Many cases of retroperitoneal hematoma related to spine surgery have been reported, however, most of them were caused by major vessel injury related to intradiscal or retroperitoneal procedures. We report an unusual case of a retroperitoneal hematoma followed by surgical excision of the lumbar extradural arachnoid cyst without intradiscal procedure. To our best knowledge, this complication without intradiscal procedure has not been described previoulsly. The pathogenetic mechanism of retroperitoneal hematoma in this report is unclear. We discuss the plausible pathophysiological mechanisms of retroperitoneal hematoma which developed after excision of arachnoid cyst. 본 논문에서는 유도전동기 속도 및 토크 제어시 고조파의 왜형률을 감소시키고, 다지털 구현이 용이하며 선형제어 영역을 증가시킬 수 있는 스위칭 방식으로 Space voltage Vector PWM(Pulse Width Modulation) 설명하였으며, 시뮬레이션하기 위한 user_tool로써 Matiabl Simulink 를 이용하여 SVPWM 을 구현하는 방법을 성명하였다. 결과적으로 개루프 운전 시 유도기에 인가되는 기준전압을 각각 비과변조와 과변조 시 전압비율 변경방식을 사용하여 나타나는 응답으로 가상스위칭 시간 T1,T2,T0 와 속도, 상전류, 토크응답을 알아보았다.

Ebstein 기형은 복잡 심기형중 하나로 치료 또한 비정상적인 삼첨판막의 교정 또는 판막치환 등으로 방법이 다양하다. 이질환의 병리소견은 삼첨판의 중격엽과 후엽이 하방전위되어서 심방화된 심실을 형성하는 것으로 수술적 목표는 삼첨판의 기능이상을 교정하고 심방화된 우심실을 주름성형하는 것이다. 본원에서는 매우 심한 삼첨판막 폐쇄 부전증이 동반된 Carpentier type A의 Ebstein기형으로 진단된 12개월된 여아에 대해 심방화된 우심실을 수직방향으로 주름성형하고 판막을 새로이 형성된 판륜에 재부착시키는 이른바 변형적 Carpentier방법으로 성공적으로 수술하였다. 수술후 환아는 심장의 크기가 매우 줄어들었고 심초음파상 미미한 정도의 삼첨판 폐쇄부전증만 남아 술후 14일째 퇴원하였다. Ebstein's anomaly is a complex malformation that can be treated by various surgical techniques, either repair or replacement of the abnormal tricuspid valve, with variable results. The essence of the malformation is the downward displacement of the septal and posterior leaflets into the ventricle, resulting in the formation of an atrialized portion of the right ventricle. The aim of surgical repair is to correct the tricuspid valve dysfunction and to plicate the atrialized portion of the right ventricle A 12-months old female was admitted with the diagnosis of Carpentier type A of Ebstein's anomaly with severe tricuspid regurgitation. She successfully underwent operation with vertical plication of right ventricle and reimplantation of tricuspid leaflets. Postoperatively cardiac size was significantly reduced and tricuspid regurgitation was trivial in echocardiography. She was diacharged the 14th postoperative day.

Between 1985 and 1993, 29 children from 1 to 15 years of age have undergone cardiac valve replacements at Buchon Sejong Hospital. The patients were composed of 20 males and 9 females and 17 patient had congenital heart disease and 12 patients had acquired heart disease. Two of these patients have had second valve replacements due to paravalvular leakage and valve thrombosis. Single valve replacements were 29 and double valve replacements were 2. All the patients had received prosthetic valves except one. Among the 25 patients who had definite post-operative records, the overall mortality was 12%[4% was early mortality and 8% was late mortality].25 patients were followed up with coumadin anticoagulation for total 633 patient-months[minimum 2 months to maximum 93 months, mean 25.3 months] and actuarial survival rate was 88.5 $\pm$ 6.3% at 7 years and event free rate was 70.3 $\pm$ 11.7% at 7 years. These results suggest that pediatric valve replacements can now be performed at a low operative risk although various problems are still remained and the choice of valve is prosthetic valve mainly due to its durability at the present time.

From January 1990 to January 1996, we analyzed outcome according to the distribution of aneurysms the type of surgical treatment the timing of operation, and the patient's pre-operative status in a total of 54 cases of multiple intracranial aneurysms. The frequency of multiple intracranial aneurysms was 13.4% and the sex ratio was 1 : 2 with female being predominant. The frequency of aneurysmal location was MCA(35.0%), P-comm, artery(25.0%) and A-comm artery (15.0%) in that order. The number of ruptured aneurysms was in the order of MCA aneurysms A-comm aneurysms, and F-comm aneurysms. The size of ruptured aneurysms was 6-10mm if 34 cases(64.2%). The unilateral distribution of aneurysms was 31 cases(57.4%) and the bilateral was f8 cases(42.6%). Depending on how many times the patient had the operation, there were 29(57.4%) cases of complete single operation, 17 cases(31.5%) of complete two-stage operation, and 8 cases(14.8%) of partial operation. Surgical outcome was good(GOS≤4) in 42 cases(77.8%). and six patients(11.1%) expired. Based on the results of the above study we have concluded that initial Hunt-Hess grade has a close relationship with the outcome, but the operation type, the timing of operation, the number and distribution of aneurysms do not significantly effect the outcome.

The rhabdomyosarcoma(RMS), a leading malignant soft tissue tumor in the pediatric population, occurs most commonly in the craniocervical region RMS has involved the temporal bone approximately 7% in the literature. Petrous bone invasion was very rare and the overall outcome was always fatal, in spite of the efficacy of multimodality therapy until recent years. The authors report a case of primary RMS in the temporal bone invading petrous bone and the result of multimodality treatment.
Endoscopic Spinal Surgery for Herniated Lumbar Discs

Objective : So called "minimally invasive procedures" have evolved from chemonucleolysis, automated percutaneous discectomy, arthroscopic microdiscectomy that are mainly working within the confines of intradiscal space to transforaminal endoscopic technique to remove herniated epidural disc materials directly. The purpose of this study is to assess the result of endoscopic spinal surgery and favorable indication in the thoracolumbar spine. Methods : The records of 71 patients, 73 endoscopic procedures, were retrospectively analysed. Yeung Endoscopic Spine Surgery system with 7 mm working sleeve and $25^{\circ}$ viewing angle was used. The mean follow up period was 6 months [range, 3-9]. Results : Operated levels were from T12-L1 disc down to L5-L6 of S1 disc. Of 71 cases, 2 patients underwent transforaminal endoscopic surgery twice due to recurrence after initial operation. MacNab's criteria was used to assess the outcome. Favorable outcome, excellent of good, was seen in 78% [57 procedures] of the patients. Among 11 fair outcomes, only 1 procedure was followed by secondary open procedure, laminectomy with discectomy. Two of 5 poor outcomes were operated again by same procedure which resulted in fair outcomes. One patient with aggravated cauda equina syndrome remained poor and a lumbar fusion procedure was performed in other patient with poor outcome. There were 2 postoperative discitis that were treated with conservative care in one and anterior lumbar interbody fusion in the other. Conclusion : Evolving technology of mechanical, visual instrument enables minimal invasive procedure possible and effective. The transforaminal endoscopic spinal surgery can reach as high as T12-L1 disc level. The rate of favorable outcome is mid-range among reported endoscopic lumbar surgery series. Authors believe that the outcome will be better as cases accumulate and will be able to reach the fate of standard open microsurgery.

In this study, according to the increase in earthquakes around the world to develop a new system that combines the body and restore the rubber disc, the resilient friction disk bearing has been developed. In this paper, the width of the body in response to changes in rubber recovery, and behavior of shear stiffness properties were tested. ISO 22762-1 testing utilizing frequency-dependent shear strain dependence study, stiffness was measured. The larger the width of the rubber, which increases stiffness. Dependence of the frequency dependence of large strain was confirmed that low.

Two novel type I catechol 1,2-dioxygenases inducible on aniline media were isolated from Acinetobacter lwoffii K24. Although the two purified enzymes, CDI₁and CDI₂, had similar intradiol cleavage activities, they showed different substrate specificities for catechol analogs, physicochemical properties, and amino acid sequences. Two catA genes, catA₁and catA₂, encoding by CDI₁ and CD I₂, respectively, were isolated from the A. lwoffii K24 genomic library by using colony hybridization and PCR. Two DNA fragments containing the catA1 and catA₂ genes were located on separate regions of the chromosome. They contained open reading frames encoding 33.4-and 30.4-kDa proteins. The amino acid sequences of the two proteins matched well with previously determined sequences. Interestingly, further analysis of the two DNA fragments revealed the locations of the catB and catC genes as well. Moreover, the DNA fragment containing catA₁ had a cluster of genes in the order catB₁-catC₁-catA₁ while the catB₂-catA₂-catC₂ arrangement was found in the catA₂ DNA fragment. These results may provide an explanation of the different substrate specificities and physicochemical properties of CDI₁ and CDI₂.
