Juvenile polyp, also known as a retention polyp because they contain mucin cysts, is a relatively common hamartomatous lesion in chlidhood. It occurs in 1-2% of children and young adults; however it is uncommon in adulthood. Juvenile polyp is rare in the first year of life and is thus presumed to be acquired and not congenital. It is pedunculated, 2-3 cm in size, red-tan in colour, smooth and always shows fluid filled cystic spaces on cut surface. Histologically, Juvenile polyps are hamartomatous with distended, mucus-filled glands, often with cystic dilatation and edematous lamina propria containing abundant vasculature. The most common manifestation is rectal bleeding produced by auto-amputation of the polyp. Juvenile polyp has been considered not to be at increased risk of developing carcinoma. However, atypical epithelium is observed in some lesions, especially in larger polyps, which may give rise to adenoma and subsequent malignant transformation. Therefore the diagnosis of solitary juvenile polyp should not be made only with colonoscopy and biopsy, but should be totally resected endoscopically for pathologic evaluation. About 90% of the lesions are solitary and localized within 20cm from anal verge. In Korea, there are some reports of juvenile polyps located in the rectosigmoid colon. We report one case of juvenile polyp in adolescent that presented as a hematochezia & abdominal pain, which is located at right colon.

We have designed an embedded controller for the control of a diesel generator using an embedded system. The generator is monitored and controlled remotely via the internet in real-time. The proposed digital controller is designed to handle precisely the distortions and noises of the signals emanating from the diesel generator, and enables abnormal operation of the diesel generator to be notified to the remote manager using the Short Message Service (SMS) of the Internet, which enables the appropriate personnel to take action by remote control according to the incoming messages.

다단계 상호연결 네트워크는 다중 처리기 시스템에서 프로세서들 상호간 혹은 메모리와의 통신기능을 제공해 준다. 이들 네트워크에서의 source와 destination사이의 연결 형태는 일반적으로 스위칭 소자의 제어 특성과 상호연결함수에 의해 결정된다. 본 논문에서는 다단계 상호연결 네트워의 일종인 베이스라인를 대상으로, 임의의 source와 destination사이에 존재할 수 있는 다양한 연결을 제공하기 위하여 분산제어 방식을 도입한 새로운 라우팅 방법을 제안하였으며, 또한 이것을 시뮬레이션 시스템으로 구현하여 그 과정 및 결과를 보였다.

창해역사 신화의 문화적 기억

이 글은 강릉지역에 전승되는 창해역사 신화의 전승 맥락을 파악하고 그 속에 함축되어 있는 상징을 풀어 지역 문화와의 연관성을 살피고 있다, 창해역사 신화는 두 가지 이야기를 가졌다. 하나는 냇가로 떠내려 온 알에서 태어난 얼굴이 검고 기골이 장대한 용사가 호랑이를 죽이고 큰 종을 옮기는 등 괴력을 보여 임금의 상객이 되었다는 영웅담이고, 또 다른 이야기는 장량과 함께 박랑사에서 진시황 암살을 시도한 용사의 무용담이다. 신화는 예국穢의 영웅담을 중심으로 전승되었으나 『사기』의 「유후세가」에 기록된 ‘창해 용사’ 등등의 모티브와 연결되면서 중국 진시황 암살시도를 삽입, 강조되어 구전되고 있다. 창해역사는 부래浮來와 난생卵生이라는 범상치 않은 탄생설화를 가진 인물이다, 그는 호랑이를 죽였고, 임금을 위해 무거운 종을 옮겼다. 예국穢의 신 神 호랑이의 죽음은 곧 예국穢의 멸망을 의미하며 종은 불교의 상징, 새로운 문명을 뜻하는 것으로 이해된다. 그리고 종을 설치하는 임금의 존재는 신진세력이 강릉지역을 복속했다는 의미로 해석할 수 있다. 이는 곧 예국穢의 제천의례 무천이 막을 내리고 단오를 중요시 했던 신라의 국행제가 자리매김한다는 개연성을 인정할 수 있다. 따라서 강릉의 공동체의례가 10월 무천에서 5월 단오제로 이동하는 단서가 될 수 있다. This article examines the connection with local culture by gasping the context of the transmission of the legend of the history of Changhaeyeoksa, which is passed down in the Gangneung area, and solving the symbols implied symbols. The myth of Changhaeyeoksa history had two stories. One is a hero’s story that a black-faced warrior born from and egg floating down the stream killed a tiger and moved a large bell, and the other is a story of a warrior who attempted to assassinate Qin Shi Huang at Bakrangsa Temple along with Jangryang. The myth was handed down around the hero story of Yeguk, but as motifs such as “Changhae and Yongsa” recorded in “Yufu Sega” of “Sagi” were connected, attempts to assassinate Qin Shi Huang of China were inserted and emphasized. Changhaeyeoksa History is a character with an extraordinary birth story of Burae and Nansaeng. He jilled a tiger and moved a heavy bell for the king. It is understood that the death of tiger, the god of ‘ye’, means the destruction of ‘ye’, and bell means the acceptance of a new civilization, a symbol of Buddhism. In addition, the existence of the king who installs the bell can be interpreted as meaning that the new forced inherited the Gangneung area. This can be acknowledged as the probability that the ritual of Jecheon in ‘Ye’ ended and the national rite of Silla, which valued Dano, was established. Therefore, Gangneung’s community ritual can be a clue to the move from Muchean in October to Danoje in May.

Internal Fixation Using Clavicle Hook Plates for Distal Clavicle Fractures

Background: To report the radiological and clinical outcomes of internal fixation using distal clavicle hook plates for distal clavicle fractures. Methods: From April 2008 to December 2012, 32 patients with distal clavicle fractures underwent surgery using an AO hook plate. The reduction was qualified and evaluated according to the radiological findings. The evaluation of the clinical outcomes was performed with the University of California at Los Angeles (UCLA) score, the Korean Shoulder score, and the visual analogue scale (VAS) pain score. Results: By radiological evaluation, we found that 31 of 32 patients showed anatomical reduction and solid bone union. Although we obtained satisfactory UCLA scores, Korean Shoulder Scale scores, and VAS pain scores, 12 cases of complications were present. We found 4 cases of osteolysis of the acromion, 1 case of nonunion, 3 cases of periprosthetic fractures, 3 cases of subacromial pain, and 1 case of skin irritation. We performed re-operations in 2 patients. Conclusions: To avoid complications associated with clavicle hook plates, choosing the appropriate hook size and bending of the hook according to the slope of the acromion undersurface is critical. Also, we believe that early removal of clavicle plates may help reduce complications.

In Korean Women, High Plasma Level of Enterolactone Reduces Risk of Type 2 Diabetes and This Effect is Enhanced in Equol-Producers

Diabetes, if left untreated, can cause many health complications. Type 2 diabetes is known to be primarily due to lifestyle and thus, efforts have been made to reduce its risk by diet particularly of vegetables containing various antioxidants. In the present study, this possibility was investigated by examining association between the plasma level of enterolactone, a metabolite of food lignans and the risk of type 2 diabetes and the association was compared between producers and non-producers of equol, a metabolite of soy isoflavone. The study population was composed of 693 cases and 698 matched controls within the Korean Genome Epidemiology Study. Logistic regression models were used to compute the odds ratio (OR) and 95% confidence intervals (CI). Stratified analysis was done to examine the association between plasma enterolactone concentration and risk of type 2 diabetes, and then, this association was examined in terms of sex and status of equol producing ability. In female, compared with the reference level, a marginally significant risk reduction was found at the highest concentration of enterolactone (for the highest category: OR, 0.68; 95% CI, 0.46–1.01). In equol producers among women, the highest concentration of enterolactone significantly decreased the risk of type 2 diabetes (OR, 0.58; 95% CI, 0.34–0.96). In equol non-producers among women, however, the concentration of enterolactone was not associated with the risk of type 2 diabetes. In male, enterolactone concentration was not associated with risk of type 2 diabetes regardless of equol producing status. In conclusion, high plasma concentrations of enterolactone were associated with a decreased risk of type 2 diabetes particularly, in equol producing female.

Immunotoxicological Effects of Aripiprazole: <em>In vivo</em> and <em>In vitro</em> Studies

Aripiprazole (ARI) is a commonly prescribed medication used to treat schizophrenia and bipolar disorder. To date, there have been no studies regarding the molecular pathological and immunotoxicological profiling of aripiprazole. Thus, in the present study, we prepared two different formulas of aripiprazole [Free base crystal of aripiprazole (ARPGCB) and cocrystal of aripiprazole (GCB3004)], and explored their effects on the patterns of survival and apoptosis-regulatory proteins under acute toxicity and cytotoxicity test conditions. Furthermore, we also evaluated the modulatory activity of the different formulations on the immunological responses in macrophages primed by various stimulators such as lipopolysaccharide (LPS), pam3CSK, and poly(I:C) via toll-like receptor 4 (TLR4), TLR2, and TLR3 pathways, respectively. In liver, both ARPGCB and GCB3004 produced similar toxicity profiles. In particular, these two formulas exhibited similar phospho-protein profiling of p65/nuclear factor (NF)-κB, c-Jun/activator protein (AP)-1, ERK, JNK, p38, caspase 3, and bcl-2 in brain. In contrast, the patterns of these phospho-proteins were variable in other tissues. Moreover, these two formulas did not exhibit any cytotoxicity in C6 glioma cells. Finally, the two formulations at available <em>in vivo</em> concentrations did not block nitric oxide (NO) production from activated macrophage-like RAW264.7 cells stimulated with LPS, pam3CSK, or poly(I:C), nor did they alter the morphological changes of the activated macrophages. Taken together, our present work, as a comparative study of two different formulas of aripiprazole, suggests that these two formulas can be used to achieve similar functional activation of brain proteins related to cell survival and apoptosis and immunotoxicological activities of macrophages.

N/A Researches on the characteristics of fatality-prone asthmatics have been performed in western countries, but there are few reports in this field in Korea. The purposes of this study were to clarify the magnitude of the problem and to identify the risk factors of near-fatal asthma(NFA). Methods : The records of patients admitted due to asthma attack were analyzed retrospectively. Results :1) The subjects had NFA in 50.6% and PaCO2 > 45mmHg in 22.9%. Five(11.1%) among NFA patients and 3 out of 19(15.8%) subjects with PaCO2 > 45mmHg required mechanical ventilation. 2) There were no significant differences between NFA and non-NFA in age, sex, resident place, academic career, familial and personal history of atopic diseases, serum total IgE level, positive skin prick test to house dust mites, accompanying allergic rhinitis and aspirin intolerance, emphysema on chest high resolution computerized tomogram, dyspnea perception, previous asthma education, regular OPD follow-up, and use of antiinflammatory drugs. 3) However, cigarette smoking(62.2% vs 38.6%), accompanying paranasal sinusitis(66.7% vs 45.3%), chronic asthma severity(severe persistent: 55.5% vs 29.5%), hospitalization frequency(2.93 vs 1.58), and duration of recent asthma exacerbation(6.6 vs 18.8 days) in NFA were significantly different from those in non-NFA.4) The relative risk for NFA was high in patients with history of hospitalization > 3, severe persistent asthma, exacerbation period < 3 days, smoking, or sinusitis in the order of frequency. Conclusion : Near-fatal asthma is a prevalent problem in clinical practice and asthma admission history, severe persistent asthma, short exacerbation period < 3 days, smoking, and paranasal sinusitis are the risk factors warning near-fatal asthma attack in advance.
