주요 식이섬유질원이 첨가된 식이가 노화 흰쥐의 장내효소 및 유해산물에 미치는 영향

한국에서 주로 섭취되고 있는 주요 식이섬유질원으로서 장내 유익균의 생육을 촉진한다고 알려진 사과, 미역, 쑥, 머위를 천연 형태로 건조분말화하여 식이내에 5%, 15%가 되게 첨가한 후 노화가 진행된 흰쥐에게 4주간 급여하였다. 그 결과 β-glucuronidase는 셀를로스군과 미역 15% 첨가군에서 가장 낮은 활성을 나타냈으며 β-glucuronidase의 경우는 쑥 15% 첨가군에서 유의적으로 현저한 증가를 보였다. 부패산물인 인돌 함량 변화는 쑥 섭취군에서 현저하게 낮은 경향을 보여 β-glucuronidase와는 다른 양상을 보였다. 수분함량은 미역섭취군과 쑥5%, 머위15% 섭취군에서 높았으며 pH는 쑥 15% 섭취군이 가장 낮은 값을 보여주었다. 쑥 15% 섭취군이 장내균총 조성을 개선시킬 수 있을 것으로 보고되어 그 유의성을 밝히고자 하였으나 본 실험에서는 부패산물, 유해효소등과 장내균총의 조성과의 뚜렷한 관계는 발견하지 못하였다. This study was performed to investigate the influence of various dietary fiber sources in Korea for activities of bacterial enzymes (β-glucosidase, β-glucuronidase) and amounts of putrefactive product (indole) in aged rats β-Glucosidase activity in the intestinal content was significantly lower in the sea mustard 15% group than in other groups whereas the activity of β-glucuronidase was higher in the mugwort 15% group than other experimental groups. The amount of indole and pH in the intestinal content of aged rats were significantly lower in mugwort groups than in other groups.

Background Patients with chronic obstructive pulmonary disease (COPD) are at high risk of lung cancer development. Due to the higher rates of complications from invasive procedures for the diagnosis of lung cancer, detecting circulating tumor DNA (ctDNA) might be useful in this population. Given the low sensitivity of ctDNA detection in early stage lung cancer, this study aimed to investigate the factors associated with ctDNA detection in COPD patients with lung cancer, to identify those who are most likely to benefit from ctDNA analysis. Methods A total of 174 patients with spirometry-defined COPD and newly diagnosed lung cancer were prospectively recruited from a single referral center. Plasma ctDNA were genotyped using targeted deep sequencing method. Demographics, comprehensive clinical variables regarding COPD and lung cancer were collected. We also measured emphysema index using chest computed tomography. Variables were analyzed using logistic regression. Results At least one ctDNA mutation were detected in 48(27.6%) patients (ranging from 6.8% in stage I to 62.5% in stage IV). In univariable logistic regression model, currents smoking, dyspnea (mMRC ≥ 2), lower emphysema index, C-reactive protein (CRP), tumor stage, and central tumor location were associated with ctDNA detection. In multivariable model, CRP and tumor stage remained significantly associated with ctDNA detection, whereas emphysema index could not meet the statistical significance (adjusted odds ratio 0.91, 95% confidence interval 0.81 - 1.00). Conclusion In addition to strong correlation with the tumor stages, we newly found an inverse relationship between emphysema and ctDNA detection in COPD patients with newly diagnosed lung cancer. While this might be attributable to impaired pulmonary vasculature in emphysematous lung, further confirmatory study is required to fully understand this signal.

Purpose: This study compared the performance for the prediction of three-level versus five level triage tool (Emergency Severity Index version 4, ESI) on the hospital outcome of emergency patients. Methods: This was an observational study that included all patients >15-year-of-age visiting an urban tertiary hospital emergency department (ED) in Korea from July 2007 to December 2008. We collected data from the electronic medical records, which included demographic factors, hospital outcome including admission to intensive care unit (ICU) and hospital mortality, and result of triage at arrival to ED. A three-level triage tool was used in the first year (July 2007 to June 2008), divided in four 3-month periods (threelevel phase). For 5 weeks, the three-level tool and ESI were used simultaneously (test phase). For the last 4 months, ESI was used for triage (five-level phase). We described the demographic findings of each study phase and compared the performance for the prediction of admission to ICU as well as hospital mortality, using the area under the receiver operating characteristic curve (AUC). Results: A total of 34,342 patients during three-level phase, 3,371 during the test phase, and 11,048 during five-level phase were involved. Demographic findings about gender, age, week and time of visiting to ED, use of ambulance service, injury or illness, and results after ED management were similar without significant difference. Proportion of admission to ICU and hospital mortality at ED and on the ward was 6.1% in period 1, 6.1% in period 2, and 5.6% in period 3 of the three-level phase, 5.8% in the test phase, and 5.8% in the five-level phase. The calculated AUC of the three periods in the three-level phase was 0.747 (95% Confidence Interval, CI, 0.729~0.765), 0.786 (95% CI, 0.769~0.804), and 0.786 (95% CI, 0.769~0.804). During the test phase, the AUC of the three-level was 0.820 (95% CI, 0.786~0.854) and that of the five-level was 0.842 (95% CI, 0.809~0.874). During five-level phase using ESI, AUC of ESI was 0.826 (95% CI, 0.809~0.844). Conclusion: ESI showed greater ability to predict hospital mortality than the three-level triage tool of emergency patients in a Korean ED, where the emergency care system and insurance coverage differs from North America.
Clinical Factors Related to Upper Extremity Shortness in Children With Hemiplegic Cerebral Palsy

목적 : 본 연구는 편마비성 뇌성마비 환아들의 편마비측 상지길이 단축의 정도를 평가하고, 길이 단축과 근경직도 및 운동기능과의 연관성을 알아보고자 하였다. 연구방법 : 30명의 편마비성 뇌성마비 환아(남아 13명, 여아 17명, 평균연령 6세 4개월)와 17 명의 대조군(남아 9명, 여아 8명, 평균연령 6세 2개월)을 대상으로 하였다. 상지의 상완골, 척골, 요골, 두 번째와 다섯 번째 중수골의 길이를 단순방사선 촬영을 통하여 길이 측정을 하였고, 건측(대조군에서는 우성측) 대비 편마비측(대조군에서는 비우성측)의 단축의 정도는 백분율로 나타내었다. 근경직도는 Modified Ashworth Scale (MAS)를 이용하였고, 상지의 운동기능은 Quality of Upper Extremity Skills Test (QUEST)를 사용하였다. 결과 : 대조군에 비하여 환자군에서는 편마비측 상지길이의 백분율이 의미있게 단축되어 있었다 (p<.05). 이러한 상지길이의 단축 정도와 근경직도의 증가 및 운동기능의 저하는 의미있는 연관성을 보였다(p<.05). 결론 : 편마비성 뇌성마비 환아에서 보이는 편마비측 상지길이의 단축은 근경직 및 운동기능과 의미있게 연관되어 있다. 추후 이러한 환아들에게서 근경직도의 완화 및 운동기능의 향상을 도모하는 재활치료 후 변화되는 상지길이의 단축 정도를 연구한다면 재활치료의 효과를 좀더 객관적으로 증명하고, 치료의 방법과 정도를 정립하는데 도움이 될 것이다. Objective : The purpose of this paper is to investigate the upper extremity shortness in people with hemiplegic cerebral palsy (HCP) compared with a normal control group, and evaluate the correlations between extremity shortness, muscle tone, and functional activity. Methods : This study included 30 children with HCP (13 males, 17 females; mean age, 6.33 yr) and 17 normal children as a control (9 males, 8 females; mean age, 6.17 yr). The length of each limb segment composed of the humerus, ulna, radius, and the second and fifth metacarpal bones was measured for both the patients and and control group using a radiograph. The discrepancy in side-to side length was calculated as a percentage. The Modified Ashworth Scale (MAS) was used in the evaluation of spasticity, and the Quality of Upper Extremity Skills Test (QUEST) was used in the evaluation of functional ability. Results : Upper extremity shortness differed significantly between the patients and control group (p<.05). The percentage differences in bone length correlated positively with the MAS point, and negatively with QUEST score in each limb segment (p<.05). Conclusion : The hemiplegic upper limb length discrepancy correlated with the functional activity and spasticity of the upper limb. Further studies to evaluate the change in upper extremity shortness after functional improvement and spasticity management will help to demonstrate the effect of rehabilitative treatment and determine the method and intensity of therapy required.

<▼1><P>The surface of proteins can be charged with zinc ions and the anomalous signals from these zinc ions can be used for structure determination of proteins.</P></▼1><▼2><P>Zinc is a suitable metal for anomalous dispersion phasing methods in protein crystallography. Structure determination using zinc anomalous scattering has been almost exclusively limited to proteins with intrinsically bound zinc(s). Here, it is reported that multiple zinc ions can easily be charged onto the surface of proteins with no intrinsic zinc-binding site by using zinc-containing solutions. Zn derivatization of protein surfaces appears to be a largely unnoticed but promising method of protein structure determination.</P></▼2>

<P>Mutations in the DJ-1 gene have been implicated in the autosomal recessive early onset parkinsonism. DJ-1 is a soluble dimeric protein with critical roles in response to oxidative stress and in neuronal maintenance. However, several lines of evidence suggest the existence of a nonfunctional aggregated form of DJ-1 in the brain of patients with some neurodegenerative diseases. Here, we show that inorganic phosphate, an important anion that exhibits elevated levels in patients with Parkinson disease, transforms DJ-1 into filamentous aggregates. According to the 2.4-A crystal structure, DJ-1 dimers are linearly stacked through P(i)-mediated interactions to form protofilaments, which are then bundled into a filamentous assembly.</P>
