급성 전골수성 백혈병(APL)의 진단을 위한 면역표현형 검사 Panel에 대한 연구

배경 : 급성 전골수구성 백혈병(APL)은 전형적인 FAB-M3형과 비전형적인 소과립형 FAB-M3v로 나누어 진다. APL에 대한면역표현형 검사는 빠르고 정확한 진단에 필수적인 검사이다. 저자들은 1987년부터 APL의 진단을 위해 panel 검사를 도입하여424 서정욱김신규2003년까지 각 시기에 따라 panel을 발전시켜 왔다. 이에 각 panel의 결과를 기존의 FAB 분류법과 비교하여 그 유용성을 검증하고자 하였다. 방법 : APL의 면역학적 진단을 위해 각 연대별로 발전한 I, II, III 단계의 다음과 같은 panel들을 사용하였다. 즉, Panel I [HLADR(-), CD15(-)](1987-1991), Panel II [HLA-DR(-), CD13(+), CD33(+), CD14(-)](1992-1994), Panel III [HLA- DR(-), CD34(-), CD13(+), CD33(+), CD14(-)](1994-2003)이면서 림프구성 표지자는 음성인 것으로 하였다. 총 570명의 급성백혈병 환자에게 표준 FAB 분류법과 직접면역형광법으로 면역표현형 검사를 실시하여 그 결과를 비교하였다. 결과 : 저자들이 확립한 APL의 진단을 위한 면역표현형 panel검사는 AML-M3와 M3V에 동일하게 적용되며 진단기준을 HLADR(-), CD34(-), CD13 and/or CD33(+), CD14(-)이면서 림프구성 표지자(CD19, CD10, CD20, CD22, CD3, CD5 및 CD7)가 음성인 것으로 하였다. 이는 기존의 형태학적, 세포화학적 분류인 APL을 진단하는데 높은 민감도(100%)와 특이도 (99%)를 보여주었다. 결론 : 본 논문은 APL의 진단을 위한 면역표현형 검사 panel의 발전상을 보여주었으며 이로써 확립된 panel은 APL을 진단함에 있어 신속한 참고자료가 될 것으로 사료된다. Background : Acute promyelocytic leukemia (APL) has two subtypes, a typical French-American- British (FAB)-M3 type and an atypical FAB-M3v type described as microgranular variant, and mmunophenotyping is a rapid and accurate method for the diagnosis of APL. We tried to define immunological criteria for the diagnosis of APL in each different period from 1987 to 2003. The purpose of this study was to compare the discrimination capacity of several panels with FAB classification. Methods : We applied immunophenotyping panel I, II, and III for the diagnosis of APL in each of the following three different periods: Panel I [HLA-DR(-), CD15(-)] (1987-1991); Panel II [HLA-DR(-), CD13(+), CD33(+), CD14(-)] (1992-1994); and Panel III [HLA- DR(-), CD34(-), CD13(+), CD33(+), CD14(-)] (1994-2003) with negative lymphoid markers in all panels. Standard FAB classification and direct immunofluorescence method were applied to diagnosis in 570 cases of acute leukemia. Results : The immunophenotyping to identify FAB subtype AML-M3 and M3v was established as the panel of [HLA-DR (-), CD34 (-), CD13 and/or CD33 (+), CD14 (-)] with negative lymphoid markers (CD19, CD10, CD20, CD22, CD3, CD5, and CD7). The sensitivity and specificity of this panel for the diagnosis of APL was 100% and 99%, respectively. Conclusions : Our study demonstrates the evolution of immunophenotyping panel from a primitive to advanced design. This immunophenotyping panel can be a ‘‘quick reference’’ for the diagnosis of APL without extra effort. (Korean J Lab Med 2004; 24: 421-5)
메틸브로마이드 훈증제 작업자의 요 중 브로마이드 이온을 이용한 생물학적 노출평가

Objective: Methyl bromide (MB) fumigant has been shown to be fatal to human when was exposed. However, it were still used the significant amount in quarantine disinfection sites. The purposes of this study were to assess the MB exposure status and characteristics by fumigation-related workers and to provide supporting data for management plan. Methods: For this study, the three groups related to fumigation work were composed. A total of 107 workers were directly exposed, 20 field inspectors were indirect, and 20 general quarantines were not exposed. The urinary bromide ion concentrations in each group were analyzed by using HPLC/ICP-MS, and the working characteristics were identified using the structured questionnaire. Results: The urinary bromide ion concentration in the exposed group of fumigation workers was higher than the indirect and non-exposed groups. In the work characteristics of workers, there was a significant tendency to increase urinary bromide ion concentrations with higher fumigation work years (≤4 years: 2.84 (1.13-7.11) mg/g cr, >4-15 years: 5.36 (4.37- 6.57) mg/g cr, >15-37 years: 6.69 (5.27-8.49) mg/g cr, p=0.034). In the comparison of the average number of working days per month, the more working days, the higher the urinary bromide ion concentration was statistically significant (≤12.5 days: 2.59 (1.19-5.65) mg/g cr, >12.5-19.25 days: 5.46 (4.62-6.44) mg/g cr, >19.25-27.25 days: 7.93 (5.93-10.59) mg/g cr, p=0.002). Conclusion: This study was the first nationwide survey including biological monitoring in workers exposed to methyl bromide. The results of this study were expected to be used as a reference for workers' health rights in relation to fumigation, prevention of addiction accidents, and safe management plan.

Conventional methods for providing electrical wiring from the base joint to the distal joint and the end effector include a dress pack, a hollow joint, and a slip ring. However, finding one that enables predictable wiring shape and easy maintenance is challenging. In this paper, the use of geared rolling joints is proposed to provide stable electrical wiring to the end of the robot, and their mechanical characteristics are analyzed. Furthermore, a cable-driven prototype of a parallel four-bar linkage comprising the unit rolling joints with gears is manufactured to confirm that the wiring path and driving torque exhibit predictable repeatability even when the electrical wires are installed. Moreover, the proposed joint structure may have additional advantages of easy addition and modification of the wiring. Therefore, the proposed rolling joint structure should provide a new option for designing robot joints. .
