그러나 bFGF는 발현하지 않았다. bFGF는 골 재생을 촉진하였으나, 근육 재생에서 bFGF의 작용은 불분명하였다. 결론: 빠른 신연 골 연장술시 근육에서 성장인자 분비가 촉진되며 성숙되지 않은 근육은 신연에 대한 적응력은 강하나 성숙된 근육은 약한 것으로 판단된다. 이는 성숙한 근육이 골신연시 국소 성장인자 유리와 myosatellite cell 증식과 분화를 활성화하는 능력이 저하되어 있기 때문으로 사료된다. Purpose: To evaluate the effect of growth factors on muscle regeneration related to the distraction rate and age on bone distraction. Materials and Methods: This study examined the effects in the tibialis anterior (TA) and soleus muscles after tibial bone distraction in 6 young and mature rats. The young and old rats were 6 weeks old (average 250 ㎎) and 6 months old (average 450 ㎎), respectively. In all rats, the right tibial bone was distracted, and the left was used as a control group. The development pattern of IGF-1, PDGF, bFGF was assessed using immunohistochemical techniques. The distraction rate was 0.7 ㎜/day (2 times) and the total extension ratio was 20% in all cases. Results: In the TA muscle, the development of IGF-1 and PDGF was inversely proportional to age (p<0.05). In the soleus muscle, the development of IGF-1 and PDGF was higher in the old rats but the difference was not statistically significant. BrdU positive cells were expressed in the myosatellite cells and the basement membrane of myocytes. IGF-1 and PDGF was also expressed in myosateilite cells but bFGF was not. bFGF facilitated bone regeneration but the mechanism for its effects on muscle regeneration were not identified. Conclusion: Distraction osteogenesis facilitates the release of growth factors. Immature muscles adapt to the distraction but mature muscles do not. This suggests that mature muscles are less able to activate the proliferation and differentiation of myosatellite cells and the local release of growth factors when bone distraction is performed.

        보중익기탕가오가피(補中益氣湯加五加皮)가 항피로(抗疲勞)에 미치는 영향(影響)

        Objects : This experiment was performed into investigate the effects of Bojungikgitang(Buzhongyiqitang) and BojungikgitanggaOgapi(Buzhongyiqitangjiawujiapi) on the recovery of fatigue in the swimming loaded rats by the lactic acid levels, creatine phosphokinase(CPK) activities, lactate dehydrogenase(LDH) activities and glucose levels, which are known as materials related to fatigue in blood. Methods : On one hour before swimming exercise, the extracts of Bojungikgitang(Buzhongyiqitang) and BojungikgitanggaOgapi(Buzhongyiqitangjiawujiapi) were orally administered to the rats(Sprague-Dawley) which had been in the environment controlled rearing system for 2 weeks, and the control group were fed with equal amount of normal saline. And the lactic acid levels, creatine phosphokinase(CPK) activities, lactate dehydrogenase(LDH) activities and glucose levels in blood in rats were measured immediately and at thirty minutes after swimming exercise. Results : 1. The lactic acid levels in BojungikgitanggaOgapi(Buzhongyiqitangjiawujiapi) group were decreased immediately after exercise and statistically showed the significant difference compared with the control group(P<0.05). 2. The creatine phosphokinase(CPK) activities in Bojungikgitang(Buzhongyiqitang) group were decreased immediately after exercise and statistically showed the significant difference compared with the control group(P<0.05), and in BojungikgitanggaOgapi(Buzhongyiqitangjiawujiapi) group were decreased immediately and at 30 minites after exercise and statistically showed the significant difference compared with the control group(P<0.05). 3. Lactate dehydrogenase(LDH) activities and glucose levels in Bojungikgitang(Buzhongyiqitang) group and BojungikgitanggaOgapi(Buzhongyiqitangjiawujiapi) group did not show the significant difference compared with the control group. Conclusion : Based on the above results, we know that Bojungikgitang(Buzhongyiqitang) and BojungikgitanggaOgapi(Buzhongyiqitangjiawujiapi) have effects on the recovery of fatigue.
