Guillain-Barre syndrome after generalized tetanus infection

Guillain-Barre syndrome (GBS) is an auto-immune disease of peripheral nerve system. It occurs mainly after preceding infection such as upper respiratory or gastrointestinal infection and other antecedent events as tetanus vaccinations. However, any case of GBS after tetanus infection has not been reported. Recently, when analyzed the clinical aspects of 13 tetanus patients including ours, 2 GBS occurred after tetanus infection. We report the neurological and electrophysiologic findings of two cases of Guillain-Barre Syndrome after generalized tetanus.

Vertigo, dizziness, and disequilibrium are common symptoms following concussion or traumatic brain injury. Dizziness and vertigo may be the result of trauma to the peripheral vestibular system or the central nervous system, or, in some cases, may be due to anxiety, depression, or posttraumatic stress disorder; these mechan-isms are not mutually exclusive. While most peripheral vestibular disorders can be identified by testing and examination, those without inner ear causes that have persisting complaints of dizziness and motion sickness are more difficult to under-stand and to manage. Some of these patients exhibit features compatible with vestibular migraine and may be treated successfully with migraine preventative medications. This paper reviews the neurological causes of persisting dizziness, the possible mechanisms, and the pathophysiology, as a framework for patient management and for future research.

Castleman's disease is a condition characterized by masses composed of benign lymphoid tissue. Castleman's disease usually occur as a solitary lesion in the mediastinum and less frequently in the neck, and rarely in retroperitoneum and pelvis. Castleman's disease is classified into hyline-vascular type and the plasma cell type. We report a case of 52-year-old woman with Castleman's disease in the retroperitoneum. She was admitted to our hospital with the complaint of right lower quadrant pain for 3 days. Ultrasonography, computed tomography, magnetic resonance imaging and angiography were performed. The mass was located in the retroperitoneum, had a size of 6 X 5 cm with internal calcification and it was completely removed by operation. Pathological review showed a byline-vascular type of giant lymph node hyperplasia(Castleman's disease) in portocaval space of retroperitoneum. Follow up physical examination and abdominal CT scan 3 years after operation revealed no recurrence of disease.

Pulmonary metastases form uterine leiomyomas are extremely rare despite of high frequency of unterine leiomyoma. Metastatic uterine leiomyomas are seen in asymptomatic middle-aged female patients, who has a history of previous uterine leiomyoma. The lung metastases appear histologically benign as are the primary uterine tumor, so they are called benign metastasizing leiomyomas. We experienced a case of benign metastasizing leiomyoma and hereby report after 5 years of follow-up.

Pancreatic cystadenoma is very rare, slow growing tumor and generally preoperative differential diagnosis may be difficult. A 43-year-old female was admitted to our hospital with the complaint of abdominal discomfort in the epigastrium for about preceeding 5 years. Ultrasonography and Computed tomography were performed. When endoscopic retrograde cholangio-pancreatogram was performed, displacement of distal pancreatic duct by huge mass was noted and injected dye was drained irregulary into the cavity of mass. Distal pancreatectomy was performed with splenectomy. The mass size of distal pancreas was 7X4cm, and the cyst contained with yellowish thick mucoid fluid. Diagnosis was confirmed the mutinous cystadenoma of the pancreas on histologic examination.
