      • Impact of Interferon-Based Treatment on Quality of Life and Work Related Productivity from the Korean Cohort in the MOSAIC Study

        Aims: Chronic Hepatitis C Virus (HCV) infection increases the risk for progressive liver disease, hepatocellular carcinoma and negatively impacts the patient’s quality of life. HCV treatment is evolving with direct acting antivirals but IFN based therapy has been the standard of care for many years and remains available in some countries. The MOSAIC study aims to characterize patients with chronic HCV infection and assess the impact of IFN-containing treatment on health-related quality of life, work related productivity and health care utilization. Methods: MOSAIC is an international prospective multicenter observational study that has been conducted in 20 countries. Consecutive patients with chronic HCV infection were enrolled and those who initiated an IFN based regimen were prospectively followed for 48 weeks. We report results from the Korean cohort Results: 100 patients were enrolled: 86 were treatment naïve and 14 were treatment experienced. 33 patients initiated an IFN based regimen: 6 patients started IFN + RBV, 26 patients started Peg-IFN + RBV, none started Peg-IFN + RBV + DAA and 1 patient received other treatment. Among the treated cohort, demographic and disease characteristics were the following: the mean age was 54.5 years; 14 patients were male. 14 had minimal or no fibrosis, 2 portal fibrosis, 3 bridging fibrosis and 6 patients suffered from cirrhosis. HCV Genotype distribution was as follows: genotype 1: 11; genotype 2: 19 and genotype 3: 3. Table 1 describes the results at baseline and changes over 4, 12 and 48 weeks and end-of-treatment (EOT) for the quality of life and work productivity outcome measures (EQ-5D-5L, HCV-PRO and WPAI). Conclusions: Results from the Korean cohort of the MOSAIC study show a moderate trend for deterioration of health-related quality of life and work productivity associated with IFN based treatment for patients with chronic HCV infection during treatment period.         <P>An all‐solution‐processed flexible piezoelectric nanogenerator, composed of polycrystalline ZnO thin film and functional polymer layers such as P3HT/PCBM and PEDOT:PSS, generates energy through a mechanical rolling and muscle stretching system. On page 6022, Youn Sang Kim, Sang‐Woo Kim, and co‐workers show that this all‐solution‐processed nanogenerator is feasible as a piezoelectric patchable device and is promising for use in future energy harvesters such as wearable human patches and mobile electronics. </P>

      • The Results of Curative Radiotherapy for Carcinoma of Uterine Cervix

        One-hundred five patients with invasive carcinoma of the uterine cervix treated with curative radiotherapy from March 1983 through October 1989 at the Department of Therapeutic Radiology, Kang-Nam St. Mary's Hospital. Among them, 78 patients received radiotherapy alone and 42 patients treated with neoadjuvant chemotherapy followed by radiotherapy and 15 patients were lost to follow up. All patients had follow up from 2 to 106 months (median; 62 months). Age of the patients ranged from 32 to 79 years at presentation (median; 59 years). The overall 5-year survival rates was 50.8%, and the 5-year survival rates by stage ⅠB, ⅡA, ⅡB, ⅢA, ⅢB, ⅣA was 47.7%, 70.2%, 64.1%, 40.0%, 23.1%, 14.3%, respectively. The 5-year survival rates was noted 51.2% of radiotherapy alone and 50.4% of neoadjuvant chemotherapy followed by radiotherapy. The overall failure rate was 18.3% (22/120) including 11.7% (14/120) locoregional failure, 5.8% (7/120) distant metastasis and 0.8% (1/120) locoregional failure with distant metastasis. Treatment failure rates by the stages were 15% (3/20) in stage ⅠB, 10.5% (2/19) in stage ⅡA, 10.2% (5/49) in stage ⅡB, 20% (1/5) for stage ⅢA, 61.5% (8/13) in stage ⅢB, and 28.6% (4/14) in stage ⅣA. The overall complication rate was 34.2% (41/120), including wet desquamation 7.5% (9/120), diarrhea 6.7% (8/120), radiation proctitis 5.8% (7/120) in decreasing order. A multivariate analysis of factors influencing the survival showed patient age (P=0.0291), FIGO stage (P=0.0001), Karnofsky performance status (P=0.0043), initial hemoglobin level (P=0.0001), and intracavitary radiation (P=0.0004), but, no significancy in histology (P=0.29) and teatment method (P=0.87).

        The Results of Curative Radiotherapy for Carcinoma of Uterine cervix

        가톨릭의과대학 강남성모병원 치료방사선과에서 1983년 3월부터 1989년 10월까지 79개월 동안에 자궁경부암으로 근치적 방사선치료를 받았던 135명의 환자들 중에서 추적이 가능하였던 120명의 환자들을 대상으로 치료결과및 예후에 영향을 미치는 인자에 대하여 후향적 분석을 하였다. 방사선 단독으로 치료한 환자는 78명이었고 유도 화학요법을 방사선 치료전에 시행한 환자는 42명이었다. 대상 환자들의 추적 조사기간은,2개월에서 106개월이었고 중간 추적조사 기간은 62개월이었다. 환자들의 나이는 32세부터 79세까지의 분포를 보였다(중앙값, 59세). FIGO 병기별 분류에 의하면, IB 기가 20명 ($16.7{\%}$), IIA 기가 19명 ($15.8{\%}$), IIB기가 49명 ($40.8{\%}$), IIIA 기가 5명 ($4.2{\%}$), IIIB기가 13명 ($10.8{\%}$), IVA 기가 14명 ($11.7{\%}$)이었다. 전체환자의 5년 생존율은 $50.8{\%}$였다. 병기별 5년 생존율은 IB 기가 $47.7{\%}$ IIA 기가 $70.2{\%}$, IIB 기가 $64.1{\%}$, IIIA 기가 $40.0{\%}$, IIIB 기가 $23.1{\%}$, IVA 기가 $14.3{\%}$였다. 치료방법에 따른 5년 생존율은 방사선 단독으로 치료한 환자가 $51.2{\%}$였고, 유도화학요법을 방사선 치료전에 시행한 환자는 $54.0{\%}$였다. 치료후 재발은 22명 ($18.3{\%}$,)에서 관찰되었고, 이중 14명 ($11.7{\%}$)에서 국소재발이, 7명 ($5.8{\%}$)에서 원격전이가, 1명 ($0.8{\%}$)에서 국소재발과 원격전이가 함께 발생하였다. 그리고, 치료에 의한 합병증은 41명 ($34.2{\%}$)에서 관찰되었으며 9명 ($7.5{\%}$)에서 습낙설, 8명 ($7.5{\%}$)에서 설사, 7명 ($5.8{\%}$)에서 방사선 직장염의 순으로 발생하였다. 예후와 관련된 생존율에 영향을 주었던 인자로는 나이 (p<0.0291), 병기(p<0.0001), 전신상태(p<0.0041), 초기 혈색소 수치 (p<0.0001), 강내 조사(p<0.0004)였고, 조직학적 소견(p<0.29), 유도 화학요법과의 병행치료(p<0.87)는 통계학적으로 유의하지 않았다. According to FIGO classification, there were 20 ($16.7{\%}$) in stage IB, 19 ($15.8{\%}$) in stage IIA,49 ($40.8{\%}$) in stage IIB, 5 ($4.2{\%}$) in stage IIIA, 13 ($10.8{\%}$,) in stage IIIB,14 ($11.7{\%}$) in stage IVA. The pathological classification showed 96 ($80.0{\%}$) squamous cell carcinomas, 5 ($4.2{\%}$) adenocarcinomas and 19 ($15.8{\%}$) proven by cytology. The overall 5-year survival rates was $50.8{\%}$, and the 5-year survival rates by stage IB, IIA, IIB, IIIA, IIIB, IVA was $47.7{\%},\;70.2{\%},\;64.1{\%},\;40.0{\%},\;23.1{\%},\;14.3{\%}$, respectively.         Noncancer-Related Health Events and Mortality in Head and Neck Cancer Patients After Definitive Radiotherapy : A Prospective Study

        <▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P>The survival of patients with head and neck squamous cell carcinoma (HNSCC) can be affected not only by progression of the original cancer or occurrence of a second cancer but also by noncancer health event (NCHE). In this study, we evaluated the prognostic significance of early NCHEs in HNSCC patients after definitive radiotherapy (RT) or chemoradiotherapy (CRT).</P><P>The prospective study cohort comprised 190 HNSCC patients who underwent definitive RT (n = 75) or CRT (n = 115). An early NCHE was defined as an event requiring hospital readmission of the patient within 12 months after treatment. Univariate and multivariate analyses were performed to identify clinicopathologic factors associated with early NCHEs, and competing and all-cause mortalities.</P><P>Thirty-three patients suffered an NCHE (17.3%) and 8 succumbed to a competing cause of mortality (4.2%). Twenty-two (11.6%) patients had an early NCHE: respiratory (22.8%), cerebrovascular (13.7%), gastrointestinal (13.7%), and others (50.0%). In multivariate analysis, hypoalbuminemia (<I>P</I> = 0.022, hazard ratio [HR] = 3.66, 95% confidence interval [CI] = 1.21–11.1), chemotherapy (<I>P</I> = 0.047, HR = 3.02, 95% CI = 1.01–8.98), and tumor recurrence (<I>P</I> = 0.024, HR = 2.66, 95% CI = 1.14–6.22) were independent predictors of an early NCHE. Patients with early NCHEs were at high risk of competing mortality (<I>P</I> < 0.001, HR = 22.6, 95% CI = 4.21–121.00) and all-cause mortality (<I>P</I> = 0.002, HR = 4.44, 95% CI = 1.76–11.2).</P><P>Early NCHEs are a major contributor to competing and all-cause mortality in HNSCC patients receiving RT or CRT. The risk factors identified could be used to predict early NCHEs.</P></▼2>

        Use of lactic acid bacteria as a biological agent against the cyanobacterium Anabaena flos-aquae

        <P>In the present study, we assessed whether lactic acid bacteria (LAB) with antimicrobial activity could be used to effectively control Anabaena flos-aquae growth. In our screening of cyanobacteriacidal bacteria from 14 LAB strains belonging to 11 species, we selected six candidate strains that could lyse 90 % or more A. flos-aquae cells compared with a control. Of those, Lactobacillus paraplantarum KCTC 5045(T) had the strongest cyanobacteriacidal activity showing complete lysis of cyanobacterial cells at the initial densities of a parts per thousand yen10(4) cells mL(-1). A host range assay revealed that L. paraplantarum strongly inhibited A. flos-aquae, Anabaena crassa, Stephanodiscus hantzschii, and Peridinium bipes but weakly inhibited or stimulated the growth of Scenedesmus actus, Pediastrum sp., Cyclotella meneghiniana, Coelastrum reticulatum, Chlamydomonas sp., and Microcystis aeruginosa. In the microcosm experiment using natural freshwater including abundant phytoplankton assemblage, moreover, the LAB under inoculation of about 10(5) cells mL(-1) could completely terminate the co-growth of A. flos-aquae, A. crassa, Anabaena circinalis, and other Anabaena spp. (over 10(4) cells mL(-1)) without a secondary bloom by another algal and cyanobacterial species. Our observation revealed that the cyanobacteriacidal bacterium releases two or more extracellular compounds possessing cyanobacteriacidal activity without cell-to-cell contact when the host cyanobacterium necessarily exits in surrounding water. Property study of cyanobacteriacidal substance revealed the release of heat stable and hydrophobic chemical substances that do not appear to be protein and peptide compound. Taken together, our results indicate that LAB may be a potential bio-agent for future use in controlling freshwater Anabaena blooms without causing the problem of pathogenicity if there is no secondary bloom by a resistant species to this bacterium.</P>

        Structural Health Monitoring Based on Electrical Impedance of a Carbon Nanotube Neuron

        <P>This paper introduces a new sensor design based on a carbon nanotube structural neuron for structural health monitoring applications. The carbon nanotube neuron is a thin and narrow polymer film sensor that is bonded or deposited onto a structure. The electrochemical impedance (resistance and capacitance) of the neuron changes due to deterioration of the structure where the neuron is located. A network of the long carbon nanotube neurons can form a structural neural system to provide large area coverage and an assurance of the operational health of a structure without the need for actuators and complex wave propagation analyses that are used with other SHM methods. The neural system can also reduce the cost of health monitoring by using biomimetic signal processing to minimize the number of channels of data acquisition needed to detect damage. The carbon nanotube neuron is lightweight and easily applied to the structural surface, and there is no stress concentration, no piezoelectrics, no amplifier, and no storage of high frequency waveforms. The carbon nanotube neuron is expected to find applications in detecting damage and corrosion in large complex structures including composite and metallic aircraft and rotorcraft, bridges, and almost any type of structure with almost no penalty to the structure.</P>

        Experimental Investigation of Pulsatility Effect on the Deformability and Hemolysis of Blood Cells

        <P>Abstract</P><P>In this study, we investigated the differences between pulsatile cardiopulmonary bypass (CPB) procedure and nonpulsatile CPB procedure in terms of their effects on hemolysis and deformability of red blood cells (RBCs) under various shear stress conditions. In order to research the effects on hemolysis and deformability, four parameters—free hemoglobin (fHb) concentration, normalized index of hemolysis (NIH), deformability index (DI) of RBCs, and elongation index of RBCs—have been deeply investigated. For these investigations, two randomly assigned adult mongrel dog groups—nonpulsatile group (NP, <I>n</I> = 6) and pulsatile group (P, <I>n</I> = 6)—were examined. According to our results, both types of perfusion did not show any statistical differences in terms of the concentrations of fHb as well as NIH. In addition, there were no significant differences in RBC deformability between perfusion types within an operation time of 3 h. Therefore, our studies suggest that pulsatile perfusion has no significant difference from nonpulsatile perfusion in terms of hemolysis and deformability of RBCs.</P>
