Purpose : Causes of acute lower respiratory tract infections(ALRI) in children are diverse. But virus is the most common cause of ALRI, so it is important to under- stand the etiology and epidemiology of ALRI. This study was performed to investigate the etiologic organisms, age distribution, clinical manifestations and seasonal occurrence of ALRI in hospitalized children. Methods : We confirmed viral etiologies using nasopharyngeal aspirates in 377 patients of the ages of 15 years or younger who were hospitalized for ALRI from March, 1996 to February, 1999 at Samsung Seoul Hospital, Seoul, Korea. Viral agents were detected by virus isolation and antigen detection by indirect immunofluorescent staining. Results: The viral pathogens identified `were influenza A(22.2%), influenza B(9.0%), adenovirus(21.2%), parainfluenza virus type 1(8.8%), type 2(3.4%), type 3(15.1%) and respiratory syncytial virus(RSV)(20.2%). The occurrence of ALRIs was high under 2 year old. The clinical patterns of viral ALRI include pneumonia (49.9%), croup (20.2%), bronchiolitis(22.0%), tracheobronchitis(8.0%). The specific viruses are frequently associated with specific clinical syndrome of ALHI. The respiratory agents and associated syndromes frequently have characteristic seasonal patterns. Conclusion: This study will help us to estimate the etiologic agents of ALRI, and to avoid inappropriate antibiotic therapy. An annual nationwide survey is necessary to understand the viral epidemiology associated with respiratory illness.

This research paper aims firstly, to consolidate general negotiation theories into military negotiation theories with the military perspectives; secondly, to identify four determinants as independent variables affecting to enhance the negotiation power of military decision-makers; thirdly, to analyze a historical case successfully negotiated by military decision-makers throughout the history of Korea. Research results in this paper are as follows: First, General negotiation theories can be conceptualized as military negotiation theory. Secondly, We can identify four determinants to affect the negotiation power of military decision-makers. Thirdly, Understanding that human/international relations that form background of negotiation is a type of interaction, and that it is impossible to find appropriate solutions to a problem without appreciating its essence. An analysis on the shared ‘behavioral patterns’ of military decision- makers equipped with the power of successful negotiation through the learning and experiences. Fourthly, Analyzing the legacy of notable figures in Korean history and assaying the ‘behavioral patterns’ of military decision-makers with successful record of overcoming the tide of change and conflict would help us foster an ability to correct generally observable errors in negotiations. Effective military decision-makers had displayed professional expertise, touted national interest and survival as the great cause, and did not scruple to utilize whatever means and methods to achieve a win-win in negotiations of critical importance. Fifthly, The importance of‘learning and experience’through military curricula focused on our future negotiators with professional expertise. Thus Korean military authorities should establish the academic courses of negotiation theories and applications in the military curricula for teaching officers.

Liver transplantation is curative therapy for hepatocellular carcinoma especially if ,within Milan criteria, 4 year survival and recurrence-free survival was reported to be 85% and 92%, respectively. Herein we report a patient who experience rapid recurrence following living donor liver transplantation (LDLT) for hepatocellular carcinoma within Milan criteria. A 52 year-old-men patient with known liver cirrhosis associated with hepatitis B virus was admitted for the treatment of hepatocellular carcinoma (HCC). Abdominal CT revealed two nodules less than 3 cm in right hepatic lobe. After single session of transcatheter arterial chemoembolization (TACE), the patient underwent LDLT. After seven months following transplantation, recurrent HCC was detected on transplanted liver with concurrent metastatic nodule in lung. Although TACE and metastsectomy were performed for recurrent intrahepatic mass and lung metastasis, recurrent HCC showed rapid progression and patient died of progressive tumor after 10 months following LDLT.

The prognosis of young patients with hepatocellular carcinoma is remains controversial. Here we report a case of advanced hepatocellular carcinoma in twenty, successfully treated with transarterial chemolipidolization (TACL), systemic chemotherapy, radiation therapy and surgical resection. Previously healthy 28 years old woman was admitted for treatment of hepatocellular carcinoma. Abdominal CT showed a diffuse infiltrative HCC involving both lobes with intrahepatic bile duct invasion and pericardial lymphadenopathy. She was treated TAC with systemic chemotherapy and external beam radiotherapy. 6 months after these treatments, main tumor and the pericardial lymph node were decreased in size. And then extended left lobectomy and systemic chemotherapy were done. The pericardial lymph node was markedly decreased. The patient has been followed for 10 months without evidence of regional tumor recurrence.
차별 발현 역전사중합효소연쇄반응법을 이용한 원발 간세포암종의 유전자 발현 분석

목적: 원발 간세포암종에서 특이하게 과발현되는 특정유전자를 규명하는 것은 간세포암종 발암 기전의 연구나, 암의 조기진단 및 암 특이 유전자를 표적으로 한 유전자 치료에 매우 중요한 정보를 제공한다. 이에 이번 연구에서는 차별 발현 역전사중합효소연쇄반응법을 활용한 유전자 분석을 통해 간세포암에서 특이하게 과발현되는 특이 유전자를 밝혀내고 이 유전자의 발현율을 여러 시료에서 확인하여 간세포암 관련 유전자들의 기능을 탐색하고자 하였다. 대상 및 방법: 원발 간세포암종으로 수술을 시행 받았던 3명의 간세포암 환자의 간 적출물에서 간세포암종 조직과 암주변의 비 간세포암 조직 3쌍을 채취한 후 차별 발현 역전사중합효소연쇄반응법을 이용하여 간세포암종 조직에서 과발현되거나 소실된 유전자 혹은 저발현되어 있는 유전자를 겔로부터 DNA를 용출하여 이를 클로닝한 후 염기서열분석을 통하여 나타난 유전자의 서열을 유전자 검색프로그램에 넣어 그 유전자의 이름이나 특성들을 탐색하였다. 이후 새로 규명된 유전자의 기능을 검증하기 위하여 11쌍의 간세포암종 및 비간세포암종 조직과 인간 암세포주인 간세포암종 세포주[HepG2, Hep3B, Huh7], 자궁암 세포주[HeLa], 대장암 세포주[HT1299], 섬유육종 세포주[HT1080], 폐암 세포주[A549]에서 실시간 중합효소연쇄반응을 이용하여 이들 유전자의 발현을 확인한 후 분석하였다. 결과: 이번 연구를 통해서 단백 대사, 유비퀴틴 의존성 단백 대사, 탄수화물 대사, 지방대사, 유전자복구, 염증 반응 등의 다양한 기능을 가지는 21종의 유전자들을 탐색하였다. 결론: 이번 연구를 통하여 간세포암종 조직과 비간세포암종 조직에서 발현차이를 보이는 유전자를 탐색하였고, 이러한 유전자의 특성 규명은 간세포암종의 발암 기전을 규명하거나 조기 간세포암을 진단하는 바이오 마커의 개발 혹은 분자표적을 대상으로 하는 유전자 치료의 개발에 기초자료로 유용할 것으로 생각한다. Background/Aims: The investigation of a specific tumor marker for hepatocellular carcinoma (HCC) is needed to examine the carcinogenesis and to select the patients for treatment options. The aim of this study was to find the genes related to HCC. We also examined the expression level of these genes in cancer cell lines and tissue specimens. Methods: Three pairs of HCC tissue and non-neoplastic hepatic tissue around the HCC were collected from three patients who underwent resection for HCC. Differential display reverse transcriptase-PCR (DD RT-PCR) using GeneFishing(TM) PCR was used to detect the differences in the gene expression between in HCC tissue and non-neoplatic tissue. Up- or down-regulated genes in HCC tissue were identified through BLAST searches after cloning and sequencing assays. Real-time RT-PCR assay was employed to detect the expression rate in 11 HCC tissues and human cancer cell lines. Results: Differentially expressed 21 genes were identified, and they were classified as genes involved in protein metabolism, ubiquitin-dependent protein catabolism, carbohydrate metabolism, lipid metabolism, DNA repair, and inflammatory response. Conclusions: We identified differentially expressed genes in HCC, and these genes may play an important role in the study of hepatocarcino-genesis, development of biomarker, and target therapy for HCC.
