뇌신경질환에 사용된 레이저 및 광선치료에 대한 임상 연구 고찰

Objectives: Recently, many studies have reported beneficial effects from the application of laser and light-emitting diode (LED) therapy for cerebral nervous disease. Transcranial laser therapy and LED therapy may be an effective method to treat diseases of the cerebral nervous system. This study aims to discuss the possibility of laser and LED therapy for cerebral nervous disease by reviewing literature about its effectiveness. Methods: We searched papers using PubMed, Science Direct, CINAHL, KTKP, Oasis and NDSL, using the keywords "Laser therapy, low-level", "Transcranial laser", "Transcranial light emitting diode" and "stroke", "traumatic brain injury", "dementia", "anxiety", "cognitive", "emotional effects", "psychiatric disorder", "multiple sclerosis", "Parkinson's disease". The search range included randomized controlled trials (RCTs) and clinical case series. Reviews and animal experiments were not included. Studies not matched with inclusion criteria were excluded. Results: A total 1,119 studies were found. 1,100 were excluded from scanning titles and abstracts and finally 9 articles were selected. Among the 9 articles, 5 were RCTs, one was a controlled study, and the other 3 were case reports. They reported that transcranial laser therapy and LED treatment had beneficial effects from photobiomodulation to the cerebral nervous system. Clinical evaluation factors showed favorable trends. Conclusions: Transcranial laser therapy and LED therapy seem to be effective to the cerebral nervous system and they may be a favorable choice for cerebral nervous disease.
DVD-ROM 시스템에 적용한 신경망 등화기에 관한 성능

차세대 저장매체로서 이미 그 규격이 확정된 재생 전용 디지털 다기능 디스크 시스템에 등화기를 적용하여 그 성능을 분석한다. 재생 전용 디지털 다기능 디스크 시스템에서는 정보 기록 시에 비선형 왜곡현상을 유발할 수 있으므로 이의 보상을 위해 비선형 입출력 사상능력이 뛰어나다고 알려져 있는 신경망 등화기를 적용한다. 또한, 결정궤환 구조의 등화기는 변조부호와 결합하여 사용하면 성능 개선을 보장할 수 없다는 것을 확인하기 위한 실험을 한다. 두 종류의 시스템에 대한 등화 실험으로 신경망을 이용한 등화기가 기존의 등화기에 비해 더 좋은 등화성능을 보이며 이는 채널의 비선형성이 증가할수록 확연해지는 결과를 얻었다. 또한, 랜덤한 데이터열의 간섭 보상에는 결정궤환 구조의 등화기가 우수한 성능을 보이지만 변조 부호화된 데이터열의 간섭 보상에는 결정궤환이 없는 구조와 비교하여 성능 개선이 없다는 결과를 얻었다. Several equalizers are applied to the DVD-ROM(Digital Versatile Disc-Read Only Memory) system. Because imperfect writing process may cause nonlinear distortion in the reply signal, neural equalizers which have strong nonlinear mapping capability are applied to the system to compensate the nonlinear distortion. Experimental results to verify that the combination of decision-feedback type equalizers and modulation code is formidable are also given. The experimental results shwo that the SNR gain of the neural equalizers over the conventional equalizers becomes much as the nonlinearity in the channel increases. Furthermore, the decision-feedback type equalizers outperform the equalizers which have no decision-feedback in eliminating ISI(Intersymbol Interference) of random data sequence but there is no performance gain of the decision-feedback type equalizers over the equalizers without decision-feedback when these are applied to compensate the ISI of modulation-encoded data sequence.
DC 급전계통의 디지털 보호계전기의 합리적 운용방안 검토

최근 서울메트로 1,2호선 변전설비는 장기 사용한 부분부터 년차적으로 설비를 교체하고 있다. DC 급전계통 보호계전기도 76I, 50, 85, 64P 4개정도에 불과했으나 76I, 76D, DDL-I, DDL-T(Imin), 85, 64P등 다양한 종류의 디지털 보호계전기로 세분화되었으며, 각종 이벤트 및 사고파형 등을 기록?저장하여 모든 동작사항을 검토 · 분석할 수 있도록 기능이 다양화 되어있다. 그러나 보호설비를 본래 보호목적에 맞게 사용하지 못하여 장애가 확대된 사례가 있어 본 논문에서는 보호계전기 종류별 보호목적, 방향, 설정치 등을 검토하여 향후 DC차단기 보호계전기 운영에 활용함으로서 사고확대를 방지하고, 직류 보호계전시스템 운영에 있어 합리적인 방법을 제시하고자 한다. Lately, Seoul metro is changing transformation facilities which have been operated in subway line 1 and 2 for a long duration of time. Although the exiting protection relays in DC feeder system have such several functions as 76I, 50, 85, and 64P, the new protective relay have a great variety of functions such as 76I, 76D, DDL-I, DDL-T(Imin), 85, and 64P, as well as record and save various events and accident wave in order to review and analyze the working causes of the protective system. However, because the new digital relays are not used properly for protective propose, there are the cases that the electric accident is deteriorated more. Therefore, in this paper, we will describe that making the use studying installation intention, direction, and setting up value of the protection relay in DC breaker operation not only prevents from making the electric accident worse but also shows the efficient operation method of direct current protection system.
위암 수술 후 발생한 문합부 누출에 대한 경장 및 정맥 혼합 영양 지원의 효과

Purpose: The effectiveness of enteral nutrition for patients with anastomotic leakage after gastric cancer surgery is controversial. The purpose of this study is to compare effectiveness between combined enteral nutrition with parenteral nutrition (EPN) and total parenteral nutrition (TPN). Methods: Patients who underwent gastric cancer surgery for primary gastric cancer from April 2010 to August 2012 were reviewed. Clinicopathologic characteristics, complication, laboratory tests, and body weight (Bwt) were compared between EPN and PN. Results: Among patients with postoperative leakage within postoperative 1 month (n=43), 13 patients were supported by EPN and 23 patients by TPN. Clinicopathologic characteristics, including preoperative Bwt, body mass index, nutritional status, other complications, and TNM stage were similar. Preoperative serum albumin and Bwt were similar between EPN and TPN. However, after 1 week of nutritional support, albumin at EPN was significantly higher than that of PN (3.52±0.3 and 3.25±0.3; P=0.010). Adjusted by preoperative Bwt, preoperative nutritional status, and difference in Bwt between preoperative and pre-nutritional support period, decrease of Bwt between pre-nutritional support and discharge was significantly less at EPN than at TPN (.4.5±5.4% and .6.3±4.1%; P=0.001). Conclusion: In terms of the maintenance of serum albumin and Bwt during nutritional support, EPN may be a better supportive option than TPN for patients with anastomotic leakage after gastric cancer surgery.

The present study examined inhibitory effects of 20 efficient experience prescriptions on platelet aggregation induced by collagen in human whole blood using the impedance method of aggregometry. Among them, a hot water extract of KIOM 2003-080 was selected to be the most effective candidate. In an in vivo study using a mouse acute thrombosis model, the anti-thrombotic effects of the KIOM2003-080 crude extract were also observed. In addition, we accessed bio-marker of platelet activation using thromboxane B2 by ELISA assay. A significantly decrease in thromboxane B2 production was seen in the presence of KIOM2003-080. Consequently, the results from this experiment provide pharmacological evidence for the traditional use of KIOM2003-080 prescription, suggesting that its hot water extracts could be used to prevent platelet aggregation and thrombosis disease.

Congenital onychodysplasia of the index fingers (COIF, Iso and Kikuchi syndrome) is a congenital disorder characterized by various forms of nail dysplasias mainly-involving the index fingers. Its etiopathogenesis is still unknown, but ischemia of the palmar digital arteries has been suggested to play a role in this disorder. Although not specific to it, a Y-shaped bifurcation of the distal affected phalanx is a characteristic finding of this syndrome. In this review, we report a case of COIF who presented with bilateral micronychia with a Y-shaped bifurcation of the distal phalanx. (Korean J Dermatol 2010;48(8):729~731)
