Object : The purpose of this study is to observe the correlation between change in pattern identification symptoms and scandinavian stroke scale in ischemic stroke patient who had receive traditional Korean medical treatment and conservative treatment. Methods : 43 subjects were recruited from patients with ischemic stroke within 30 days of onset. We chose the subjects who had at least one follow up session and had checked the score between last follow up session and first session in pattern identification and scandinavian stroke scale. We also assessed the correlation between pattern identification and scandinavian stroke scale. Results : There were significant negative correlation between pattern identification and scandinavian stroke scale in Fire-heat pattern and positive correlation in Dampness-phlegm pattern. Conclusions : This study provides evidence that collaborative treatment maybe effective in improving neurologic symptoms in ischemic stroke patients diagnosed as Fire-heat pattern. Further studies with larger scale and longer observation period, more neurologicscales scales, control group would be required.

Object : The purpose of this study is to observe the changes of symptoms and signs diagnosised by differentiation of the pattern identification in stroke patients. Methods : Seventy subjects were recruited from patients with stroke within ten days of onset. We chose twenty-nine subjects diagnosised as same differentiation of the pattern identification, and who had at least on follow up session. We had investigated change of symptoms and signs diagnosised by differentiation of the pattern identification. Results : There were five symptoms and signs (thick fur, dry fur, difficult defecation, heat vexation and aversion to heat, normal pulse) and ten (heavy-headedness, frequency of defecation, hard defecation, feel heavy, slippery pulsem, dry mouth, bitter taste in the mouth, feel lazy, look lazy) symptoms and signs that were statistically significant improved in fire-heat patterns and dampness-phlegm pattern, respectively. Conclusions : This study provides evidence that collaborative treatment is effective in improving some symptoms and signs in acute ischemic stroke patients diagnosised by fire-heat pattern and dampness-phlegm pattern. Further studies with larger scale, longer observation period would be required.

Neuropsychiatric disorder is one of the symptoms developed after traumatic brain injury. The main symptoms of neuropsychiatric disorder are delirium, anxiety, depression and so on. We treated the patient with herbal medicine, acupuncture, moxibustion and other appropriate traditional Korean medical treatment. The patient only appeared neuropsychiatric disorder, not another neurologic symptoms. We consider it is the rare case in traditional Korean medicine hospital. So we report the patient with the review of literatures regarding neuropsychiatric disorder induced by traumatic brain injury.

■ Objectives This clinical study is to report the effect of Korean medicine on patient with chronic benign fasciculation syndrome on a both legs. ■ Methods A patient who suffered from involuntary fasciculation of Quadricepts femoris and Gastrocnemius was treated with herbal medicine Gami SSanghwa-Tang and Yang Eui Gongjin-dan, acupuncture and moxibustion. Degree of fasciculation and general condition was measured. ■ Results After taking Gami Ssanghwa-Tang and Yang Eui Gongjin-Dan, degree of fasciculation have reduced and patient's general condition improved. ■ Conclusion This clinical case study showed the effect of Korean medicine on symptom of fasciculation.
급성기 허혈성 뇌경색 환자들의 변증 분형과 위험 요인들간의 관련성 연구

Object : The purpose of this study was to evaluate the relationship between pattern identification (PI) and stroke risk factors, such as hypertension, diabetes mellitus, dyslipidemia, stroke history, obesity, abdominal obesity and metabolic syndrome. Methods : 46 patients with acute ischemic stroke were recruited from May 2012 to November, 2012. We analyzed the data of 32 patients, and pattern identification was identified by resident and specialist of Korean medicine. We analized patient's PI and risk factor by Fisher's exact test. Results : We found that Dampness-phlegm group was more related with patient's metabolic syndrome than non Dampness-Phlegm group. And Yin deficiency group had less relationship with patient's metabolic syndrome, obesity, abdominal obesity and dyslipidemia than non Yin deficiency group. Conclusions : According to the analysis, these results provide evidence for relationship between the Dampness-phlegm group, Yin deficiency and metabolic syndrome.

Objectives : The purpose of this study was to report the clinical characteristics of glossodynia patients and effects of traditional Korean treatment on glossodynia and to gather data in order to carry out future study. Methods : We surveyed 40 glossodynia patients visiting the Department of Korean Internal Medicine of Stroke and Neurological Disorders Center, Kyung Hee University Hospital at Gangdong and analyzed their glossodynia questionnaires, examination results, and progression of symptoms. Results : In Glossodynia patients the average age was 62.4 years old. Glossodynia was more prevalent in females than males (5.67:1). Red tongue color, white fur, and fissured tongue were observed most often. The average glossodynia duration was 36.1 months and the average glossodynia degree was VAS 5.6. The most commonly appearing symptom was burning sense, and dryness of tongue. In Korean medicine diagnosis, Yin, deficiency, Tae-eum was most commonly diagnosed. In herbal medicine, Gamichungsim-tang was the most frequently prescribed. In laboratory tests, zinc deficiency was the most common result. In heart rate variability (HRV), average SDNN was 34.03 and average LF/HF ratio was 2.11. Of 40 patients, 23 completed treatment and their pain was relieved from average VAS 5.52 to VAS 3.63. Pre-and post-treatment results showed a significant difference. The average duration of treatment was 34.91 days. Conclusions : The results of this study suggest that Korean medical approach and treatment for glossodynia could be effective. Further study should be conducted for more valuable information.

Central poststroke pain (CPSP) refers to pain resulting from a primary lesion or dysfunction of the central nervous system after stroke. CPSP has diverse sensory symptom and its pathophysiology is not well understood yet. We treated a patient with CPSP from cerebral hemorrhage with CS yakchim and evaluated pain severity using numerical rating scale(NRS). After 6 injections for 8 days, NRS decreased from 9 to 1. The patient discharged with no pain and there were no CPSP recurrence after discharge. We suggest that CS yakchim could be used for CPSP patients.

Objectives: Postural instability and gait difficulty(PIGD) can develop in all Parkinson`s disease patients, especially late in the disease course, but does not respond well to conventional dopamine treatment. This study aimed to report three cases of PIGD in Parkinson`s disease patients treated with Korean and Western medicine treatment. Methods: We used acupuncture, bee venom acupuncture, herbal medicine and moxibustion to treat patients during hospitalization. They continued Western medication and received rehabilitation treatment. We observed the changes of PIGD using the Unified Parkinson`s Disease Rating Scale(UPDRS). Results: After treatment, PIGD symptoms improved and UPDRS scores were decreased. Especially, scores of falling, walking, gait and postural stability subsections related with PIGD were decreased. In addition, general conditions of patients were improved. Conclusions: This study suggests that Korean medicine treatment could be effective in the treatment of PIGD in Parkinson`s disease patients.

Objectives: CS has been used as effective pain reducing pharmacoacupunture in clinical fields, however its constituents and physiological activity have not been yet certified by experiment. This experimental research was to analyze the component of CS Yakchim and prove its anti-oxidant and anti-inflammatory effect. Methods: Content of protein, glucose, crude fat and crude ash was calculated. Anti-oxidant effect was measured using 1,1-diphenyl-2-picryl hydrazyl(DPPH) free radical scavenging activity. Anti-inflammatory effect was measured with COX-2 kit and RAW 264.7 macrophage. the concentration of TNF-α and IL-6 was measured by Enzyme-linked immuno-sorbent assay. Results: CS Yakchim is consist of 0.17% of protein, 0.32% of glucose, 95.19% of crude fat and 0.32% of crude ash. CS showed better anti-oxidant effect than its control sample. CS COX-2 inhibiting capacity was better than its Indomethacin control sample. Inflammatory cytokine production was reduced but not at transcription level. Conclusions: These results showed that CS Yakchim is consist protein, glucose, crude fat and crude ash. CS Yakchim anti-oxidant and anti-inflammatory effect.
