        해군 의무시설 근무자의 감염관리 표준주의지침 수행 영향요인

        Purpose: The purpose of this study is to identify the influencing factors on the practice of standard precautions for infection control among healthcare workers in naval and marine medical units. Methods: This study is a descriptive study where 151 physicians, dentists, registered nurses, nursing assistants, medical technicians, and uncertified assistants were sampled to respond to a self-reporting questionnaire from July to October 2020. Results: The knowledge score had a significant difference among the licensed healthcare professional group, unlicensed but certified health workers group, and uncertified assistants group (F=12.35, p<.001). The practice score was significantly different between men and women (t=-3.85, p<.001). The safety climate score was found to have a positive correlation with the practice score (r=.23, p=.004). The multiple regression analysis presented that the safety climate score influenced the score of Standard Precautions practice (ß=.36, p=.005). The model explained 34.3% of Standard Precautions practice (F=7.06, p<.001). Conclusion: It is important to build a safe climate to promote healthcare workers’ practice of standard precautions in naval and marine medical units.

        군 병원 간호관리자가 지각하는 간호관리역량의 중요도와 수행도

        Purpose: The purpose of this study is to examine the Republic of Korea's military nurse managers' perceived importance and performance of nursing managerial competencies and to measure the gap between them. Methods: With a questionnaire asking clinical nursing managerial competencies categorized into 42 competencies under 5 nursing management processes, the authors analyzed a total of 146 data collected from first-line, middle and top managers working in military hospitals. One-way ANOVA and Importance-Performance Analysis were mainly conducted to identify the current status of the managers' perceptions and the discrepancy between them. Results: The average perception scores on the importance and performance were 4.36 and 3.66, respectively. There were significant differences in the perceived importance and performance on nursing managerial competencies among the different manager groups. An IPA demonstrated that top managers needed to concentrate on developing competencies of vision-building, motivation, and performance management, middle managers on vision-building, human resources development, professionalism, and self-management, and first-line managers on change management, staffing management, human resources development, leadership, professionalism, motivation, and development of nursing standards. Conclusion: To strengthen the clinical nursing managerial competencies, education/training programs customized for each management group should be developed and delivered.

        국군간호사관생도의 여가제약, 스트레스 정도와 삶의 질

        Purpose: The purpose of this study is to understand the leisure restriction, stress, and quality of life of cadets in Korea Armed Forces Nursing Academy (KAFNA) and to analyze its correlation. Methods: This cross-sectional descriptive survey was conducted to 228 KAFNA cadets in August, 2018, using self-report questionnaires. Collected data were analyzed using t-test, ANOVA, Scheffé test, and Pearson correlation coefficient. Result: The average of leisure restriction was 2.78±0.48, stress was 2.67±0.55, and the quality of life was 3.72±0.44. Leisure restriction and the quality of life as well as stress and the quality of life showed negative correlations. In contrast, leisure restriction and stress showed a significantly positive correlation. Conclusion: The results indicate that it is in need to develop various programs to alleviate leisure restriction of KAFNA cadets considering quality of life and stress.

        항당뇨 물질 Aloe QDM complex의 세포내 포도당 흡수촉진 효능

        Previous studies have shown that Aloe QDM complex, which is consisted of chromium (Cr), aloesin (ALS) and processed Aloe vera gel (PAG), exert antidiabetic activity in a high fat diet-induced mouse model of type 2 diabetes. In this study we examined the mechanism of the antidiabetic activity of the Aloe QDM complex. Rat myoblast cell line L6 cells were cultured in the presence of Cr, ALS, and PAG alone and in combinations, and then the capability of the cells to uptake glucose was examined using radiolabeled glucose. All of the 3 agents, Cr, ALS and PAG, exerted glucose uptake-enhancing activity in L6 cells. The most potent capability to uptake glucose was observed when L6 cells were cultured with the Aloe QDM complex. The activity of the Aloe QDM complex to enhance glucose uptake was prominent in conditions where existing insulin concentrations are low. We also examined the effects of the Aloe QDM complex on the plasma membrane expression of GLUT4 in L6 cells. The Aloe QDM complex increased the content of GLUT4 in the plasma membrane, while decreasing the content of GLUT4 in the light microsome. Taken together, these results show that the antidiabetic activity of the Aloe QDM complex is at least in part due to the stimulation of glucose uptake into the muscle cells, and this activity of the Aloe QDM complex is mediated through the enhancement of the translocation of GLUT4 into the plasma membrane.

        Dietary Aloe Reduces Adipogenesis via the Activation of AMPK and Suppresses Obesity-related Inflammation in Obese Mice

        Background: Metabolic disorders, including type II diabetes and obesity, present major health risks in industrialized countries. AMP-activated protein kinase (AMPK) has become the focus of a great deal of attention as a novel therapeutic target for the treatment of metabolic syndromes. In this study, we evaluated whether dietary aloe could reduce obesity-induced inflammation and adipogenesis. Methods: Male C57BL/6 obese mice fed a high-fat diet for 54 days received a supplement of aloe formula (PAG, ALS, Aloe QDM, and Aloe QDM complex) or pioglitazone (PGZ) and were compared with unsupplemented controls (high-fat diet; HFD) or mice fed a regular diet (RD). RT-PCR and western blot analysis were used to quantify the expression of obesity-induced inflammation. Results: Aloe QDM complex downregulated fat size through suppressed expression of scavenger receptors on adipose tissue macrophages (ATMs) compared with HFD. Both white adipose tissue (WATs) and muscle exhibited increased AMPK activation through aloe supplementation, and in particular, the Aloe QDM complex. Obesity-induced inflammatory cytokines (IL-1β and -6) and HIF1α mRNA and protein were decreased markedly, as was macrophage infiltration by the Aloe QDM complex. Further, the Aloe QDM complex decreased the translocation of NF-κB p65 from the cytosol in the WAT. Conclusion: Dietary aloe formula reduced obesity-induced inflammatory responses by activation of AMPK in muscle and suppression of proinflammatory cytokines in the WAT. Additionally, the expression of scavenger receptors in the ATM and activation of AMPK in WAT led to reduction in the percent of body fat. Thus, we suggest that the effect of the Aloe QDM complex in the WAT and muscle are related to activation of AMPK and its use as a nutritional intervention against T2D and obesity-related inflammation.

        Dietary Aloe QDM Complex Reduces Obesity-Induced Insulin Resistance and Adipogenesis in Obese Mice Fed a High-Fat Diet

        Obesity-induced disorders contribute to the development of metabolic diseases such as insulin resistance, fatty liver diseases,and type 2 diabetes (T2D). In this study, we evaluated whether the Aloe QDM complex could improve metabolic disorders related to blood glucose levels and insulin resistance. Male C57BL/6 obese mice fed a high-fat diet for 54 days received a supplement of Aloe QDM complex or pioglitazone (PGZ) or metformin (Met) and were compared with unsupplemented controls (high-fat diet; HFD) or mice fed a regular diet (RD). RT-PCR and western blot analysis were used to quantify the expression of obesity-induced inflammation. Dietary Aloe QDM complex lowered body weight,fasting blood glucose, plasma insulin, and leptin levels, and markedly reduced the impairment of glucose tolerance in obese mice. Also, Aloe QDM complex significantly enhanced plasma adiponectin levels and insulin sensitivity via AMPK activity in muscles. At the same time, Aloe QDM decreased the mRNA and protein of PPARγ/LXRα and scavenger receptors in white adipose tissue (WAT). Dietary Aloe QDM complex reduces obesity-induced glucose tolerance not only by suppressing PPARγ/LXRα but also by enhancing AMPK activity in the WAT and muscles, both of which are important peripheral tissues affecting insulin resistance. The Aloe QDM complex could be used as a nutritional intervention against T2D.

        한·미 여군 건강연구 동향 분석(2015~2019년)

        Purpose: The purpose of this study was to compare the research trends on female soldiers' health in Republic of Korea and the United States from 2015 to 2019 by analyzing published articles. Methods: A total of 56 studies from Korean Journal of Military Nursing Research and Military Medicine were analyzed. Research design, funding status, conceptual/theoretical framework, ethical considerations, research topic, research subjects' characteristics, characteristics of quantitative and qualitative study were analyzed. Also the frequency of use of keywords were categorized into 10 domains. Results: Female soldiers' health research in United States accounted for 91.1% and those in Korea accounted for 8.9% respectively. Quantitative and qualitative research accounted for 83.9% and 8.9% of these 56 studies analyzed, respectively. Mixed method research was 7.2%. The most frequent studies who have health issues subjects were psychological, physical, pregnant issues, reproductive issues in descending order. The most frequent keyword domain involved 'health related concept (37.4%)' among ten domains. Conclusion: This study suggests that efforts and policy support are needed to encourage more studies on female soldiers' health with a variety of perspectives and topics.
