      • 한국인 심부전증 환자 심근에서의 인형 거대 세포 바이러스 감염

        Objectives: In order to evaluated the prevalence and the site of infection of cytomegalovirus in terminally failing heart, cytomegaloviral DNA was detected in the explanted hearts of transplantation recipients. Methods: DNA extractions were performed from explanted failing hearts(N=22) and normal hearts(N=5) and polymerase cain reactions(PCRs) were done for detection of late gene sequence coding pp150 glycoprotein. The products were confirmed by electrophoresis on the 1% agarose gel. n order to improve the detectability of cytomegaloviral genome, nested PCRs were executed with the primers designed for the original 607 bp products. In situ PCRs also were done with the samples which were confirmed as positive for CMV viral genome by nested PCRs. Results: All patients had IgG anti-cytomegalovirus antibody and did not have IgM anti-cytomegalovirus antibody. Cytomegaloviral genomes in myocardium were detected by polymerase chain reaction. The 607bp products by PCRs were found in both explanted failing heart(3 cases/22. 13.5%) and normal hearts( 1 case/5, 20.0%). In nested PCRs, 186bp products were found in both failing hearts(LV 4/22, LA 3/2, RV 4/22, RA 0/17) and normal hearts(LV 2/5, LA 1/4, RV 2/5, RA 2/5). These was no significant change in positivity of cytomegaloviral DNA genome between failing and normal hearts. Total positivity of cytomegaloviral genome in explanted hearts was 44.4% according to the nested PCR results. The positivity of cytomegaloviral DNA by n situ PCR was 33.3%(4/12), and the site of positive reaction was the nuclei of the myocardial cells. Conclusion: Cytomegalovirus was rarely observed in explanted hearts of terminal heart failure and nested PCR could enhance the sensitivity of cytomegaloviral genome detection. The result of the in situ PCR showed the site of the cytomegaloviral infection was nuclei of the myocardial cells. Cytomegalovirus, however, might have no direct causal relationship in the development of terminal heart failure.

      • Multiple myeloma 환자에서 Bortezomib치료 후 발생한 마비성 장폐색증 1례

        Bortezomib is the first clinically available proteasome inhibitor that is clinically effective in the treatment of multiple myeloma. A proteasome inhibitor acts through multiple mechanisms to arrest tumor growth, tumor spread, and angiogenesis. The main adverse effects of bortezomib are gastrointestinal symptoms, cytopenia, fatigue, and peripheral neuropathy. To date, severe paralytic ileus has not been reported as a toxic effect of bortezomib treatment in multiple myeloma. Bortezomib is a novel agent that has only been used clinically for 30 months, so a need exists to further evaluate its toxicity. We report a case of grade Ⅲ (NCI CTCAE v3.0) or grade Ⅳ(SWOG toxicity criteria) paralytic ileus in a 65-year-old man with relapsed multiple myeloma who underwent one cycle of single-agent bortezomib treatment scheduled in a 21-day cycle (1.3 mg/㎥ as a single i.v. bolus on days 1, 4, 8, and 11).

      • Trigger point를 利用한 口眼괘斜 患者 10例에 대한 臨床的 考察

        From July 1th 2001 to October 31th 2001, the Clinical investigation studies were carried out 10 cases of patient with Bell's palsy using Trigger point treatment group, who had been treated in Han-seung Oriental Medical Clinic. The results were like below : 1. The distribution of sex : Male were 4cases, female were 6case and the female to male ratio was 1.5 : 1. 2. The distribution of contributing factor : Unknown reason was most predominant 4 cases, and then were revealed to labor too hard, menttal stress(3 cases), trauma·common cold(1 case) in turn. 3. The distribution of perid to fall ill : Under a week was the most number 6cases, and then were revealed a week under two weeks(3 cases), over two weeks(1 case) in turn. 4. About period of treatment : The Trigger point treatment group is shorter than the Acupuncture treatment group. 5. The distribution of grade on admission in descending order : GrⅠ was the most number and then GrⅡ, GrⅢ in turn. 6. The distribution of clinical symptoms on adimission : Mastoid pain was the most number 9 cases and then lacrimal gland disorder 8 cases, dysgeusia 4 cases, hyperacusis 2 cases, the others 2 cases in turn. 7. About effect of treatment : The Trigger point treatment group is more excellent than the Acupuncture treatment group. 8. The more patient age is young, the more effect of treatment was excellent, had nothing to do with both groups. 9. The more perid to fall ill is short, the more effect of treatment was excellent, had nothing to do with both groups. 10. About period of improved frontal belly : The Trigger point treatment group is more excellent than the Acupuncture treatment group.

        Splenic marginal zone lymphoma is very rare B-cell lymphoma, characterized by an indolent clinical course. Clinical features are moderate-to severe splenomegaly, absolute lymphocytosis, and bone marrow intrasinusoidal infiltration of lymphocyte. We report the case of a 37-year-old male with SMZL. He complained of LUQ pain, who had a massive splenomegaly and moderate lymphocytosis in peripheral blood. Immunophenotyping findings and morphologic findings were consistent with SMZL. He underwent splenectomy and received chemotherapy with cyclophosphamide for eight months. Now, one year later after surgery he has a nearly normal blood count with no treatment.

        야뇨증 환아들의 심리사회적 특성에 대한 다기관 연구 : 행동 및 정서 문제를 중심으로

        Objectives : The aim of this study was to examine the behavioral and emotional problems associated with nocturnal enuresis in Korean children. Methods : Three hundred eighteen children with nocturnal enuresis, together with their parents, completed the Child Behavior Checklist (CBCL), Disruptive Behavior Disorder Scale according to DSM-IV (DBDS), Children's Depression Inventory (CDI), State-Trait Anxiety Inventory for Children (STAIC), and Piers-Harris Children's Self-Concept Scale (PHCSC). Ninety-three normal students were selected as the control group. Results : Compared to the normal control group, the mean scores with regard to the withdrawn, social problems, attention problems, delinquent behavior, aggressive behavior, externalizing problems and total problems profiles were significantly higher in the nocturnal enuresis group according to the CBCL results. The nocturnal enuresis group also scored significantly higher in the ADHD and ODD profiles of the DBDS. The nocturnal enuresis group was more depressed and anxious than the control group according to the results of the CDI and STAI. The mean score of the PHCSC was significantly lower in the nocturnal enuresis group when compared to the normal control group. Conclusion : The results of this study suggest that children with nocturnal enuresis in Korea have clinically relevant behavioral and emotional problems. The findings support the link between nocturnal enuresis and psychopathology in Korean children.

      • 넙치, Paralichthys olivaceus(TEMMINCK et SCHLEGEL)의 性分化와 生殖巢發達

        本 硏究는 넙치를 대상으로 孵化直後에서 成熟段階에 이르기까지 始原生殖細胞의 출현과 原始生殖巢의 形成, 雌雄性分化와 配偶者形成過程을 組織學的으로 조사하여, 이들의 性分化過程과 生殖巢 발달과정을 究明하였다. 1. 始原生殖細胞는 孵化後 3일 전장 3.1㎜된 前期仔漁에서 腸管과 脊索사이의 體壁中胚葉上皮에서 크기가 7.2∼7.4㎛인 2∼3개의 細胞가 纖維性 間充織에 묻혀 식별되었다. 2. 孵化 11일(전장 4.1㎜)의 後期仔魚期에 中腎管과 밀착된 體腔上皮에서 結體纖維性 生殖上皮가 분화되고, 이들 상피상에 섬유성 간충직에 쌓인 수 개의 生殖原細胞가 위치한다. 3. 孵化 22일경의 後期仔魚(전장 9.1㎜)에서 生殖原細胞들은 後復膜쪽으로 이동하며, 孵化後 43일된 着低期椎魚(전장 2.2㎝)에서는 有絲分裂중인 소수의 生殖原細胞와 풍부한 纖維性 間充織으로 구성된 線狀의 原始生殖巢가 발달한다. 4. 孵化後 46일된 椎魚(전장 2.2㎝)에서 原始生殖巢의 髓質層은 葉狀으로 分離하여 卵巢腔이 形成되고, 孵化後 78일 된 椎魚(전장 7.9㎝)에서는 卵巢小囊의 構造가 완성된다. 135일(전장 11.2㎝)이 지나면 난소소낭 구조사 성체와 유사하게 발달하고, 生殖上皮의 기능이 활발하게 되어 난원세포의 분열증식과 함께 初期卵母細胞가 출현한다. 5. 精巢의 初期 分化는 卵巢에 비해 늦어 孵化後 80일된 椎魚(전장 7.1㎝)에서 최초로 정소의 皮質層에서 精原細胞들의 分裂增殖이 식별되었고, 孵化後 135일된 개체(전장 11.9㎝)에서 精巢小囊이 형성되었다. 6. 原始生殖巢로부터 난소와 정소로 분화 발달하면서 각각 分布位置와 出現量이 다른 4종류의 體細胞性 間質細胞가 식별되었다. 7. 수컷은 만 13개월만에 完熟精子를 포함한 정소를 형성하고, 암컷은 만 2년만에 完熟卵母細胞를 가진 卵巢를 형성하였다. 8. 性分化에 따른 性比는 암수 3 : 1로 암컷의 出現率이 높았다. To study sex differentiation and process of gonadal development in the flounder, Paralichthys olivaceus, this research was conducted according to a histological method; the appearance of primordial germ cells, formation of primitive gonads, sex differentiation of female and male and process of gametogenesis from newly hatched larva to the mature stage of adult fish. The primordial germ cell is identified as 7.2∼7.4㎛ in cell size, among 2∼3cell which are buried under fibrous mesenchymal tissue between gut duct and notochord of pre-larva, at a total length of 3.1㎜, 3days after hatching. Germinal epithelium of fibrous connective tissue was differentiated in the epithelium of the coelom closed in the mesonephric duct, a few gonial cell were located in the germinal epithelium. covered with fibrous mesenchymal tissue of post larva, 11 days(TL 4.1㎜) after hatching. Gonial cells migrated in the retroperitonium of the post larva, with a total length of 9.1㎜, 22 days after hatching and a few mitotic gonial cells and primitive, lineshaped gonads were composed, with fibrous mesenchyme development, at the settlement stage with a total length of 2.2㎝, 43 days after hatching. The ovarian cavity is formed according to the medullary layer of the primitive delaminated gonad, in a juvenile with a total length of 2.2㎝, 46 days after hatching, and a total length of 7.9㎝, 78 days after hatching, construction of the ovarian sac was thus completed. A total length of 11.2㎝, 135 days after hatching is commensurate with construction of the ovarian sac, which is now similar to that of the adult, and so the mitotic division of oogonia and early oocytes appeared, with the germinal epithelium of functional development. Early differentiation belatedly compared the ovary with a juvenile testis whose total length was 7.1㎝, 80 days after hatching, with the testis of the cortical layer, spermatogonia of mitotic division were first recognizable. In a juvenile with a total length of 11.9㎝, also 135 days after hatching and testis sac was developed. Somatic interstitial cells of four different types were identified by distribution and appearance, accorking to the differentiated development of the testis and ovary in primitive gonad. The male appeared to have a testis that contained ripe spermatozoa, 13 months after hatching and the female appeared to have an ovary that contained ripe oocytes, 2years after hatching. According to sex differentiation, sex ratio is female 3 : male 1, the appearance of female was high.

      • 황놀래기, Pseudolabrus japonicus(HOUTTUYN)의 생식주기와 성전환

        Reproductive cycle and sex reversal of Pseudolabrus japonicu s were investigated histologically on the gonadal development, sex change of the gonad and studied by gonadosomatic index(GS1). color patterns. Sample were collected at the costal area of Ham duck in Cheju-do. from December, 1990 to November 1992. In male and female, GSI began to increase from August and reached it's malimum value in October, respectively. It began to decrease from December, the low temperature season, thereafter, maintained relatively low values until July of next year. The annual reproductive cycle of this species could be classified into four sucessive developmental stage : growing stage (August-September), mature stage (September-October), ripe and spent stage (October-December), degenerative and resting stage (December-July). The species is a diandric, protogynous hermaphrodite. and has two color patterns : red-yellow (A) and dark-green (B). A-fish were less than 14.5cm SL and consisted of females (92.7%), secondary males (3.6%), and fish with transitional gonad (3.6%). B-fish consited of secondary male (80.2%). primary male (5.5%). female (5.5%) and fish with transitional gonad (8.8%). In P.japonicus, individuals with hermaphrodic gonad were observed in 11.5-16.0 cm SL. Sex reversal of this species could be occured from 11.5 to 16.0cm SL.

      • Phenylephrine이 토끼 유두근의 수축력과 활동전압에 미치는 영향

        In order to clarify the receptor types and mechanisms underlying the positive inotropic effect of phenylephrie on the mammalian ventricular myocardium, the action potential, its first derivatives and isometric contraction of the rabbit papillary muscle were recorded using a free transducer and glass capillary microelectronics filled with 3M KCI, and results were analyzed. The results were as follows; l. In normal Tyrode solution, the contractile force was increased with increments of phenylephrine concentration and maximum effect was reached at the stimulation frequency of 1 Hz in 10^-5 M phenylephrine. 2. In 22 mM K^+-Tyrode solution, the maximum rate of rise (Vmax) of action potential, overshoot and the duration oction potential were significantly increased with increments of phenylephrine concentration (10^-6 ~ 10^-4 M). 3. The negative inotropic effect induced by verapamil was reversed by phenylephrine. 4. Phentolamine (3x10^-6 M) shifted the dose response curve for phenylephrine to the right more than propranolol (10^-6 M) did. The above results may be interpreted as follow; The positive inotropic effect of phenylephrine is caused by increase in slow inward current (Ca^2+influx into the myocardial cell), and is mediated through α- and β-adrengergic receptors in the rabbit pailary muscle
