        Mid-Term Outcomes of Anterior Cruciate Ligament Reconstruction with Far Anteromedial Portal Technique

        purpose: The purpose of this study was to evaluate the mid-term outcomes of anatomic anterior cruciate ligament (ACL) reconstruction using two anteromedial (AM) portals by comparing with short-term follow-up results.Materials and Methods: Fifty patients who were treated by ACL reconstruction using a two AM portal technique were evaluated retrospectively. The follow-up period was at least 5 years. The mean follow-up period was 68.5±13.9 months. The mid-term clinical outcomes were compared with short-term (≥12 months) results. For the assessment of knee stability, anterior tibial translation was evaluated using the Lachman test and the KT-2000. Rotational stability was evaluated using pivot shift test. For clinical assessment, the Lysholm and International Knee Documentation Committee scores were used. results: The average anterior translation was 2.1±1.4 mm at the short-term follow-up and 2.8±1.8 mm at the mid-term follow-up. Stability and mid-term clinical outcomes were not significantly improved compared to the short-term follow-up results. At the mid-term follow-up, anteroposterior (AP) instability assessed by the KT-2000 was slightly increased, but still acceptable. On the other clinical physical evaluation, there was no statistically significant difference.conclusions: The short-term and mid-term outcomes of ACL reconstruction using the two AM portal technique were not significantly different except for AP stability although the value was less than 3 mm at both follow-ups. Therefore, this operative technique could be considered a satisfactory alternative for ACL reconstruction.

        대형 국가산업단지의 생산성이 지역 유해 환경 변화에 미치는 영향

        본 논문에서는 대형 국가산업단지의 생산량이 지역 유해물질의 발생량에 미치는 영향을 단지규모, 산업 특성을 기반으로 분석하였다. 서울의 경우에는 제조업 기반보다는 지식기반산업 중심으로 서울 전체의 대기환경에 악영향을 미치는 유해물질의 발생비중은 타 지역 산업단지에 비하여 상당히 적었다. 반면에 기계, 금속, 전기, 전자 등 제조업 기반의 타 국가산업단지는 업종별 특성과 유통물류 및 지역적 특성이 복합적으로 영향을 미치는 것으로 조사되었다. 전체적으로는 대형 국가산업단지의 생산성 증대가 지역전체의 유해물질의 증가에 비례하지는 않는 것으로 조사되었다. 또한, 중화학공업 기반인 경북의 경우에는 업종의 특성으로 인하여 타 지역과 달리 유해물질 종류의 비중이 다른 지역과 다르게 나타나는 것으로 조사되었다. In this paper, the effects of large national industrial complexes on the generation of local hazardous substances were analyzed based on their size, industrial characteristics. In Seoul, the proportion of hazardous substances, which adversely affect the atmosphere of the entire city, was much less than that of other regional industrial complexes, centered on knowledge-based industries. On the other hand, other national industrial complexes based on manufacturing industries, such as machinery, metal, electricity, and electronics, were found to have a combination of industry characteristics and logistics and regional characteristics. Overall, it was found that the increase in productivity in large national industrial complexes was not proportional to the increase in hazardous substances throughout the region. In addition, it was also found that in North Gyeongsang Province, which is based on the heavy chemical industry, the percentage of hazardous substances is different from other regions due to the characteristics of the industry.

      • 석회화 건염의 보존적 치료와 조기 관절경적 치료 결과

        목적: 석회화 건염에 대하여 보존적 치료 및 조기 관절경을 이용한 수술적 치료 후 통증과 임상경과를 비교 관찰하였다. 대상 및 방법: 2002년 2월부터 2008년 5월까지 석회화 건염으로 진단받은 30예 중 6개월 이상 추시관찰이 가능하였던 스테로이드 주사치료 15예 및 수술 치료 15예를 대상으로 하여 석회질의 통증과 운동 범위의 호전을 2주, 12주, 24주째 각각 비교 관찰하여, Constant-Murley score를 이용하여 평가하였다. 결과: 통증 정도는 Constant-Murley score상 보존적 치료군의 경우 수술 전 평균 3.2에서 치료 이후 2주, 12주, 24주째 각각 13.6점, 14.5점, 14.7점이었으며, 수술적 치료군은 수술 전 평균 3.3에서 수술후 2주, 12주, 24주째 각각 10.2점, 13.0점, 14.3점이었다. 운동 범위는 보존적 치료군의 경우 수술 전 평균 14.3에서 치료 이후 2주, 12주, 24주째 각각 21.7점, 31.3점, 35.7점이었으며, 수술적 치료군은 수술 전 평균 14.4에서 수술후 2주, 12주, 24주째 각각 33.1점, 35.8점, 36.4점이었다. 치료 2주째 및 12주째 두 간의 비교에서는 수술적 치료 군이 유의하게 우수하였으나, 최종 추시시는 통계학적인 차이가 없었다. 결론: 석회화 건염의 조기 관절경적 치료는 환자의 만족도를 높이고 조기에 일상 생활로 복귀할 수 있는 효과적인 치료법으로 사료된다. Purpose: We evaluated the level of pain and clinical presentation of calcific tendinitis after treatment with conservative and early arthroscopic operation. Materials and Methods: We reviewed 30 patients with calcific tendinitis with the minimum of 6 months of follow up period, treated from February 2002 to May 2008. We implemented Constant-Murley score to evaluate the pain of calcification and improvement of range of motion at the 2nd week, 12th week and 24th week with the patients who had treatment with steroid injection in 15 cases and operation in the other 15 cases. Results: Evaluating the level of pain using Constant-Murley score, we found that conservative treatment group had the scale of 3.2 before treatment and 13.6, 14.5 and 14.7 at 2nd, 12th, and 24th week, respectively since started treatment. The operation group, on the other hand, had the scale of 3.3 preoperatively and 10.2, 13.0, and 14.3 at 2nd, 12th and 24th week postoperatively. The range of motion in the conservative treatment group showed 14.3 on average before the treatment and 21.7, 31.3 and 35.7 at 2nd, 12th and 24th week after treatment. The operation group had 14.4 on average preoperatively, and 33.1, 35.8 and 36.4 at 2nd, 12th and 24th week postoperatively. The operation group had statistically significant improvement compared to the conservative group at the 2nd and 24th week, but showed no difference between the two groups at the final follow up. Conclusion: Early arthroscopic treatment of calcific tendinitis seems to be an excellent option that can increase the level of satisfaction of the patients and an efficient way of putting the patients back to their normal life promptly.

        산업폐기물 해양배출 관련 규제 연구

        There was an issue of marine pollution in 2016 when a power plant located on the shoreline directly discharged hazardous chemicals into the sea. The power plant was accused of breaking the laws of the Marine Environment Management Act by a citizen and environmental organization in Korea. Ministry of Oceans and Fisheries (MoF) claimed the power plant discharged dimethylpolysiloxane, which is regulated as a noxious liquid substance by MARPOL73/78. Dimethylpolysiloxane (PDMS) is normally used to reduce the formation of foam in the cooling system during the washing process, and the plant’s flow of discharge wastewater is connected to the sea, directly. The aim of this study was to research the possible effects on the industry when the laws are applied to all industrial facilities along the shoreline. The smallto medium-sized companies can be affected by financial duties tied to complying with the law. This study investigated how to treat and discharge waste containing hazardous chemicals from a facility, focusing especially on power plants and companies along the shoreline. Direct or indirect discharges of waste from these facilities may cause marine pollution and are major sources of ecocide. However, there is no standard to apply to noxious liquid substances and no one knows whether or not the power plant is in the scope of the marine facilities of that law. Therefore, the government must manage industrial waste efficiently, with proper policies and regulations. Furthermore, it needs to create reasonable standards related to discharging industrial waste.

        하향 전파하는 예혼합 화염에서 루이스 수에 따른 열음향 불안정성의 동적거동 변화

        The effects of Lewis number on acoustic instability of downward propagating flames in a tube were studied experimentally. The discussion first treated the coupling effects between cellular, Darrieus-Landau and primary acoustic instabilities in non-equidiffusive flames. Low Lewis number tended to generate stronger primary acoustic sound and to suppress cellular instability under the fixed laminar burning velocity. Also, displacement flame velocities declined with low Lewis number because of diminished flame surface area with absence of combustion instability. We then described the effects of Lewis number on secondary acoustic instability. To generate secondary acoustic instability at desired time, we adopted the CO₂ laser irradiation method to alter the shape of the flame front. The results showed that for a Lewis number below (above) unity, relatively stronger (weaker) acoustic sound is produced under the same laminar burning velocity, because the combustion wave becomes sensitive (insensitive) to pressure fluctuations in the acoustic field due to curvature effects. Therefore, shorter (longer) transition time was required from secondary acoustic instability to turbulent motion.

        PHAST를 이용한 흡입독성유해물질에 대한 초기이격거리 산정

        Chemical spill accident in Korea is increasing in various forms, with 45 percent of them occurring in the process of transporting chemicals. Initial response around the accident point is more important than anything else, especially in the case of inhalation toxic hazardous substance leakage accidents, the Initial Isolation distance can seriously affect a person"s life. Therefore, in this study, the Process Hazard Analysis Software Tool(PHAST) program, which is used internationally as a risk assessment program, was used to calculate the initial isolation distance when an accident occurs during the transportation. Using PHAST programs to compare the initial isolation distance with the range of toxic effects shows that the initial isolation distance of most substances belongs to ERPG-1 and ERPG-2. Since it is necessary to specify a wide range of initial isolation distances for safer response to the initial chemical accident, this is considered as a realistic range considering safety and to be a practical scope for the chemical transport accident.

      • 레디믹스트 콘크리트의 환경성적표지인증 개선에 관한 연구

        녹색건축인증(G-SEED) 개정 이래, 전과정평가(Life Cycle Assessment-LCA)에 기반한 환경성적표지인증(Environmental Product Declaration-EPD)를 획득한 건축자재를 사용하는 경우 가점을 부여함에 따라 건축자재의 EPD가 급격하게 증가되었다. 특히, 레디믹스트 콘크리트(RMC-레미콘)의 EPD 누적건수는 2018년 18건 2019년 109건 2020년 501건 2021년 809건으로 매년 증가하고 있다. 본 연구에서는 현재 EPD 결과의 신뢰도를 높이기 위해 EPD에서 규정하고 있는 레미콘의 환경성적표지 개별지침을 분석하여 레미콘 환경성적표지인증의 개선안을 제시하고자 한다. PCR 분석 결과로는 공정수의 할당방법의 세분화 필요성이 있었고, 개별공장의 배출량 산정의 표기의 필요성이 있었다. 이러한 레미콘 PCR 분석을 통해서 추후 EPD의 일반제품 작성지침, 건축자재 작성지침, 레미콘 작성 지침 개정에 검토자료로 활용될 것으로 사료된다. Since the revision of the Green Standard for Energy and Environmental Design (G-SEED), Environmental Product Declaration (EPD) of building materials has increased rapidly as additional points have been given to use building materials that have obtained EPD based on Life Cycle Assessment (LCA). In particular, the cumulative number of (Environmental Product Declaration-EPD) of ready-mixed concrete (RMC) is increasing every year from 18 in 2018, 109 in 2019, 501 in 2020, and 809 in 2021. In order to increase the reliability of the current EPD results, this study aims to present an improvement plan for the certification of EPD of RMC by analyzing the RMC Product Category Rules (PCR) as stipulated by the EPD. As a result of PCR analysis, there was a need to subdivide the method of allocating the number of processes, and there was a need to indicate the calculation of emissions of individual factories. Through this RMC PCR analysis, it is expected to be used as a review data for revising EPD's general product preparation guidelines, building material preparation guidelines, and remicon preparation guidelines in the future.

        수부에서 근위 지간 관절 골두의 부분 골연골 결손에 대한 자가 족지 골두 이식술

        Osteochondral injury due to the trauma of the hand is relatively common. If the size of the osteochondral fracture fragment is large, open reduction and internal fixation are often feasible in treating these problems. However, arthroplasty using osteochondral graft is more preferred when the particle is small and articular surface is comminuted or fully defected. There are many reports of osteochondral graft using the costal osteochondral graft but the osteochondral graft using the interphalangeal joint of the toe is rarely reported. Thoroughly reviewed with relevant articles, this report presents a case of a 33 year old male who was successfully treated with osteochondral autograft using the proximal interphalangeal joint of the toe due to the traumatic osteochondral defect in the head of the second proximal phalanx.
