배경: 대동맥 근부를 보존하는 심장수술이 점차 늘어남에 따라 대동맥 근부의 해부학적 구조에 대한 중요성 또한 증가하고 있다. 본 연구는 외과적 관점에서 필요한 대동맥 근부를 한국인 해부학 실습용 시신에서 실측하여 대동맥 근부의 진단 및 수술적 처치의 기준으로 하고자 한다. 대상 및 방법: 사인이 심혈관 질환이 아닌 시신 62구 중 대동맥 판막이 쌍엽인 1구, 기술적 오류로 대동맥 근부가 손상된 4구, 고정기간 중 변형이 심한 10구를 제외한 47예를 대상으로 하였다. 각 교련사이 간격, 대동맥동의 높이, Sinotubular junction 및 aortic annulus의 길이를 측정하여 비교하였다. 결과: 시신의 사망 나이는 평균 61.3이었으며 남성이 31, 여성이 16구였다. 각 교련사이 간격은 Right coronary sinus 20.73 2.23 mm, Non coronary sinus 19.34 2.08 mm, Left coronary sinus 18.58 2.15 mm였다. 각 대동맥동의 높이는 Right coronary sinus 20.59 2.48, Non coronary sinus 18.61 2.26 mm, Left coronary sinus 17.95 19 mm였으며, 통계 분석에서 교련사이에 길이 및 대동맥동의 높이는 Right coronary sinus가 가장 컸으며, Non coronary sinus, Left coronary sinus 순으로 나타났다. Sinotubular junction의 길이는 70.73 5.94 mm, aortic annulus는 77.94 5.63 mm였으며 Sinotubular junction의 길이, 대동맥 판륜의 길이와 대동맥 판륜에 대한 Sinotubular junction의 길이의 비를 각각 나이와 비교 분석했을 때 상관관계가 없었다(p=0.920, p=0.111, p=0.073). 대동맥 판륜과 sinotubular junction의 기울기는 2.03o에서 7.77o (평균=4.90o)였다. 결론: 각 교련 사이 간격 및 대동맥동의 높이는 non-parametric ANOVA test의 결과 Right coronary sinus, Non coronary sinus, Left coronary sinus의 순으로 유의한 크기차이를 보였다(p=0.00). Sinotubular junction의 둘레 길이는 대동맥 판륜의 길이보다 9.3% 작았고 sinotubular junction이 aortic annulus에 대해 좌후측으로 기울어져 있었다.
Developing a Dementia Platform Databank Using Multiple Existing Cohorts

This study was conducted as a pilot project to evaluate the feasibility of building an integrate dementia platform converging preexisting dementia cohorts from several variable levels. The following four cohorts were used to develop this pilot platform: 1) Clinical Research Center for Dementia of South Korea (CREDOS), 2) Korean Brain Aging Study for Early Diagnosis and Prediction of Alzheimer’s disease (K-BASE), 3) Environmental Pollution-induced Neurological Effects (EPINEF) study, and 4) a prospective registry in Dementia Platform Korea project (DPKR). A total of 29916 patients were included in the platform with 348 integrated variables. Among participants, 13.9%, 31.5%, and 44.2% of patients had normal cognition, mild cognitive impairment, and dementia, respectively. The mean age was 72.4 years. Females accounted for 65.7% of all patients. Those with college or higher education and those without problems in reading or writing accounted for 12.3% and 46.8%, respectively. Marital status, cohabitation, family history of Parkinson’s disease, smoking and drinking status, physical activity, sleep status, and nutrition status had rates of missing information of 50% or more. Although individual cohorts were of the same domain and of high quality, we found there were several barriers to integrating individual cohorts, including variability in study variables and measurements. Although many researchers are trying to combine pre-existing cohorts, the process of integrating past data has not been easy. Therefore, it is necessary to establish a protocol with considerations for data integration at the cohort establishment stage.

Nafamostat mesilate (NM) is a serine protease inhibitor with anticoagulant and anti-inflammatory effects. NM has been used in Asia for anticoagulation during extracorporeal circulation in patients undergoing continuous renal replacement therapy and extra corporeal membrane oxygenation. Oxidative stress is an independent risk factor for atherosclerotic vascular disease and is associated with vascular endothelial function. We investigated whether NM could inhibit endothelial dysfunction induced by tumor necrosis factor-α (TNF-α ). Human umbilical vein endothelial cells (HUVECs) were treated with TNF-α for 24 h. The effects of NM on monocyte adhesion, vascular cell adhesion molecule-1 (VCAM-1) and intracellular adhesion molecule-1 (ICAM-1) protein expression, p38 mitogenactivated protein kinase (MAPK) activation, and intracellular superoxide production were then examined. NM (0.01∼100 μ g/mL) did not affect HUVEC viability; however, it inhibited the increases in reactive oxygen species (ROS) production and p66shc expression elicited by TNF-α (3 ng/mL), and it dose dependently prevented the TNF-α -induced upregulation of endothelial VCAM-1 and ICAM-1. In addition, it mitigated TNF-α -induced p38 MAPK phosphorylation and the adhesion of U937 monocytes. These data suggest that NM mitigates TNF-α -induced monocyte adhesion and the expression of endothelial cell adhesion molecules, and that the anti-adhesive effect of NM is mediated through the inhibition of p66shc, ROS production, and p38 MAPK activation.

폐동맥 슬링은 좌 폐동맥이 우 폐동맥으로부터 이상 기시하여 기관의 후방으로 주행하는 혈관기형으로 주로 기관협착에 의한 호흡기 증상을 일으키는 매우 드문 선천성 질환이다. 심혈관 기형이나 기관의 기형과 동반되는 경우가 많고, 드물게 호흡기 증상 없이 청소년기나 성인에서 우연히 발견되는 경우도 있다. 본 증례는 14세 환아에서 우연히 발견된 이중 폐동맥 슬링(double pulmonary artery sling)을 경험하고 수술 치료하였기에 보고하는 바이다.
스팀을 이용한 중국산 신화 석탄 촤 가스화 반응에 관한 연구

In this study, carbon conversion was measured using an electronic mass balance. In a lab scale furnace, each coal sample was pyrolyzed in a nitrogen environment and became coal char, which was then gasified with steam under isothermal conditions. The reactivity of coal char was investigated at various temperatures and steam concentrations. The VRM(volume reaction model), SCM(shrinking core model), and RPM(random pore model) were used to interpret experimental data. For each model the activation energy(Ea), pre-exponential factor (A), and reaction order(n) of the coal char-steam reaction were determined by applying the Arrhenius equation into the data obtained with thermo-gravimetric analysis(TGA). According to this study, it was found that experimental data agreed better with the VRM and SCM for 1,000 and 1,100℃, and the RPM for 1,200 and 1,300℃. The reactivity of chars increased with the increase of gasification temperature. The structure parameter(Ψ) of the surface area for the RPM was obtained.
자동차 에어컨 시스템에서 이중관 열교환기가 성능에 미치는 영향에 관한 실험적 연구

Mobile air –conditioning system efficiency become more important to improve fuel efficiency and reduce green housegas emissions, so internal heat exchanger(IHX) applications become necessary for R134a refrigerant air-conditioning system as well as anR1234yfrefrigerant air-conditioner. In this study, R134arefrigerant air-conditioning system was investigated with three different type of coaxial IHX. The experimental results showed three different types of IHX havethe similar cooling capacity, but spiral type IHX shows slightly better coefficient of performance(COP) than other type of coaxial type IHXs. Type 3 IHX increas COP about 0.5~1.5% and 0.9~1.0% than Type 1 IHX and Type 2 IHX respectively.

It is important to develop an evaluation technique for design and performance optimization of coal gasifiers using a numerical simulation model, and to confirm the validity of the model. The coal gasification process of an entrained-flow gasifier for integrated coal gasification combined cycle(IGCC) plant, so called as Shell type gasifier, was numerically predicted with a commercial code, FLUENT. By classifying the complicated process into several simplified stages of slurry evaporation, coal devolatilization, and chemical reactions coupled with turbulent flow and heat transfer, a comprehensive numerical model was constructed to simulate the coal gasification process. In this work, we have used Finite rate/eddy dissipation model to have a modeling of chemical/flow interaction. In this model, concentrations of CO and H₂ were calculated for various oxygen to coal ratios(0.29~0.87) at steam to coal ratio, 0.11, and length to diameter ratio(2~4) at oxygen to coal ratios, 0.72, and steam to coal ratio, 0.11. The results showed the operating point for the maximum gasifier efficiency was the O₂/coal ratio, about 0.5, and L/D ratio that was similar from 2 to 4.
