Gout is the most common inflammatory arthritis in men characterized by extremely painful recurrent acute and chronic arthritis. Gout is associated with various comorbidities, such as, chronic renal disease and metabolic syndrome, including dyslipidemia, diabetes mellitus, cardiovascular disease and obesity. Thus it has an impact on morbidity and mortality. The goals of treatment for gout are fast pain relief and the prevention of future gout attacks and other comorbidities. In every gout treatment, dietary and life style modification can be recommended. For the proper management of gout, definite diagnosis should come first. In this article, a current classification for the diagnosis of gout and management strategies for gout will be described.

Chip curling occurred during cutting process for difficult-to-cut material detracts product qualities and productivity. Among of method preventing the phenomenon, high pressure injecting cutting oil is an alterative. In this study, the optimal nozzle was designed by CFD method and it was conducted to analyse on the effect of high pressure injection on chip shape generated during cutting process and wear of insert by experimental method. As the result, it could be confirmed that high pressure injection is favorable for preventing chip curling and insert from wearing.

강직성 척추염은 몸통 뼈대와 큰 말초 관절 그리고 근육힘줄뼈 부착부위에 발생하는 만성 염증성 류마티스 질환이다. Adalimumab은 anti-tumor necrosis factor(TNF) 중 하나로 강 직성 척추염의 치료에 투여한다. Adalimumab 이 강직성 척추염의 징후와 증상의 치료에 효과적이기는 하지만, 강직성 척추염 환자에 서 뼈의 발생 혹은 재형성과 TNF 사이의 뚜렷한 연관성은 아직 밝혀지지 않았다. 그 리고adalimumab 치료 후에 이전에 손상되었던 천장관절의 회복이 영상의학적으로 증명 된 적은 극히 드물다. 여기서 우리가 소개하는 29세 남자는 이전에 천장관절의 염증성 변화를 보이는 강직성 척추염을 진단 받았던 자로, adalimumab 치료 전후에 영상의학적 변화를 비교해 보았다. 컴퓨터 단층촬영 영 상에서 adalimumab 치료 후 양측 천장관절 의 관절강이 좁아진 소견없이 미란성 변화들 이 거의 회복된 것을 볼 수 있었다. 이것은 adalimumab 치료 후에 강직성 척추염의 징 후 와 증상의 호전뿐만 아니라 이와 함께 천 장관절의 골미란이 관절강이 좁아지는 소견 없이 회복된 첫 번째 증례라고 할 수 있다. Ankylosing spondylitis(AS) is a chronic rheumatic inflammatory disease of the axial skeleton, large peripheral joints, and entheses. Adalimumab is one of anti-tumor necrosis factor(TNF) and is used for the treatment of AS. Although adalimumab is effective in treating the signs and symptoms of AS, a clear relationship between TNF and bone formation or remodeling in patients with AS has not yet been established. And radiological evidence of reparative change of a previously damaged sacroiliac (SI) joint by adalimumab treatment is extremely rare. Here, we present a 29-year-old man, who was diagnosed AS, with sacroiliac joint inflammation. We compared radiological difference before and after adalimumab treatment. Computerized tomography showed full recovery and repair of bilateral SI joint space erosions without joint-space narrowing after adalimumab treatment. This is the first report of a patient who showed an improvement of AS signs and symptoms-coupled with reparative changes to SI joint bone erosions occurred without joint-space narrowing after adalimumab treatment.
Klebsiella pneumoniae 로 부터 nif A Gene 의 크로닝 및 Azospirillum 에서의 발현

The nifA gene in the 3.0 kb SalI fragment from the nifA gene cluster of Klebsiella pneumoniae was cloned into BglII site of pRK290 and XhoI site of pVK100, a derivative of a wide host range plasmid RK2 (Inc P-1 group) and named pMK23 and pMK26, respectively. The constructed pMK23 and pMK26 carring nifA gene could be introduced into Azospirillum by transformation and conjugation with the helper plasmid pRK2013 and maintained stably in the host in constrast to some instability of RSF1010 (Inc Q group) or pSa (Inc W group)-derived vector. The nifA gene in the former recombinant nifA plasmid was expressed under the Tc^r gene promoter of pRK290 and the latter under the Km^r gene promoter of pVK100 in the Azospirillum host. The degree of derepression of the nitrogenase activity by the nifA expression from K. pneumoniae in the presence of 16 mM ammonia was about 18% in A. lipoferum (pMK23) and 10% in A. lipoferum (pMK26) compared to the nitrogenase activity in the absence of fixed nitrogen. Different level of MoFe-protein (nitrogenase component I) and Fe-protein (nitrogenase component II) synthesis between nifA plasmid introduced Azospirillum transconjugant and wild type were observed in the cells grown with the fixed nitrogen.

Cloning of nitA Gene from Klebsiella pneumoniae and Expression in Azospirillum

Klebsiella pneumoniae로부터 nifA 유전자를 크로닝하여 Azospirillum에 도입 발현시킴으로서 암모니아 비제어 질소고정을 유도하고자 하였다. Azospirillum 숙주에서 안정하게 복제 유지될 수 있는 vector로서 RK2 (IncP-1 group plasmid) 변형 vector인 pRK290과 pVK100를 선별하였으며 pKT230 (RSF1010 변형 vector, IncQ group) 및 pSa151(pSa 변형 vector, IncW group)는 불안정하였다. nifA 유전자 크로닝은 nifA 3.0 kb Sal1 절편을 pRK290의 Bgl II 위치에 그리고 pVK100의 Xho1 위치에 각각 크로닝하고 pMK23 및 pMK26으로 명명하였다. pMK23에서 nifA의 발현은 $Tc^r$ 유전자 promoter pMK 26에서는 $Km^r$ 유전자 promoter에 의하여 constitutive expression을 유도하였다. A lipoferum KY 6 에서는 16mM의 암모니아 첨가로 질소고정효소의 발현이 완전히 억제되었으나 A.lipoferum KY6 (pMK23) 및 A.lipoferum KY6 (pMK26)에서는 암모니아를 첨가하지 않았을 경우의 질소고정력의 18% 및 10%를 나타내므로써 K. pneumoniae nifA 유전자의 Azospirillum에서 발현이 확인되었다. 한편 암모니아 존재하에서의 질소고정 효소 합성도 전기영동 결과 확인되었다. 베체트병 환자에서 발생한 Lenticulostriate 동맥류 파열 1예

베체트병의 환자에서 뇌동맥류는 흔치 않은 중추신경계침범 임상상이며, 특히 lenticulostriate 동맥류가 발생하는 경우는 매우 드물다. 저자들은 활동성 베체트병에의한 lenticulostriate 동맥류 파열로 발생한 급성 좌측 편마비환자에서 고용량 글루코코르티코이드제제 투여로 호전된 국내첫 번째 증례를 경험하였기에 문헌고찰과 함께 보고한다. Behcet`s disease (BD) is characterized by recurrent oro-genital ulcers, skin lesions, and intraocular inflammation, but can also affect various internal organs. Vascular BD usually presents with luminal stenosis, thrombosis, or aneurysm formation in aorta and peripheral arteries. However, intracranial artery involvement has been uncommonly reported in patients with BD and BD cases with lenticulostriate artery aneurysm have been rarely described in the English-language literature. We hereby reported the first case of a Korean BD patient presenting with a ruptured lenticulostriate artery aneurysm, who received medical treatment, and reviewed the literature on reported cases of BD with intracranial aneurysms. 저자들은 혈액 및 활막액 호산구증가증이 동반된 급성 무릎 단관절염 환자에서 개회충증 감염을 진단하고 글루코코르티코이드 관절강내 주사와 함께 알벤다졸 투약을 통하여 치료한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Eosinophilic synovial effusion is rarely observed in patients with inflammatory nature of synovial fluid, and the differential diagnosis includes parasitic arthritis. Toxocariasis is the one of the most common forms of helminthiasis worldwide and has been reported as a common cause of peripheral blood eosinophilia in Korea. However, joint involvement has been rarely reported in adults with toxocariasis in the English-language literature. Here, we report the first Korean case of a female presenting with acute monoarthritis with an increased number of eosinophils in the peripheral blood and synovial fluid, who was finally diagnosed with toxocariasis.
