전기자극에 의해 유발된 통증에 대한 말초 전침자극의 진통효과

The present experiment was performed in 35 normal male volunteers to evaluate the effect of electroacupuncture on the human nociception more clearly and to demonstrate the effect of different parameters of electrical stimulation through acupuncture needles. The threshold of the pain(Tp) and the nociceptive flexion reflex(Tr), the threshold for intolerable pain(Tip) and that for obtaining maximal reflex response(Tmr) were studied before and after electroacupuncture performed on the acupoints of tsusanli and hsuanchung. 1) For the pricking pain, electroacupuncture with intermittent stimulation induced a significant decrease in Tp which recovered after removal of the needles. There was no significant change in other thresholds. 2) For the dull pain, electroacupuncture with intermittent stimulation produced a significant increase in Tp which continued after removal of the needles. But there was no signifcant change in Tip. Electroacupuncture with continuous stimulation induced a slight increase in Tp. 3) After resting without electroacupuncture, Tp and Tip of the dull pain were slightly decreased. These results suggest that electroacupuncture had no significant analgesic effect on the pricking pain induced by electrical stimulation of the foot skin. However, electroacupuncture with intermittent stimulation had significant analgesic effect on the weak dull pain and it had slightly greater analgesic effect than electroacupuncture with continuous stimulation.

Analgesia resulting from stimulation of brain(stimulation-produced analgesia) has been extensively studied. It has been shown that the anterior pretectal nucleus(APT) is involved in somatosensory motor integration, although the pretectal complex is traditionally known to be involved in visual information processing. Stimulation of the APT inhibits tail-flick and jaw-opening reflexes and produces antinociception. But descending modulatory circutis involved in the antinociceptive effects of APT stimulation are not clear. Male Spraue Dawley rats were subjected as experimental animals. Each rat was anesthetized with mixture of α-chloralose and urethane and placed in a stereotaxic frame. The jaw-opening reflex(JOR) which was elicited by electrical stimulation of the upper lip was monitored by recording the evoked digastric muscle activity. Electrolytic lesions of the ventral medulla(VM) at the level of the inferior olivary nuclei(IO) or microinjections of lidocaine into the VM significantly reduced the APT-induced inhibition of the JOR. Electrical stimulation of the VM at the level of the IO or microinjection of glutamate into the VM significantly inhibited the JOR. These findings suggest that the inhibitory effects of APT stimulation on the JOR were mediated in part by a relay station in the VM at the level of the IO.
특발성 만성 타액선염 진단에 있어서 자기공명 타액선 조영술의 유용성

Background and Objectives The symptoms of salivary diseasess are often nonspecific, and a variety of investigative methods can be employed. Conventional sialography, which is still widely used for diagnosis of salivary ductal pathologies, has the withdrawback of invasiveness and radiation exposure, and thus ultrasound and magnetic resonance (MR) sialography can replace the conventional tools. This study was performed to evaluate the usefulness of MR sialography for the diagnosis of idiopathic chronic sialadenitis. Subjects and Method From November 2013 to June 2014, we have retrospectively analyzed 26 patients who have had swelling and pain of salivary glands and undergone MR sialography for further diagnosis of the idiopathic salivary obstructive symptom. We analyzed the symptom scores, salivary flow rate (SFR) and parameters of salivary gland scintigraphy. Then we evaluated correlation among MR sialography findings (duct visualization, grade of stenosis at main duct, degree of sialectasis and glandular volume size). Results Among the 26 patients, stenosis of salivary duct was observed in 14 patients (53.8%), chronic sialadenitis without stenosis in 6 patients (23.1%), Sjögren’s syndrome in 3 patients (11.5%), Juvenile reccutent parotitis in 1 patient (3.8%), and 2 patients were norma (7.7%). The degree of sialectasis was significantly correlated with Tmin (time interval, in minutes, from stimulation to minimum count), maximum secretion (p<0.05), and glandular volume size was also significantly correlated with unstimulated SFR (p<0.05). But others did not show any significant correlations. From these findings, we report three cases that were useful to diagnose the gland disease using MR sialography. Conclusion Resutls show that MR sialogarphy indirectly reflects the salivary gland function. Therefore MR sialography can be helpful when the differential diagnosis of idiopathic chronic sialadenitis is difficult with conventional tools.
