        Background: The purpose of this study was to evaluate changes in the distance between the tip of endotracheal tube(ETT) and the carina by the pneumoperitoneum with Trendelenburg lithotomy position during lapaorscopic gynecologic surgery. Methods: We selected clinical patients who received laparoscopic gynecologic surgery on 20 degree of Trendelenburg lithotomy position under about 12㎝ H_2O of abdominal insufflation pressure. We checked a distance from the tip of ETT to the carina using a fiberoptic bronchoscope. Group 1 (n = 24) was measured the distance after the intubation and the pneumoperitoneum with trendelenburg lithotomy positions 10 minute later, respectively. Group 2 (n = 20) was measured the distance after intubation. And then measurements were taken in the pneumoperitoneum with trendelenburg lithotomy position 10 minute later after the positioning of the ETT fixed with 1 ㎝ shortly. Results: There was a significantly difference in the distance from the ETT to the carina within the Group 1. In group 2, the distance of between the tip of the ETT and carina was not altered significantly. Conclusion: The ETT has been shown to migrate towards the carina during laparoscopic gynecologic surgery. We suggest that the safety position for the ETT during laparoscopic gynecologic surgery should be shorter than fixation length of the ETT in supine position.

      • 정신분열병 입원 환자에서 단축형 증상관리 훈련의 효과:예비적 연구

        연구목적: 정신분열병 환자에서의 재활치료는 개인의 극복 기술과 능력을 향상시키고 재발을 유발시킬 수 있는 스트레스의 부정적인 영향을 중화시킴으로써 질병의 만성화를 막고 사회기능을 호전시킬 수 있기에 점차 그 중요성이 대두되고 있다. 본 연구는 만성 정신분열병 입원 환자에서 재활치료의 하나인 단축형 증상관리 훈련이 정신장애에 대한 자기 인식에 미치는 효능을 알아보기 위하여 시행되었다. 방법: 정신분열병으로 진단받고 입원 중인 환자들에게 재발경고징후를 규명하는 것, 재발경고징후를 관리하는 것, 지속증상을 극복하는 것, 술과 불법 약물을 피하는 것 등으로 구성된 승인-UCLA 사회기술 및 독립적인 생활기술훈련 단위 중 증상관리단위를 기초로 한국실정에 맞게 축약한 증산관리 훈련을 주 1회, 60분씩, 4∼5주간 실시하였고 실시 전후에 전신장애 비인식 평가 척도(Scale to Assess Unawareness ofMental Disorder; 이하 SUMD)로 자신의 정신장애에 대한 인식을 평가하였으며 임상적 전반적 인상(Clinical Global Impression;이하 CGI)로 임상 상태를 조사하였고 각 인구학적 변인들을 조사하였다. 결과: 환자 30명이 훈련을 완수하였으며 이들의 평균 연령은 36.6±7.6세였고 남자가 14명(46.7%), 여자가16명(53.3%)이었다. SUMD 점수는 훈련 전 16.9±4.0점에서 훈련 후 12.9±4.2점으로 유의하게 줄어 정신장애에 대한 자기 인식 정도가 향상되었으며(p<0.01), CGI도 훈련전의 3.7±0.9점에서 훈련 후 2.8±0.8점으로 유의하게 호전되었다(p<0.01). 환자의 연령, 발병시 연령, 유병 기간, 입원 기간, 항정신병약물 사용 용량, 교육 정도, CGI로 평가한 임상 상태등은 정신장애의 자기 인식 정도와는 관련성이 없었다. 결론: 정신분열병 입원 환자에서 단축형 증상관리 훈련은 정신장애에 대한 자기 인식을 증가시킬 수 있는 방법이 될 가능성을 확인하였으며 이 방법이 정신분열병 재활치료의 중요한 요소가 될 수 있음을 알 수 있었다. Objectives : The purpose of this study was to examine whether Symptom Management Module-Korean Brief Version(SMM-B) is effective as a psychosocial treatment of the inpat-ients with chronic schizophrenia. We also questioned which of demographic and clinical characteristics could be predictors of differential treatment response. Methods : The subjects were composed of 30 inpatients with chronic schizophrenia, who were trained with weekly session of SMM-B for 5 weeks. Before and after the training, the level of psychopathology, knowledge about schizophrenia were assessed as dependent variables by using Clinical Global Impression(CGI) and Scale to Assess Unawareness of Mental Disorder(SUMD). Results : Overall, patients showed improvement over the treatment period on psychopath-ology as well as knowledge about psychosis. Total scores of SUMD were changed from 16.9±4.0 before training to 12.9±4.2 after training(p<0.01) and scores of CGI were improved from 3.7±0.9 to 2.8±0.8<p<0.01). Howver age, gender, years of education, age of onset, duration of illness, duration of admission, numbers of psychiatric admission, and clinical status evaluated by CGI were not correlated with unawareness of psychosis. Conclusion : These results suggest that SMM-B could be a reliable method to improve self-aw-areness of psychosis in patients whth chronic schizophrenia and one of important elements in rehabilitation.

        These studies were carried out to investigate the effect of Radix Morindae water extract on the inhibitory contractile action of histamine in guinea pig. The results of these studies were as follows; 1. Contractile force of histamine from trachea smooth muscle in guinea pig was inhibited by Radix Morindae. 2. Radix Morindae did not changed the contractile force of histamine pretreated propranolol, indomethacin, and methylene blue. 3. Contractile force of uterus in rat was inhibited by Radix Morindae. 4. Contractile frequence of uterus in rat was inhibited by Radix Morindae.

      • 기혼여성의 신용카드사용과 제품구매에 관한 연구

        This study is concerned with the credit card and the buying behavior of the urban females. The samples are composed of 750 female consumers filtered by the purposive sampling. In order to achieve study objects, the statistics used for data analysis are the frequency distribution and the chi-square analysis. The buying behaviors of the urban females showed the significant differences between the social demographic variables(age, education, the numbers of children, the residence type and the family type). The buying behaviors of the urban females showed the significant differences between the economic variables(family income, savings and occupation of females). The buying behaviors of the urban females showed the significant differences between the credit card related variables(the number of credit card usage, the spend money of credit card usage, the place of consumption) Findings of this study suggest the females have concerns of the credit card. They already have consumed in the use of the credit card. In order to improve the economic and the social wellbeing of the urban females, any type of consumers should have an opportunity to improve their knowledge and the information about the credit card.

      • 결핵균 단세포군항체에 의한 항산균항원의 면역화학적 분석에 관한 연구

        β-antigen is one of the major protein of Mycobacterium tuberculosis. We purified reference β-antigen from the unheated culture filtrate of Mycobacterium tuberculosis by serial ammonium sulfate precipitation, gel filtration on Sephacryl S-200 column and preparative polyacryamide gel electrophoresis. Nine monoclonal antibodies against β-antigen were produced by hybridoma technique. The nine monoclonal antibodies were divided into two groups according to their reaction patterns on Western blots. Four monoclonal antibodies(group Ⅰ) reacted with two major bands of 83 and 52 kDa, and two minor bands of 110 and 56 kDa molecules while the other five monoclonal antibodies(group Ⅱ) reacted with a major band of 83 kDa and a minor and of 110 kDa molecule. The interspecies reactivities of nine monoclonal antibodies on the sonicated antigens from 21 species of mycobacteria were also examined by dot blots. Two monoclonal antibodies, designated 4G5E10(group Ⅰ) and 5F3F2(group Ⅱ), showed a specificity restricted to Mycobacterium tuberculosis complex. β-antigien from the cell extract of Mycobacterium tuberculosis was purified by affinity chromatography with 5F3F2 monoclonal antibody and checked its molecular characterization. Affinity purified β-antigen showed two distinct bands of 83 and 52 kDa molecules on SDS-PAGE. These results indicating that β-antigen is a associated protein with molecular weights of 83 and 52 kDa, and contains at least two different specific epitopes for Mycobacterium tuberculosis complex on β- antigen molecules. Affinity purified and reference β- antigens were applied for its potential diagnostic utility for determination of LgG antibody in the cerebrospinal fluids and sera of tuberculosis patients by ELISA. The mean ELISA values of IgG antibody activities to affinity purified β-antigen at the CSF dilution of 1 : 5 were 0.866±0.607 in 12 patients with tuberculous meningitis and 0.058±0.040 in 24 controls(P<0.001). The mean ELISA values of IgG antibody activities to reference β-antigen at the serum dilution of 1 : 100 were 0.861±0.350 in 97 patients with minimal tuberculosis, 1.052±0.443 in 51 patients with moderately advanced, 1.381±0.396 in 49 patients with far advanced and 0.454±0.248 in 138 controls(P<0.01). An indirect sandwich ELISA using specific biotinylated 5F3F2 monoclonal antibody as tracer was designed to detect β-antigen in the culture filtrate of Mycobacterium tuberculosis and sputum samples from patients with pulmonary tuberculosis. The detection limit of indirect sandwich ELISA was 0.1㎍ culture filtrate/㎖. The mean ELISA values of β-antigen reactivities in sputum samples from 52 patients with pulmonary tuberculosis and 25 controls were 0.766±0.404 and 0.453±0.133(not significant).

      • THE PARTICLE SIZE EFFECT ON COMBUSTION BEHAVIOR OF CELLULOSE INSULATION

        The combustion of cellulose insulation treated with Borax, Boric acid and Aluminum Sulfate as combustion retardants is examined by candle type combustion tester. The cellulose fibers in cellulose insulation are classified by diameter as less than 0.2mm, 0.2mm-0.5mm, 0.5mm-2mm and more than 2mm. The burning behavior of cellulose insulation are studied by LOI (Limit Oxygen Index: Beginning point of smoldering), L- point (Lower point of combustion transition from smoldering- flaming to flaming combustion), LOI, L-point and H-point rise with the increasing particle size of cellulose fibers because thermal decomposition rate of cellulose fiber decreases. The phenomena of combustion transition from smoldering to flaming combustion are determined by the generating rate of combustible gas and the formation rate of combustible gas mixture within the zone of cellulose fiber heated.

      • 면역친화 크로마토그라피법에 의한 결핵균 알파항원의 정제와 폐결핵 환자에서 결핵균 항체의 측정에 관한 연구

        In order to purified alpha antigen, one of the major protein antigens of Mycobacterium tuberculosis, anti-α serum was obtained from rabbit immunized with alpha-antigen purified from the unheated culture filtrate of M. tuberculosis by ammonium sulfate precipitation, hydroxylapatite chromatography, gel filtration and preparative natural-PAGE. From the culture filtrate concentrated by ultrafiltration, alpha antigen was purified by affinity chromatography using the anti-alpha serum and checked its molecular characterization by SDS-PAGE. For the determination of antibody activities to affinity purified alpha antigen in the IgG, IgM and IgA class in sera from patients with pulmonary tuberculosis, ELISA was performed on 48 patients with pulmonary tuberculosis and 102 healthy controls. The results were summarized as follows: 1. The immunoaffinity purification resulted in recovery of 35% the alpha antigen and achieved 4.9-fold purification in Ouchterlony immunodiffusion titer. The purified alpha antigen showed a distinct alpha antigen band of 30 KDa molecule and another contaminated band of 32 KDa molecule on SDS-PAGE. 2. The mean ELISA values of antibody activites to affinity purified alpha antigen in sera of 13 patients with minimal tuberculosis, 26 patients with moderately advanced, 9 patients with far advanced and 102 controls were the following:(1) the IgG activity of each groups were 0.7514±0.3934, 0.7163±0.3452, 1.0621±0.4301 and 0.3138±0.1068, respectively, (2) the IgM ac-tivity were 0.5410±0.2375, 0.6083±0.2998, 0.6656±0.3442 and 0.4981±0.2118(not signifi-cant), respectively, (3) the IgA activity were 0.1210±0.0691, 0.1685±0.1396, 0.2681±0.1544 and 0.0716±0.0860. The significant difference were observed in the value of IgG and IgA an-tibody. 3. When antibody activities of AFB positive group and AFB negative were compared to those of control, the significant difference were observed in the value of IgG and IgA antibody but was not observed in value of IgM antibody. 4. According to the F value of analysis of variance, the activity level IgG antibody was more significant difference than that of IgA antibody.

        Inhibitory Effect of Clavicepitaceae on Serotonin Release out of Human Platelets and Human Platelet Aggregation

        We have investigated the effects of hypha-water extracts (HWE), fruit body-water extracts (FWE) and cordycepin from Cordyceps militaris on serotonin release out of human platelets and human platelet aggregation. HWE and FWE inhibited the release of [^(3)H]-serotonin from human platelet stimulated by thrombin (2 U/ml) or collagen (20 ㎍/ml) in a dose-dependent manner. Furthermore, cordycepin, a major component of Cordyceps militaris, inhibited the human platelet aggregation induced by collagen (10 ㎍/ml) in a dose-dependent manner. These results suggest that cordycepin containing in HWE and FWE may inhibit the serotonin release by suppressing the collagen-induced human platelet aggregation. Accordingly, our data demonstrate that HWE and FWE containing much cordycepin might have antithrombotic and antimigrainous functions.

        Inhibitory Effect of Cordycepin on Human Platelet Aggregation

        Cordycepin separated from Cordyceps militaris is a major physiologic active component in Cordyceps militaris. The platelet aggregation is stimulated by Ca^(2+), which is either mobilized from intracellular endoplasmic reticulum or transported from extracellular space. cGMP antagonizes the actions of Ca^(2+). Based on these facts, we have investigated the effects of cordycepin on the mobilization of Ca^(2+) and the production of cGMP on collagen (10 ㎍/ml)-induced human platelet aggregation. Cordycepin potently stimulated the human platelet aggregation induced by collagen (10 ㎍/ml) in a dose-dependent manner. Cordycepin (500 μM) inhibited also the collagen-induced human platelet aggregation in the presence both 1 mM and 2 mM of CaCl_(2). These are in accord with the results that cordycepin inhibited the Ca^(2+)-influx on collagen-induced human platelet aggregation. These results suggest that cordycepin decrease the intracellular Ca^(2+) concentration to inhibit collagen-induced human platelet aggregation. Besides, cordycepin increased the level of cGMP on collagen-induced human platelet aggregation. This result is related with the decrease of intracellular Ca^(2+) concentration, because cGMP inhibits the mobilization of Ca^(2+). In addition, cordycepin inhibited the human platelet aggregation induced by LY-83583, inhibitor of guanylate cyclase. This result suggested that cordycepin inhibit the platelet aggregation by stimulating the activity of guanylate cyclase. In conclusion, we demonstrated that cordycepin might have the antiplatelet function by inhibiting Ca^(2+)-mobilization via the stimulation of the production of cGMP.

        The various clinical features of tuberculosis are mediated by diverse cytokines produced by various immune cells which are initially triggered by M. tuberculosis antigens. CD4+ T cells can be classified into two subsets according to the patterns of cytokines they produce; Thl cells give rise to cell-mediated immunity and are characterized by the production of IL-2 and IFN-y, whereas Th2 cells are more efficient in mediating antibody production and secrete II-4, IL-5, IL-6 and II-10, Thl cells can control Th2 cell and vice versa. Peripheral blood mononuclear cells of healthy, cured and chronic refractory tuberculosis patients were stimulated with PPD, TSP and PHA antigen, and lymphoproliferative response and expression of II-2, IL- 10, TNF-α mRNAs were measured. Lymphoproliferative responses to PPD, TSP and PHA antigen were depressed in chronic refractory case compared with others expression of IL-2 mRNA was depressed in chronic refractory case stimulated with all antigens. Expression of IL-10 and TNF-α were depressed in cured and chronic refractory cases stimulated with PPD and TSP antigens.
