        Serum Insulin-Like Growth Factor Binding Protein 7 as a Potential Biomarker in the Diagnosis and Prognosis of Esophagogastric Junction Adenocarcinoma

        Background/Aims: Esophagogastric junction adenocarcinoma (EJA) is a malignant tumor associated with high morbidity and has attracted increasing attention due to a rising incidence and low survival rate. Pathological biopsy is the gold standard for diagnosis, but noninvasive and effective tests are lacking, resulting in diagnoses at advanced stages. This study explored the diagnostic value of insulin-like growth factor binding protein 7 (IGFBP7) in EJA. Methods: A total of 120 EJA patients and 88 normal controls were recruited, and their serum levels of IGFBP7 were measured by enzymelinked immunosorbent assay. Receiver operating characteristic (ROC) curve analysis was used to assess the diagnostic value, and Pearson chi-square analysis was used to evaluate the correlation between IGFBP7 and clinical parameters. Kaplan- Meier survival analysis was carried out to assess the effect of IGFBP7 on overall survival (OS). Results: The levels of IGFBP7 were higher in both early- and late-stage EJA patients than in normal controls (p<0.001). The area under the ROC curve for EJA patients was 0.794 (95% confidence interval [CI], 0.733 to 0.854), with a cutoff value of 2.716 ng/mL, a sensitivity of 63.3% (95% CI, 54.0% to 71.8%) and a specificity of 90.9% (95% CI, 82.4% to 95.7%). For the diagnosis of early-stage EJA, the same cutoff value and specificity were obtained, but the sensitivity of IGFBP7 was 54.3% (95% CI, 36.9% to 70.8%). Patients with low IGFBP7 protein expression had lower OS than those with high expression (p=0.034). The multivariate analysis showed that IGFBP7 is an independent prognostic factor for EJA (p=0.011). Conclusions: Serum IGFBP7 acts as a potential diagnostic and prognostic marker for EJA.

        Sensitivity of Lavender to Proton, Electron, and Gamma Radiation

        While ion beams are widely used in plant breeding, little is known about the sensitivity of Lavandula angustifolia (lavender) to ionizing radiation. To compare the biological effects of different types of ionizing radiation on the germination and survival rates of lavender, we exposed lavender seeds to gamma rays, 3 MeV electron beams, and 1.89 MeV proton ion beams. We observed that the seed germination rate decreased with increasing dosages of all three types of ionizing radiation. The malformation rate of lavender seedlings exposed to electron beams and gamma rays increased with increasing radiation dosage. By contrast, the effect of the accelerated proton beams on the malformation rate was negatively correlated with the dosage used. The survival rate of lavender seedlings exposed to the three types of ionizing radiation decreased in a dose-dependent manner. In addition, the survival rate of seedlings irradiated with proton and electron beams decreased more slowly than did that of seedlings irradiated with gamma rays. The half-lethal dose of gamma rays, electron beams, and proton beams was determined to be 48.1 Gy, 134.3 Gy, and 277.8 Gy, respectively, and the most suitable proton-ion energy for lavender seeds in terms of penetration depth was determined to be 5 MeV. These findings provide valuable information for the breeding of lavender by radiation mutation.

        Influence of selected host plants on biology of castor whitefly, Trialeurodes ricini (Hemiptera: Aleyrodidae)

        The development time, immature survivorship, immature size, tertiary sex ratio, pre-oviposition period, fecundity,and preferences of the castor whitefly, Trialeurodes ricini on four host plant species (castor, eggplant, cottonand green bean) were evaluated under laboratory conditions. Development time from egg to adult emergencewas the longest on cotton (33.3 days), the shortest on green bean (25.4 days), and intermediate on eggplant(28.5 days) and castor (28.3 days). The survival rate was the highest on castor (92.5%), followed by those ongreen bean (80.2%) and eggplant (73.8%), and the lowest on cotton (42.6%). The sex ratio was the highest on cotton(♀:♂ = 2.45:1.00), the lowest on eggplant (♀:♂ = 0.75:1.00), and intermediate on castor and green bean(♀:♂= 1.04:1.00). T. ricini immatures and adultswere generally largerwhen reared on castor and eggplant thanon other plants. The net reproduction rate (R0) was 57.656 females per female per generation, the generationtime(T) was 35.9 days, the intrinsic rate of increase (rm)was 0.1128 eggs per female per day, the gross reproductionrate (GRR) was 108.04 eggs per female per generation, the finite rate of increase (λ) was 1.1194 females perfemale per day, and the doubling time (DT) was 6.1149 days.         The Prognostic Impact of Heat Shock Proteins Expression in Patients with Esophageal Cancer: A Meta-Analysis

        Purpose: Heat shock proteins (HSPs) are highly conserved molecular chaperones. There are various studies that assess the prognosticvalue of HSPs in patients with esophageal cancer, but the conclusion remains controversial. This is the first meta-analysisstudy aiming to summarize the evidence on the suitability of HSPs to predict patients’ survival. Materials and Methods: Searching PubMed, Web of science and Medline until May 31, 2014, data were compared for overall survivalin patients with down-regulated HSPs level with those with up-regulated level. We conducted a meta-analysis of 9 studies(801 patients) that correlated HSPs levels with overall survival. Data were synthesized with hazard ratios (HRs). Results: The estimated risk of death was 2.93-fold greater in HSP27 negative patients than HSP27 positive patients [95% confidenceinterval (CI), 1.12–7.62]. When limited to esophageal squamous cell carcinoma (ESCC), the risk of death in HSP27 negativepatients seemed more significant (HR, 3.90; 95% CI, 2.35–6.49). Decreased expression of HSP70 was also associated with worsesurvival in esophageal cancer (HR, 2.83; 95% CI, 1.90–4.23) and, when limited to ESCC, HR was 3.21 (95% CI, 1.94–5.30). Data collected,however, were not sufficient to determine the prognostic value of HSP90 in patients with ESCC nor esophageal adenocarcinomas(EADC). Conclusion: In this meta-analysis, reduced HSP27 and HSP70 expressions were associated with poor survival in patients withesophageal cancer, especially esophageal squamous cell carcinoma.

      • Diagnostic value of Thyroglobulin Measurement with Fine-needle Aspiration Biopsy for Lymph Node Metastases in Patients with a History of Differentiated Thyroid Cancer

        Purpose: The aim of this study was to evaluate the diagnostic value of FNA-Tg for detecting lymph node metastases in patients with a history of differentiated thyroid cancer (DTC). Materials and Methods: A total of 58 patients with DTC diagnosis and evidence of single or multiple suspicious cervical lymph nodes were assessed. All underwent total or near-total thyroidectomy with (35 cases) or without (23 cases) radioiodine (RAI) ablation, followed by thyroid stimulating hormone (TSH) suppression therapy. A total of 68 lymph nodes were examined by ultrasound-guided fine needle aspiration (US-FNA) for both cytological examination and FNA-Tg measurement. Serum Tg and anti-thyroglobulin antibody (TgAb) levels were also measured. Diagnostic performance including sensitivity, specificity, accuracy, positive (PPV) and negative predictive value (NPV) of FNAC and FNA-Tg were calculated and compared. The Spearman's rank correlation coefficient was used to estimate the relationship between FNA-Tg and serum TgAb. Results: The FNA-Tg levels were significantly higher with DTC metastatic lymph nodes (median 927.7 ng/mL, interquartile range 602.9 ng/mL) than non-metastatic lymph nodes (median 0.1 ng/mL, interquartile range 0.4 ng/mL) (p<0.01). Considering 1.0 ng/mL as a threshold value for FNA-Tg, the sensitivity, specificity, accuracy, PPV and NPV of FNA-Tg were 95.7%, 95.5%, 95.6%, 97.8% and 91.3%, respectively. The sensitivity and accuracy of the combination of FNAC and FNA-Tg were significantly higher than that of FNAC alone (p<0.05). The diagnostic performance of FNA-Tg was not significantly different between cases with or without RAI ablation, and the serum TgAb levels did not interfere with FNA-Tg measurements. Conclusions: Measurement of FNA-Tg is useful. The combination of FNAC and FNA-Tg is more sensitive and accurate for detecting lymph node metastases in patients with a history of DTC than FNAC alone. Serum TgAbs appear to be irrelevant for measurement of FNA-Tg.

        Fishbone Oscillations in the Experimental Advanced Superconductivity Tokamak

        A fishbone oscillation was observed in the neutral beam injection plasma at Experimental Ad- vanced Superconductivity Tokamak (EAST). This m = 1/n = 1 (m, n: poloidal, toroidal mode numbers, respectively) typical internal kink mode travels in the ion-diamagnetism direction in the poloidal section with a rotation speed close to the ion diamagnetic drift frequency. A high thermal plasma beta and high amounts of energetic ions are necessary for the mode to develop. Fishbone oscillations can expel heavy impurities in the core, which favors sustaining a high-performance plasma. The born frequency of the fishbone oscillation is the ion diamagnetic drift frequency and the chirping down of the frequency during the initial growth phase is the result of a drop in ion- diamagnetic drift frequency. The excitation energy is thought to be due to the thermal plasma pressure gradient; however, the development of a fishbone oscillation is related to energetic ions.         Intravenous Tenecteplase for Acute Ischemic Stroke Within 4.5–24 Hours of Onset (ROSE-TNK): A Phase 2, Randomized, Multicenter Study

        Background and Purpose Intravenous tenecteplase (TNK) efficacy has not been well demonstrated in acute ischemic stroke (AIS) beyond 4.5 hours after onset. This study aimed to determine the effect of intravenous TNK for AIS within 4.5 to 24 hours of onset. Methods In this pilot trial, eligible AIS patients with diffusion-weighted imaging (DWI)-fluid attenuated inversion recovery (FLAIR) mismatch were randomly allocated to intravenous TNK (0.25 mg/kg) or standard care within 4.5–24 hours of onset. The primary endpoint was excellent functional outcome at 90 days (modified Rankin Scale [mRS] score of 0–1). The primary safety endpoint was symptomatic intracranial hemorrhage (sICH). Results Of the randomly assigned 80 patients, the primary endpoint occurred in 52.5% (21/40) of TNK group and 50.0% (20/40) of control group, with no significant difference (unadjusted odds ratio, 1.11; 95% confidence interval 0.46–2.66; <i>P</i>=0.82). More early neurological improvement occurred in TNK group than in control group (11 vs. 3, <i>P</i>=0.03), but no significant differences were found in other secondary endpoints, such as mRS 0–2 at 90 days, shift analysis of mRS at 90 days, and change in National Institutes of Health Stroke Scale score at 24 hours and 7 days. There were no cases of sICH in this trial; however, asymptomatic intracranial hemorrhage occurred in 3 of the 40 patients (7.5%) in the TNK group. Conclusion This phase 2, randomized, multicenter study suggests that intravenous TNK within 4.5–24 hours of onset may be safe and feasible in AIS patients with a DWI-FLAIR mismatch.
