Pulmonary Langerhans cell histiocytosis is a hetergenous group of conditions of unknown etiology characterized by an abnormal proliferation of antigen presenting cell of bone marrow derivation known as Langerhans cells that invade and destroy distal bronchioles. The outcome is highly variable, ranging from rapid spontaneous resolution to irreversible respiratory failure. We describe a patient with biopsy-confirmed pulmonary Langerhans cell histiocytosis who presented with large tension cyst, treated with pneumothorax tube with Heimlich flutter valve.
Effect of Intercritical Heat Treatment on J-R Fracture Resistance of SA508 Gr.1A Low-Alloy Steels

SA508 Gr.1A low-alloy steel is used for main steam line piping in nuclear power plants. The yield strength and J-R fractureresistance of the piping material must be high in order to apply the leak-before-break concept. In this study, intercritical heattreatment (IHT) was applied to two SA508 Gr.1A low-alloy steels having different chemical compositions, namely, samplesC and P; the latter containing Mo and V. Their microstructures, tensile properties, impact properties, and J-R fractureresistances were evaluated, and the effects of IHT on the mechanical properties of the steels were analyzed. After IHT, finegrains formed at grain boundaries and coarse cementite decomposed, which greatly improved toughness without reducingthe strength. Further, a model to predict the J-R fracture resistance (JIc) of the SA508 Gr.1A low-alloy steel was developedby considering the microstructural and mechanical factors that affect the J-R fracture resistance. According to the JIc resultsof 12 kinds of SA508 Gr.1A low alloy steel, subjected to different heat treatments, the JIc was linearly proportional to thenumber of effective grains (N) contained in the plastic zone, which showed that N is the main factor affecting JIc. Furthermore,the yield and tensile strengths were considered to compensate for the strength difference owing to the different chemicalcompositions. The JIc prediction model was derived considering the effective grain size, plastic zone size, and tensile properties. The predicted JIc values agreed well with the test JIc values.

Purpose: Refractory small-cell lung cancer (SCLC) hasa poor prognosis, and current salvage chemotherapy forrefractory SCLC, such as CAV (cyclophosphamide, adriamycin,vincristine) or topotecan, has an unsatisfactoryoutcome, with a response rate and overall survival of lessthan 10% and 6 months, respectively. This phase II studyevaluated the role of topotecan combined with etoposidein SCLC patients that have progressed, or relapsed,within 3 months following completion of the initial chemotherapy.Materials and Methods: Twenty-seven patients wereentered into this study. Eligible patients had an ECOGperformance status of less than, or equal to, 2, at leastone bidimensionally measurable lesion and adequate endorgan function. IV topotecan, 1.0 mg/m2/d for 5 consecutivedays, and etoposide, 100 mg/m2/d through days1 to 3, were administered every 3 weeks until diseaseprogression or undue toxicity.Results: The major toxicity was myelosuppression.Grade 3/4 anemia, granulocytopenia, and thrombocytopeniaoccurred in 14.2, 34.8, and 27.3% of cycles,respectively. There was no treatment-related death, andother non-hematologic toxicities were generally mild. Fourpatients achieved partial responses, with a response rateRR of 14.8%. The progression-free survival PFS rangedfrom 1 to 7 months, with a median of 2.0 months (95%confidence interval 1.22~2.78 months). Twenty-five patientsdied, with a median overall survival of 5.5 months(ranging from 1 to 21 months, 95% CI 4.32~6.68 months),and the 6-month survival rate was 32.1% (95% confidenceinterval 14.4~49.8%).Conclusion: The combination of topotecan and etoposidechemotherapy showed a modest response rate, butfailed to prolong survival of refractory SCLC patientscompared to topotecan monotherapy. Atopic dermatitis (AD) is an allergic, inflammatory skin disease characterized by chronic eczema and mechanical injury to the skin, caused by scratching. Korean red ginseng (RG) has diverse biological activities, but the molecular effects of RG on allergic diseases, like AD, are unclear. The present study was designed to investigate whether RG inhibits 1-chloro-2,4-dinitrobenzene (DNCB)-induced AD in a mouse model. DNCB was applied topically on the dorsal surface of Balb/c mice to induce AD-like skin lesions. We observed the scratching behavior and examined the serum IgE level and interleukin (IL)-4 and IL-10 in splenocytes compared with dexamethasone. We also evaluated the DNCB-induced mitogen-activated protein kinases (MAPKs), NF-${\kappa}B$, and Ikaros activities after RG treatment using reverse transcriptase-polymerase chain reaction, Western blotting, and ELISA. Our data showed that the topical application of RG significantly improved the AD-like skin lesions and scratching behavior. RG decreased not only the mRNA expression of IL-4 and IL-10, but also the secretion of IL-4 protein and serum IgE in mice. Additionally, RG treatment decreased the DNCB-induced MAPKs activity and subsequent Ikaros translocation irrespective of NF-${\kappa}B$. We suggest that RG may be useful as a therapeutic nutrition for the treatment of AD.
