Background: The aim of our study was to evaluate the “diagnosis changed” rate in patients notified as tuberculosis (TB) on the Korean TB surveillance system (KTBS). Methods: A total of 1,273 patients notified as TB cases on the KTBS in one private tertiary hospital in 2011 were enrolled in the present study. Patients were classified into three groups: “diagnosis maintained”, “diagnosis changed” (initially notified as TB, but ultimately diagnosed as non-TB), and “administrative error” (notified as TB due to administrative errors). Results: Excluding 17 patients in the “administrative error” group, the “diagnosis maintained” and “diagnosis changed” groups included 1,097 (87.3%) and 159 patients (12.7%), respectively. Common causes of “diagnosis changed” were nontuberculous mycobacterial (NTM) disease (51.7%, 61/118), and pneumonia (17.8%) in cases notified as pulmonary TB, and meningitis (19.5%, 8/41) and Crohn`s disease (12.2%) in cases notified as extrapulmonary TB. Being older than 35 years of age (odds ratio [OR], 2.18) and a positive acid-fast bacilli stain (OR, 1.58) were positive predictors and a TB-related radiological finding (OR, 0.42) was a negative predictor for a “diagnosis changed” result via multivariate logistic regression analysis in pulmonary TB cases. Conclusion: Because of a high “diagnosis changed” rate in TB notifications to the KTBS, the TB incidence rate measured by the KTBS may be overestimated. Considering the worldwide trend toward increased NTM disease, the “diagnosis changed” rate may increase over time. Thus, when reporting the annual TB notification rate in Korea, the exclusion of “diagnosis changed” cases is desirable.

How We Have Treated Severe to Critically Ill Patients With Coronavirus Disease 2019 in Korea

Since 2019, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread worldwide, and the coronavirus disease 2019 (COVID-19) pandemic currently continues. In response to this unprecedented pandemic, several researchers and medical staff have struggled to find appropriate treatments for COVID-19. Patients with mild symptoms can recuperate with symptomatic care, however establishing treatment for severe to critically ill patients who can have a high mortality has been essential. Accordingly, the guidelines for COVID-19 treatment have evolved through numerous trials and errors and have been relatively well established to date. In the Republic of Korea, several evidence-based guidelines for COVID-19 treatment were released and revised, reflecting various research and regional medical conditions. To date, approximately 3 years after the beginning of the COVID-19 pandemic, we are reflecting on the changes in the guidelines thus far and have summarized the treatment experience of severe to critically ill patients with COVID-19. The Korean guidelines for COVID-19 treatment have been updated continuously as the National Institutes of Health (NIH) guidelines have changed. Dexamethasone is currently used as the backbone for the treatment of severe to critically ill patients with COVID-19, and remdesivir, baricitinib, and tocilizumab can be added depending on a patient’s situation. In addition, venous thromboembolism prophylaxis is one of the important adjunctive therapies for patients with severe COVID-19. In the clinical field, treatment of severely ill patients with COVID-19 based on guidelines is widely practiced by medical staff and established currently.

Injuries to the sternoclavicular joint are not frequent, and the posterior dislocation of the sternoclavicular joint is much more rare. As the diagnosis is not easy with simple radiographic technique, special techniques are required. A case of the traumatic posterior dislocation of the sternoclavicular joint had been reported in our literature up to now. We also experienced a case of the retrosternal dislocation of the clavicle which had been caused by a ski injury and had a satisfactory clinical result with a closed reduction in a long-term follow-up. So we are reporting this case with a review of the literatures along with the injury mechanism.

Syringoadenoma papilliferum occurs as a solitary lesion appearing usually on the scalp. It's origin is not definitely known, and about one-third of cases are associted with nevus sebaseous. We describe a case of syringoadenoma papillferum associated with nevus sebaseous in a 29 year-old female who had developed bean sized verrucotic nodules on the Rt. temporal area of the scalp since 6 years after removal of nevus sebaceous. Histopathologic findings showed hyperkeratosis, acanthosis and villi that projected into a cystic lumen. And the villi were surrounded by two cell layers.

An advanced Fuzzy Controller to cruise on the longitudinal road intelligently is proposed in this paper. Typically the vehicle model is represented by velocity, due to its dynamic property and non-linearity. Linear acceleration controller described as the first Fuzzy Controller of most effective control method is applied mainly and the second one based on linguistic description for riding sensitivity is linked with the first and applied. A designed controller is evaluated under MATLAB SIMULINK simulation after it being adapted for virtual vehicle model.

This paper describes an intelligent cruise control system for automobile. With the remarkable numericalincrease of automobiles on the roal, the optimized traffic flow control using the cruise control is one of the very important traffic problems to overcome the limitation of an existing road capacity. Based onthis idea that minimize the fuel cost and teh air pollution, and accept a driver's needs for driving, we have developed an intelligent cruise control system for vehicle. This proposed intelligent fuzzy cruise controller was successfully implemented using the fuzzy algorithm, the i80c196 μ-controller board and the throttle valve actuator. The field test results on an linear road was introduced.

This study has suggested the 2 DOF(Degree of Freedom) PID (Proportional Integral Derivative) controller that is more synthetic and analytic than previously designed ones to solve out trade off between tracking and regulating problem and has concretely realized the structure of the controller to apply the practical systems.
