Presurgical Naso-Alveolar Molding Appliance for Unilateral Cleft Lip and Palate

본 연구의 목적은 1)편측성 순구개열 신생아 환자에 대한 술전 비치조 정형장치(PNAM)치료를 소개하고 2) 이 장치의 치조골 정형 효과를 평가하는 것이다. 서울대학교 병원 교정과에 내원하여 PNAM장치를 사용하여 치료받은 4명의 편측성 순구개열 호나자(남자 3명, 여자 1명, 평균 초진연령: 23.2일)을 대상으로 하였으며, 치조골에서 대분절과 소분절 파열부간의 평균거리는 8.27mm였고 치조골 정형치료의 평균기간은 9.7주였다. 초진시(Ti)와 성공적인 치조골 정형술이 이루어진 시기(T2)에 이들 환자의 인상을 채득하여 모형을 제작한 후, 사진촬영과 digital caliper를 사용하여 7개의 길이 항목과 5개의 각도항목을 계측하였다. SPSS win. ver 7.5프로그램을 사용하여 통계처리 하였고, 평균치의 차이를 비교하기 위하여 paired t-test를 사용하였다. 1. 신생아 치조골 후방부는 치조골 정형술을 시행하는 동안 안정된 구조물이었다. 2. 치조골 정형술에 의하여 대분절의 전방 성장이 억제되었다. 3. 치조골 정형술에 의한 파열부의 패쇄는 주로 대분절 전방부의 내측과 후방 굴곡 및 소분절 전체의 외측 굴곡에 의한 것이었다. The goals of this study were to present presurgical naso-alveolar molding(PNAM) appliance in unilaterla cleft lip and palate treatment and to evaluate the effects of PNAM appliance on alveolar molding. Samples were consisted of 4 unilateral cleft lip and palate infants(3 males and 1 female, mean age=23.2 days after birth) who were treated with PNAM appliances in Department of Orthodontics, Seoul National University Dental Hospital. Average alveolar cleft gap between the greater and lesser segment was 8.27 mm and average duration of alveolar molding treatment was 9.7 weeks. These patients' models were obtained at initial visit(T1) and after successful alveolar molding(T2). Seven linear and five angular variables were measured by using photometry and digital caliper. All statistical analyses were performed by SPSS win ver. 7.5 program. Paired t-test was used to compare the mean values. 1. The posterior part of alveolar segments are the stable structures during alveolar molding treatment period in infants. 2. Forward growth of th greater segment may be hindered by the action of alveolar molding. 3. The closure of cleft gap during alveolar molding were usually due to inward and backward bending of the anterior part of the greater segment and outward bending of the whole lesser segment.
In vivo alternative testing with zebrafish in ecotoxicology

Although rodents have previously been used in ecotoxicological studies, they are expensive, time-consuming, and are limited by strict legal restrictions. The present study used a zebrafish (Danio rerio) model and generated data that was useful for extrapolating toxicant effects in this system to that of humans. Here we treated embryos of the naive-type as well as a transiently transfected zebrafish liver cell line carrying a plasmid (phAhREEGFP), for comparing toxicity levels with the well-known aryl hydrocarbon receptor (AhR)-binding toxicants: 3,3’,4,4’,5-pentachlorobiphenyl (PCB126), 2,3,7,8-tetrachlorodibenzo- p-dioxin, and 3-methylcholanthrene. These toxicants induced a concentration-dependent increase in morphological disruption, indicating toxicity at early life-stages. The transient transgenic zebrafish liver cell line was sensitive enough to these toxicants to express the CYP1A1 regulated enhanced green fluorescent protein. The findings of this study demonstrated that the zebrafish in vivo model might allow for extremely rapid and reproducible toxicological profiling of early life-stage embryo development. We have also shown that the transient transgenic zebrafish liver cell line can be used for research on AhR mechanism studies.

We first established a transgenic zebrafish line carrying a plasmid (phAhRE-EGFP) for detecting developmental toxicities caused by aryl hydrocarbon receptor (AhR) binding toxicants. The plasmid contains the 5` human cytochrome P4501A1 (CYP1A1) upstream sequence, which AhR ligands target, and enhanced green fluorescent protein (EGFP) gene, an accurate and quick, inducible reporter. To evaluate this system, we treated embryos of wild-type and the transgenic fish with four well-known AhR binding toxicants: 3,3`,4,4`,5-pentachlorobiphenyl (PCB126), 2,3,7,8-tetra-chlorodibenzo-p-dioxin (TCDD), 3-methylcholanthrene (3-MC), and β-naphthoflavone (β-NF). These toxicants induced a dose-dependent increase in morphologic disruption, indicating toxicity at early life-stages. The transgenic zebrafish were sensitive enough to these toxicants to express the CYP1A1 regulated EGFP. Moreover, fluorescence-based quantification of the reporter gene was consistent with dose dependent toxicity of the four model toxicants. In conclusion, the stable transgenic line will allow for extremely rapid and reproducible toxicological profiling of early life-stage embryo development in both aquatic organisms and humans.
The Clinical Features of Korean Patients with Duane`s Retraction Syndrome

Purpose: To describe the clinical features of Duane`s retraction syndrome (DRS) in Korean patients. Methods: We retrospectively analyzed the 78 DRS cases that presented to our department between 1995 and 2004. The clinical features investigated included sex distribution, laterality, type of presentation, deviation in primary position, anomalous vertical movements, face turn, amblyopia and anisometropia. Results: There were 38 (48.7%) affected males and 40 (51.3%) females. Left eye predominance (83.3%) was observed, as was typeⅠ presentation (85.9%). Orthotropia was found to be the most common primary position in 46 cases (59.0%). Face turn in unilateral DRS was noted in 13 patients (17.1%). There were 6 cases (7.7%) with anisometropia and 4 (5.1%) with amblyopia. Conclusions: The clinical manifestations of DRS in our study were different from those of equivalent Caucasian studies yet similar to those previously reported for Asian groups. Racial and regional differences were noted, for which further research is needed to elaborate the reasons and mechanisms.

Scalp hair loss has increased recently in young-aged people as well as middle-aged people, and although there are no serious in health problems, scalp hair loss may significantly affect a variety of psychological and social experiences and the individual's quality of life. DREAMMO scalp lotion is composed of several plant extracts which are known to be used as oriental medicine to prevent hair loss or promote hair growth in alopecia. This study was carried out to investigate the effect of DREAMMO scalp lotion on hair growth in an alopecia model of C57BL/6 mice. Mice were divided into three experimental groups including control, DREAMMO scalp lotion and 3% Minoxidil (MXD). The test compounds were topically treated with 0.2 ㎖ per mouse per day for 4 weeks. The hair growth was determined photographically and histologically in an alopecia model of C57BL/6 mice. After 3-4 weeks, DREAMMO scalp lotion significantly increased hair growth compared to the controls in the dorsal skin of C57BL/6 mice. DREAMMO scalp lotion also promoted the elongation of hair follicles than the controls. These results suggest that DREAMMO scalp lotion has a hair growth activity and can be useful for the treatment of alopecia.

<P>Cinobufagin (CBG) is a cardiotoxic bufanolide steroid secreted by the skin and parotid venom glands of the Asiatic toad <I>Bufo bufo gargarizans</I> (called Chan-Su). Although CBG is known to exhibit anti-cancer activities, very little is known about its potential mechanism(s) of action. In this study, we investigated whether CBG mediates its effect through the modulation of the mitogen-activated protein kinases (MAPKs) signaling pathway in human multiple myeloma (MM) U266 cells. We found that CBG caused the significant activation of ERK, JNK and p38 MAPK in U266 cells. CBG showed much higher cytotoxicity against U266 cells as compared to peripheral blood mononuclear cells (PBMC). Induction of CBG increased reactive oxygen species (ROS) generation from mitochondria, which is associated with the induction of apoptosis as characterized by increased sub-G1 DNA contents of cell cycle, positive Annexin V binding, activation of caspase-3 and cleavage of PARP. Inhibition of ROS generation by <I>N</I>-acetyl-<SMALL>L</SMALL>-cysteine (NAC) significantly prevented CBG-induced ERK, JNK and p38 MAPK activation and apoptosis. CBG also down-regulated the expression of various downstream gene products that mediate cell proliferation, survival, angiogenesis and metastasis. Interestingly, ERK, JNK and p38MAPK pharmacological inhibitors blocked CBG-induced MAPKs activation and ERK inhibitor (PD98059) also prevented the CBG-induced caspase-3 activation and PARP cleavage in U266 cells. Taken together, these findings suggest that CBG can act as a potent anticancer agent against MM and possibly exerts its effects through the ROS-mediated activation of ERK, JNK and p38 MAPK leading to the activation of caspase-3 in U266 cells.</P>

Background: To investigate whether pretreatment serum carbohydrate antigen 19-9 (CA 19-9) levels are associated with pathological responses to preoperative chemoradiotherapy (CRT) in patients with rectal cancer. Methods: In total, 260 patients with locally advanced rectal cancer (cT3-4NanyM0) who underwent preoperative CRT and radical surgery were analyzed retrospectively. CRT consisted of 50.4 Gy pelvic radiotherapy and concurrent chemotherapy. Radical surgery was performed at median 7 weeks after CRT completion. Pathological CRT response criteria included downstaging (ypStage 0-I) and ypT0-1. A discrimination threshold of CA 19-9 level was determined using a receiver operating characteristicsanalysis. Results: The median CA 19-9 level was 8.0 (1.0-648.0) U/mL. Downstaging occurred in 94 (36.2%) patients and ypT0-1 in 50 (19.2%). The calculated optimal threshold of CA 19-9 level was 10.2 U/mL for downstaging and 9.0 U/mL for ypT0-1. On multivariate analysis, CA 19-9 (= 9.0 U/mL) was signifi cantly associated with downstaging (odds ratio, 2.089; 95% confi dence interval, 1.189-3.669; P = 0.010) or ypT0-1 (OR, 2.207; 95% CI, 1.079-4.512; P = 0.030), independent of clinical stage or carcinoembryonic antigen. Conclusions: This study fi rstly showed a signifi cant association of pretreatment serum CA 19-9 levels with pathological CRT responses of rectal cancer. The CA 19-9 levels are suggested to be valuable in predicting CRT responses of rectal cancer before treatment.
