구기지황탕 투여 후 호전된 경도인지장애 환자 1례에 대한 증례보고

Objective: Alzheimer's disease is characterized by progressive, irreversible brain damage and cognitive decline. Although the diagnosis and treatment of the prodromal symptoms of dementia are important, no treatment for mild cognitive impairment has been currently established. Herein, we report the case of an 80-year-old female patient with memory complaints treated with Gugijihwang-tang, a traditional Korean medicine herbal formula, as an add-on medication. Case Presentation: The patient was diagnosed with mild cognitive impairment based on clinical examinations using the Mini-Mental State Examination (MMSE), the Consortium to Establish a Registry for Alzheimer's Disease (CERAD), Activities of Daily Living (ADL) Scale, Global Deterioration (GDR) Scale, and Clinical Dementia Rating (CDR) Scale. She was treated with Gugijihwang-tang bis in die for 12 months while continuing her original medications, including 5-mg donepezil and 590-mg acetyl-l-carnitine. The MMSE score in the Korean Version of the CERAD Assessment Packet increased from 21 to 27 during the 12-month treatment period, and the CERAD 2 score increased from 33 to 62. The instrumental ADL scale score improved from 11 to 5. Other clinical examination results also showed improvement. The patient was satisfied and experienced no significant adverse events related to the Gugijihwang-tang treatment. Conclusion: This case suggests that Gugijihwang-tang could be considered as a treatment method for patients with mild cognitive impairment.

The existing flood runoff models, Complex Hydrograph and HEC-1, have some problems which do not properly represent runoff characteristics on the Korean paddy basin and their basin slopes. In this study, FAS(Flood Analysis System) was developed in order to supplement those problems, which was built calibrating runoff curve number for paddy basin and applying basin slopes to classify 5 levels. And also the FAS can synthesize the flood hydrographs of subbasin and analyze flood routing along a stream. To verify the applicability of the FAS, the computed flood hydrographs were compared with the observed hydrographs from the four watersheds. In the small basin smaller than 10 ㎢, the results of the FAS did not completely agree with the observed ones due to concentration time delay of paddy storage effect while in the medium and large size basin showed good agreements between the observed and computed ones. Therefore, it was concluded that the FAS could be applied for the flood analysis of Korean watershed which was characterized by paddy storage effect.

Background : It is very important to understand the drug interaction in total intravenous anesthesia. We conducted this study to find the effects of plasma fentabyl concentrations on intraoperative propofol requirements, emergence from anesthesia and relief of postoperative pain. Methods : Total 60, ASA physical status I-II patients who underwent spine fusion were studied. The patients were randomly assigned to four study groups according to the expected intraoperative plasma fentanyl concentration. Group I received an infusion of saline and Groups II, III and IV received fentanyl infusions to maintain the blood level at 1.5, 3.0 and 4.5 ng/ml, respectively. An infusion rate of propofol was adjusted to keep the mean arterial pressure (MAP) within 15% of the control value. Inspired nitrous oxide concentration was maintained at 67% throughout the surgical procedure. The following items were investigated in each group: 1) an average propofol infusion rate, 2) time to spontaneous eye opening and recovery of orientation (name, date and place) and 3) total dosage of fentanyl used for 24 hr after admission to post-anesthetic care unit. Results : Average propofol infusion rates were 10.1±2.5 (mean±SD), 7.5±1.2, 5.7±1.1, and 4.9±1.2 mg/kg/h, in Group I, II, III and IV, respectively. Groups receiving fentanyl infusion had significantly less administration rates (P< 0.01) than the group receiving saline infusion. Among the three fentanyl infusion groups, Group II (P< 0.01) had more than Groups III and IV. Time to spontaneous eye opening and the recovery of orientation were prolonged in the order of plasma fentanyl concentration. There was no significant difference between Groupss III and IV of the plasma fentanyl level of 3 and 4.5 ng/ml. Total amount of IV-PCA fentanyl during postop 24 h increased significantly in the reversed order of plasma fentanyl concentrations: 913.1±58.4, 553.4± 129, 222.7±73.4 and 135.1±69.5 mcg in Groups I, II, III and IV, respectively. Conclusion : These results confirmed that the addition of fentanyl infusion had analgesic ceiling effects, which in turn reduced the average propofol infusion rate to plasma fentanyl concentration level of 3 4.5 ng/ml. This had an indirect influence on the recovery of consciousness after surgery, but it did not show similar ceiling effect in postoperative analgesia. (Korean J Anesthesiol 1999; 37: S 1∼S 9)
분광타원법을 이용한 스퍼터된 Ta<sub>2</sub>O<sub>5</sub> 박막의 광학적 특성

본 연구에서는 RF 파워, 기판의 종류, 산소분압비의 다양한 제작조건으로 RF 마그네트론 스퍼터링법을 이용하여 $Ta_{2}O_{5}$ 박막을 제작하였다. 제작된 $Ta_{2}O_{5}$ 박막의 분석을 위해 위상변조방식의 분광타원계를 이용하여 타원상수를 $1.0{\sim}4.0eV$ 영역에 걸쳐 측정하였고, Tauc-Lorentz 분산관계식을 이용하여 박막의 두께와 광학상수를 분석한 결과 제작조건에 따른 광학상수의 크기와 분간형태의 변화가 나타났다. 또한 분산관계식에 의해 분석된 박막의 두께와 광학상수를 이용하여 얻은 투과율 스펙트럼을 UV-Vis 분광광도계에 의해 측정된 값과 비교하여 타원상수 분석을 통해 얻은 두께와 광학상수의 신뢰성을 확인하였다. $Ta_{2}O_{5}$ thin films were deposited by RF magnetron sputtering method under various RF power, substrates and oxygen partial pressure. Elliptic constants were measured by using a phase modulated spectroscopic ellipsometer and analyzed with the Tauc-Lorentz dispersion formula and best fit method in the range of 310$\sim$1239 nm. Also, transmittance spectra of the films were measured by UV -Vis spectrophotometer in the range of 300$\sim$1000 nm. From these data, thickness of $Ta_{2}O_{5}$ and surface layer were analyzed and changes of magnitude and shape of dispersion of optical constants according to fabricated conditions were measured. Also, to evaluate thickness and optical constants data analyzed by Tauc-Lorentz dispersion formula, the measured and analyzed transmittance spectra were compared. In result of the comparison, two spectra were in good agreement each other. Accordingly, it indicates that our ellipsometric analysis is valid.

In order to analyze quantitatively the diffraction of light from CD and DVD, in this study, the diffraction patterns from grating, CD and DVD were compared. The lattice distance of the samples was compared by the SEM measurements and the theoretical lattice distance was calculated by the measurements of the incident and diffraction angles. Also, the diffraction angle and the intensity of the 1st-order diffraction light, which was generated by an Ar-ion laser with various wavelength, were measured, and the results were quantitatively compared. in the comparison, superior diffraction was observed in the DVD with more shorter lattice distance and larger intensity of the diffraction light comparing with the grating and the CD.

Objectives: The Aim of this study was to investigate the clinical characteristics of male and female patients who have been suffered from headaches. The investigation was undertaken based on Qi-section(Methods of prescribing oriental herbal medicine) from DongEuiBoGam ( ). Methods: A statistical survey was conducted to compare the difference in clinical characteristics between male and female patients experiencing headaches. Oriental medical Diagnosis was used to classify all parients under the following categories; Qi-deficiency (氣虛), Jing-dificiency (精 ), Food-stagnation (食積), Damp-accumulation (痰飮), Qi-stagnation (氣滯). Patients were treated using acupuncture therapy and herbal medicines. Results: 1. The ratio between male and female patients was about 1:3. 2. It showed that periods of illness were longer in female patients than in male patients. 3. The total number of treatments received was higher in female patients than in male patients. 4. Statistically, most female patients had Metopodynia and Migraines(Rt.), while male patients suffered mostly from Laryngalgia. 5. Male patients had feelings of strain in the head and female patients had splitting headaches. 6. Musculo-skeletal System and General Symptoms were frequently observed in male patients. Digestive System and Nervous System were frequently observed in female patients. 7. Results from oriental medical Diagnosis showed that male patients mostly had Qi-deficiency, Jing-deficiency while the majority of female patients had Food-stagnation, Damp-accumulation, Qi-stagnation. 8. The recovery time for male patients was shorter than it was for female patients. The recurrence rate of the headaches were higher for female patients. Prognosis was better than in female patients in comparison to male patients. Conclusions: The statistical survey conducted was based on Qi-section of DongEuiBoGam. It had significant differences in clinical characteristics between male & femal patients.
