        발달장애아에서 안과질환의 임상양상

        Purpose: To evaluate ocular abnormalities in children with developmental disability and to find out whether any correlation exists between developmental disability and surgical outcome. Methods: Totally 43 patients with the diagnosis of developmental disability were enrolled in this retrospective study. Results: Mean follow-up was 4.54 ± 2.35 year. 20 patients had exodeviation, 15 patients had esodeviation, 10 patients had dissociated vertical deviation and 4 patients had dyskinetic strabismus. 21 patients had surgery and the mean deviation angle was decreased from 36.67 ± 15.70 PD to 5.33 ± 5.93 PD in exotropia and 56.25 ± 8.54 PD to 5.75 ± 4.65 PD in esotropia. Refractive error was found in 33 patients (88.4%). 26 patients (60.5%) had monocular amblyopia and 17 patients (65.38%) showed improvement of visual acuity after treatment. Conclusions: Exotropia is the most common type strabismus in patients with developmental disability and dyskinetic strabismus is found in cerebral palsy. Strabismus Surgery for patients with stable angle deviation and amblyopia treatment is effective in children with developmental disability. However decision for surgery should be made after a long follow up period. J Korean Ophthalmol Soc 2014;55(2):261-266

        조대술을 이용한 결막 봉입 낭종의 치료 1예

        Purpose: We present a case with conjunctival inclusion cyst at inferior fornix treated by marsupialization. Case summary: A 23-year-old woman visited our clinic complaining of left lower eyelid swelling. Ophthalmologic examination and CT scan showed a cystic mass from inferior conjunctival fornix to anterior orbit with shallow fornix and focal symblepharon. The cyst was effectively removed with marsupializaion. Postoperatively, there was no recurrence of cyst and the fornix was deepened. Conclusions: Marsupialization can be a considerable treatment option in conjunctival inclusion cyst, especially when accompanied by shallow fornix and symblepharon.       • Coronary artery vasospasm after atrial septal defect surgery

        Coronary vasospasm is one of the fatal complications that may occur in patients undergoing open heart surgery. To date, however, there are not many cases in this series and no definite pathophysiology has been documented. We experienced a case of coronary artery vasospasm after atrial septal defect (ASD) surgery and then successfully treated it with both transbrachial intra-aortic balloon pump and percutaneous cardiopulmonary support. Only several hours after ASD surgery, the patient exhibited the cardiovascular collapse, the ST-segment elevation, followed by ventricular fibrillation and normal coronary angiography findings. It is important to make a differential diagnosis of coronary artery vasospasm in patients presenting with ST-segment elevation who had no notable coronary artery diseases. This case indicates that clinicians should be aware of the possibility that the coronary artery vasospasm may also occur in patients undergoing ASD surgery.

        Tri-n-butyl phosphate와 액상고분자 지지액막을 이용한 페놀의 분리

        다양한 수질 오염원 중 페놀과 같은 유기화합물은 배출조건이 비정상적인 경우 기존의 처리방법으로 제거하거나 파괴하기 어렵다. 수용액으로부터 페놀을 분리하기 위한 최근의 연구들은 활성탄에 의한 흡착, 용매 추출 등에 의해 수행되어져 왔으며, 물-기름 유화에 기초를 둔 액막법이 기존의 기술들을 대체하기 위한 방법으로 시험되어져 왔다. 본 연구에서는 tri-n-butyl phosphate(TBP)와 액상 고분자를 지지액막의 액막용액으로 선정하여 수용액상의 페놀을 분리하였다. 공급측 페놀 농도를 변화시키고 다양한 액막용액을 사용하여 이들이 페놀분리에 미치는 효과를 연구하였다. TBP를 운반체로 사용한 경우의 실험결과들은 용매추출에서 주로 사용되고 있는 methyl isobutyl ketone(MIBK)보다 높은 물질전달속도를 보여주었으며, 선택한 액상고분자 중 polypropylene glycol 4000(PPG-4000), polybutylene glycol 500(PBG 500)의 경우 MIBK와 유사하거나 조금 높은 값을 나타내었다. Among various water contaminents, organic compounds like phenol are difficult to be removed or destroyed by conventional methods under the unusual discharge conditions. The separation of phenol from aqueous solution has been carried out by several methods recently: absorption by an activated carbon, solvent extraction and liquid membrane technology. The liquid membrane based on water-oil emulsification has been tested as an alternative technology of the conventional technology. In this work, tri-n-butyl phosphate(TBP) and liquid polymers were examined as a liquid membrane in the supported liquid membrane(SLM). The feed concentration of phenol was varied and various types of liquid membranes were used to examine their effects on separation of phenol. It was found that TBP, polypropylene glycol 4000(PPG 4000) and polybutytene glycol 500(PBG 500) were proper carriers because mass transfer rates through them were much higher than or similar to that through methyl isobutyl ketone(MIBK) which was used as a conventional solvent in a solvent extraction process.

        아토스 스마트사이트수술의 임상 결과 보고

        Purpose: To evaluate the clinical outcomes of a new refractive corneal lenticule extraction method (SmartSight) using a Schwind ATOS femtosecond laser (Schwind Eye-Tech-Solutions, Kleinostheim, Germany). Methods: The medical records of 338 eyes of 170 patients who underwent SmartSight treatment from April to September 2022 and were followed-up for at least 3 months were retrospectively analyzed. The patients were followed-up at 1 day, 1 week, and 1 and 3 months after operation. Refraction, visual acuity, and intraocular pressure were measured at each visit. Results: Preoperatively, the uncorrected distant visual acuity was 1.26 ± 0.33 logarithm of minimum angle or resolution (logMAR) and the spherical equivalent was -4.52 ± 1.77 diopters. The 3-month, postoperative, uncorrected distant visual acuity was 0.01 ± 0.03 logMAR and the spherical equivalent was -0.12 ± 0.22 diopters. The 3-month data were as follows: efficacy of refractive surgery, 0.99 ± 0.11; safety, 1.00 ± 0.05; predictabilities at ±0.5 and ± 1.0 diopters, 97% and 100%, respectively. Conclusions: Myopic astigmatism correction using the SmartSight method was effective, safe, and predictable in terms of visual outcomes.

        덧눈꺼풀 환아에서 눈꺼풀 틈새의 수직길이와 각막 난시와의 관계

        목적: 덧눈꺼풀 환아에서 환아의 난시의 정도와 눈꺼풀 틈새의 수직길이를 조사하고, 그 연관성을 알아보고자 하였다. 대상과 방법: 2012년 9월부터 2013년 7월까지 외래를 방문한 총 34명 68안의 덧눈꺼풀군과 같은 기간 외래를 방문한 덧눈꺼풀이 없는 환아 총 44명 88안의 대조군을 대상으로 두 군 간에 눈꺼풀 틈새의 수직길이와 난시를 비교하였다. 또한 덧눈꺼풀 군에서는 수술 전 각막미란의 정도를 분류하여 난시와 연관이 있는지 관찰하였다. 결과: 덧눈꺼풀군의 나이는 5.6 ± 2.2세, 대조군의 나이는 6.1 ± 1.5세로 유의한 차이를 보이지 않았다(p=0.339). 덧눈꺼풀군의 난시는 2.28 ± 1.54 diopter (D), 대조군은 0.91 ± 1.07D로 덧눈꺼풀군에서 더 심하였다(p<0.001). 덧눈꺼풀군의 눈꺼풀 틈새의 수직길이는 6.70 ± 1.19 mm, 대조군은 7.63 ± 1.06 mm로 덧눈꺼풀군에서 더 작았다(p<0.001). 또한 덧눈꺼풀군에서 눈꺼풀 틈새의 수직길이가 7 mm보다 작은 경우에 그 이상인 경우보다 난시가 컸고(p=0.026), 난시값이 클수록 눈꺼풀 틈새의 수직길이가 작았다(p=0.022). 결론: 덧눈꺼풀군에서 대조군에 비해 유의하게 각막난시의 양이 컸고, 이는 낮은 눈꺼풀 틈새의 수직길이와 관련이 있는 것으로 나타났다. Purpose: To study the effect of plapebral fissure height on astigmatism in epiblepharon patients. Methods: The study consisted of 68 eyes of 34 patients who were diagnosed with epiblepharon and 88 eyes of 44 patients who had normal eyelids from September 2012 to July 2013. Data on palpebral fissure height and refractive errors were compared between the epiblepharon group and the control group. Epiblepharon patients were further divided into two subgroups depending on the degree of preoperative corneal erosion in order to study the effects of corneal erosion on corneal astigmatism. Results: The mean age was 5.6 ± 2.2 years in the epiblepharon group and 6.1 ± 1.5 years in the control group (p = 0.339). The mean astigmatism was 2.28 ± 1.54 D in the epiblepharon group and 0.91 ± 1.07 D in the control group. The epiblepharon group showed higher astigmatism than the control group (p < 0.001). The mean palepebral fissure height was 6.70 ± 1.19 mm in the epiblepharon group and 7.63 ± 1.06 mm in the control group. The epiblepharon group exhibited smaller palpebral fissure height than the control group (p < 0.001). In the palpebral fissure height subgroups of the epiblepharon group, the <7.0 mm group showed higher astigmatism than the ≥7.0 mm group (p = 0.026). Higher astigmatism was associated with smaller palpebral fissure height (p = 0.022). Conclusions: Patients with epiblepharon had significantly higher corneal astigmatism, and higher astigmatism was associated with smaller palpebral fissure height.
