To ellucidate immunologic activities of ginseng saponin, the splenic leukocytes of BALB/c mice were incubated in RPMI medium for 48hr with or without ginseng total saponin(GTS), and then analyzed flow cytometrically. Upto the concentration of 100㎍/㎖, GTS exherted stimulatory effect on the immune cells most prominently on B cells: the percentage of the surface Immunoglobulin (sIg) -positive cells (B cells) of the control was 47.l%, while that of GTS treated was 58.9%. The percentages of monoclonal antibody(mAb) J1j-reactive cells (T cells) were 28.8% and 3l.3%, those of mAb Gkl.5-reactive cells (CD4' T cells) were l8.5% and l9.l%, and those of mAb M1/70-reactive cells (mostly macrophages) were 4.4 and 5.0% at the concentration of 0 and 100㎍/㎖, respectively. However, at the concentration of 300㎍/㎖ or higher, GTS was found to be toxic to the immune cells especially to B cells in that the flow rate of the cells (gated viable cells) significantly decreased and the percentage of B cells droped from 58.9% to 31.2% and 25.4% at l00, 300, and 1000㎍/㎖, respectivelly. These data strongly suggests that GTS would exhert B-cell mitogen-like activity at physiological concentrations of 100㎍/㎖ or lower.
Potassium Currents in Isolated Deiters' Cells of Guinea Pig

Deiters' cells are the supporting cells in organ of Corti and are suggested to play an important role in biochemical and mechanical modulation of outer hair cells. We successfully isolated functionally different $K^+$ currents from Deiters' cells of guinea pig using whole cell patch clamp technique. With high $K^+$ pipette solution, depolarizing step pulses activated strongly outward rectifying currents which were dose-dependently blocked by clofilium, a class III anti-arrhythmic $K^+$ channel blocker. The remaining outward current was transient in time course whereas the clofilium-sensitive outward current showed slow inactivation and delayed rectification. Addition of 5 mM tetraethylammonium (TEA) further blocked the remaining current leaving a very fast inactivating transient outward current. Therefore, at least three different types of $K^+$ current were identified in Deiters' cells, such as fast activating and fast inactivating current, fast activating slow inactivating current, and very fast inactivating transient outward current. Physiological role of them needs to be established.

Risk factors for poor prognosis in nosocomial infective endocarditis

Background/Aims: The aim of our study was to compare the characteristics of nosocomial infective endocarditis (NIE) with community-acquired infective endocarditis (CIE) and to determine independent risk factors for in-hospital death. Methods: We retrospectively reviewed the medical records of 560 patients diagnosed with infective endocarditis. NIE was defined by a diagnosis made > 72 hours after hospital admission or within 2 months of hospital discharge. Results: Among the 560 cases reviewed, 121 were classified as NIE. Compared with patients with CIE, patients with NIE were older (mean ± SD, 51.30±18.01 vs. 59.76±14.87, p < 0.001). The in-hospital death rate of the NIE group was much higher than that of the CIE group (27.3% vs. 5.9%, p < 0.001). More patients with NIE had central intravenous catheters, and were undergoing hemodialysis (p < 0.001). Methicillin-resistant Staphylococcus aureus (MRSA) was the most common causal microorganism of NIE, and MRSA (p < 0.001) and fungus (p = 0.002) were more common in NIE compared with CIE. On multiple analysis, age, liver cirrhosis, cancer chemotherapy, central intravenous catheter, hemodialysis, and genitourinary tract manipulation were independent clinical risk factors for NIE. Among the patients with NIE, 33 died during their hospital admission. The independent risk factors for in-hospital death were older age (adjusted odds ratio [OR], 1.04; 95% confidence interval [CI], 1.01 to 1.07; p = 0.037) and chemotherapy for malignancy (adjusted OR, 3.89; 95% CI, 1.18 to 12.87; p = 0.026). Conclusions: Because of the considerable incidence of NIE and its poor prognosis, we should pay attention to early diagnosis and active management of NIE, especially for older patients and patients receiving chemotherapy.
