        Effect of Total Hip Arthroplasty on Ipsilateral Lower Limb Alignment and Knee Joint Space Width: Minimum 5-Year Follow-up

        Background: This study aimed to 1) assess the effect of total hip arthroplasty (THA) on coronal limb alignment, namely, the hip–knee–ankle angle (HKA), 2) identify factors that determine changes in the HKA, and 3) determine whether alignment changes influence the knee joint space width. Methods: We retrospectively evaluated 266 limbs of patients who underwent THA. Three types of prostheses with neck shaft angles (NSAs) of 132°, 135°, and 138° were used. Several radiographic parameters were measured in the preoperative and final radiographs (at least 5 years after THA). A paired t-test was used to confirm the effect of THA on HKA change. Multiple regression analysis was performed to identify radiographic parameters related to HKA changes following THA and changes in knee joint space width. Subgroup analyses were performed to reveal the effect of NSA change on the HKA change, and the proportion of total knee arthroplasty usage and changes in radiographic parameters between maintained joint space and narrowed joint space groups were compared. Results: The preoperative mean HKA was 1.4° varus and increased to 2.7° varus after THA. This change was related to changes in the NSA, lateral distal femoral angle, and femoral bowing angle. In particular, in the group with a decrease in NSA of > 5°, the preoperative mean HKA was largely changed from 1.4° varus to 4.6° varus after THA. The prostheses with NSA of 132° and 135° also led to greater varus HKA changes than those with an NSA of 138°. Narrowing of the medial knee joint space was related to changes in the varus direction of the HKA, decrease in NSA, increase in femoral offset. Conclusion: A large reduction in NSA can lead to considerable varus limb alignment after THA, which can have adverse effects on the medial compartment of the ipsilateral knee.

        Causes and Predictive Factors Associated with "Diagnosis Changed" Outcomes in Patients Notified as Tuberculosis Cases in a Private Tertiary Hospital

        Background: The aim of our study was to evaluate the "diagnosis changed" rate in patients notified as tuberculosis (TB) on the Korean TB surveillance system (KTBS). Methods: A total of 1,273 patients notified as TB cases on the KTBS in one private tertiary hospital in 2011 were enrolled in the present study. Patients were classified into three groups: "diagnosis maintained", "diagnosis changed" (initially notified as TB, but ultimately diagnosed as non-TB), and "administrative error" (notified as TB due to administrative errors). Results: Excluding 17 patients in the "administrative error" group, the "diagnosis maintained" and "diagnosis changed" groups included 1,097 (87.3%) and 159 patients (12.7%), respectively. Common causes of "diagnosis changed" were nontuberculous mycobacterial (NTM) disease (51.7%, 61/118), and pneumonia (17.8%) in cases notified as pulmonary TB, and meningitis (19.5%, 8/41) and Crohn's disease (12.2%) in cases notified as extrapulmonary TB. Being older than 35 years of age (odds ratio [OR], 2.18) and a positive acid-fast bacilli stain (OR, 1.58) were positive predictors and a TB-related radiological finding (OR, 0.42) was a negative predictor for a "diagnosis changed" result via multivariate logistic regression analysis in pulmonary TB cases. Conclusion: Because of a high "diagnosis changed" rate in TB notifications to the KTBS, the TB incidence rate measured by the KTBS may be overestimated. Considering the worldwide trend toward increased NTM disease, the "diagnosis changed" rate may increase over time. Thus, when reporting the annual TB notification rate in Korea, the exclusion of "diagnosis changed" cases is desirable.

        TRANSIENT ISCHEMIA-INDUCED EXPRESSION AND CHANGES OF TYROSINE KINASE A IN THE HIPPOCAMPAL DENTATE GYRUS OF THE GERBIL

        <P>The present study examined ischemia-related changes in tyrosine kinase A (trkA) immunoreactivity and its protein content in the dentate gyrus after 5 min of transient forebrain ischemia in gerbils. One day after ischemic insult, cresyl violet-positive polymorphic cells showed ischemic degeneration. The ischemia-induced changes in trkA immunoreactivity were found in the polymorphic layer (PL) and granule cell layer (GCL) of the dentate gyrus. In the sham-operated group, trkA immunoreactivity in the dentate gyrus was very weak. From 30 min after ischemia, trkA immunoreactivity was increased in the dentate gyrus and peaked in the dentate gyrus at 12 h after ischemia-reperfusion. Thereafter, trkA immunoreactivity was decreased time-dependently after ischemia-reperfusion. Four days after ischemic insult, trkA immunoreactivity was similar to that of the sham-operated group. In addition, it was found that ischemia-related changes in trkA protein content were similar to the immunohistochemical changes. These results suggest that the chronological changes of trkA in the dentate gyrus after transient forebrain ischemia may be associated with ischemic damage in polymorphic cells of the dentate gyrus.</P>

        북서태평양에 서식하는 살오징어(Todarodes pacificus) 계군 분석에 대한 고찰

        This paper reviews comparison analysis of current and latest application for stock identification methods of Todarodes pacificus, and the pros and cons of each method and consideration of how to compensate for each other. Todarodes pacificus which migrates wide areas in western North Pacific is important fishery resource ecologically and commercially. Todarodes pacificus is also considered as 'biological indicator' of ocean environmental changes. And changes in its short and long term catch and distribution area occur along with environmental changes. For example, while the catch of pollack, a cold water fish, has dramatically decreased until today after the climate regime shift in 1987/1988, the catch of Todarodes pacificus has been dramatically increased. Regarding the decrease in pollack catch, overfishing and climate changes were considered as the main causes, but there has been no definite reason until today. One of the reasons why there is no definite answer is related with no proper analysis about ecological and environmental aspects based on stock identification. Subpopulation is a group sharing the same gene pool through sexual reproduction process within limited boundaries having similar ecological characteristics. Each individual with same stock might be affected by different environment in temporal and spatial during the process of spawning, recruitment and then reproduction. Thereby, accurate stock analysis about the species can play an efficient alternative to comply with effective resource management and rapid changes. Four main stock analysis were applied to Todarodes pacificus: Morphologic Method, Ecological Method, Tagging Method, Genetic Method. Ecological method is studies for analysis of differences in spawning grounds by analysing the individual ecological change, distribution, migration status, parasitic state of parasite, kinds of parasite and parasite infection rate etc. Currently the method has been studying lively can identify the group in the similar environment. However It is difficult to know to identify the same genetic group in each other. Tagging Method is direct method. It can analyse cohort's migration, distribution and location of spawning, but it is very difficult to recapture tagged squids and hard to tag juveniles. Genetic method, which is for useful fishery resource stock analysis has provided the basic information regarding resource management study. Genetic method for stock analysis is determined according to markers' sensitivity and need to select high multiform of genetic markers. For stock identification, isozyme multiform has been used for genetic markers. Recently there is increase in use of makers with high range variability among DNA sequencing like mitochondria, microsatellite. Even the current morphologic method, tagging method and ecological method played important rolls through finding Todarodes pacificus' life cycle, migration route and changes in spawning grounds, it is still difficult to analyze the stock of Todarodes pacificus as those are distributed in difference seas. Lately, by taking advantages of each stock analysis method, more complicated method is being applied. If based on such analysis and genetic method for improvement are played, there will be much advance in management system for the resource fluctuation of Todarodes pacificus. 본 종설논문은 살오징어의 기존 및 최근에 새롭게 적용되고 있는 계군 분석방법들을 비교 분석하여 각 분석방법의 장단점과 분석방법간의 상호보완에 대하여 고찰하였다. 살오징어는 북서태평양의 넓은 지역을 회유하는 어종으로 생태계 및 상업적으로 중요한 자원이다. 살오징어는 해양환경변화의 생물학적 지표로서의 가능성을 평가 받고 있으며, 장단기적인 어획량 및 분포역의 변화가 환경 변화와 함께 나타난다. 예를 들어, 1987/1988 무렵에 발생한 기후체제전환 이후 한류성 어종으로 분류되는 명태의 어획량은 급감하여 현재까지 그 영향이 지속되고 있는 반면, 살 오징어 어획량은 크게 증가하였다. 현재까지 명태 어획량의 감소에 대하여 남획과 기후변화에 초점이 맞추어진 해석이 있으나, 뚜렷한 원인 분석은 이루어지지 않고 있다. 그 이유 중 한 가지는 계군 분석에 근거한 생태, 환경적 측면에 대한 정확한 원인 분석이 이루어지지 않고 있는 것과 관련이 된다. 계군은 유사한 생물학적 특징을 가진 개체들이 제한된 영역 내에서 유성생식과정을 통하여 동일한 유전자 풀(gene pool)을 공유하는 집단으로, 동일 계군을 형성하는 개체들은 산란에서 자원으로 가입 후 다시 재생산 과정에 이르기까지 시간 및 공간적으로 각기 다른 환경의 영향을 받을 수 있다. 따라서, 종에 대한 정확한 계군 분석은 자원의 효과적인 관리 및 급격한 변화에 대한 중요한 대응 방안의 역할을 할 수 있다. 살오징어 계군 분석에 적용된 주요 방법은 크게 4가지로 형태학적 방법, 생태학적 방법, 표지방류법, 유전학적 방법이 있다. 형태학적인 방법은 분석방법이 가장 간단하고 다수의 개체를 비교적 쉽게 분석할 수 있지만 각 형질들은 성장기간 동안 환경에 의해 영향을 많이 받게 되어 개체간의 차이가 생긴다. 생태학적 방법은 주로 개체의 생리적인 변화와 분포 및 회유상태, 기생충의 기생상태나 종류 및 기생률 등을 분석, 산란장의 차이를 알아보는 연구이며, 현재 활발히 연구되고 있는 방법으로 유사한 환경에서 생활하는 집단을 알 수 있지만 유전적으로 같은 집단인지는 알기 어렵다. 표지방류법은 직접적인 방법으로 계군의 회유 및 분포, 산란장의 위치를 파악할 수 있지만 수거가 어렵고 초기 단계에는 표식을 하기 어렵다. 수산생물의 계군 분석을 위한 유전학적 방법은 자원관리학적 연구에 관한 기본적 정보를 제공해 왔다. 계군 분석을 위한 유전학적 방법은 이에 사용하는 유전자 마커(marker)의 감도에 따라 결정되며, 유전자 마커의 다형성이 높은 것을 선택해야 한다. 계군 분석을 위한 유전자 마커로는 오랜 기간 동안 동위효소 다형이 사용되어졌으며, 최근에는 mitochondria, microsatellite와 같이 DNA 염기배열 중에서도 변이성이 높은 영역을 선택하여 마커로 이용한 연구가 증가되고 있다. 기존의 형태학적 방법, 표지방류법, 생태학적인 방법들은 살오징어의 생활사, 회유경로, 산란장의 변화 등을 밝혀내어 계군을 파악하는데 많은 기여를 하였지만 여전히 각 해역에 분포하는 살오징어의 계군을 파악하기에는 어려움이 있다. 최근에는 기존의 계군 분석이 지닌 장단점을 비교 분석하여 복합적인 방법의 계군 분석이 이루어지며, 이러한 정보들을 바탕으로 유전학적 방법을 보완한다면 살오징어 자원의 변동에 대한 관리 방안을 마련하는데 도움을 줄 것이다.

      • 韓國産 미꾸리에 關한 育種·繁殖學的 硏究 : Ⅵ. Histological Changes of Pituitary Gland and Oocytes from March to May Ⅵ. 계절에 따른 암컷의 腦下垂體와 卵母細胞의 組織學的 變化

        1992년 3월부터 1992년 5월까지 자연적인 채광상태에서 溫度를 유지시킨 후 채취된 미꾸리 암컷의 腦下垂體, 肝 및 卵巢 組織의 번식주기에 일어나는 미세구조의 변화를 광학 및 투과형 전자현미경을 이용하여 살펴보고, 이것들의 相互 聯關性을 알기 위하여 본 연구를 실시하였다. 본 연구에서 얻어진 결과는 다음과 같다. 성숙된 개체의 뇌하수체는 수많은 작은 분비과립과 커다란 분비구 및 불규칙한 형태의 과립들이 관찰되었다. 핵소체가 핵막 주변에 위치헤 있는 후기 단계(LMO)의 난모세포가 난황이 축적되는 성숙초기 단계(EMO)의 난모세포로 변형되는 시기를 난황의 형성이 개시되는 시기로 본다. 난모세포가 성장함에 따라 핵소체의 크기와 난황과립의 수가 증가하게 된다. 난황과립은 결정상의 과립으로 난세포질에 치밀하지 않은 상태로 축적된다. 성숙초기(EMO) 및 말기단계(LMO)에 난소는 커지고 투명하며 과립형태로서 색깔이 연노란색을 나타낸다. 따라서 난 내부를 쉽게 관찰할 수 있다. 방사대(ZR)는 성숙초기단계(EMO)에 단층으로부터 입방형태로 변화된다. 과립막 세포와 난모세포로 부터 유래된 미세융모는 난황형성이 진행되는 동안에 성장하게 되며, 방사대의 pore canals의 구멍을 통해서 서로 연결되어 있다. 따라서 외부로부터 물질의 이동이 있게 된다. 난모세포가 완전하게 성숙되어 방사대가 단단해 지면 미세융모는 그 기능이 상실된다. This study was undertaken to understand the development stages of pituitary gland, liver and ovary of Korean loach(Misgurnus anguillicaudatus) under natural circumstances from March to May 1922. Expecially, the light microscopic and ultrastructural changes of ooplasm and follicular membranes of oocytes, were observed by light and transmission electron microscope during the reproductive cycle. In pituitary gland of mature individuals, there were observed a number of secretory granules,large globules, and irregular masses.The vitellogenic phase began as these late perinucleolus oocytes became transformed into early maturing oocytes through the accumulation of yolk. The size of the nucleoli and number of the yolk granules increased as the oocyte growed. Yolk granules were loosely deposited in the oocyte as crystalline granules. Due to the presence of early and late maturing oocytes, their ovaries were enlarged transparent, granular and yellowish in color. Zona radiata change a squamous into cuboid shape in EMO stage. Microvilli from the granulosa cells and from the oocyte grow and make contact with each other in the pore canals of the zona radiata during vitellogenesis, but are withdrawn as the zona radiata becomes more compact during oocyte maturation.

      • 濟州道 겨울 作物(채소류)의 流通 및 對日 輸出增大에 대한 硏究

        (1) The mean atmospheric timperature of Jeju area is higher than that of Seoul by 3'c and in the coldest month by 8'c, so Jeju-do is regarded as important place to cultivate subtropical plants. Such a natural state has accelerated the crop production suitable to warm region front time immemorial. Jeju-do has become a main producing district of tingerines, sweet potatos, barleys and rapes since the beginning of 1960's and Jeju-do's latest main products are wintering vegetables such as cabbages, carrots, garlics and onions. Such a term favorable to an off-crop season is developing Truck Farming. (2) A milder winter causes us to save petroleum needed to cultivate plants under vinyl plastic hot houses, where some 200cc of petroleum is needed to cultivate a tomato, some 100cc to cultivate a cucumber, some 18cc to cultivate a melon, some 200cc to cultivate a tangerine, and some 50-60cc to cultivate an eggplant. (3) A case study on sales method of carrots, garlics and cabbagges shows that 64-60% of farmers sell out a crop estimate on vegetable garden before harvesting. (4) A case study on distribution margin shows that gross margin rate is 66.2% in carrots, 77.2% in cabbages and 53.6% in garlics, and the income rate of above-mentioned products in a farmhouse is 33.8%, 22.8% and 46.4% respectively, while transporting expenses…cabbages (23.2%), carrots (12.1%) and garlics (8.7%)…are given much more weight than any other expense. (5) Japan limitted the total output of vegetables with 16 million ton of vegetables in 600,000 ha of vegetable culture area to keep the change of demand safe in 1979, but the increase of vegetable culture area, both by changing rice fields and by the propensity to consume energy, is becoming primary factors in the alteration of vegetable product stucture. (6) The amount of annual vege table consumption per headof population has changed into 110kg, and the vegetables consumed over 5kg are cabbages, onions and radishes and sweet potatos. Cucumbers are staying between 4kg and 5kg. The present propensity to consume vegetables represents multiformity, annual plants and high-grade food, with western vegetables as the central one. (7) The amout of Jepanese in agricultural, forestry and marine fields came up to $ 29,055, 000,000 in 1980. The income amount in agricultural products reaches $ 14,911,000,000 wich is equal to 51.3% of total. The income amount in fruits and vege tables reaches $ 1,420,000,000 which is equal to 95%, America holds the foremost place as importer Country of agricultural and marine producss and Taiwan the sixth place, but Korea fakes 11th place, earning $ 738,000,000 annually. The income amount from Korea in agricutural comes up to $ 152,000,000. (8) The income amount of vegetables in Japan reached $ 107,800,000,000, and the change of vegetable income (1965-1978) tells that income amount has increased the quantity by 17 times. Main articles of vegetables are frozen vegetables and salted ones. (9) No importer countries surpass America and Taiwan. The export share of fish and vegetables in Taiwan and Korea shows that Taiwan exports 24.3% of total Japanese import (2nd position) , while Korea does 2.8% (5th position). For frozen vegetables, America holds the first position, exporting 48.6% and Taiwan the 2nd position, exporting 31.4% , while for dried vegetables, Taiwan holds the first position, exporting 42.9% and Korea 5th position, exporting 2,8%. Also, for salted vegetables Taiwan exports 63.4% (1st) and Korea 3.1% (3rd), while for prepared vegetables, Taiwan exports 78% (nst) , and Korea 2% (5th). (10) The agricultural effort amount of Korea came up to $182,000,000 which corresponded to 0.9% of total export, $20,993,000,000, while the export amount of Jeju-do reached $ 593,000, corresponding to 0.3%. The raw silks of Korean agricultural products in 1975 were main export articles, corresponding to 58% of total agricultural products, but they are remarkably decreasing into 14.3%. The vegetable export share was 2.2% in 1975, but 11.8 percent increase in 1981 is a characteristic found only in the vegetable export. Thus, the fact that vegetables are given much weight in the export structure of agricultural products shows an important change and the probablility to develope in the future. Therefore, in the aspect of vegetable export, diversified studies are needed to prepare good vegetable-producing center and to foster special artfole like dried, salted, frozen, and manufactured vegetables.

        입술의 돌출을 해소하고자 하는 환자들은 자신의 안모 개선을 안모의 측면보다는 거울로 볼 수 있는 정면을 통하여 느끼는 것이 보편적이다. 안모의 정면에서 평가가 이루어질 때에는 입술이 후방으로 들어간 정도와 함께 입술과 그 주위 조직의 수직적 변화에 의해서도 큰 영향을 받을 수 있으므로 교정치료 진단이나 치료 후 평가를 위한 측모두부방사선 사진 분석 시 전통적으로 중요시되고 있는 입술의 시상적 이동뿐만 아니라 흔히 간과하기 쉬운 수직적인 변화에 관해서도 평가가 이루어져야 한다. 따라서 본 연구에서는 입술 돌출을 주소로 내원한, 성장이 완료된 성인 여성 61명을 대상으로 측모두부 방사선 사진을 촬영하여 전치의 후방견인시 나타나는 입술과 주위 연조직의 수직적 변화를 계단식 다중 회귀 분석법을 이용하여 조사한 결과 다음과 같은 결론을 얻었다. 1.상, 하순의 philtrum길이 (SnLs, LiB') 는 전치의 후방견인 후 증가하였고, 상, 하순의 vermilion height (LsStms, StmiLi) 와 vermilion length (LsLi) 는 감소하였다. 2.상, 하순의 길이 (SnStms, StmiB')와 연조직의 하안면 고경 (SnMe')은 치료 후 유의성 있는 변화를 보이지 않았다. 3.상순의 philtrum길이 증가에 상악 전치의 정출 △UIV 이 가장 큰 영향을 주었으며 하순의 philtnlm길이 증가에는 치료 전 overjet이 가장 큰 영향을 주었다. 4.상, 하순의 vermilion height감소에 상순의 두께 감소가 가장 큰 영향을 주었다. Patients who want to reduce their lip protrusion usually estimate the severity of the lip protrusion on the frontal aspect. Most orthodontists have a perplexed experience of a reduced thin line of vermilion border on the frontal aspect as incisors are retracted, even though the lip protrusion is thought to be reduced favorably on the sagittal aspect. Some patients also look older after orthodontic treatment because of severe lip thinning. This unaesthetic reduction of vermilion border urges us to study the vertical lip change during orthodontic procedure. The purpose of this study was to evaluate the vertical lip and perioral soft tissue changes in respect to incisor retraction in an effort to analyze which factors might be responsible for their vertical changes, using the multiple regression analysis. The results were as follows. 1.Upper and lower lip philtrum length(SnLs, LiB') were increased after retraction of anterior teeth, where as upper and lower vermilion height(LsStms, StmiLi), and vermilion length(LsLi) were decreased. 2.Upper and lower lip length(SnStms, StmiB'), and soft tissue lower anterior facial height(SnMe') did not show any significant difference after treatment. 3.The increase of the upper lip philtrum length was mainly influenced by the extrusion of upper anterior teeth(△UIV), and the increase of the lower lip philtrum length was mainly influenced by the initial overjet before treatment. 4.The decrease of the upper and lower lip vermilion height was mainly influenced by the decrease of upper lip thickness.

        Ischemia-Related Changes in Galanin Expression in the Dentate Hilar Region after Transient Forebrain Ischemia in Gerbils

        <P>Although galanin (GAL) protects hippocampal neurons from ischemic damage, no study has examined ischemia-related changes in endogenous GAL in the hippocampal dentate gyrus. We investigated the chronological changes of GAL, well-known as the potassium channel opener, expression in the dentate gyrus at various times after 5 min of transient forebrain ischemia in gerbils. A few GAL-immunoreactive (IR) neurons were found in the polymorphic layer of the sham-operated group. Three hours after ischemia-reperfusion, the pattern of GAL immunoreactivity was similar to that of the sham-operated group and the number of GAL-IR neurons and immunoreactivity were highest 12 h after ischemic insult. At this time, GAL-IR neurons in the polymorphic layer showed strong GAL immunoreactivity. Thereafter, GAL-IR neurons and immunoreactivity significantly decreased in the dentate hilar region. Four days after ischemic insult, GAL-IR neurons were not detectable. In addition, the results of a Western blot study showed a pattern of GAL expression similar to the immunohistochemical changes. GAL protein content also was highest 12 h after ischemia. In conclusion, the increased expression of endogenous GAL in the dentate gyrus after ischemia is related to response to the ischemic damage.</P>

        This study is for research on the cause of behavioral change in relation to health of Koreaans in order to enhance the quality of life. For this We have surveyed against 3,000 adults of 8 cities and provinces nationwide. Looking the result of this study synthetically, most of the subjects of study were thinking that they are 'jealthy'(62.9% for male, 50.0%) and of drinker(83.2%) formale, which harm our health are high. The ratio of having stress often were high for voth male and female(27.1% for male 31.4% for female), and the main cause of the stress were shown as wok related to their company(56.7%) for male and as family problem (44.1%) for female, They responded to resolve it mainly in 'patient'(36.5% for male, 51.0% for female). This shows that they need something positive way of resolving the stress. Korean people likes modicines too much and it was surveyed to have worsened their health due to ovenuse of medicines. They hardly have medical check up and even there were people who don't know is medical check up, so it was shown to nood activ promotion of it. As the result of study on te cause of behavioral change related to health of Koreans for the enhancement of people[s' lives, We would like to suggest some guidelines for healithy life. A. It needs to promote and educate for reducing smoking and drinking which harm our health B. The plan or programs for resolving stress of the people must be studied concretely on the level of gorvernment. C. For the checking condition of health well, ti must be done and consulted with specialists. D. The eating habits of our people must be changed in consulting with specialists in order to protcet and cure from the most serious digestive troubles.
