      • Gullian Barre Syndrome 患者 1例에 대한 症例報告

        Gullian Barre Syndrome is an acute symmetric polyradiculoneuropathy that typically presents as a progressive flaccid paralysis. The pathology is believed to be caused by both cellular and humoral immune processes The possibility of death is 3-5% and About 20% of patients require mechanical ventilaton. Recently We have experienced a patient who had suspected Gullian Barre Syndrome for 60days, his condition weakness and pain was impoved through the herb and acupucture Therefore I reported it for the treatment.

      • 원자력 발전소 근로자에서 업무상 질병으로 인정받은 급성 골수성 백혈병 1예

        저자들은 10여 년 동안 원자력 발전소 협력업체에 종사하면서 저선량의 방사선에 노출된 근로자로 급성 골수성 백혈병으로 진단 받고 사망 후 업무상 질병으로 인정된 증례를 보고하는 바이다. 증례에 대한 의무기록과 혈액검사 및 조직검사, 세포유전학 검사를 검토하였으며 환자의 작업 내역, 방사선 피폭량 및 동료 근로자에 대하여 조사하였다. 환자는 35세 남자로 1997년 11월 20일 경북 포항시 소재 D 대학병원에서 용접공에 대하여 실시하는 기타 중금속 특수건강진단(망간)에서 백혈구 수가 μL당 108.9×10³개, 혈색소는 8.6g/dL로 급성 백혈병을 의심하였다. 11월 24일 대구광역시에 위치한 K 대학병원을 방문하여 급성 골수성 백혈병으로 확진되었다. 골수검사에서 이소성 B 임파구성 표지자를 지닌 AML M2로 진단되었고, 염색체 검사에서 필라델피아 염색체가 발견되었으며, 46 XY t(1:4)(p32:p16)의 소견을 보였다. 항암 화학요법을 시행하였고 주기적인 항암 약물요법과 추적 관찰을 시행하였다. 1999년 5월 30일 급성 골수성 백혈병에 의한 합병증인 폐렴 및 패혈증으로 사망하였다. 본 증례의 방사선 피폭량은 1,853 mRem (18.53 mSv)이었으며, 방사선에 의한 암발생 원인확률은 미국 기준으로 5.4-6.1%이었다. 우리나라는 업무상 질병인정 기준에 방사선 노출에 의한 백혈병이 인정된다고 명시하고 있어 업무상 질병으로 인정되었다. 그러므로 우리나라 실정에 맞는 전리방사선 피폭량에 따른 업무상 질병 인정기준을 설정하여야 하며 방사선 피폭 근로자에 대하여 방사선 질환의 감시체계를 운영하여야 한다. Acute myelogenous leukemia (AML) has been known as an occupational disease induced by irradiation. We report a case of acute myelogenous leukemia with a history of low dose irradiation in a nuclear power plant. The authors reviewed the clinical and hematologic features of the case. We also reviewed the job contents of his workplace. We also interviewed other workers in the same workplace. The amounts of radiation exposure were reviewed. The worker was a 35-year-old male who had worked in a nuclear power plant for 11 years (1987-1997). Total amounts of radiation exposure during the work period was 1,853 mRem. He had been diagnosed as AML on November, 1997 at a routine periodic health examination. He died of refractory leukemia on May, 1999. The AML developed in this patient seems to be related to the exposure to radiation. To prevent irradiation injury, we must modify work procedures, begin comprehensive medical surveillance, educate workers on risks of radiation, and increase workers awareness of safety regulations in the workplace.

        According to the case study of patient with seguela of C.V.A who was treated by Percutaneous Endoscopic Gastrotomy(PEG), his chief complaint is the dysphagia. We treated him by a viewpoint of oriental medicine and got the good clinical symtom.

      • 철근콘크리트 연속보에서 부분프리스트레스 도입에 의한 휨보강 효과에 관한 연구

        A strengthening technique for reinforced concrete beams using external unbonded reinforcement offers advantages in speed and simplicity of installation over other, established, strengthening techniques. The purpose of this paper is to investigate the capabilities of a new retrofitting technique, namely external prestressing(Bow), for flexural strengthening of beams. The paper provides a general description of structural behavior of beams strengthened using the technique. Results of 2 physical tests on strengthened reinforced concrete continuous beams are reported and compared. It is shown that the technique can provide greater strength enhancement to lightly reinforced sections and that provision of deflectors enhances efficiency.

      • Supra Pubic Catheter를 施行한 腦卒中 患者 1例에 對한 證例報告

        According to the literary study on catheterization of bladder in one case of patient with C.V.A. who was treated by supra pubic catheterization, the results were as follows. 1. If not immediately treat ischuria, emergency disease, very great danger to life can happen by loss of kidney function. So we must treat ischuria instantly by consideration of treatment from the factor. 2. Catheterization of bladder generally acted in emergency room, is emergency treatment only in the lower part of urethral obstruction. 3. Catheterization of bladder was put on record in Son Saimiao's Qianjinyaofang, which is the first book on it. It had been developed by Luo Qianyi, Zhang Jiepin, and Wang Dao. 4. Ancient times, an urethral catheter was made of green onion leaf, pig bladder, goose feather and so on. 5. Nelaton is catheterization of bladder in functional or momentary ischuria. Indwelling catheterization is treatment keeping in urethral canal for a long time. 6. Supra pubic catheterization is a surgical operating catheterization when urethral catheterization is impossible, is very easy and can decrease risk of infection. 7. After catheterization, for decreasing risk of the infection, a complication of catheterization, it is must be taken precedence, ascetic treatment, constantly management and sterilization, anti-biotic medication. 8. At times, increasing patient of emergency disease, as C.V.A etc., the needing of catheterization of bladder is increasing also for preventing infection and bed sore by ischuria and incontinence of urine. Therefore a specialty knowledge and skill about it is necessary.

      • 반건식탈황법을 이용한 SOx/NOx의 제거에 관한 연구

        This study has been carried out to investigate the variation of removal efficiencies with SOx/NOx concentrations and the improvement of removal efficiencies with additives addition. The results are summarized as follows ; 1. For the SOx cases, the removal efficiencies of 95%, 95% and 90% were achieved at 300ppm, 1000ppm and 1500pm respectively with 15 Ca/S stoichiometric ratio in SDA-FF Process 2. For the NOx cases, the removal efficiencies of SDA were obtained extremely low levels. But the removal efficiencies of SDA-FF were achieved approximately 50% regardless of Ca/S stoichiometric rations and concentration loadings. 3. For the cases in variation of NOx removal efficiencies with additives addition, the improvement of removal efficiencies were obtained 5%, 10% by NaOH, NH₄Br respectively with 1.0 Ca/S stoichiometric ratio in SDA-FF.

      • 강관구속된 철근콘크리트 기둥의 변형성능에 관한 실험연구

        A new method to prevent reinforced concrete columns from brittle failure. The method is called transversely reinforcing method in which only the critical regions are confined in steel tube. The steel tube can change the failure mode of the latter columns from the shear to the flexure. The steel tube also increase the compressive strength, shear strength and deformation capacity of the in-filled concrete. This paper described results of an investigation conducted to study the ductility of high-strength RC rectangular short columns subjected to constant axial and reversed cyclic lateral load. The variables in the test program were the amount of lateral steel, and shear span to depth ratio.

      • 일정축력 및 반복 횡하중을 받는 철골철근콘크리트 기둥의 내력과 변형성능

        An experimental investigation was conducted to investigate the strength and deformation capacity of Wide Flange Steel Encased Reinforced Concrete(SRC) Columns subjected to constant axial compression and cyclic lateral loading conditions. Six one-third scale specimens were tested, each consisting of a structural steel size encased in reinforced concrete. The parameters studied in the test program included the effects of B/t ratios of encased steel(=13, 18, 23), ratios of axial force(=1/3, 1/6). As a result, capacity prediction based on superimposing the moment-axial-load interaction surfaces of the steel shape and reinforced concrete produced good results; the average of the experimental-to-predicted capacity ratio being equal to 0.99∼1.20. The observed hysteretic behavior of the test specimens indicated that adequated ductility can be achieved and the concrete core must be inhibited to buckling of wide flange.

      • 도시하천 악취물질의 발생원조사에 대한 연구

        This study has been carried out to investigate odor characteristics and removal methods on the water quality and sulfuric material of four urban sewage. The results from these experiments are as followings ; 1. For sediment, N contents(%) was shown 0.068mg/l at S-1, 0.109mg/l at S-2, 0.043mg/l at S-3, 1.233mg/l at S-4 and S contents(mg/g) was shown 2.192mg/l at S-1, 2.410mgA at S-2, 1.564mg/l at S-3, 15.146mg/l at S-4. The sample of S-4 was indicated the worst value to 1.233mg/l 2. NH₃, one of the representative odor materials, which was showed the highest value at S-1 in the water, showed the highest value at the S-4 in the air. It means that the cause of odor comes from the disintegration of sediment orgamisms, which has more important effect than the water pollution. 3. To control the odor materials, it is desirable to minimizes the inflow of sewage from pollutant source itself. One of the considering odor method is H₂O₂ or activated carbon input into the sewage, which has several advantages as chemical treatment of odor materials is to be desirable.
