        정상 한국인에서 연령증가에 따른 혈장 Dehydroepiandrosteronesulfate ( DHEA-S ) 농도 및 DHEA - S / cortisol 비의 변화

        Backgroud: DHEA-S is the most abundant steroid hormone in circulation, and primarily secreted from the adrenal cortex, but its physiological role is little known. One of the characteristic features of DHEA-S is progressive decrement of plasma DHEA-S level with advancing age, in contrast, plasma levels of other adrenal hormones are not chaging or littie decreasing. To grasp the trends of plasma DHEA-S level and DHEA-S/cortisol ratio by age in healthy Korean, we measured the plasma DHEA-S levels and DHEA-S/cortisol ratios in healthy Korean. Methods: Healthy Korean(men: 99, women: 102, age range: 15-97 year old)were studied. Subjects were not taking drugs(such as glucocorticoid or androgenic medication) or cigarettes known to modify the plasma level of DHEA-S and cortisol, and had no evidence of hepatic, renal disease or hyperlipidemia as determined by serum lipid, bilirubin, SGOT, SGPT, BUN, creatinine. Data were analyzed by 10-year age group for men and women: i.e, 10-19, 20-29, 30-39, 40- 49, 50-59, 60-69, 70-79, 80-89 and 90 year or more. Plasma DHEA-S levels were measured by using a commercially available RIA kit with 125I labeled-DHEA-SO4(Coat-A Count DHEA-SO4), and for the measurement of plasma cortisol levels, commercial Gamma Coat TM[125I] Cortisol Radioimmunassay Kit was used. Results:. 1) In both men and women, plasma DHEA-S level showed high interindividual variation within the same age group. 2) There were individual sex differences in plasma levels of DHEA-S, in all age groups, plasma DHEA-S levels were significantly higher values for men than for women. 3) Maximum plasma DHEA-S levels(men; 237+-3.35 ug/dL, women; 108+-17.5 ug/dL) were at third decade in both men and women. 4) Both men and women showed the continuous decline in plasma DHEA-S level with age. These age-related decline was more prominent in men than in women(men; y= -3.152 * +292.6, r2= 0.8459, P$lt;0.05, women; y= -1.417 * +143.3, r2 = 0.7278, P$lt; 0.05). 5) As an index of aging, there was no stastical difference between DHEA-S and DHEA- S/cortisol ratio. Conclusion: In healthy Korean, there were high interindividual variation of plasrna DHEA-S levels. In both men and women plasma DHEA-S level was peak at third decade, and from when it declined progressively with age. These results suggest that although the reliability of single plasma DHEA-S measurement are limited, the decline of DHEA-S with advancing age might be a specific marker of endocrinologic hormonal milieu(aging index). Also, concerning to individual adrenal secreting capacity, we measured DHEA-S/cortisol ratio. But we did not found that plasma DHEA-S/cortisol ratio is superior to the plasma DHEA-S level as an aging index. (J Kor Soc Endocrinol 12:245-254, 1997)

        Double pyrolus, one of the relatively rare anomalies of the gastrointestinal tract, is a fistulous communication between the gastric antrum and the duodenal bulb. The first case of a double Pylorus was reported in 1969 by Smith, since then several cases have been reported. Recently, as the frequency of examining upper gastrointestinal series and endoscopy increases, it occurs more frequently and a few cases have been described in our country. According to the reports previously published, it resulted only from cysts, diverticla, atresia, band, membrane malformation of the pylorus, however, it might be also caused by penetration, complication of peptic ulcer. We report a case of double pylorus in a 67-year old man with prolonged administration of corticostetoids due to lumbago who has complained epigastric pain which was confirmed by endoscopy and biopsy.

        Abstract: As known in clinical trial like DCCT, UKPDS, tight glycemic control is essential to prevent complications from diabetes mellitus(DM). In type 2 diabetes taking multiple medication, nonadherence to medications is common problem. The aim of our study is to investigate adher-ence rate of patients with type 2 DM and to evaluate association between many factors including adherence rate and HbA_(1C). 441 patients with type 2 DM who visited Busan National University Hospital endocrine internal department from Aug 13 to Sep 2, 2008(3weeks) were enrolled. We conducted a man-to-man questionnaire survey about general knowledge about diabetes mellitus, medicine and adherence, and measured HbA_(1C). Among them, 305 patients taking oral hypo-glycemic agent(OHA) were analyzed to investigate correlation between adherence rate and glycemic control. We studied 305 patients(male 126, female 179, average age 61.2±9.8 years, DM duration 8.6±6.4 years, average adherence 88.8±16.2%, average HbA_(1C) 7.03± 1.18%). HbA_(1C) showed significant relation with DM duration(r=0.137, p=0.017), number of aHA in regimen (r=0.135, p=0.018), dosage frequency(r=0.132, p=0.026), Medication Refill Adherence(MRA)(r=-0.124, p=0.030) and adherence(r=-0.168, p=0.003). Adherence had significant relation with MRA(r=0.148, p=0.010) but didn't have statical corelationship with DM duration, number of aHA and dosage frequency. Patients that their adherence rate exceed 90% took fewer OHA(p=0.011) and went on a better dietary treatment(p=0.007). In patients controlling diabetes mellitus with aHA, when adherence rate and MRA was higher, HbA_(1C) was lower. As a result, to improve adher-ence rate, it is necessary to enhance patient education, do careful consideration about prescrip-tion medicine number and dosage frequency

        위성신호 Delay추가를 통한 위성 DMB 서비스 경계지역 품질 개선

        위성DMB 서비스 시스템에서는 음영 지역을 커버하기 위한 중계기인 갭필러를 운용하고 있다. 하지만, 갭 필러의 커버리지 영역에 한계가 있가 때문에 위성DMB 단말기가 갭필러의 커버리지 영역에서 벗어나면 위성으로부터 직접 신호를 수신한다. 여기서, 갭필러신호 서비스 커버리지와 위성신호 서비스 커버리지 사이에 경계지역이 발생하게 되며, 이러한 경계지역에서는 위성으로부터의 선호와 갭필러로부터의 신호간에 단말기의 복조창보다 차이가 큰 지연시간이 발생하므로 서비스 품질 저하가 발생하게 되는 문제점이 있다. 본 논문에서는 위성신호와 갭필러신호 사이에 발생되는 지연시간을 극복할 수 있도록 위성신호와 갭필러신호에 Delay를 추가하여 경계지역의 품질을 개선하는 방법을 제안한다. 갭필러시설이 전국적으로 확대되지 않은 서비스 초기단계에서 이 방법은 품질개선활동에 많은 도움이 되리라 사료된다. Satellite Digital Multimedia Broadcasting(SDMB) needs Gap Fillers(GFs) to cover the non-service areas. However, SDMB mobile phones which are not located in GFs service area receive signals directly from the satellite because it is impossible to cover all SDMB service with GFs. Therefore, there is a border between the GFs service coverage and the satellite service coverage and it causes a time delay in signals which are from the GFs or the satellite. The time delay is the reason why SDMB service deteriorates. In this paper, I show how to reduce the deterioration of SDMB service in the border area between the GFs and the satellite adding the time delay to the satellite signal and the GFs signal. I think that this method is helpful in reducing the deterioration, especially in the early stages of SDMB service, which the GFs are not spread all over the nation yet.

        뇌동맥류 수술시 Thiopental Sodium 정주 후 경정맥 산소포화도(SjVO2)의 변화

        Background: Thiopental has a profound impact on the cardiovascular system. The effects of hemody-namics after intravenous thiopental on the balance of cerebral metabolism with cerebral blood flow is unklown. The purpose of this study was to monitor hemodynamic change, cerebral arterial-jugular ve-nous oxygen content difference (AVDO2) and jugular venous oxygen saturation (SjVO2) after a thiopental injection for brain protection during cerebral aneurysm surgery. Methods: Twenty patients received a standard anesthetic consisting of isoflurane, vecuronium and fentanyl with a PaCO2 of 30-35 mmHg. Hemodynamics, arterial and jugular venous blood gases were measured at 3 time points: I; Just before thiopental injection; II; Electroencephalographic (EEG) burst suppression after a 4-5 mg/kg intravenous thiopental injection; and III. EEG recovery. Results: Intravenous thiopental (4-5 mg/kg) induced an EEG burst suppression for 6.5 ± 1.7 mi- nutes. Hemodynamics and arterial blood gas analysis were not significantly different among the different time points, but mean arterial pressure (68.4 ± 7.2 mmHg) and systemic vascular resistance (1027.0 ± 300.9 dynes · sec/cm3) in II were significantly (P < 0.05) decreased compared with I (84.3 ± 9.6, 1169.1 ± 304.5) and III (89.1 ± 10.6, 1288.6 ± 426.1). SjVO2 (71.6 ± 11.8%) was significantly (p < 0.05) decreased within the normal value compared with I (75.1 ± 5.6) and III (76.1 ± 10.1), but AVDO2 was not significantly different among the 3 time points. There was no evidence of cerebral ischemia or infarction in computed tomographic (CT) findings of the 20 patients after surgery.       • Efficient Production of Gene Edited Bovine Embryos

        Transgenic animals has been produced by using microinjection method for target gene editing. However, the producing efficiency of gene edited embryos in livestock is very low. Therefore, the objective of this study is production of high quality blastocyst and establishment of appropriate microinjection condition for gene editing embryos. Frist, we treated the cytochalasin B (CB) within injection medium for reduction of microinjection damage, and microinjected embryos were cultured in CR1aa medium supplemented with 0.1 μM melatonin. As a result, blastocysts development of microinjected embryos in CB treated group was significantly higher than that of non-treated group (p<0.05, CB+, 18.5±2.0% vs CB-, 11.2±3.5%). After microinjection under CB treated condition, blastocyst formation was significantly increased (p<0.05) in Mela treated group. Next, to investigate DNA damage in blastocyst stage embryos, fluorescence expressions of H2AX139ph (DNA damage protein) and RAD51 (DNA repairs protein) were observed by using immunofluorescence staining. DNA damage in developing blastocysts was reduced in the CB and Mela treated embryos. Finally, we identified the blastocyst development after microinjection by using CRISPR/Cas9 system according to protein or mRNA of Cas9. Green fluorescence expressions by GFP were increased in developing blastocysts derived from Cas9 protein microinjected embryos. Moreover, gene targeting rate in GFP expressed blastocysts was increased in Cas9 protein injected group (p<0.05; Cas9 protein, 30% vs Cas9 mRNA, 20%). These results demonstrated that positive effect of CB and Mela treatment improves the blastocysts qualities and their developmental competence via reduction of microinjection damage in gene editing bovine embryos. We also verified the improvement of knock-in efficiency in microinjected bovine embryos by using Cas9 protein.

        A Case of Korean Medicine Treatment for a Patient with Somatic Symptom Disorder Hospitalized after a Traffic Accident

        Objectives: The purpose of this study was to report the effect of Korean medicine treatment on a patient with somatic symptom disorder hospitalized after a traffic accident. Methods: The subject for this study was a patient diagnosed with somatic symptom disorder that worsened after a traffic accident. During hospitalization, herbal medicine, acupuncture, psychotherapy, and other treatments were performed. Clinical effect was evaluated using Heart Rate Variability (HRV), BDI (Beck Depression Inventory)-II, BAI (Beck Anxiety Inventory), Global assessment (G/A), and Numerical rating scale (NRS). Results: After 36 days of treatment, BDI-II score was reduced from 17 to 5. BAI score was reduced from 23 to 8. Overall somatic symptoms and pain showed decreases. Conclusions: Korean medicine treatment might be effective for somatic symptom disorder that is worsened after a traffic accident.

        Shprintzen-Goldberg syndrome with a novel missense mutation of SKI in a 6-month-old boy

        The Shprintzen-Goldberg syndrome (SGS) is an extremely rare genetic disorder caused by heterozygous variant in SKI. SGS is characterized by neurodevelopmental impairment with skeletal anomaly. Recognition of SGS is sometimes quite challeng-ing in practice because it has diverse clinical features involving skeletal, neurological, and cardiovascular system. Here we report a case of a 6-month-old boy who initially presented with developmental delay and marfanoid facial features including prominent forehead, hypertelorism, high arched palate and retrognathia. He showed motor developmental delay since birth and could not control his head at the time of ἀrst evaluation. His height was above 2 standard deviation score. Arachnodac-tyly, hypermobility of joints, skin laxity, and pectus excavatum were also noted. Sequencing for FBN1 was negative, however, a novel missense variant, c.350G>A in SKI was identiἀed by sequential whole exome sequencing. To our knowledge, this is the ἀrst case with SGS with phenotypic features of SGS overlapping with those of the Marfan syndrome, diagnosed by next generation sequencing in Korea.
