        Multiple indices based drought analysis by using long term climatic variables over a part of Koel river basin, India

        The present study demonstrates changes in vegetation pattern and climatic variability in past years in the parts of Koel basin in Jharkhand state of India by considering the spatial climatic variability, NDVI anomaly and satellite based drought indices viz Vegetation Condition Index (VCI), Temperature Condition Index (TCI) and Vegetation Health Index (VHI). Least square statistical method has been used for assessment of long term climatic fluctuation of major four climatic parameters viz maximum temperature of summer season, minimum temperature of winter season, rainfall of monsoon season, and solar radiation of Rabi and kharif season. Analysis of climate extremes has been done for 26 locations in the study area and then interpolated spatially in Geographical Information System platform. Long term NDVI anomaly shows adverse effect of climate extreme in past 20 years in the study area. The climatic variability exhibits that average maximum temperature during 1979–2017 fluctuates with an increase of 0.50–0.81 C contrary to a decrease of 0.32–0.15 C in various parts of study area. Similarly rainfall fluctuates with a decrease of 26–90 mm contrary to an increase of 19–230 mm. Drought prone zones as delineated from spatial overlaying map of VCI, TCI and VHI indicated value from 23 to 55. Major part of the study area severely affected by drought facing water scarcity and mediocre vegetation condition. These areas need proper planning and soil moisture management to overcome the recurrent drought conditions perceived in upcoming years.

        Human Leukocyte Antigen-DQ Genotyping in Pediatric Celiac Disease

        Purpose: The purpose of this study was to determine the pattern of human leukocyte antigen (HLA)-DQ genotype in children diagnosed with celiac disease (CD) (biopsy proven), and to compare this with a control group; and secondarily, to correlate HLA genotypes with clinical profiles of CD. Methods: This cross-sectional comparative observational study included 26 controls and 52 patients diagnosed with CD who presented at Sir Padampat Mother and Child Health Institute, Jaipur, from May, 2017 to October, 2018. HLA DQ genotype was assessed for each patients and correlated with clinical profiles. Results: HLA DQ2/DQ8 genotypes were significantly more common in CD (present in 100.0% cases) than in controls (23.1%) in Northern India (Rajasthan). When HLA DQ2.5 and DQ8 were present together, individuals had significantly more atypical presentations and severe findings on duodenal biopsy. Similarly, patients with the HLA DQ 2.5 genotype were also predisposed to more severe endoscopic findings, while HLA DQ2.2 predisposed them to less severe biopsy findings. HLA DQ8 was significantly associated with later age at diagnosis (>5 years) and shorter stature. The highest HLA DQ relative risk (RR) for CD development was associated with HLA DQ2.5 and DQ2.2 in combination, followed by HLA DQ2.5 and DQ8 in combination, while HLA DQx.5 and HLA DQ2.2 together had the lowest risk. Conclusion: HLA DQ2/DQ8 genotypes are strongly associated with pediatric CD patients in northern India. These genotypes and their combinations may be associated with different clinical presentations of CD, and may help predict severity of CD.

        Expression profiling of cultured podocytes exposed to nephrotic plasma reveals intrinsic molecular signatures of nephrotic syndrome

        Background: Nephrotic syndrome (NS) is a common renal disorder in children attributed to podocyte injury. However, children with the same diagnosis have markedly variable treatment responses, clinical courses, and outcomes, suggesting molecular heterogeneity. Purpose: This study aimed to explore the molecular responses of podocytes to nephrotic plasma to identify specific genes and signaling pathways differentiating various clinical NS groups as well as biological processes that drive injury in normal podocytes. Methods: Transcriptome profiles from immortalized human podocyte cell line exposed to the plasma of 8 subjects (steroid-sensitive nephrotic syndrome [SSNS], n=4; steroid-resistant nephrotic syndrome [SRNS], n=2; and healthy adult individuals [control], n=2) were generated using microarray analysis. Results: Unsupervised hierarchical clustering of global gene expression data was broadly correlated with the clinical classification of NS. Differential gene expression (DGE) analysis of diseased groups (SSNS or SRNS) versus healthy controls identified 105 genes (58 up-regulated, 47 down-regulated) in SSNS and 139 genes (78 up-regulated, 61 down-regulated) in SRNS with 55 common to SSNS and SRNS, while the rest were unique (50 in SSNS, 84 genes in SRNS). Pathway analysis of the significant (P≤0.05, -1≤ log2 FC ≥1) differentially expressed genes identified the transforming growth factor-β and Janus kinase-signal transducer and activator of transcription pathways to be involved in both SSNS and SRNS. DGE analysis of SSNS versus SRNS identified 2,350 genes with values of P≤0.05, and a heatmap of corresponding expression values of these genes in each subject showed clear differences in SSNS and SRNS. Conclusion: Our study observations indicate that, although podocyte injury follows similar pathways in different clinical subgroups, the pathways are modulated differently as evidenced by the heatmap. Such transcriptome profiling with a larger cohort can stratify patients into intrinsic subtypes and provide insight into the molecular mechanisms of podocyte injury.

      • Effectiveness of Neurodynamic Mobilization versus Conventional Therapy on Spasticity Reduction and Upper Limb Function in Tetraplegic Patients

        Study Design: The study employed a pre- and post-test experimental design.Purpose: This study was designed to assess the effect of neurodynamic mobilization of the median nerve on upper limb spasticity in tetraplegic patients.Overview of Literature: Spasticity is a common and potentially disabling and bothersome complication in patients with spinal cord lesion; this disorder can negatively influence the quality of life by restricting the patient’s ability to perform activities of daily living. Neural mobilization is currently used for reducing the spasticity in individuals with neurological disorders.Methods: Twenty subjects with traumatic spinal cord injury (level C5–C8) and upper limb spasticity in the finger and wrist flexors were enrolled. They were randomly allocated to two different groups using a computer-generated randomization schedule: group I comprised the neurodynamic mobilization group (n=11) and group II was the conventional therapy group (n=9); the subjects were administered therapy for 5 days every week for a period of 4 weeks. Upper limb spasticity was assessed using the Modified Ashworth Scale for wrist and finger flexors; F-wave amplitude, latency, and F-wave/M-wave amplitude ratio (F/M ratio) were examined using the F-wave scores of the median nerve; and upper limb function was determined using the Capabilities of Upper Extremity (CUE) Questionnaire.Results: After 4 weeks of intervention, between-group comparisons showed a significant difference in the pre-intervention and postintervention scores on the Modified Ashworth Scale score for wrist flexors (−1.64±0.67), Modified Ashworth Scale score for finger flexors (−1.00±0.63), F-wave amplitude (−154.09±220.86), F/M ratio (−0.18±0.24), and CUE scores (17.82±13.49).Conclusions: These results suggest that neurodynamic mobilization of the median nerve may be effective for upper limb spasticity control and upper limb functional improvement in tetraplegic patients.

        Psychosocial Factors Associated With Irritable Bowel Syndrome Development in Chinese College Freshmen

        Background/Aims The role of psychosocial factors on irritable bowel syndrome (IBS) in adolescents is incompletely understood. The aim of this study is to investigate the association between the psychosocial factors and the risk of developing IBS in college freshmen. Methods A cross-sectional survey was conducted in a randomly selected freshmen population in Wuhan China (n = 2449). Questionnaire evaluated demographics and psychosocial risks. The population was divided into 3 groups: non-discomfort, chronic abdominal discomfort and IBS. The association between the development of IBS and psychosocial factors was analyzed by ordinal and multiple logistic regression analysis. Results A total of 2053 (83.8%) completed this survey (mean age, 18.2 ± 0.9 years; female, 35.6%). Among them, 82 (4.0%) fulfilled the Rome III criteria for IBS. Female (odds ratio [OR], 3.31; 95% confidence interval [CI], 2.47–4.45), experience of abuse (OR, 2.44; 95% CI, 1.10–5.56), and suicidal intention (OR, 2.17; 95% CI, 1.15–4.17) were more likely to have IBS. Compared with chronic abdominal discomfort, however, depression (OR, 5.55; 95% CI, 1.36–22.71) was the only dependent risk factor for IBS. Conclusion The prevalence of IBS in college freshmen is 4.0%, and to the freshmen, psychosocial factors such as experience of abuse, depression, and suicidal intention were associated with high risk of developing IBS.

        DNA barcoding and description of new species of Paradorydium Kirkaldy (Cicadellidae: Deltocephalinae: Eupelicini) from India

        A new species of leafhopper Paradorydium kirkaldyi sp. nov. (from Himachal Pradesh: Kinnaur: Powri) from India, is described. Detailed photographic illustration, annotated checklist and modified key to the species of India is also provided. This species closely resembles P. khasianum Viraktamath but differs in male genitalia characters and molecular analysis using COI gene confirmed the difference and also the taxonomic positon of species in the tribe is established with Histone H3 gene.

        Conflict or Cooperation? India and Pakistan in Shanghai Cooperation Organisation

        South Asia is one of the least integrated regions in the world, with persistent India–Pakistan rivalry acting as a major stumbling block to regional cooperation through the South Asian Association for Regional Cooperation (SAARC). Nonetheless, both India and Pakistan continue to experiment with multilateral arrangements and both became members of the Shanghai Cooperation Organisation (SCO) in 2017. While it is too early to predict the impact of the India–Pakistan rivalry on the SCO, dominant scholarship highlights the likelihood of a negative influence. This paper would like to present an alternative possibility – of the SCO providing opportunities for collaboration in the areas of security and counterterrorism, which in turn may positively influence Indo–Pak relations. Further, the likelihood of the Indo–Pak conflict negatively impacting the SCO is low primarily because, unlike SAARC, the SCO is led by China and Russia, who are actively invested in keeping the SCO influential to serve their key interest in challenging the dominant Western world order.
