        Objectives : To observe the effect of acupuncture treatment in chronic shoulder pain patients. Methods : 36 voluntary patients were randomly assigned to an acupuncture treatment group(E GrouP, n=18) and a control group(C GrouP, n=18). The E Group patients received acupuncture treatment on LI_(15), TE_(14), GB_(21) and Master Dong's acupuncture points, Shin-gwan and Gyun-joong, twice a week for four weeks. The C Group patients received no treatment. All patients in both groups were instructed to practise self exercise in their daily lives. Evaluations were made at baseline and after four weeks of study. The Constant Shoulder Assessment(CSA), Shoulder Pain and Disability Index(SPADI) and the patient's subjective pain was measured by Visual Analogue Scale(VAS). The obtained data was analyzed. Results '. The E Group showed significant(p<0.05) improvement in CSA, SPADI and VAS after four weeks of treatment. The C Group showed significant(p<0.05) improvement in CSA, but the change of SPADI and VAS was insignificant(P>0.05). CSA and SPADI of E Group significantly(p<0.05) improved compared to the C GrouP, but the difference of VAS change in the two groups was insignificant(p>0.05). Conclusions : Four weeks of acupuncture treatment significantly improved CSA, SPADI and VAS. The improvement of CSA and SPADI was significant(p<0.05) compared to untreated patients.

        Purpose: This study aims to test if the age of pregnant women affects the perception, experience, and attitude towards complementary alternative therapy (CAT), by investigating the perception, experience, and attitude of pregnant women under the age of 35, and above 35 (Advanced Maternal Age). The study will 1) find the general characteristics of pregnant women under and over the age of 35 2) compare the perception, behavior, and experience of pregnant women under and over the age of 35, and 3) find the comparison of the perception, behavior, and experience according to the general characteristics of pregnant women under and over the age of 35. Method: The subjects of this study are composed of 44 women under the age of 35 and 44 women above the age of 35, totaling 88 pregnant women visiting midwiferies, obstetrics and gynecology, and baby fairs in Seoul and Gyeong-Gi area. Data for this study was collected from November 28, 2015 to January 6, 2016 for 40 days. The collected data analyzed mean, standard deviation, t test, ANOVA, and chi-square by using SPSS 23.0. Result Results show that there is a significant difference in the perception, attitude, and experience of the two-sample groups in the use of CAT. The question of “what is most necessary when applying CAT in a hospital clinical environment” for the perception criteria came out with the results of t=9.980, p<.05. For the attitude, results showed that the answers of pregnant women under the age of 35 displayed a more positive attitude towards CAT. (t=2.421, p<.05) Lastly, in the experience section, pregnant women under the age of 35 showed more CAT use than the other sample group. Conclusion Results have shown that there is a difference in the perception, behavior, and experience between the two sample groups. Therefore further research on the efficacy and the adverse effects of CAT needs to be done.

        Background: Limited studies have been identifi ed about nursing and healthcare-associated pneumonia (NHCAP) requiring intensive care unit (ICU) admission. We evaluated the clinical and microbiological characteristics of NHCAP patients admitted to the ICU. Methods: NHCAP was categorized as four groups. Reviewed database included age, gender, comorbid diseases, laboratory data and microbiological Results: The severity of pneumonia was assessed by using the pneumonia severity index (PSI) and CURB-65. The 30-day mortality and duration of ICU stay were evaluated as endpoints Results: Total 428 patients (men, 67.1%) were reviewed. The mean age was 71.2 ± 11.9 year-old. Thirty-day mortality was 25.5%, and duration of ICU stay was 13.8 ± 13.3 days. Mortality (P = 0.349) had no signifi cant difference among four groups although duration of ICU stay (P = 0.008) was signifi cantly longer in group C and D. When we performed multivariate logistic analysis using signifi cant variables, PSI score (OR 1.015 95% CI 1.004-1.026, P = 0.009), serum HCO3- level (OR 0.954 95% CI 0.918-0.993, P = 0.020), duration of ICU stay (OR 0.971 95% CI 0.950-0.993, P = 0.010), multidrug-resistant (MDR) pathogens including ESBL-producing K. pneumoniae (OR 2.688 95% CI 1.237-5.840, P = 0.013) and MDR A. baumannii (OR 3.081 95% CI 1.504-6.311, P = 0.002) were signifi cantly associated with 30-day mortality. Conclusions: ESBL-producing K. pneumoniae and MDR A. baumannii were more often isolated and associated with mortality in NHCAP in ICU patients, therefore these MDR pathogens as well as PSI score should be considered as prognostic factors in NHCAP.

      • Anti-inflammatory Effects of Effective Microorganism Fermentation Substance (EM-S) on Atopic Dermatitis-like NC/Nga Mouse Model

        Atopic dermatitis (AD) is a chronically relapsing prurtttc inflammatory skin disease. The effective microorganism fermentation extract (EM-X) is an antioxidant cocktail derived from the fermentation of plant material with effective microorganisms, it shows an anti-inflammatory, anticancer, and antioxidant effects. To find new anti¬inflammatory products for skin inflammatory disease such as AD and contact dermatitis, we produced the effective microorganism fermentation substance (EM-S) by fermentation of medicinal plants with effective microorganisms including photosynthetic bacteria, lactic acid bacteria and yeast, screened the effects of EM-S on mouse models of acute and chronic inflammatory responses. The effects of EM-S treatment inhibited 12-O-tetradecanoylphorbol 13-actate (TPA)-induced ear edema in Balb/c mice. Topically applied EM-S significantly reduced ear thickness, severity score and histological grade in AD-like NC/Nga mouse model by the house dust mite antigen (Dermatophagoides farinse extract) sensitization. In addition, the serum IgE and tissue Th2 chemokine levels were significantly reduced by EM-S. These results demonstrate that topical application of EM-S may be improve the AD-like skin lesion by suppressing IgE and Th2 chemokines.

        Design of Low-Power Variable Gain Amplifier

        This paper presents a low-power variable gain amplifier (VGA) for the automotive radar. The proposed VGA comprises a gain control stage and an amplifier stage with active inductors. The gain control stage on the circuit is designed to be adjustable in 5 steps from 0 dB to 20 dB. The chip area can be reduced using an active inductor. This circuit also has a lower power dissipation, which reduces slow vibrations. The VGA is fabricated by TSMC 0.13 μm CMOS process. In comparison to conventional research results, the proposed VGA showed a lower power consumption of 17.88 mW, a low noise figure of less than 3.424 dB, and a relatively low active area of 0.034 mm².

      • Mer signaling increases the abundance of the transcription factor LXR to promote the resolution of acute sterile inflammation

        <P><B>Resolving inflammation with Mer</B></P><P>Unchecked inflammatory responses in the body can do more harm than good; thus, the body has mechanisms for resolving inflammation and restoring normal tissue function. Choi <I>et al</I>. found that mice with impaired signaling by the receptor tyrosine kinase Mer had exacerbated inflammation in response to acute sterile tissue damage, as well as decreased abundance of liver X receptor (LXR) transcription factors (which are implicated in inhibiting macrophage responses) in their macrophages, spleens, and lungs. Conversely, treating macrophages in culture with an agonist of Mer increased LXR abundance and activity. Furthermore, an LXR agonist reduced inflammation in mice even in the context of reduced Mer signaling. The finding that Mer-dependent increases in LXR abundance lead to the resolution of inflammation might be exploited therapeutically.</P><P>The receptor tyrosine kinase Mer plays a central role in inhibiting the inflammatory response of immune cells to pathogens. We aimed to understand the function of Mer signaling in the resolution of sterile inflammation in experiments with a Mer-neutralizing antibody or with Mer-deficient (<I>Mer</I><SUP>−/−</SUP>) mice in a model of sterile, zymosan-induced acute inflammation. We found that inhibition or deficiency of Mer enhanced local and systemic inflammatory responses. The exacerbated inflammatory responses induced by the lack of Mer signaling were associated with reduced abundance of the transcription factors liver X receptor α (LXRα) and LXRβ and decreased expression of their target genes in peritoneal macrophages, spleens, and lungs. Similarly, treatment of mice with a Mer/Fc fusion protein, which prevents the Mer ligand Gas6 (growth arrest–specific protein 6) from binding to Mer, exacerbated the inflammatory response and decreased the abundance of LXR. Coadministration of the LXR agonist T0901317 with the Mer-neutralizing antibody inhibited the aggravating effects of the antibody on inflammation in mice. In vitro exposure of RAW264.7 cells or primary peritoneal macrophages to Gas6 increased LXR abundance in an Akt-dependent manner. Thus, we have elucidated a previously uncharacterized pathway involved in the resolution of acute sterile inflammation: Enhanced Mer signaling during the recovery phase increases the abundance and activity of LXR to inactivate the inflammatory response in macrophages.</P>

      • Modified Reactive Hemophagocytic Syndrome Diagnostic Score as a Prognostic Factor in Lung Transplantation Patients

        Background Lung transplantation (LTX) has been established as a current therapy for patients with end-stage lung disease. Despite year of experience with LTX, the rate of mortality still remains high. Calcineurin inhibitors sometimes have induced reactive hemophagocytic syndrome and influenced on mortality. Objective of this study was to search prognostic factors using modified reactive hemophagocytic syndrome diagnostic score (mrHScore) associated with mortality after LTX. Methods We conducted a retrospective analysis of 100 patients who underwent LTX at Severance Hospital from October 2012 to July 2018. We classified into two groups by high (n=52, mrHScore ≥78.5) and low mrHScore (n=48, mrHScore <78.5). The optimal cut-off value of mrHScore for the prediction of postoperative overall survival was set at 78.5 using receiver operating characteristics analysis. Five biologics (i.e., triglyceride, ferritin, serum glutamic oxaloacetic transaminase, fibrinogen and cytopenia) were used in calculating mrHScore. We analyzed the basal characteristics and mortality rates in low and high mrHScore groups as prognostic factors of LTX outcomes. Results The primary lung disease was idiopathic pulmonary fibrosis (n=51, 51%), interstitial lung disease (n=14, 14%), and bronchiectasis (n=5, 5%). High mrHScore group showed significantly cytopenia, hyperferritinemia, hypertriglyceremia, high LDH levels and high C-reactive protein levels compare to those of low mrHScore group. On Kaplan-Meier analysis, mortality rate was significantly increased in high mrHScore group than in low mrHScore group (hazard ratio: 2.074, p=0.015). Multivariate regression analysis showed that high mrHScore was significantly associated with postoperative mortality, even after adjusting for other confounding factors. Conclusion Our Results show that high mrHScore provides clues for adverse outcome during follow up period in LTX patients. The mrHScore can be used to estimate prognosis of post LTX patients, and provide useful information for predicting postoperative mortality.

      • Development of the Composite Biologic Scoring as a Predictor of Longterm Prognosis of Lung Transplantation

        Background Survival after lung transplantation (LT) has steadily improved but its prognosis is still lower than other solid organ transplantation. It is important to predict prognosis in patients who underwent LT but currently there is no specific tool for prediction. Methods In the single-center retrospective study, we reviewed data from patients who underwent LT at Severance Hospital in South Korea from October 2012 to September 2020. Patients who underwent multi-organ transplantation (n=5), re-transplantation (n=5), age ≤18 years old (n=4) and early acute-phase patients within 3 months after LT (n=31) were excluded. A total of 249 LT patients were included for constructing the prediction model for prognosis called the Composite Biologic Scoring (CBS), using the numeric biologic markers around 3 months as a reference which were highest among observation period. Cox proportional hazards model was used to calculate the weight of each criterion included in the CBS, and the predictive accuracy and discriminative ability were measured by concordance index (C-index) and Kaplan Meier survival curve. Results Among 249 patients, mean age was 54.5±11.6 years, and 161 (64.7%) were men. Eight independent prognostic factors were identified and points were assigned to each variable based on Cox proportional hazards regression analysis including white blood cells, hemoglobin, platelet, ferritin, fibrinogen, aspartate aminotransferase, triglyceride, and lactate dehydrogenase (Fig. 1). The C-index for cox proportional hazards model was 0.809 and the calibration curves for the probability of 2, 3, and 5-year overall survival showed close approximation between nomogram prediction and actual observation. Three stratified risk groups according to CBS in post LT patients demonstrated significant distinction between Kaplan-Meier survival curves (Fig. 2 & 3; p-value<0.001). Conclusion The CBS is a useful prediction model for long-term prognosis of LT patients. Further validation study is needed to confirm.
