      • Clinical Study of Thalidomide Combined with Dexamethasone for the Treatment of Elderly Patients with Newly Diagnosed Multiple Myeloma

        Objective: To investigate the relationship between the efficacy and safety of different doses of thalidomide (Thal) plus dexamethasone (Dex) as the initial therapy in elderly patients with newly diagnosed multiple myeloma (MM). Methods: Clinical data of 28 elderly patients with newly diagnosed MM who underwent the TD regimen as the initial therapy were analyzed retrospectively. The patients were divided into two groups according to the maximal sustained dose of Thal: lower dose (group A) and higher dose (group B). The overall response rate (ORR), progression free survival (PFS), overall survival (OS), and adverse events (AES) were compared between the two groups. Results: A total of 28 patients were followed up with a median of 18 months. The ORR was 60.1%. The median response time and PFS were 2.0 and 17.0 months, respectively. The mean sustained dose of Thal in group B was significantly higher than group A (292.9 mg v 180.4 mg, P=0.01). There was no significantly difference in ORR (57.1% v 64.3%, P=1.00) and PFS (9.63months v 17.66 months, P=0.73) between groups A and B. During the follow up, only five patients died (<40%) and, therefore, median OS values were not available. It is estimated, however, that the mean survival time in the two groups was 35.6 and 33.4 months (P>0.05), respectively. All of the patients tolerated the treatment well. The incidence of AES in patients with a grading above 3 in group B was significantly higher than in group A (P=0.033). Conclusions: The TD regimen results in a high response rate and manageable AES as the initial therapy in elderly patients with MM. TD should be considered as the front line regimen for the treatment of elderly patients with MM in areas with financial constraints. The clinical response can be achieved at a low dose Thal with minimal toxicity.

        Redox-responsive PAEFc-b-PDMAEMA amphiphilic block copolymer self-assembly micelles: Physicochemical properties and anticancer drug controlled release

        Ferrocene-containing amphiphilic block copolymers, poly(2-acryloyloxyethyl ferrocenecarboxylate)-block-poly(2-(dimethylamino) ethyl methacrylate) (PAEFc-b-PDMAEMA), were synthesized via sequen-tial ATRP, and self-assembled into globular nanoscaled micelle aggregates. The copolymer micellesexhibited reversible redox on-off switch feature, which was mediated by using H2O2, KMnO4, NaClO andFeCl3 as a model of oxidants and ascorbic acid as a model of reductants. The micelle nanoparticles wereused to load and deliver anticancer drug, 10-hydroxycamptothecine,finding that the encapsulated drugwas rapidly delivered by selectively responding to redox environments in cancer cells. MTT assays wereperformed to uncover cytotoxicity of the developed copolymer micelles and potentiality used for cancertherapy.

        Volatilization of Nerolidol in Tea and Tea-soup

        Nerolidol is one of most important content in tea aroma, especially in Oolong tea. This experiment focuses on the impact of time and medium on nerolidol volatilization. The results of nerolidol volatilization with infusion time showed that the peak areas of nerolidol increased from 1 to 3 min, then decreased. The maximum peak area of nerolidol volatilization was 25630.78 at 3 min. Tea polyphenol had strong promotion impact on fragrant compound volatize, caffeine had limited impact on volatilization, while polysaccharide inhibited nerolidol volatilization.

        Lifetime data modelling and reliability analysis based on modified weibull extension distribution and Bayesian approach

        Satellite reliability has long been a critical issue in space industry. In-orbit lifetime data modelling and the corresponding reliability analysis are indispensable aspects for satellite reliability. However, limited by available in-orbit lifetime data, these two aspects have not been well studied. Aiming to mitigate this limitation, we conduct a coherent research on satellite reliability analysis based on an in-orbit database of 1584 satellites. A nonparametric reliability analysis is firstly implemented on the data set to identify the characteristics of failure rate. Then, the modified Weibull extension distribution is used to model the satellite reliability. Point estimation and interval estimation are carried out by utilizing a Bayesian method. Posterior distributions of model parameters are obtained and reserved for further reliability analysis of newly launched satellites. The results presented in this paper can provide helpful information to the space industry for design improvement and test planning of new satellites.

        Influence of the Incorporation of Aluminum on the Optical Properties of MCM-41

        Different ratios of aluminum were incorporated into MCM-41. PL measurements revealed a blue shift and an enhancement in intensity by 2 orders of magnitude as the Si : Al ratio was decreased to 14. By comparing the PL and the photoluminescence excitation (PLE) spectra of aluminosilicate MCM-41 to silica MCM-41 and to published reports on similar materials, we are able to identify the origin of the photoluminescence enhancement.

        High-mass Star Formation through Filamentary Collapse and Clump-fed Accretion in G22

        <P>How mass is accumulated from cloud-scale down to individual stars is a key open question in understanding highmass star formation. Here, we present the mass accumulation process in a hub-filament cloud G22 that is composed of four supercritical filaments. Velocity gradients detected along three filaments indicate that they are collapsing with a total mass infall rate of about 440M(circle dot) Myr(-1), suggesting the hub mass would be doubled in six free-fall times, adding up to similar to 2 Myr. A fraction of the masses in the central clumps C1 and C2 can be accounted for through large-scale filamentary collapse. Ubiquitous blue profiles in HCO+. (3-2) and (CO)-C-13. (3-2) spectra suggest a clump-scale collapse scenario in the most massive and densest clump C1. The estimated infall velocity and mass infall rate are 0.31 km s(-1) and 7.2 x. 10(-4)M(circle dot) yr(-1), respectively. In clump C1, a hot molecular core (SMA1) is revealed by the Submillimeter Array observations and an outflow-driving high-mass protostar is located at the center of SMA1. The mass of the protostar is estimated to be 11-15M(circle dot) and it is still growing with an accretion rate of 7 x. 10(-5)M(circle dot) yr(-1). The coexistent infall in filaments, clump C1, and the central hot core in G22 suggests that pre-assembled mass reservoirs (i.e., high-mass starless cores) may not be required to form high-mass stars. In the course of high-mass star formation, the central protostar, the core, and the clump can simultaneously grow in mass via core-fed/disk accretion, clump-fed accretion, and filamentary/cloud collapse.</P>

        <P><I>Clostridium difficile</I> is the major cause of hospital-acquired infectious diarrhea and colitis in developed countries. The pathogenicity of <I>C. difficile</I> is mainly mediated by the release of 2 large potent exotoxins, toxin A (TcdA) and toxin B (TcdB), both of which require neutralization to prevent disease occurrence. We have generated a novel chimeric protein, designated mTcd138, comprised of the glucosyltransferase and cysteine proteinase domains of TcdB and the receptor binding domain of TcdA and expressed it in <I>Bacillus megaterium</I>. To ensure that mTcd138 is atoxic, 2 point mutations were introduced to the glucosyltransferase domain of TcdB, which essentially eliminates toxicity of mTcd138. Parenteral immunizations of mice and hamsters with mTcd138 induced protective antibodies to both toxins and provided protection against infection with the hyper-virulent <I>C. difficile</I> strain UK6.</P>

        The Characteristics and Survival Outcomes in Patients Aged 70 Years and Older with Nasopharyngeal Carcinoma in the Intensity-Modulated Radiotherapy Era

        Purpose We aim to examine nasopharyngeal carcinoma (NPC) characteristics and survival outcomes in patients aged 70 years and older in the intensity-modulated radiotherapy (IMRT) era. Materials and Methods From 2006 to 2013, 126 non-metastatic NPC patients aged  70 years who were treated with IMRT +/ chemotherapy were included. Adult Comorbidity Evaluation 27 (ACE-27) was used to measure patient comorbidities. The overall survival (OS) and cancer-specific survival (CSS) were calculated with the Kaplan-Meier method, and differences were compared using the log-rank test. The Cox proportional hazards model was used to carry out multivariate analyses. Results For the entire group, only two patients (1.6%) presented stage I disease, and up to 84.1% patients had stage III-IVB disease. All patients had a comorbidity score of 0 in 24 (19.0%), 1 in 45 (35.7%), 2 in 42 (33.3%), and 3 in 15 (11.9%) patients. The main acute grade during radiotherapy was 3-4 adverse events consisting of mucositis (25.4%), bone marrow suppression (16.7%), and dermatitis (8.7%). After treatment, four patients (3.2%) developed temporal lobe injury. Five-year CSS and OS rates were 67.3% (95% confidence interval [CI], 58.6% to 77.4%) and 54.0% (95% CI, 45.6% to 63.9%), respectively. Five-year OS was significantly higher for ACE-27 score 0-1 than ACE-27 score 2-3 (72.9% and 39.9%, respectively; p < 0.001). Multivariate analyses showed ACE-27 score 0-1 was significantly associated with superior OS (hazard ratio [HR], 3.02; 95% CI, 1.64 to 5.55; p < 0.001). In addition, the rate of OS was higher for stage I-III than that of stage IV, with borderline significance (HR, 1.67; 95% CI, 0.99 to 2.82; p=0.053). But no significant advantage was observed in OS when chemotherapy was used (p > 0.05). Conclusion Our findings suggest IMRT +/– chemotherapy has a manageable toxicity and provides an acceptable survival in patients aged  70 years with NPC. ACE-27 score was significantly associated with survival outcomes in this group population.

        Circulating Low Absolute CD4+ T Cell Counts May Predict Poor Prognosis in Extranodal NK/T-Cell Lymphoma Patients Treating with Pegaspargase-Based Chemotherapy

        Purpose Extranodal natural killer/T-cell lymphoma, nasal type (ENKTL) is a rare subtype of non- Hodgkin lymphoma, and asparaginase-based regimens are the best first-line treatments. Data on the role of specific circulating lymphocyte subsets in the progression of ENKTL are limited. The aim of this study was to investigate the clinical correlation and distribution of circulating absolute CD4+ T-cell counts (ACD4Cs) in ENKTL. Materials and Methods We retrospectively searched medical records for 70 newly diagnosed ENKTL patients treated with pegaspargase-based regimens. Comparison of ACD4Cs as a continuous parameter in different groups was calculated. Univariate and multivariate analyses were used to assess prognostic factors for overall survival (OS) and progression-free survival (PFS). Results Stage III/IV, B symptoms, elevated lactate dehydrogenase, monocytopenia, high-intermediate and high risk International Prognostic Index (IPI) and Korean Prognostic Index (KPI), high risk Prognostic Index of Natural Killer Lymphoma (PINK), and lower lymphocytes were significantly associated with low ACD4C at diagnosis. With a median follow-up time of 32 months, patients who had an ACD4C < 0.30109/L had a worse OS. Median OS was 11 months and median PFS was 5 months in the low ACD4C cohort. There were significant differences in both OS and PFS between the two cohorts. Moreover, multivariate Cox analysis identified ACD4Cs as an independent predictor for OS and PFS. Conclusion Low ACD4Cs were associated with poorer survival and could act as a negative predictor for ENKTL patients treated with asparaginase-based regimens.

        Medium- and long-chain triglyceride propofol reduces the activity of acetyl-coenzyme A carboxylase in hepatic lipid metabolism in HepG2 and Huh7 cells

        Medium- and long-chain triglyceride (MCT/LCT) propofol is widely used as an intravenous anesthetic, especially in the intensive care unit. The present study aimed to assess whether MCT/LCT propofol is safe in the hyperlipidemic population for long-term use. Free fatty acids (FFAs) were used to establish high-fat stimulation of HepG2 and Huh7 cells. Subsequently, these cells were treated with propofol at the concentration of 0, 4, or 8 μg/ml for 24 and 48 h. The results indicated that the cell viability was notably decreased when the cells were stimulated with 2 mmol/L FFAs and treated with 12 μg/ml MCT/LCT propofol. Accordingly, we chose 2 mmol/L FFAs along with 4 and 8 μg/ml MCT/LCT propofol for the subsequent experiments. Four and 8 μg/ml MCT/LCT propofol inhibited FFA-induced lipid accumulation in the cells and significantly reversed acetyl coenzyme A carboxylase (ACC) activity. In addition, MCT/LCT propofol not only significantly promoted the phosphorylation of AMPK and ACC, but also reversed the FFA-induced decreased phosphorylation of AMPK and ACC. In conclusion, MCT/LCT propofol reverses the negative effects caused by FFAs in HepG2 and Huh7 cells, indicating that MCT/LCT propofol might positively regulate lipid metabolism.
