        방광의 조영 증강 이중시기 나선식 CT

        Purpose : To determine optimal scan time for early phase of two-phase spiral CT and to evaluate is usefulness in the detection and assessment of extension of urinary bladder lesions.Materials and Methods : In four normal adults, we performed dynamic scanning and obtained time-density curves for internal and external iliac arteries and veins, and the wall of the urinary bladder. Sixty patients with 68 lesions of the urinary bladder or prostate underwent precontrast and two-phase spiral CT scanning. After injection of 100ml of noninonic contrast material, images for the early and delayed phases were obtained at 60 seconds 5 minutes, respectively. We measured CT H.U. of the wall, the lesion, and lumen of urinary bladder as seen on axial scanning, in each image in which the lesioin was best shown. For the detection of bladder lesions and assessment of their extension, precontrast, early-, and delayed phase images were compared.Results : Dynamic study of normal adults showed maximum enhancement of bladder wall between 60 and 100 seconds. The differene of CT H.U. between bladder wall and the lesion was greatest in the early phase. The best detection rate(98.5%) was seen during this phase, and for the detection of bladder lesion, this same phase was superior or equal (66/68, 97.1%) to the delayed phase. The precontrast image was also superior or equal (31/68, 45.6%) to that of the delayed phase. For the assessment of extension of bladder lesion, the early phase was superior (36/68. 52.9) to the delayed phase, and precontrast image was superior (1/68, 1.5%) to that of the delayed phase. For determining the stage of bladder cancer, the early phase was most accurate if the stages was below $B_2$ or D, while for stage C, the delayed phase was most accurate.Conclusion : In two-phase spiral CT scanning, we consider the optimal time for the early phase to be between 60 and 100 seconds after injection of contrast material. For the detection and assessment of extension of urinary bladder lseion, the early phase was superior to the late phase, and for evaluation of the ureter, the delayed phase was useful. The precontrast image was inferior to that of the delayed phase. We suggest that for the detectioin and assessment of extension of urinary bladder lesion without scanning of the precontrast image, two-phase spiral CT is reliable.

      • Trehalose의 노화 억제 효과

        시차주사열량기와 색도계를 이용하여 Trehalose의 가래떡 노화에 미치는 영향을 검토하였다. 가래떡의 노화정도는 저장온도와 무관하게 경시적으로 증가하였는데 Trehalose가 노화를 억제하는 것으로 나타났다. 특히 저온에서 노화가 실온보다 심하게 일어났으며 저장초기에 저장온도에 대한 영향이 큰 것으로 나타났다. 그리고 L*값은 실온저장(25℃)시 냉장저장(4℃)의 경우보다 시간에 따른 변화가 컸으며, 냉동보관시 a*값은 저장 1일째 까지는 변화가 없다가 5%를 첨가한 가래떡을 실온에서 저장한 경우에는 가장 낮은 a*값을 나타내었다. b*값의 경우에는 Trehalose 첨가량과 저장온도에 따라 크게 변하지 않았다. 색깔안정화의 효과는 Trehalose 첨가농도가 증가할수록 큰 것으로 나타났으며, 특히 냉장보관시에 효과가 두드러지는 것으로 관찰되었다. This studies were investigated to provide the influence of trehalose on retrogradation of Korean rice cake by differential scanning calorimeter and tristimulus values of CIE color Croma Meter. Having no connection with storage temperature, degree of retrogradation of Korean rice cake was increased with storage time. Degree of retrogradation of Korean rice cake was decreased in case of addition of trehalose to the Korean rice cake paste. Degree of retrogradation of Korean rice cake stored at low temperature(4℃) was higher than that of Korean rice cake stored at room temperature(25℃). In particular, degree of retrogradation of Korean rice cake was shown to be clearly influenced by storage temperature at the beginning of storage. The value of L of Korean rice cake at room temperature was indicated to be significantly changed compared with that of Korean rice cake at low temperature. The value of a of Korean rice cake was not changed until one day of storage, and shown to be minimized in storage at room temperature(25℃) in case of addition of 5% trehalose to the paste of Korean rice cake. The change of b value in general was not significantly observed even if storage temperature and amount of trehalose have been variable. However effects of trehalose in color stability of Korean rice cake was remarkably exhibited in storage at low temperature(4℃).

        사례 분석을 통한 국내 BIM 적용 문제점 및 대안 도출에 관한 연구

        건축물의 초고층화, 복합화, 비정형 및 생산성 증대, 지속가능성 등의 해소방안으로 BIM(Building Information Modeling)에 대한 관심도가 증가되었고, 국내에서도 BIM에 대한 관심이 높아져 건설관련업계에는 하루가 다르게 BIM 확산이 진행되고 있다. 따라서, BIM의 정의와 특징을 파악하고 국내와 해외의 사례를 검토하여 BIM의 성공적인 도입 방안에 대한 내용을 본 논문에서 분석하고자 하며, 아울러, 국내 BIM적용 사례를 비교분석 함으로써 국내 BIM적용의 문제점과 해결 방안을 모색하고자 한다. When faced with new projects, such as high-rise, complex, free-formed, and sustainable buildings, numerous participants in the building industry use Building Information Modeling (BIM) technology. In 2009, the use of BIM spread to more contractors than ever before. This paper aims to study the definition and characteristics of BIM, and to analyze Korean BIM application problems. Solutions for the application of BIM to the Korean Construction Industry suggested, by studying cases from developed counties and from Korea.

      • 경막외 통증조절 후 경막외 카테테르의 포착 -증례 보고-

        The insertion of an epidural catheter into the epidural space to control postoperative pain is very common, as the technique is safe, effective and easy to perform. The numbers of documented problems are remarkably few, particularly those encountered during removal of the catheter. A 65-year-old man with stomach cancer was scheduled for a subtotal gastrectomy. The patient requested epidural analgesia for postoperative pain control. The patient was placed in the right decubitus position, and a 17-gauge Arrow Tuohy needle was inserted midline at the T12-L1 vertebral interspace, with the epidural space located using the loss-of-resistance technique. A 19-gauge Arrow Flextip PlusTM epidural catheter was inserted and advanced 5 cm into the epidural space, without difficulty or resistance. Two days after the administration of epidural analgesia, when the effects of local analgesics and opioid had worn off, an attempt was made to remove the catheter in the sitting position. Increased pressure was applied, but the catheter stretched and the wire reinforcement within the catheter began to uncoil. The removal of the epidural catheter was stopped at this point. After three hours, the patient was placed in the right lateral decubitus position, with his legs flexed to his chest, and another attempt to remove the catheter was successful. Herein, a case of difficulty in removing an obstinate epidural catheter is reported.
