기관지 협착 환자에서 풍선카테터를 이용한 기도확장 1례 보고

기관지 협착증은 호흡곤란, 기침 및 객담, 반복성 폐렴, 협착 원위부에 기관지 확장증, 그리고 심한 경우 급성 호흡부전 등의 원인이 되므로 협착부위의 제거 또는 확장 등의 근본적인 치료가 요구되는 질환이다. 저자들은 기관지 결핵의 합병증으로 기관 및 좌측 주기관지 기시부 협착증이 발생한 40세 여자 환자에서 혈관성형술용 풍선 카테터를 이용하여 기관지 확장술을 시행한 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. In tracheobronchial stenosis, as any cause, productive cough, dyspnea, recurrent pneumonia, bronchiectasis and acute respiratory failure in serious case can be developed, so that the resection or dilatation of the stenotic regions are required. Various modalities of therapy Include surgical methods-resection and anastomosis, tracheal prostheses, tracheoplasty, bronchoscopic curettage, laser photoresection and cryotherapy, etc. -and non-surgical methods using bougies, expandable metallic stenos and balloon catheters etc. Mervyn D. Cohen has firstly described balloon dilatation in 31-month-old girl with narrowing of the upper part of the trachea and the origin of both right and left main-stem bronchi after resection of congenitally stenotic airway segments. We experienced a case of balloon catheter dilatation in tracheal and left main-stem bronchial stenosis complicating endobronchial tuberculosis. This procedure can be performed safty and effectively to alleviate the symptoms associated with bronchial stenosis, thus further studies should be accomplished.

궤양성 대장염과 크론병의 발병 기전으로 유전학적, 미생물학적, 환경적 또는 면역학적 요인 등 여러 가지 학설이 제시되고 있지만, 궁극적으로는 대장 점막에서 분비되는 cytokine에 의한 면역 조절 기능의 변화와 염증 반응을 통하여 발병한다. Chemokine은 proinflammatory cytokine으로서 염증 반응을 매개하는데 중요한 역할을 하며, 최근에 궤양성 대장염이나 크론병과 같은 만성 염증성 장 질환에서도 이들의 역할이 강조되고 있다. Interleukin 8(IL-8)이나 monocyte chemoattractant protein 1(MCP-1) 등이 대표적인 chemokine으로 각각 호중구와 단핵구에 대한 강력한 주성인자로 작용하여 염증 반응을 유발하거나 증폭시킨다. 본 연구에서는 궤양성 대장염과 크론병 환자의 대장 점막에서 chemokine 유전자의 발현 양상을 알아 보고, 또한 합성 RNA를 이용한 정량적 역전사 PCR법을 이용하여 이를 정량적으로도 비교해 봄으로써 이들 질환의 병인 기전에 chemokine의 역할을 알아보고자 하였다. Background/Aims: Although etiologies of ulcerative colitis(UC) and Crohn's disease(CD) remain unclear, these chronic, relapsing inflammatory conditions appear to be immunologically mediated. A number of inflammatory mediators released from activated immunocytes have been implicated in the pathogenesis of tissue injury in inflammatory bowel disease. Cytokines, such as interleukin 8 (IL-8) or monocyte chemoattractant protein 1(MCP-1) play iportant role in the recruitment and activation of neutrophils and monocytes. The objectives of this study were to explore the profile of chemokine gene expression in the colonic mucosa of patients wish UC and CD, and to quantify chemokine mRNA by way of quantitative reverse transcriptional polymerase chain reaction(RT-PCR) using synthetic standard RNA to elucidate the pathogenic roles of these cytokines in UC and CD. Methods: Colonoscopic biopsy specimens were obtained from active colonic mucosal lesions of 10 patients with UC, 13 patients with CD, and 10 normal controls(NC). After total tissue RNA was extracted, expression of IL-8 and MCP-1 mRNA was assessed by RT-PCR. The expressed transcripts for chemokine mRNA were quantified by quantitative RT-PCR using synthetic standard RNA, Results: 1) Expression of IL-8 mRNA : The expression rate was 90%(9/10) in UC, 85%(11/13) in CD, and 30%(3/10) in NC. The mean number of transcripts was 4.2×106 molecules in UC, 1.0×107 molecules in CD, and 1.6×105molecules in NC. Both the expression rate and the copy number of IL-8 mRNA were significantly higher in UC or CD than in NC(p<0.05), but there was no significant difference between UC and CD in the expression rate and the copy number of IL-8 mRNA(p>0.05). 2) Expression of MCP-1 mRNA The expression rate was 90% (9/10) in UC, 92% (12/13) in CD, and 10% (1/10) in NC. The mean number of transcripts was 7.9×105 molecules in UC, 1.9×106 molecules in CD, and 2.5×104 molecules in NC. Both the expression rate and the copy number of MCP-1 mRNA were significantly higher in UC or CD than in NC(p<0.75), but there was no significant difference between UC and CD in the expression rate and the copy number of MCP-1 mRNA(p>0.05). Conclusions: Chemokine genes such as IL-8 and MCP-1 are up-regulated In the colonic mucosa of patients wish UC and CD compared with NC. These findings suggest that IL-8 and MCP-1 may play central role in the pathogenesis of inflammatory bowel diseases through the augmentation and perpetuation of inflammatory reactions.

Hypercalcemia secondary to hyperthyroidism is common, but hypercalcemia due to concomitant hyperthyroidism and hyperparathyroidism after medical treatment of hyperthyroidism is relatively uncommon. We experienced a patient who presented with primary hyperthyroidism [triiodothyronine 5.9 nmol/L (1.2~3.4), total thyroxine 276.7 nmol/L (64.4~180.2), and TSH 0.7 mU/L (2.0~8.0)] and hypercalcemia [3.6 mmol/L (2.1~2.6)]. In spite of the treatment of hyperthyroidism, hypercalcemia persisted and hypercalcemic crisis developed. The radioimmunoassay for parathyroid hormone showed high values [2.49, 3.93, 3.48 μg/L, (C-terminal 0~0.5)]. We performed subtotal thyroidectomy and parathyroid exploration, and confirmed right inferior parathyroid adenoma as the cause of persistent hypercalcemia. The patient is healthy without any medication after operation. While there are no clinical features which can make the differentiation easy between two groups: one with hyperthyroidism with secondary hypercalcemia and one with concomitant hyperthyroidism and hyperparathyroidism, in patients with hyperthyroidism and possible hyperparathyroidism, serum parathyroid hormone levels should be measured and surgical therapy should be considered.

To investigate the water balance in a paddy rice field with the pumping station for irrigation water supply, flow measurements and analysis on various components of water balance were carried out. The investigated area is 103.7 ha, and the irrigation water was supplied for 102 days during the total irrigation period starting from June 1. It was found that the consumption rate was increased as the growing stage was progressed. The variation of evapotranspiration rate was shown same tendency as the consumption rate, while no apparent tendency was found in infiltration rate upon different growing stages. And the ground water input to the area was predominant during the early stage of growing period, while ground water output from the study area was predominant at the end of the growing stage. The range of return flow rate, the ratio of total outflow to total inflow in every decad, was 57.6 to 85.7%. These values are slightly higher than reported values from the other investigation projects.

It has been known that the synthesis of the cathode materials for the lithium rechargeable batteries by the sol-gel process has many advantages over the conventional solid-state method. It has been, however, a continuing issue that new additional steps should be introduced to commercialize this process. In this work, spray drying was introduced to the existing sol-gel process as a continuous mass production method of the pre-heat treatment precursor materials. The precursors of LiCoO2 and Li Ni0.8Co0.2O2 were continuously produced through spray drying from the solution containing stoichiometric amount of lithium, cobalt, and nickel sources as well as a chelating agent. The process variables, such as pH of the starting solution, spray drying conditions, and calcination conditions were optimized. The XRD pattern for the synthesized material indicated a good crystallinity with a layered structure.
