        뇌졸중 환자의 간호성과지표 개발

        Purpose: This study was conducted to develop nursing outcome indicators based on nursing activities done for stroke patients. Method: focus group meeting and delphi technique, which consisted of clinical nurse experts, neurologist, and nursing professors. CVI(Index of Content Validity) and user validity test was performed. Result: 12 nursing diagnoses, 29 nursing interventions and nursing outcomes were identified. The former were from NIC and the latter were developed according to nursing interventions. They were verified by experts in focus group. 199 nursing activities were identified, 133 nursing outcome indicators were developed. In user validity, usefulness and usability were tested. Conclusion: This systemic approach of measuring nursing outcomes verified nurses' positive effects in changing patients health status and nursing contributions in the health care system as a profession.

      • The Computer Algebra Systems in Mathematics Education : A Review of Recent Research

        This study examines the potential role for Computer Algebra Systems(CAS) in such an educational context, particularly their potential to get students more engaged in the learning of mathematics as well as to reduce the "strangle hold" of prerequisite algebraic skills, in courses such as functions, introductory calculus. What is the present status of and near-future outlook for computer algebra in Korean secondary schools? The answer that arises from this review of available fact is: bright with promise, but still dim. Several recent developments have set the stage for an acceleration in the dissemination rate for CAS to high schools. However, this review of some past and present conditions indicates that many years will pass before computer algebra is impacting high school mathematics as much as hand-held calculators are doing now.

        황사 기간 동안에 어린이, 성인, 노인에서의 자각증상과 예방행동에 대한 조사연구

        Purpose: Recently there are scientific evidences for the health effects of Asian dust-storms. Particularly in 2002, the daily average of PM_(10) exceeded 600 and 700 ㎍/㎥ in March and April respectively. We examined the effects of Asian dust-storms on perceived symptoms and preventive behaviors among children, adults, and elders. Method: The subjects of this study were 425 children, 444 adults and 60 elders. A questionnaire survey was carried out on the children and the adults by teachers and on the elders by interviewers to obtain information on demographic variables, perceived symptoms and preventive behaviors, previous respiratory disease, environmental tobacco smoke, and proximity between the house and the road during Asian dust-storms. We analyzed using descriptive statistics, X_(2) test and multiple logistic regression models. After adjustment for potential confounders, we estimated the odds ratios and 95% confidence intervals of the children and the elders for perceived symptoms and preventive behaviors. Result: The elders had a significantly higher prevalence of respiratory symptoms than the other groups. The children had a significantly higher prevalence than the adults in getting URI (1.51), coughing (1.68), rhinorrhea (1.46), fever (2.39) and medication for allergy or asthma (1.90). The children had better behaviors than the elders in closing windows (3.57), taking less outdoor recreational activity (2.59) and staying home (2.60).

      • 보행훈련방법이 편마비 환자의 보행요소와 족저압에 미치는 영향

        이 연구는 뇌졸중으로 인한 편마비 환자에게 보행훈련방법을 달리하였을 때, 일반적인 지면과 트레드밀, 불안정면을 이용한 보행훈련이 보행요소와 족저압에 미치는 영향을 알아보기 위하여 편마비 환자 30명을 지면과 트레드밀, 불안정면 보행훈련군으로 무작위 배정하고 각 치료군들은 주 5회, 8주 간의 보행훈련을 실시하였다. 실험 전과 후, 보행요소를 알아보기 위하여 보행속도, 보장, 보각과 족저압을 측정하여 수집된 자료를 분석하여 다음과 같은 결과를 얻었다. 이 연구의 결과를 종합해 보면 보행훈련방법에 따라 지면 보행훈련은 보행속도와 족저압 전족부, 트레드밀 보행훈련군은 보행속도와 보각, 족저압 중족부, 불안정면 보행훈련군은 보행속도와 보장, 보각에서 유의한 증가를 보였다. 이러한 결과는 뇌졸중으로 인한 편마비 환자에게 보행훈련방법을 달리함에 따라 많은 영향을 끼치어, 환경의 변화와 다양한 과제에 대한 적절한 반응을 만들어 낼 수 있음을 의미하며, 기능 회복의 임상적 접근에서 치료 중재의 시점과 기간, 제공되는 환경, 과제의 선택에 대한 광범위한 연구가 필요할 것으로 생각되어진다. Purpose : This study was performed for effects of gait component and plantar foot pressure on gait training way needed in gait with hemiplegic patients caused by cerebrovascular accident. Methods : Three groups of adult hemiplegia(n=30) were allocated randomly in this study : group I(TGT : treadmill gait training), group II(USGT : unstable surface gait training) and group III(SGT : surface gait training). The gait training was provided to experimental groups for 8 weeks(5 times a week). Measurements of pre and post experiment were gait velocity, step length, toe out angle, foot plantar pressure which demonstrate temporal-spatial parameter of gait component. Results : The results of this study showed that in comparison of pre and post changes of gait training, SGT group has statistically significant differences in gait velocity, metatarsal area of foot plantar pressure, TGT group has statistically significant differences in gait velocity, toe out angle, midfoot of foot plantar pressure. USGT group has statistically significant differences in gait velocity, step length, toe out angle. Conclusion : These results mean gait training resulted by gait training way provides effective gait, with stroke, and therefore proper response about environmental changes and various tasks could be made.

      • Homocysteine-thiolactone 이 유발시키는 소 혈관내피세포의 손상효과

        The homocysteine-thiolactone, a reactive metabolic compound of homocysteine has been implicated in human cardiovascular diseases. The increasing of total plasma homocysteine concentration lead to homocystinuria, which may cause to elevate the concentration of homocysteine-thiolactone in the blood serum. It might be one of the risk factor which is responsible for the development of several vascular diseases. The present study is to demonstrate the harmful effect of homocysteine-thiolactone induced on calf pulmonary artery endothelial (CPAE) cells. CPAE were cultured in RPMI1640 medium supplemented with 10% FBS. CPAE were grown in humidified atmosphere of 5% C₂ in air at 3TC. Cells cultured with homocysteine-thiolactone at concentrations ranging from 0.1mM, lmM, 5mM and l0mM (dissolved in dH₂O). After incubation, cells were stained with MTT. Cell viability was evaluated under the UV-visible absorbance at 540nm. Cells viability was significantly decreased by increasing the concentration of homocysteine-thiolactone, Our data provide the evidence to understand the effect of homocysteine-thiolactone on the cardiovascular system.

        <P>Tuberculosis (TB) is a contagious disease that has been responsible for the death of one billion people in the last 200 years. Until now, the only vaccine approved for the prevention of TB is Bacillus Calmette-Guérin (BCG), which is prepared by attenuating <I>Mycobacterium bovis</I>. However, one of the limitations of BCG is that its preventive effect against pulmonary TB varies from person to person. Therefore, there arises a need for a new TB vaccine to replace or supplement BCG. In this review, we have summarized the findings of current clinical trials on preventive and therapeutic TB vaccine candidates. In addition, we have discussed a novel vaccination approach using the cell-based vaccine presenting early secretory antigenic target-6 (ESAT-6), which is a potent immunogenic antigen. The role of ESAT-6 in hosts has also been described.</P>

        Implantable cardioverter defibrillator therapy in pediatric and congenital heart disease patients: a single tertiary center experience in Korea

        Purpose: The use of implantable cardioverter defibrillators (ICDs) to prevent sudden cardiac death is increasing in children and adolescents. This study investigated the use of ICDs in children with congenital heart disease. Methods: This retrospective study was conducted on the clinical characteristics and effectiveness of ICD implantation at the department of pediatrics of a single tertiary center between 2007 and 2011. Results: Fifteen patients underwent ICD implantation. Their mean age at the time of implantation was $14.5{\pm}5.4$ years (range, 2 to 22 years). The follow-up duration was $28.9{\pm}20.4$ months. The cause of ICD implantation was cardiac arrest in 7, sustained ventricular tachycardia in 6, and syncope in 2 patients. The underlying disorders were as follows: ionic channelopathy in 6 patients (long QT type 3 in 4, catecholaminergic polymorphic ventricular tachycardia [CPVT] in 1, and J wave syndrome in 1), cardiomyopathy in 5 patients, and postoperative congenital heart disease in 4 patients. ICD coils were implanted in the pericardial space in 2 children (ages 2 and 6 years). Five patients received appropriate ICD shock therapy, and 2 patients received inappropriate shocks due to supraventricular tachycardia. During follow-up, 2 patients required lead dysfunction-related revision. One patient with CPVT suffered from an ICD storm that was resolved using sympathetic denervation surgery. Conclusion: The overall ICD outcome was acceptable in most pediatric patients. Early diagnosis and timely ICD implantation are recommended for preventing sudden death in high-risk children and patients with congenital heart disease.

        Effects of Combined Treatment with Thermotherapy and Electrical Stimulation on Musculoskeletal Pain Disorders: A Systematic Review

        Objective: This review aimed to investigate the effects of combined treatment with thermotherapy and electrical stimulation simultaneously in musculoskeletal pain disorders. Method: A systematic review was performed using electronic databases including PubMed and Embase. Following search terms were used: (simultaneous OR synchronous OR combined) AND (thermotherapy OR “thermal therapy” OR heat OR “hot pack” OR ultrasound OR microwave OR shortwave OR cold OR cryotherapy) AND (“electrical stimulation” OR electrotherapy OR TENS OR “interferential current”). Studies on the simultaneous application of thermotherapy and electrical stimulation, comparing with single physical modalities or massage were included. Results: After title and abstract exclusion, four articles were selected, according to the eligibility criteria. Combined treatment did not reveal a differential effect on pain reduction when compared to a single physical modality. However, combined treatment showed superior effects on functional aspects such as range of motion (ROM) and timed up-and-go score. Conclusion: This review suggests that combined treatment with thermotherapy and electrical stimulation simultaneously seems to have a superior effect on ROM, balance, and gait ability, but not pain reduction, in musculoskeletal pain disorders. However, the number of included studies in this review was small and study designs were heterogeneous. Therefore, further research is needed to confirm these findings.

        Atomic layer deposition of Y-stabilized ZrO<sub>2</sub> for advanced DRAM capacitors

        <P><B>Abstract</B></P> <P>With accelerated shrinking of integrated circuit, the fabrication of metal-insulator-metal (MIM) capacitors having a high capacitance density and low leakage current for dynamic random access memory (DRAM) has become a challenge. In this study, we investigated Y-stabilized ZrO<SUB>2</SUB> as a novel high-<I>k</I> material for DRAM capacitors. We used atomic layer deposition (ALD) to produce Y-stabilized ZrO<SUB>2</SUB>; this technique enables easy control of the Y concentration by changing the ratio of ZrO<SUB>2</SUB> to Y<SUB>2</SUB>O<SUB>3</SUB> ALD cycles. This technique is suitable for future DRAM capacitors, as it provides superior thickness controllability and conformality. Y doping into ZrO<SUB>2</SUB> increases the oxygen vacancy content in the films and transforms the ZrO<SUB>2</SUB> crystal structure from monoclinic to cubic. As a result, the dielectric constant is significantly increased from 19.1 to 30.2. Moreover, Y doping shifts the defect level into the conduction band rather than the energy bandgap, resulting in about 60 times lower leakage current density for Y-doped ZrO<SUB>2</SUB> compared to undoped ZrO<SUB>2</SUB>. It is notable that the dielectric properties and the leakage current density are simultaneously enhanced, indicating that Y-doped ZrO<SUB>2</SUB> is a promising candidate to satisfy the requirements of future DRAM capacitors.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Significant enhanced performance of DRAM capacitor with ALD Y-stabilized ZrO<SUB>2</SUB> (YSZ) films as a capacitor dielectric. </LI> <LI> Production of YSZ films using the supercycle process of ALD. </LI> <LI> The increase in dielectric constant up to ∼1.58 times due to the change of crystallinity. </LI> <LI> The decrease in leakage currents ∼60 times due to the change of bandgap structure. </LI> <LI> In-depth study chemical composition, crystal structure, energy gap, and energy level of defect state. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
