고혈압·당뇨병 신규 환자 발견 이후 지역사회 협력을 통한 등록관리가 치료순응도 및 혈압 및 혈당 조절에 미치는 영향

인천광역시의 2개 지방자치단체에서 고혈압과 당뇨병의 관리를 위해 민간의료기관과 함께 만든 만성질환등록관리체계의 효과를 평가하였다. 만성질환등록관리체계에 등록관리가 이루어지고 있는 군이 미등록군에 비해 치료순응도와 혈압 및 혈당의 조절 정도가 좋은지를 평가하였다. 자료분석은 로짓회귀모형을 구축하여 이루어졌고, 다단계 변수 선택법을 사용하여 교차비와 95% 신뢰구간을 구하였다. 연구결과 등록관리군이 미등록군에 비해 치료순응도가 5.5배 높았고, 혈압 및 혈당의 조절이 3.78배 더 잘 되었다. 향후 이러한 효과가 인천광역시 전체 사업으로 확대된 후에도 지속되는지를 평가할 필요가 있다. Objectives : Incheon metropolitan city has set a model program of community partnership for hypertensive or diabetic patients detection and follow-up since 2005. This study examines the effects of the model on the compliance and the control of blood pressure or blood sugar level Methods : Telephone-surveys were done for 140 persons selected from 408 patients who were detected newly as hypertensive or diabetic patients at Gangwha-Gun and Seo-Gu in Incheon between January 1st and August 31th in 2006, and finally completed in 110 patients(78.6%). Survey questionnaires included socio-demographic(age, gender, and educational level), health behavioral (smoking, drinking, physical activity, and diet), and therapy-related (registration state, compliance, blood pressure or blood sugar control) variables. Odds ratio and 95% CI were derived from logistic regression model. Results : Registered group exhibited high compliance and well managed blood pressure or blood sugar level. The odds ratio of registration were 5.55(95% CI:1.83~16.89) for compliance and 3.78(95% CI:1.43~9.99) for blood pressure or blood sugar control after adjusting for age, gender, disease, and area Conclusions : It is independently related to compliance and blood pressure or blood sugar control whether the patient is registered or not. To control hypertension or diabetes mellitus in terms of compliance and control state, a community-based registration program through community partnership could be a powerful tool.

Cooking characteristics of coated rice with water homogenate of citrus fruit peels (1% for rice) were investigated. The color of the coated rice both before and after cooking was dark yellow. The total content of carotenoids, hesperidin and naringin were 10.74, 2173.12 and 1468.40 ㎎% for citrus fruit peels, 0.46, 108.65 and 73.38 ㎎% for its water homogenate, 0.12, 21.73 and 14.62 ㎎% for coated rice, and 0.05, 8.67 and 5.87 ㎎% for cooked coated rice, respectively. Citrus fruits peel contained 94.22 ㎎% of asparagine, 24.88 ㎎% of methionine, 19.64 ㎎% of alaniue, and 15.37 ㎎% of r-aminoisobutyric acid as the majority free amino acids, accounting for 70% of the total free amino acids present. Total free amino acid content of the cooked coated rice increased by 15% compared to those of cooked uncoated rice. The majority of minerals in the citrus fruit peels were K and Ca, accounting for 86% of total minerals present. The mineral content of cooked coated rice was generally higher than that of the cooked uncoated rice. The cooked coated rice showed comparable hardness, gumminess and brittleness, but higher cohesiveness and springiness than the cooked uncoated rice. There were no differences in sweet and bitter taste between the cooked uncoated and coated rice. However, the cooked coated rice showed higher sensory scores for color acceptability, savory taste and overall acceptability than the cooked uncoated rice.

地衣類의 영양학적 가치를 조사하기 위하여 地衣類(지붕, 바위, 나무)와 석이의 總脂質 含量, 脂質 분포, 脂肪酸 조성, 蛋白質 含量을 측정하였다. 그 결과 총지질 함량은 100g당 석이는 3.47g, 바위에 地衣類는 3.40g, 지붕의 地衣類는 3.17g, 나무의 地衣類는 2.73g 이었고 TLC法에 의한 脂質의 분포는 석이와 地衣類 모두 cholesterol ester와 phospholipid 등이 있었고 나무 地衣類에는 TG 및 cholesterol의 흔적을 보였다. 지방산 조성은 대부분 C_12~C_18 fatty acid였고 그 중 C_18 : 1 C_18 : 2인 것이 50% 정도로 높게 나타났다. 나무의 地衣類에는 C_18 이상의 fatty acid도 상당량 분포되어 있었으며 C_20.4 fatty acid는 18.9%였다. 단백질 함량은 석이 6.60g, 地衣類는 8.22g이었다. To investigate the nutritional value of lichens, contents of total lipid, compositions of fatty acids, distributions of lipids and contents of total proteins in lichens (on the rock, roof and tree) and sogi were examined. The results were summarized as follow. The contents of total lipid in sogi, rock, roof, and tree lichens were 3.47±0.05%, 3.40±0.09%, 3.17±0.02%, 2.73±0.04% respectively. Lipids in all them were cholestrol ester and phospholidpid, and TG and cholesterol were existed only the lichen on the tree. Fatty acids were composed of that of carbon number 12-18; C_18:1 and C_18:2 were about 50%, Contents of total protein in sogi and lichen (roof) were 6.60% and 8.22% respectively.
웨이브렛 신경회로망을 이용한 비선형 시스템의 안정한 직접 적응 제어

이 논문에서는 웨이브렛 신경회로망을 사용하여 알려지지 않은 비선형 시스템을 안정하게 적응 제어하는 문제를 다룬다. 비선형 시스템의 정확한 제어는 함수를 근사화하는 데 상용된 함수 근사화기의 정확성과 효율성에 의존한다. 이에 기준 함수의 선택이 자유롭고 함수 근사화 능력이 뛰어난 웨이브렛 신경회로망을 비선형 시스템 제어에 사용한다. 초기 웨이브렛 신경회로망 제어기를 설정하는 방법은 먼저 제어기 입력의 시-주파수 특성을 분석해서 웨이브렛 신경회로망 변수인 신축과 이동을 구하고, 다음에 Lyapunov 안정성 이론에 기초한 적응 법칙을 사용하여 연결 강도를 조절하는 것이다. 이 직접 적응 웨이브렛 신경회로망 제어기를 불안정한 비선형 시스템인 역 진자 시스템을 제어하는 데 적용한다. In this paper, we deal with the problem of controlling an unknown nonlinear dynamical system, using wavelet network. Accurate control of the nonlinear systems depends critically on the accuracy and efficiency of the function approximator used to approximate the function. Thus, we use wavelet network which shows high capability of approximating the functions and includes the free-selection of the basis functions for the control of the nonlinear system. The method of constructing an initial adaptive wavelet network controller is that we find the dilation and translation that are wavelet network parameters by analyzing the time-frequency characteristics of the controller's input, and then, the weight is adjusted by the adaptive law based on the Lyapunov stability theory. We apply this direct adaptive wavelet network controller to control the inverted pendulum system which is an unstable system.
